This week's post catches up all of 2011 for worker's that we had not previously found. We are now combining search methods so you will once again see a change in The Weekly Toll. Some workers we have more information on than others, so please understand some will have minimal information compared to others. Later today the first post for 2012 will come online.

Jan 2, OSCEOLA, AR - 83 year old Johnny H. Brooks, and employee for Roller Swift Funeral Homes, died when he fell from a ladder and hit his head while trying to store decorations at the business.

Jan 4, PORTSMOUTH, VA - 60 year old William Harris died of natural causes while he was riding in a transport van to begin his shift at a VDOT tunnel booth.

Jan 5, RICHLAND, IA - 60 year old John Hamms died when he was engulfed in a grain bin. He was a farmer. No other details are available.

Jan 6, ANN ARBOR, MI - 67 year old Kenneth Levine died when he lost his balance while working on a ladder and fell to the ground. He was employed by L&S Plumbing.

Jan 6, SAN ANTONIO, TX - 44 year old Miguel Perez died due a fall from a roof while working to replace asphalt shingles. Mr. Perez was contracted by Fry Roofing, Inc.

Jan 6, SCARBOROUGH, ME - 44 year old John Plante was descending a wooden power transmission pole when he somehow fell to the ground. He was wearing safety equipment. He was employed by Hawkeye, LLC.

Jan 16, AVONDALE, PA - 40 year old Lucio Valderrabano died when he was engulfed in grain while working for Cardile Brothers Mushrooms. No other information is available.

Jan 20, NATCHEZ, MS - Steven Kyle Jones died of an unknown cause while working for Energy Drilling. No further details were given.

Jan 20, CARLISLE, AR - 56 year old Jacques Parker died at Four Parkers Farms when he was engulfed in a grain bin.

Jan 20, ITTA BENA, MS - 33 year old Deon Armstrong died buried in material at Live Oaks Planting Co. No details are available.



Jan 25, LAGRANGEVILLE, NY - 65 year old Claude Townsend died of a medical issue while performing his duties. He was employed by Lagrange Department of Public Works.


Jan 26, OMAHA, NE - 49 year old Richard Moscato died when he was pinned between a work dock and delivery truck. He was employed by Bimbo Bakeries, a Sarah Lee bakery outlet.

Jan 26, HAMPTON, NH - 48 year old Paul "Mike" Carney died of an unspecified cause while delivery his usual mail route.

Jan 27, SALEM, ND - 38 year old Richard Ries died in an oil field explosion incident near Keene.


Jan 27, HAGERSTOWN, MD - Herman Weaver died of unknown injuries suffered while working at the Holcim Inc.




Jan 28, MILPITAS, CA - Raul Zapata of Hayward working as a carpenter died in a home construction trench collapse. General Contractor is Richard Liu of US-Sino Investments.

Jan 30, DELMAR, WI - 52 year old Patrick Mateski of Stanley was found unresponsive inside a cow pen at a farm in Delmar.

Jan 30, CONRATH, WI - 34 year old Anthony Knoll died in a in the town of Cleveland.

Jan 30, EL CAJON, CA - 41 year old Gordon Woodard was struck in the head while he was attempting to turn the propeller of his airplane. He and another employee were using the plane for a business trip. He was employed by Southern California Soil & Testing, Inc.

Feb 3, SEATTLE, WA - 39 year old William "Bull" Ben Shelby of Bremerton died in a fall at Vigor Shipyards.





Feb 7, NILES, OH - 57 year old Jeffrey Uber died of a reported head injury at Better Management Corp. of OH.

Feb 7, WILSONVILLE, OR - Susan Asher, a teacher at Inza R. Wood Middle School in Wilsonville due to a fall or medical issue on February 2.

Feb 8, ALEXANDRIA, VA - 33 year old Joshua Weissman of Bristow died in a fall from a bridge while responding to a car fire. He was employed by the Alexandria Fire Dept.

Feb 8, DENVER, CO - 41 year old Dennis Mann died when the boom he was operating at O'Brien Concrete Pumping equipment yard touched an overhead electrical line.

Feb 8, HARLINGEN, TX - 51 year old Noe Cabriales was killed when he was struck by the force of a burst line while working at Great White Energy Services oil drilling site.




Feb 9, CRETE, IL - 47 year old Edward Randall, a mechanic, was working under a tour bus that fell onto him.

Feb 11, SUWANEE, GA - 45 year old Jeffery Lee McGhee was operating a skid-steer machine at a residential property on a steep incline and it impacted a tree causing McGhee's death.

Feb 11, KLICKITAT, WA - 44 year old Harold R. Malcolm Jr. of The Dalles, OR died in a fiery tanker truck crash.

Feb 12, BRECKENRIDGE, CO - 67 year old Ted Sams died while working at a residential construction site. He cut an artery in his leg with a saw and suffered massive blood loss.

Feb 13, GORE, VA - 21 year old Zachary Whitacre died after being thrown from the back of a fire truck that had crashed into a bank along the road. He was a volunteer for the Gore Volunteer Fire & Rescue Company.


Feb 13, FREEPORT, MN - 18 year old Craig Wiechmann died when he was pinned inside a silo. Reports say he was breaking up frozen silage when he got entangled in the auger.






Feb 13, TOWSON, MD - 35 year old Timothy Bartholomew was killed due to a fall from an automated parking lift at The Palisades parking garage.

Feb 13, TEXAS CITY, TX - 26 year old Virgel James Stoker died in an incident involving industrial equipment. He worked for Dallas Group of America's Inc. Texas City facility.

Feb 13, COLORADO SPRINGS, CO - 46 year old Mark Meschelle, an employee for Allied Plumbing, was killed when he was struck by another vehicle unloading items from the back of his work van.

Feb 14, INDIANAPOLIS, IN - 64 year old Arnold O. Rexroad of Effingham, IL was killed when he was pinned between his disabled semi truck and tow truck when it was struck from behind by another semi truck on a snowy roadway.

Feb 14, SHELTON, CT - 46 year old Francisco Amaral of Bridgeport died after the roof of a condo unit in Shelton. He was J.C. Silva Remodeling Services in Bridgeport.

Feb 14, JACKSONVILLE, FL - 42 year old Peter Jerome Michnick of Carney, MI, died when he fell from the back of a boom truck, striking his head before falling into the St. John's River. He was employed by Osmore Railroad Services.

Feb 14, DUBLIN, OH - 46 year old Mohamed S. Hassan was found shot dead inside a Sunoco Gas Station by a co-worker arriving for work.

Feb 14, DENVER, CO - 29 year old Carlos R. Navarette died when he was a forklift and steel rolls on his work site. His employer is unknown.

Feb 15, ASHEVILLE, NC - Herman Glenn Wooten, a truck driver, died when his rig went off the roadway and he became trapped in the truck's cab.

Feb 15, MOSES LAKE, WA - 56 year old Bill Massey of Soap Lake died when a concrete wall collapsed and toppled over on top of him. He was working at Two Rivers Terminal chemical warehouse in Moses Lake.


Feb 15, TEMPLE, TX - 71 year old James Johnson lost his battle to survive after injuries he received when his school bus collided with a delivery truck on Jan 17. He was a driver for Academy ISD, having worked for the district for 27 years.


Feb 16, AMARILLO, TX - 58 year old Tommy Barrett was killed when his Hereford School District truck exploded when he turned the ignition. He worked as a plumber for the district for 12 years.

Feb 17, MIDDLEBURG, FL - 35 year old Detective David White was shot and killed when he was at a home investigating a meth lab. He was part of the Clay County Sheriff Department.




Feb 17, TEXARKANA, TX - 20 year old Stephen Rodney Cutchall of Atlanta, TX was killed when a heavy metal support beam broke loose and struck him in the head. He was working at Jackson Pipe & Steel in Texarkana.

Feb 18, PHOENIX, AZ - 60 year old Robert Demarco died when he became trapped between two baggage handling conveyor belts. He was an employee for US Airways, working at the Sky Harbor Airport in Arizona.

Feb 18, CLINTON, NC - 26 year old Brandon Christopher Taylor died after being overcome by toxic fumes as he cleaned out a tanker while working at Sampson pork packing plant.

Feb 18, ATLANTA, GA - 34 year old Samuel Wallace was shot and killed by a man who entered a Pizza Hut store in search of his ex-girlfriend who worked at the store. After killing Wallace the shooter turned the gun on himself. The ex-girlfriend was unharmed.

Feb 19, PORTLAND, OR - 57 year old John Summers of Pe Ell, OR died when he fell into a vat of toxic and corrosive chemicals while on a barge at Swan Island Basin. It is unclear who Summers was working for.



Feb 19, PORTLAND, OR - 30 year old Robert Greene was shot and killed while working as a bouncer/security for Grand Central Restaurant & Bowling Lounge. He was attempting to secure a rowdy group near the front door when gunshots were heard.


Feb 19, DENVER, CO - 43 year old Leonardo Soledad-Garcia died at a construction work site when he fell into a 5-foot deep trench and was impaled on a sharp object. His employer is not named.

Feb 20, LARADO, TX - 31 year old Roberto Rodriguez Jr. died after he fell from the side of building. He was doing some work on the side of the El Catan Restaurant and the rope he was using tore and he fell to the ground. It is unclear who his employer was.

Feb 20, CHICAGO, IL - 45 year old Charles Kimbrough died when he was pinned between two vehicles while attempting to unload a delivery truck. While at the back of the delivery truck, another driver caused a chain reaction when he struck another vehicle causing Kimbrough to become pinned.

Feb 21, HONOLULU, HI - Gilbert Macalma died when he was caught and partially buried in a trench collapse at a home construction work site in Waialae Iki Ridge.

Feb 21, PALM HARBOR, FL - 49 year old George Karalis and another employee were closing up the Ferman BMW dealership where they worked when Karalis fell from the back of a golf cart when the driver made a turn.

Feb 21, SAVANNAH, GA - 26 year old Fernando Aburto died of head trauma while working at Southern Metals Recycling. Reports state he cut his hand and upon seeing the blood he passed out and struck his head on the concrete.


Feb 22, MCCUTCHANVILLE, IN - 18 year old volunteer firefighter Jeremy Tighe died when the fire truck he was a passenger in ran off the roadway and overturned. He was a member of the McCutchanville Fire Department.





Feb 22, BRISTOL, VA - Clarence Collins was the victim of a farm tractor roll over while driving on a roadway. Passersby saw the tractor, stopped and called for help.


Feb 22, PORT NEWARK, NJ - 47 year old Earline Brundage of Phillipsburg died when she was crushed between two bus-sized shipping containers. She was working at the Port Newark Container Terminal and was a member of the Longshormen's Association.



Feb 23, TACOMA, WA - 44 year old Trooper Tony Radulescu was shot to death by a man he had pulled over along State Route 16. The man later turned the gun on himself when deputies went to his residence. Radulescu was a Washington State Trooper.

Feb 26, JEAN, NV - 57 year old Robert Vanwettering of Nevada died of blunt force chest trauma when his tractor trailer rig collided with the rear of another semi truck near Jean.

Feb 17- 60 year old Robert Demarco of Phoenix, AZ. Crushed in conveyor belt while working for US Airways.

Feb 20- 59 year old Ricky Paul Phillips of Oak Ridge, TN. Killed when his tractor trailer ran off the road and hit several trees. Employer: Natures Best Organics.

Feb 20- 26 year old Fernando Aburto of Savannah, GA. Fainted after he cut his hand working for StaffCo and hit his head on the concrete surface.

Feb 21- 40 year old Macalma Gilbert of Waipahu, HI. Killed when he became trapped by a collapsing mound of dirt while working for W.A. Felix Brothers.


Feb 22- 47 year old Earline Brundage of Elizabeth, NJ. Pinned by shipping containers while working at Port Newark Container Terminal.









Feb 22- 18 year oldJeremy Tighe. Employer: McCutchanville Fire Station of Evansville, IN. Killed in crash.



Feb 23- 44 year oldTony Radulescu. Was shot while working for Washington State Police of Port Orchard, WA.







Feb 23- 58 year old Michael Eugene Wells. Killed in a trench collape in Gatlinburg, TN. Contracted by NEO Corp for Veolia Water North American of Candler, NC.


Feb 23- 50 year old Sam Wilsonof New Albany, MS. Killed when he was caught in machinery while working for Best Foam Company.


Feb 24- 56 year oldJavier Hernandez of Richmond, VA was a landscape worker that was struck by a falling tree.


Feb 24- 28 year oldNickafor Zephyrineof Savannah, GA was killed after being shot in the head by a co-worker while working at OA Logistics.








Feb 24- 58 year old Ismael Rodriguezof Wimauma, FL was killed when the dumptruck he was driving for J&M Pavement overturned.



Feb 24- 58 year old Joseph Rossiof Verona, NJ was shot and killed at his business, Phoenix Amusement Company.

Feb 25- 48 year old Preston Ross Jr. Suffered a heart attack while on duty for the Chicago Police Dept of Chicago, IL.








Feb 25- 43 year old Jesse Guerrero. Employer: Supreme Vacuum Services of San Antonio, TX. Was killed when his big rig broke down and another truck struck him when he stepped out.


Feb 27- 51 year old Todd Richardsonof Colfax, WI. He was killed when he was transporting fuel for Kane Transportation INC when his truck left the road, crossed the median, crashed and caught fire.









Feb 27- 62 year old Dale Eugene Thompson of Hazleton, ID. Was killed when his semitrailer rolled off of an overpass and landed on the railroad tracks below.


Feb 27- 53 year old Estaban Leos. The construction worker from Mississippi was killed when he was installing cable barriers on the interstate and a vehicle struck him.



Feb 27- 64 year old C. Dennis Cash of Madison Heights, VA. The owner of Cash's Garage was struck by the driver of a passing 18 wheeler that wedged him between his own tow truck.







Feb 28- 38 year old Roger Dale Robertsfrom Owsley County in Kentucky was killed while working for Kendall Tree Service. He was cutting trees along the highway when a branch fell and landed on him.

Construction Worker Falls

Feb 27, CARTHAGE, MO - 67 year old Toby Pauley died after falling about ten feet while working with others on the Carthage Masonic Lodge where he was also a member. He fell headfirst from the rafters to the concrete below.



Armored Guard Killed by Partner

Feb 28, PITTSBURGH, PA - 31 year old Michael Haines was murdered by his co-worker who stole $2 million they had picked up during the day. The co-worker drove and hid the armored truck under a bridge where authorities found Haines' body.



Postal Worker Dies of Trauma

Feb 29, COMPTON, CA - 32 year old Earl Anthony Dunn, a postal worker, has died after his legs were severed in an accident. Dunn was standing behind his postal truck when another driver veered off the road and pinned him between the vehicles.


Firefighter Struck and Killed

Feb 29, COTTONWOOD, CA - 35 year old Capt. Mark Ratledge died when he was struck by an out of control driver during a storm. Ratledge was at the scene of another vehicle spin-out when a driver lost control and slid into Ratledge and a police vehicle. He was employed by the Cottonwood Fire Protection District.


Farming Incident Causes Worker Death

March 1, WESTERN, NE - 33 year old Justin Kunc died after he was involved in a farming accident. His clothing became caught in a sweep auger of a grain bin near Crete. He was rushed to a hospital where he died.


Worker Killed in Plant Incident

March 2, BOYD, KY - 27 year old Shaun Shelton of Martin died of asphyxiation in an incident that involved a conveyor belt. He was employed by Stein, Inc., contracted to crush rock and slag for AK Steel. No other details are available.


Utility Worker Pinned by Machinery

March 2, AUSTIN, TX - 52 year old Timothy Louvier died when he became pinned by a backhoe while an Austin Water Utility crew was restoring water lines. No other details are available.



Plant Worker Killed in Explosion

March 2, HARLEM, GA - 70 year old Homer Gay suffered a fatal head injury when an air compressor he was working near exploded. He was a long time employee for Tracy-Luckey Pecan plant in Harlem.



Man Dies on Oil Rig

March 2, STANLEY, ND - 22 year old Jason McDonald of Vernal, UT died when he fell from an oil derrick near Stanley. He fell approximately 80 feet from the derrick to the platform. An investigation is underway.



Mine Foreman Killed

March 3, BLAIR, KY - 32 year old James Bailey of Loyall was killed while doing work on a mine shuttle car at the Timber Tree Mine near Blair. Bailey, a Mine Foreman, was installing a protective covering over a shuttle car, called a canopy, when the canopy collapsed on him.



Industrial Incident Kills Worker

March 3, DECATUR, AL - 50 year old Joseph Andrew Sparks was killed when a tire wheel he was working on broke apart and sent debris flying. He was working on the tire of a piece of industrial equipment when the wheel broke apart. He was employed with TCI Tire, Inc.


Water Worker Struck by Car

March 3, SYCAMORE, OH - 49 year old Gary Cooper of Sycamore, an Ohio American Water employee, was working on a valve at West Davis and Wall streets when a car driven by an 83 year old woman struck him. Police say driver inattention caused the accident.



Plant Worker Hit by Pipes

March 3, UMATILLA, OR - 31 year old Thomas "TJ" Heap died after being struck in the head by a pipe. He was working at JM Eagle Manufacturing plant in Umatilla when something caused the pipe to fall. Oregon OSHA is investigating.



Employee Killed in Robbery

March 4, NEWARK, NJ - 55 year old Calvin Johnson, a 35 year employee of the family owned Golden Pizzeria & Grill, was shot to death by robbery suspect. The suspect attempted to rob the cashier when Johnson tried to intervene. It was then he was shot twice in the chest.


Store Employee Murdered

March 4, MERRILLVILLE, IN - 48 year old Judi Simpson-Beaver was working her shift at the Lucky Food Market when a robbery suspect walked in the store. Video recording shows Simpson-Beaver complied with the suspect when he jumped the counter to get to the cash drawer. But he shot her anyway, twice.


Firefighter Killed on the Job

March 4, COLBY, WI - 34 year old Jamison Kampmeyer died when he was trapped under debris of a roof collapse. Kampmeyer, a firefighter for the Colby Fire Department, was fighting a building fire when the roof collapsed, raining bricks and debris down on him and other firefighters.


Tragedy at the Race Track

March 4, OKLAHOMA CITY, OK - 24 year old Mike Priddy died while working maintenance at Remington (horse race) Park. Reports state that Priddy was closing a track gate to keep a running horse from escaping the track when the horse struck the gate. The gate swung hard into Priddy who died of internal injuries.


School Janitor Perishes in Fire

March 5, VIDRINE, LA - 58 year old Donald Thomas was a beloved 22 year employee for Vidrine Elementary School. He was in a storage supply building when a fire broke out and he was trapped inside. An investigation to what caused the fire continues.


Man Removing Logs Struck and Killed

March 5, ARCADIA, WI - 45 year old Patrick George died after he was struck in the chest by a tree branch. He was operating a skidder in the town of Trempealeau when he was struck by the log.


Tree Trimmer Electrocuted

March 5, LAKE VIEW, IA - 63 year old Edward Paul Peters died when he was electrocuted while trimming trees. Owner of Peter's Painting and Tree Trimming, he was leaning against a boom truck when the arm of the bucket touched a power line. Another employee was inside the bucket but was uninjured.


Worker Caught in Auger

March 6, MONCKS CORNER, SC - 48 year old Lamar Ravenell died after becoming tangled in a cement auger. Ravenell was performing maintenance on the machine at Sanders Brother's asphalt plant when he was caught in the machine.


Fired Employee Murders Headmistress

March 6, JACKSONVILLE, FL - Dale Regan, Headmistress for Episcopal High School in Jacksonville, was shot and killed by a former employee when then turned the gun on himself. The fired employee returned to the school with an assault rifle in a guitar case and walked into Regan's office where he shot her.


Deputy Shot by Burglary Suspect

March 6, MELBOURNE, FL - 53 year old Deputy Barbara Pill was shot and killed when she was questioning a suspect at a local motel. Pill was requesting back-up and speaking to the suspect when the suspect pulled a gun and shot the deputy. She was a 15 year veteran of the Brevard County Sheriff Department.


Worker Struck by Equipment

March 6, MASSIES MILL, VA - 28 year old Steven Evans was killed when he was struck by a large piece of machinery. Evans was working on a track loader and when he walked behind it, it rolled backwards running over him. No employer is listed.


Fuel Tanker Hit by Train

March 7, BARTOW, FL - 51 year old Thomas O'Neal Ruff of Lake Wales was killed when the fuel tanker truck he was driving was struck by a train causing a massive fire. He was driving the tanker, full of diesel, on property owned by Mosiac phosphate company when he failed to stop for the oncoming train. He was unable to escape the truck.


Steel Beam Crushes Worker

March 7, DALLAS, TX - 57 year old Jose Villanueva died when he was crushed under a falling beam. He was helping to load steel beams onto a semi truck trailer when the beam fell onto him. He was employed by Red Steel Company.


Road Worker Struck

March 9, ST. CLAIR SHORES, MI - 26 year old Justin Vallad died as the result of being struck by a vehicle. He was a construction worker in an I-94 construction zone when a semi struck the back of an SUV which was pushed into Vallad before hitting another vehicle.


Hotel Maid Found Dead

March 11, ALBUQUERQUE, NM - Romona Montoya Leon was a maid working at the Plaza Inn when she was discovered dead inside a room. Police have a grainy video that shows a possible suspect in her death.



Cab Driver Murdered

March 11, HARRISBURG, PA - 37 year old Atlas Simpson was shot in the head during a robbery. He was a cab driver for Capital Cab Co. His murderers have been been arrested.




Truck Driver Killed in Watery Crash

March 12, SCHRIEVER, LA - 30 year old Otto F. Tello of Channelview, TX drowned when his Brammer Engineering truck rolled off a bridge and into a canal. Tello was hauling a load of pipes which shifted as he was driving over the bridge causing the rig to fall into the water.


School Bus Crash Kills Two

March 12, INDIANAPOLIS, IN - 60 year old Thomas Spencer II died when the Lighthouse Charter School Bus he was driving crashed. A 5 year old student also died. It is unclear if a medical condition contributed to the crash.



Semi Truck Driver Killed in Wreck

March 13, CAMPBELL COUNTY, TN - 48 year old Steven C. Wright of Greenback died of blunt force trauma when the tractor trailer he was driving slammed into the back of another big rig. Wright may not have realized that two other tractor trailer trucks were slowing down on Interstate 75 and he ran into the back of one, causing a chain reaction. The other drivers were hurt.


Construction Worker Dies From Fall

March 14, HANOVER, NH - 53 year old Dana Lowe of Morrisville, VT died as the result of a fall at a construction site. A hook block of a crane inadvertently made contact with a scissor lift Lowe was working on which caused the lift to topple over, and he landed on a concrete deck. He was employed by CSE, Inc.


Truck Driver Dies in Violent Crash

March 14, TURKEYFOOT, PA - 31 year old Robert Dodson Jr of Duncansville was driving a large Pepsi truck when for an unknown reason he veered across the lane and struck a school bus head on. Dodson died at the scene and several students were transported to a hospital.


Logging Incident Claims Life

March 14, HOPE, IN - 71 year old Lowell Miller died of blunt force trauma to his head when a falling tree branch struck him . Miller was working with two sons and another man to clear timber. Miller was wearing a safety hard hat, face guard and ear protection at the time. He was owner of Hope Hardwoods and Hope Warehouse.


Contract Worker Electrocuted

March 14, PINE BLUFF, AR - 28 year old Ryan Bullard of Mayflower was electrocuted when a pipe he was working with accidentally came into contact with a power line. He was employed with Snyder Environmental doing work for Pine Bluff Arsenal.


Employee Shot During Altercation

March 14, SALT LAKE CITY, UT - 26 year old Steven Guadalupe Chavez, the son of C J's Smoke Shop was shot and killed during an altercation at the shop. An individual involved in a disturbance left but later returned with a gun. Police are pursuing leads.


Mechanical Failure Causes Death

March 14, MT. PLEASANT, TN - 67 year old Jerry Nance died when the lift bucket he was in fell to the ground. Nance was in the lift when mechanical issues caused the hydraulic arms failed and buckled, causing the lift to drop to the ground. Nance was an employee for Maury County Highway Department.


Fire Captain Suffers Medical Issue on Duty

March 14, HOUSTON, TX - 49 year old Sr. Captain Thomas "Bill" Dillion died after collapsing on a fire call. After extinguishing a kitchen fire in an apartment complex he began suffering chest pain.


Counselor Killed by Student

March 14, LOS ANGELES, CA - 49 year old Dwayne Alexander was allegedly stabbed to death by a student who was then restrained by another student while awaiting police. Alexander was a counselor for Job Corp. Authorities have not released a reason for the murder but the student has been arrested.


Ambulance Driver, Patient Die in Crash

March 14, CHILLICOTHE, OH - 21 year old Cody A. King and his patient were killed when his Life Ambulance collided with a lumber hauling truck. The force of the impact caused some of the lumber to propel forward, smashing through the front windshield of the ambulance.


Food Plant Worker Dies

March 15, DAKOTA CITY, NE - 37 year old Rodney Bridgett, an employee for Tyson Foods, died when a piece of machinery he was working on collapsed onto him.


Electrician Dies in Fall

March 15, CHICAGO, IL - 47 year old Jim Weinberg of McHenry was found on the ground beside his ladder at a downtown high-rise Thursday by a co-worker returning from lunch. The worker immediately called police. It is unclear if Weinberg suffered any type of medical issue that may have caused his fall.


Worker Suffocates

March 15, BRUSH, CO - 56 year old Raymond "Kat" Segura died he was buried under a mountain of pinto beans. Segura worked for Kelley Bean Co., and was working in a warehouse with a conveyor belt. It is unclear how he came to be buried.





Worker Suffers Fatal Head Injury

March 15, BERKELEY, CA - 30 year old Ariel Munoz-Garcia of San Pablo suffered a fatal head injury when the pneumatic hand grinder he was using broke apart and sent shrapnel flying. He was struck in the left side of his head.



Highway Worker Struck

March 22, WATERBURY, CT - Daniel Dinardi of Rocky Hill, was struck by a vehicle while working along I-84. He was a CT DOT employee.




Worker Killed in Robbery Attempt


March 23, DURHAM, NC - 65 year old Naomi White was working at J.T. Tire store when a robbery suspect came into the store and shot and killed her and a patron and injured two other employees.


Worker Crushed in Machine

March 27, SYCAMORE, IL - 56 year old Evaristo Alvarado of Sandwich died after he was crushed in the lifting mechanism of a forklift he was servicing. He was employed by Menards.


Death on Construction Site

March 27, KENT, OH - 49 year old Russell Hirschfelt of Barberton died on a construction site when he was pinned between a backhoe and stack of concrete slabs. He died later at a local hospital.


Worker Dies of Natural Causes

March 28, DALTON, GA - 51 year old Pascual Apodaca died of a reported heart attack while working to clean out a tank of latex at Global Textile's plant in Dalton.


Farmer Killed in Tractor Mishap

March 28, CENTERVIEW, MO - 71 year old John E. Lee was killed when he was run over by his farm tractor. A mechanical issue caused the tractor to swerve into a tree when Mr. Lee was thrown off but them the tractor rolled over him.


Painter Falls From Bridge

March 30, BRONX, NY - 35 year old John Massas died after falling from Throgs Neck Bridge into the Long Island Sound. He was adjusting his safety harness when something went wrong and he fell into the water and presumed drowned. He was employed by Nuco Painting Corp. of Long Island.



Worker Drowns in Fall From Bridge

March 28, ST. LOUIS, MO - 35 year old Andy Gammon of Park Hills, MO died when a piece of equipment he was operating toppled into the Mississippi River while working on a bridge project. He was employed by Massman Taylor Alverici, a Joint Venture.


Port Worker Killed

March 28, SAVANNAH, GA - 57 year old Dianne Pinckney Cobb was killed when she was struck by a forklift at Georgia Ports' Ocean Terminal. She was a member of the International Longshoreman's Association Local 1475.


Recycling Worker Crushed

March 29, TERREBONNE, LA - 54 year old Anthony Giles of Rockford, AL died while working at Mars Modern American Recycling Services in Terrebonne. He was using a cutting torch on the structure when it fell and trapped his lower body. He died after arriving at an area hospital.


Tree Trimmer Electrocuted

March 29, PHILADELPHIA, PA - 47 year old David Ruiz was electrocuted when he came into contact with a high tension electric line. Ruiz was trimming trees in Huntingdon Valley when he came into contact with the line.


Skydive Instructor Dies

March 30, SEBASTIAN, FL - 30 year old Jason Eisenzopf, originally from Fitchburg, MA, died after a hard landing while working for Skydive Sebastian. He is reported to have come in too fast for landing and his foot clipped a ditch and he suffered severe head trauma on landing.


Volunteer Crushed Under Tractor

March 30, SANTA FE SPRINGS, CA - 73 year old Ramon Arthur Gibson died after he was crushed under a tractor that overturned onto him. He was a long time volunteer at Hathaway Ranch and Museum.


Restaurant Employee Murdered

March 31, DELAND, FL - 43 year old Tom Walsh was shot and killed in an apparent robbery attempt while working at the Mainstreet Grill in Deland.


Ski Area Worker Suffocates

March 31, YAKIMA, WA - 22 year old KyOna Hoff, an experienced skier, died after she reportedly fell head first into a piled up section of snow called a tree well and apparently suffocated. She worked for the White Pass Ski area outside of Yakama.


Semi Truck Collides With Train

March 31, PLUMMER, MN - 65 year old Dale J. Buzzell of Isanti, MN was killed when his tractor trailer rig somehow collided with a train crossing the roadway. No other details are given.



Deputy Dies in Car Crash

March 31, CHARLES CITY, VA - 36 year old Michael Walizer was responding as back up to a traffic stop when he lost control of his vehicle, going off the roadside and back on, before crashing into a tree. He was a deputy for Charles City Sheriff Dept.

Three Employees Killed in Robbery

April 1, FARMVILLE, NC - 26 year old Nabil "Sam" Al'Mogannahi, 24 year old Gaber "David" Alawi and 16 year old Mokbell "Sam" Almujanhi, all related, were shot and killed during a robbery at the family owned convenience store Hustle Mart 3 in Farmville.

April 2, AVOCA, WI - 78 year old Alfred Albrecht died when he was attacked by a bull he owned while feeding livestock. He was a farmer.

Truck Driver Crashes Over Embankment

April 2, BUFFALO, WY - 44 year old Oscar L. Vizcarra of Lewisville, TX, a truck driver, was hauling a load of bentonite when the truck jack-knifed and went over a 300-foot embankment near Buffalo, killing him. His employer is unknown.

Truck Driver Killed in Crash

April 2, HARRIMAN, TN - 52 year old Rupert Halstead of Russellville, AR was killed when he lost control of his tractor trailer and it overturned on I-40 near Harriman. His employer is unknown but he was hauling bologna when he crashed.




School Worker Killed in Shooting Rampage

April 2, OAKLAND, CA - 24 year old Katleen Ping was an administrative assistant for Oikos University in Oakland when a man entered the building looking for a female administrator. Angry that the woman was not there, the gunman proceeded to shoot Ping and then opened fire on students before leaving and later being arrested.

Crane Collapses Onto Worker

April 3, MANHATTAN, NY - 30 year old Michael Simmermeyer of Burlington, NJ was killed and others injured when a construction boom crane collapsed to the ground and he was crushed under it. Simmermeyer was working on No. 7 Subway line for Metro Transit Authority but it is unknown who was his direct employer.

Apr 12 - MODESTO, CA - 53 year old Robert Paris died when he was shot while attempting to serve an eviction notice. He was a deputy for the Stanislaus County Sheriff.

 Hotel Employee Falls Into Elevator Shaft

Dec 29, 2011 - San Antonio, TX - 65 year old Gloria Rodriguez fell to her death down an elevator shaft while working for the Crockett Hotel in San Antonio.

Semi Truck Backs Over Employee

Jan 4, San Antonio, TX - 40 year old Solomon W. Roberts died when he was crushed when a semi pinned him up against a loading dock.  He was the manager of CertiFit Auto Body Parts. 




Sanitation Struck by Work Truck


Jan 9, Newark, NJ - 38 year old Teeshon Small of East Orange suffered fatal injuries when he was struck by the sanitation truck he was attempting to jump on. He was employed by Newark Sanitation.


Worker Crushed by Equipment

March 2, Harleyville, SC - 32 year old DeWayne C. Smith was killed when he was pinned by a boom while operating a backhoe. It is unclear who Mr. Smith worked for.   


Worker Dies From Fall

March 6, Anchorage, AK - 47 year old Paul Ackerman of Fairbanks died while working at the SouthCentral Power Project in Anchorage.  He fell about 17 feet from an elevated platform.


Worker Struck by Semi

March 13, Knoxville, TN - 46 year old Tom Kerley died when he was struck by a semi backing down the driveway of a business. Kerley worked for ASG Ameri-Lam and the driver of the truck was employed by a transport company. 


Construction Worker Dies from Falling Material
March 19, Goffstown, NH - 19 year old Tyler Walsh was killed when he was struck in the head by a piece of metal.  Walsh was working on a construction site when he heard someone yell.  As he looked up a metal staging bracket was falling from the third story and struck him in the forehead.


Worker Struck by Vehicle

March 20, Dallas, TX - 53 year old Faustino Delapaz of Dallas was struck and killed by a catering truck while working at the Dallas/Fort Worth Airport. He was struck in the chest by the truck, knocking him down and running over him.  


City Worker Dies in Manhole

March 21, Sulphur Springs, TX - 42 year old Francisco Melendez, an employee for the City of Sulphur Springs, drowned inside a manhole. Authorities say he was in the manhole attempting to remove a plug that allows water to flow. Somehow his foot got stuck and he drowned as the hole filled up with water.  


Industrial Incident Claims Worker's Life

March 23, Salt Lake City, UT - 34 year old Inoke Langi died when he was crushed under a piece of equipment.  Langi was employed with Cambelt International and he was working under a piece of equipment.  For an unknown reason, the equipment shifted, falling onto Langi. 


Worker Crushed Under Falling Material

March 23, Chino, CA - 34 year old Vays Dzhanayez of North Carolina was killed while offloading material at Herrington Industrial Plastics in Chino, CA.   He was removing straps from a load but it shifted and fell onto Dzhanayez.


 
 Worker Suffers Fatal Head Injury


March24, Wyoming County, PA - 36 year old Jason Slavish of Luzerne died in an industrial incident at the Proctor & Gamble Paper Products Plant. The cause of the incident is not known, but authorities say he died of head trauma.


Contractor Dies at Refinery

March 25, El Paso, TX - 65 year old Herbert Casias died when he was crushed by a falling pipe while working as a contractor for Western Refining.

Metal Structure Collapses on Worker

March 29, Terrebonne, AL - 54 year old Anthony Giles of Rockford died when a metal structure he was using a cutting torch on collapsed onto him.  Giles was working for Mars Modern American Recycling Services.


Construction Worker Killed by Falling Debris

April 10, Chicago, IL - 37 year old James Quigley of Evergreen Park was killed by debris falling onto him at a construction site. Authorities say the debris may have been a piece of metal.  It is unknown who Quigley worked for.  


Tank Rupture Kills Worker

April 12, Broken Arrow, OK - Wesley Little died when a pressurized compressed natural gas tank he was working on ruptured, throwing him flying about 25 feet. He was unscrewing a valve on the tank when it exploded off the tank, hitting Little in the chest. He was working in front of his CNG Oklahoma Auto Sales business when the incident happened.


Construction Worker Struck by Forklift

April 18, Plano, TX - 24 year old Michael Pishnick was killed on a Balfour Beatty construction sitewhen a lull, or large forklift struck him. Pishnick was employed by MEI Rigging and Crating.


Forklift Incident Kills Laborer

April 19, Helendale, CA - 32 year old Jose Reyes of Santa Ana died after being struck by a forklift while helping to line a waste water pond. The forklift was moving heavy rubber rolls.  No one witnessed the incident but Reyes died of head trauma. 



Man Electrocuted at Feed Yard

April 24, Allen, NE - 29 year old Derrick Roth was electrocuted while working on a utility pole at the Dixon County Feed Yard in Allen. He was working for a private contractor at the feed yard.  Authorities do not know how or why he was electrocuted. 





Two Men of Toxic Exposure

April 26, Sylvester, TX - 19 year old Nicholas Walker and 19 year old Jeremy Claxton died when they were apparently overcome by carbon monoxide fumes. The men were working with a gas-powered water pump at the bottom of a hand dug 30 foot water well at the Pfluger China Creek Ranch.  It appears one man entered the well and fell unconscious, then the other man entered the space and also fell unconscious.  One man was submerged in water. 



Steel Worker Dies

April 27, Rapid City, SD - 26 year old Adam Klynsma died when he was struck by a 12 foot, 1,600 pound steel beam while working at TrueNorth's Steel fabricator's business. It is unclear how the incident happened.

 Killed at Work

April 9, Moss Point, MS - 24 year old Christopher Hebert died due to blood loss while working at Omega Protein Seafood plant.  Circumstances are unclear but reports say he was working on a conveyor belt and seems to have become caught in the machine.   

 Worker Falls Into Machinery

April 12, 2012 - Mt. Pleasant, TN - 44 year old Keith Frierson died while working at Tennessee Aluminum Processors. He was clearing scrap metal and apparently noticed the hopper of a shredder machine clogged.  He someone slipped and fell into the machine.  

Bridge Worker Killed

May 3, Austin, IN - 50 year old Roger Lee Cox was killed while working on the construction of the Milton Madison Bridge. Authorities say it appears Cox was crushed by a piece of equipment.  Cox had been working from a bucket when he was pinned between the bucket and the object he was working on.

Industrial Incident Kills Worker

May 3, Carenco, LA - 33 year old Kevin "Slim" Benjamin died when he was pinned between two pipes that he had been working on.  Benjamin worked for Frank's Casing Crew. 

Man Killed by Falling Equipment

May 3, Amelia, LA - 22 year old Oscar Pharegood of Napoleonville,died after a piece of equipment fell on him while he was working at Allison Marine, authorities said. A crane was lowering a pressure release vessel from a platform to the ground and when Pharegood was releasing the cables, the vessel began to lean, and it fell on him.

May 7, Festus, MO - 54 year old Robert N. "Robb" Jett died when a forklift fell off a loading dock and onto him while he was working at Schroeder & Tremayne, Inc.   It is unclear if he was driving the forklift as it fell.

 Boom Truck Operator Killed at Work

May 8, Seward, AK - 52 year old Felix Morales of Lawndale, CA was killed when the boom truck he was operating tipped over, throwing him from the cab and onto a ship's deck below.  Morales was employed by Icicle Seafoods and has been traveling from California to Alaska to work the fishing season for a number of years.

 Tank Explosion Kills Worker

May 9, El Paso, TX - 36 year old Gerardo Morales Gutierrez died after a water tank he was working on exploded.  He was standing on top of the tank, welding, when the explosion occurred.  It is unclear who Gutierrez worked for.

Longshoreman Crushed to Death

May 14, Port Manatee, FL - 48 year old Robert L. Dixon of Tampa, was killed when he was crushed under a 5,000 pound crane basket at Port Manatee.  Dixon was helping to offload a ship when he became pinned under the crane basket.  Dixon was employed by Logistec USA, Inc. 

Tree Trimmer Electrocuted

May 14, Fort Worth, TX - 31 year old Jose Murcia of Arlington was electrocuted when he came into contact with power lines.  He was trimming a tree at a home in Fort Worth when the incident took place.  His work partner called 911. 

Equipment Incident Claims Life

May 15, Tallmadge, ON - 22 year old Andrew Elko of Middlefield was killed in an industrial incident at Dayton Freight Lines.  Authorities have said Elko died of significant injuries related to a tow motor.  No other details are available. 

Worker Crushed Under Building Collapse

May 15, Augusta, ME - 54 year old Robert "Bobbie" Rodriques died of crushing injuries when a building he was working under collapsed onto him.  Rodriques was kneeling under the building attempting to repair a post foundation when some support beams gave way.  His boss was also under the building and barely escaped being crushed.He was employed by Kennebec Home Improvements and the building was at a summer camp on Lovejoy Pond.

Railroad Engineer Dies 


May 18, Upper Merion, PA - 43 year old Jefferson L. "Karma" Troester of Claymont, DE died when a giant roll of newsprint pager fell onto him as he opened the rail car doorway. He was engineer for Upper Merion & Plymouth Railroad.




Exploding Equipment Kills Worker


May 21, Pascagoula, MS - 39 year old Jeffery Simpson died while working for MS Phosphates, a fertilizer plant. Authorities say it appears a piece of equipment exploded, killing Simpson.  The equipment was in a portion of the plant that was not operational at the time but maintenance was being performed. 


Three Oil Company Workers Die in Blast in Arkansas


 May 21, Norphlet, AR - 24 year old Coley Gene Lee of El Dorado, AR was killed when an oil tank he and two other workers were taking apart exploded.  Authorities believe fumes left inside the tank ignited.  All three men worked for Long Brothers Oil Company. 







May 21, Norphlet, AR - 38 year old Mercil "Shane" Chelette of Louann, AR was also killed in the blast while working to take apart an old oil tank working for Long Brothers Oil Company.







May 21, Norphlet, AR - 24 year old Lee Randall Ross of Smackover, AR was the third worker who died in the oil tank explosion while working for Long Brothers Oil Company.







Father and two Sons Die in Manure Pit Incident

May 24, Kennedyville, MD - 48 year old Glen Nolt, 18 year old Kevin Nolt and 14 year old Cleason Nolt, all of Peach Bottom, PA, were all found dead inside a manure pit on a Kennedyville, MD dairy farm.  Officials say all three died of asphyxiation but that Cleason also died of "multiple injuries".  Investigators say they believe his injuries were caused by a large propeller at the end of the auger that circulates the liquid manure in the pit.  It is not clear how all three came to be in the manure pond/pit.

January 4, 2012 - St. Landry, LA - 23 year old Johnathan Kyle West died on the job while working for Migues Deloach Company.  He was electrocuted while working with an aerial lift and scaffolding. 





Oil Field Worker Did Not Die of Natural Causes

January 7, Bismark, ND  - 21 year old Dustin Bergsing of Edgar, MT did not die of natural causes as was first reported back in January.  Bergsing, an oil field employee for Marathon Oil Corp. died of hydrocarbon poisoning due to inhalation of petroleum vapors, according to a final report.

 

Worker Dies in Rail Car Incident

January 30, - Hobert, IN - 55 year old Michael M. Shoemaker died when he became pinned between two rail cars while working for US Steel's Gary Works.

April 4,  - Corona, CA - 21 year old Connor Penhall died after being struck by a suspected drunk driver while working along a California freeway.  He was employed by Cut & Core Construction, doing work for Caltrans.



Ballpark Worker Dies in Accident

April 10, Hagerstown, MD - 18 year old Mitchell Akers was killed when he fell from the back of a truck while performing maintenance duties at Municipal Stadium.  Akers was riding atop some garbage bags in the utility truck when he fell out, striking his head.  He was an employee for the Suns Minor League baseball team.

 Construction Worker Killed in Crane Incident

April 20, DePere, WI - 58 year old Raymond Ashenbrenner of Black River Falls died when he was pinned underneath a crane arm as workers were unloading equipment from a trailer.  He was working for Lunda Construction.

Metro Bus Driver Murdered

 May 20, Los Angeles, CA - 51 year old Alan Thomas of West Hollywood, a bus driver for Los Angeles County Metropolitan Transportation Authority, was shot while aboard his bus by a lone gunman. The gunman was arrested for Thomas' murder.

 
Railroad Conductor Killed on Job

May 28, Kenmare, ND - 38 year old Robert J. Glasgow of Harvey, ND was killed while working on the CP Rail in Kenmare in a rail switching incident. He was a conductor and a member of the UTU Local 887.

 Steam Explosion Kills Worker

June 1, Pascagoula, MS - 20 year old Jeremy Moore of Moss Point became the second worker to die in two weeks time at MS Phosphates plant in Pascagoula. Moore died of an apparent steam explosion at the fertilizer plant.  The first employee, Jeffrey Simpson, was listed in last week's Weekly Toll.

Pilots Die in Plane Crash Fighting Forest Fire

 
June 4 -  48 year old Todd Tompkins (left) and 40 year old Ronnie Chambliss, both of Boise, ID, were killed when their P-2V air tanker crashed while dumping retardant on a 5,000-acre blaze near the Utah-Nevada border. The cause of the crash is unknown.




Steel Worker Suffers Fatal Crush Injuries

June 8, Claremont, NH - 41 year old Scott Bates diedafter being crushed by a trailer at the Canam Steel plant in Claremont. Bates had been trying to load a large steel span onto a trailer when he was pinned beneath its wheels.


Worker Killed From Pressurized Blast

June 11, Henderson, NV - 44 year old Thomas Turner died when he was struck in the head by a pressurized blast of grout, mud and fist-sized rocks.  He was working underground with Vegas Tunnel Constructors on the third intake project at Lake Mead. It is unclear what caused the blast.  





Tree Trimmer Falls to His Death

June 11, Waikiki, HI - 26 year old Reid Barcelona who was trimming trees at the Hilton Hawaiian Village in Waikiki fell to his death. Barcelona was working for a tree trimming service working for the hotel.


Utility Worker Dies in Fall
 
June 12, Hazard, NE - 46 year old Kurt Keaschall of Pleasanton died after being injured while working on a power pole near Hazard. Keaschall was working in a utility truck bucket when it broke away from its boom. Keaschall fell an estimated 25 feet. Keaschall was employed with Dawson Public Power District.


Worker Suffers Electrocution

June 13, Augusta, GA - 33 year old Javon Maloy, of Bonaire,  died after receiving an electrical shock while working on Richmond County's newest magnet school. Maloy was working in the ceiling area when he was injured.  He was working for a sub contractor for MEJA Construction Company.


Worker Crushed Under Equipment

June 13, Greenville, SC - 28 year old Pedro Alberto Miguel Perez died at a construction site.   Crews were drilling a trench for a water line when a heavy piece of equipment fell on top of Perez and injured another worker who were inside a 16 foot deep trench. The men worked for Payne, McGinn & Cummins, Inc.


Heavy Equipment Crushes Worker

June 14, Madison, WI - 45 year old Terry Hudzinski of Middleton was struck in the head and crushed by an 800-pound hydraulic press being removed from a truckHis employer is unknown. 


Fall From Roof Kills Worker

June 14, Camillus, NY - 46 year old Harvey J. Seymour suffered fatal head injuries when he fell from the roof of a home.  He was placing shingles on the roof when he fell.  He was employed by Landscaping A-Z. 


Shooting Range Owner Struck and Killed

June 15, Colorado Springs, CO - 51 year old Terri Flanell was killed by two smoke bombs flying at 150mph being used for special effects in a new TV show about military vehicles. Flanell owned the Dragon Man shooting range with her husband. The smoke bombs were being used to create a smoky atmosphere for the show when they accidentally detonated and flew toward the film crew.




Shifting Pipes Strike Worker

June 15, Kemper County, MS - 46 year old Emery "Hoyt" Ferguson of Willard, MO was killed while transporting pipes to Mississippi Power's Ignite coal power plant. Ferguson, employed by M.G. Dyess, Inc. died when the pipes in the back of the truck shifted and smashed through the cab of the truck.  


Worker Crushed Under Equipment

June 15, Forsyth, GA - 37 year old Jose Rodriguez of Buford died while working at Lakewood 400 Antiques Market. When authorities arrived at the scene they found Rodriguez laying under a 'pill pusher' also known as a dye press.  


Worker Killed by Shifting Load

June 16, London, KY - 40 year old Doug Price was killed in an industrial incident while unloading equipment. Price, owner of Cold Hill Iron Works, was operating a hydraulic lift, unloading equipment weighing in excess of 3000 pounds when, apparently, the load shifted while it was being moved from a utility trailer and struck him. 


Torch Sets Off Deadly Blast

June 19, Arvin, CA - 24 year old Cesar Martinez was killed and a co-worker injured when an oil tank they were attempting to dismantle exploded.  The two employees were working for Skybrand Services and were using a torch to dismantle pipes on top of one tank, which held petroleum vapor. The torch ignited the vapor, blasting the tank about 30 feet into the air.

June 19, San Leandro, CA - 66 year old  Evangelina "Eva" Macias died after being struck by a bulldozer while working at a Waste Management plant.  Macias was struck by the vehicle while working as a traffic director in the plant's public drop-off zone.
  

Dump Truck Strikes, Kills Worker

June 19, Brentwood, Washington, DC - 84 year old Leroy Cook was not working as a mail runner for Fort Myer Construction because he needed to, but because he like to be around people. Cook died after he was struck by a dump truck and his company's asphalt plant.  The dump truck was not owned by the company no other details to the incident are available.  
 





Man Falls From Elevated Lift

June 19, Bakersfield, CA - 51 year old Luis Roberto Minjarez of Los Angeles died when he fell approximately 50 feet from an elevated basket.  He was working for Cleveland Wrecking from Covina helping to dismantle the abandoned Pacific Gas & Electric Kern Power Plant.  A falling beam hit the basket he was in, causing him to fall.  

 
Trench Collapse Causes Death

June 21, Staten Island, NY - an unidentified 29 year old man died when he was buried in a trench collapse and subsequently suffered a heart attack. The worker was in the process of replacing piping for a septic system inside the trench for Reliable Plumbing.  A search online has not produced the worker's identity.


Brake Failure Causes Fatal Dump Truck Incident

June 21, Manlius, NY - 49 year old Terry Johnson was killed and another worked injured when the dump truck Johnson was driving crashed off the roadway. Reports say the truck was coming down an incline from the T.H. Kinsella Quarry when the truck lost its brakes and went off the road. Johnson and his passenger jumped from the runaway truck but the truck ran over Johnson.  


Ranger Killed During Rescue Attempt

June 21, Seattle, WA - 34 year old Nick Hall was killed in a fall while rescuing two climbers who had fallen into a crevasse. He was preparing them for a helicopter evacuation when he fell 3,700 feet from the mountain's northeast side. He was not moving after his fall, and attempts to contact him were unsuccessful.


Man Killed by Electric Shock

June 22, West Gardiner, ME - 53 year old Ronald Hickey, owner of Hickey Tree Service, died when he came into contact with a live power line. He was in a boom clearing tree growth away from power lines in front of a home when the incident happened.  


Utility Worker Struck and Killed

June 22, Reading, MI - 56 year old Jeffery Henry Creel was struck by a vehicle as he prepared to assess damages caused by a downed tree limb. He was an employee for Consumer's Energy and was behind his work truck when a resident drove into him.  


Restaurant Owner Dies 

June 24, Schenectady, NY -  30 year old Israel "Bobby" Silva was killed when he was struck in the head by a dumbwaiter at the Bangkok Bistro he co-owned.  The exact circumstances are not known but reports say he was making on the food elevator and it was either inadvertently activated by an employee or it was still operational while he was working on it.


Head On Train Collision Kills Three

June 24, Guymon, OK - 50 year old Conductor Brian L. Stone of Dalhart, TX, and Engineers Dan Hall and John Hall, presumably from Oklahoma, were all killed when the train they were on slammed head on into another freight train causing a massive diesel fuel-fed fire.  


Worker Dies of Heat Stroke

June 25, Fayetteville, AR -  39 year old David Lopez of Mount Pleasant, TX died of a heat stroke suffered while working for Symmetry Turf of Mount Pleasant.  He was working at the University of Arkansas campus when he became ill and was rushed to a local hospital where his body temperature reached 109 degrees.


Auto Worker Crushed by Equipment

June 26, Parma, OH - 52 year old Phil Niewiadomski of Painesville, an employee at the General Motors plant in Parma, died when he was somehow crushed under a heavy piece of equipment. It is unclear at this time what caused the incident.  



Worker Dies of Asphyxia

June 26, Richburg, PA - 33 year old Shawn Merritt of Lancaster is reported to have died from asphyxia while working at ATI Allvac plant. According to the coroner, Merritt was in an area of the plant that filled with super-cooled liquid nitrogen where temperatures in that area can fall to 200 degrees below zero. Any further details are unavailable at this time.


Cell Tower Worker Dies in Fall

June 26, Solway, MN - 19 year old Jeremy Jo Froemke of Lisbon, ND died when he fell 153 feet from a cell tower in Solway. It is unclear how he fell, but he is reported to have been equipped with fall protection. Froemke was employed by Midwest Steeplejacks of Fargo, ND.  


Man Killed in Farm Incident

June 26, Grandview, WA - 57 year old James Womachwas killed when he became pinned between a tree and his tractor while spraying a cherry orchard. One of his workers and a passerby attempted to free him but were unable to move the tractor.




Ship Worker Dies in Fall

June 28, Houston, TX - an unidentified 30 year old man died when he fell from the top of the cargo bay while working on the Thorco Atlantic cargo ship.  He was working at the Houston Ship yard but it is unclear who he was employed by.

Worker Who Fell 19 Feet Back on the Job

August 8, 2012, Madison, NJ - 23 year old Thomas Block of Wantage is back to work after he was injured on May 17th when he fell 19 feet while erecting a steel frame for a business building. Block's mother said her son's recovery is quite amazing.  Block was an employee for the family business, Vandermeyde Welding and Iron Works, Inc. who received a citation from OSHA resulting from Block's fall.

Construction Worker Struck by Truck

July 2, Hannibal, MO - 31 year old Larry Dye, a construction worker for Chester Bross Construction which had been hired as a contractor by MoDOT, was killed when a fully loaded asphalt truck backed over him as he was picking up a construction highway sign. He died at the scene.

Bridge Worker Dies After Being Struck

July 3, Hampton, NH - James Hayes of Seabrook, a bridge attendant, was struck by a vehicle at the end of his shift on the Neil Underwood Bridge. He was air lifted to an area hospital when he died.

Brake Failure on Utility Truck Causes Crash

July 3, Milton, FL - 57 year old Jacqueline Green, a Gulf Power employee, was assisting in a clean up effort after a storm when her work bucket truck lost its brakes. While driving she had to swerve around other trucks but when she got to an intersection she stuck a semi and then into another truck. Green died at the scene, her passenger was hospitalized.  

Death at Fish Processing Plant

July 4, Akutan, AK - 55 year old Edgardo P. Sanchez of California died while working for Trident Seafood processing plant.  Reports states he became short of breath and ill as he was starting his shift.  It is unclear the reason for his death. 

Farm Worker Run Over by Tractor

July 4, Wapato, WA 44 year old Francisco Espinoza died when he was run over by a running farm tractor while he was working on an attached trailer. Espinoza died at the scene.

Deadly Timber Incident

July 5, Jackman, ME - 46 year old Bruno Cloutier of Quebec Canada died when he was pinned between a timber harvester and a service truck as the harvester was being moved. Cloutier was a heavy equipment mechanic. 

Bridge Construction Worker Killed

July 5, Oshkosh, WI - 35 year old Joseph R. Bidler of Green Bay died and a co-worker was injured while working on the Butte des Morts bridge in Oshkosh when a crane collapsed. Bidler, who was the driver of the semi truck that delivered the massive concrete girder was crushed under the boom which fell as the crane collapsed. Bidler was an employee for Spancrete.

Another Farm Worker Tragedy

 
July 7, Wapato, WA - 23 year old Francisco Javier Mendez of Toppenish was killed when a forklift rolled and he was pinned underneath it. Mendez was an employee for Consolidated Fruit.  

  

Crane Collapse Kills Two

July 7, Dallas, TX - 58 year old Thomas Fairbrother, Jr. of Austin and 50 year old Terry Weaver of Grand Saline were killed when a crane being dismantled collapsed. It is unclear what caused the collapse.  Both men were employed by Harrison Crane & Hoist, Inc.  

Child Farm Worker Killed

July 9, Deer Grove, IL - 15 year old Curvin Levi Kropf of Tampico, was killed when he was run over by a piece of farm machinery.  Kropf working in a seed-corn field and was riding on a machine to cut individual cornstalks in the field when he fell off and was run over. 

Construction Worker Killed at AZ Airport

July 9, Phoenix, AZ - 58 year old Cesar Rodriguez Montoya was struck and killed while working at the Phoeniz-Mesa Gateway Airport. Montoya was preparing to fuel up a water tanker at a construction site at the airport, and was standing next to his truck when the water tank struck him and pinned him against the truck. He was employed by a construction company doing work at the airport.

Trash Collection Worker Dies

July 10, Panama City Beach, FL - 25 year old Sean Philbrook died when he was run over by a tractor while collecting trash along the beach. Philbrook, and two others, were in a trailer attached to the tractor. When he attempted to step back on the trailer, he lost his footing and slipped under the tires of the tractor.  

Two Teenage Farm Workers Die

July 12, Middleville, MI - 17 year old Francisco Mendez Martinez and 18 year old Victor Perez were found dead inside a silo at Yankee Springs Dairy Farm. The young men were power-washing the silo. The farm foreman, who went looking for the boys, found their ladder askew and the boys inside the silo.  It is not clear how they came to be inside the silo.  
  
Entergy Worker Electrocuted


July 12, Houma, LA - 25 year old Roland Lafont III of Montegut, was electrocuted while working on an electrical box about noon outside a newly built home in Southland Woods subdivision. Lafont worked as a line mechanic for Entergy.


Construction Worker Electrocuted

July 12, Stamford, CT - 40 year old Juan Orozco was electrocuted while operating heavy machinery to lift a steel beam at a construction site. Witnesses told police the beam came into contact with a live wire hanging from a light fixture and saw sparks shoot out when the beam or machinery came into contact with the wire. Orozco worked for Welding Services, Inc.

July 13, Alpine, UT - 41 year old Ricardo Flores of Pleasant Grove, died when an autoclave door either blew off its hinges or opened and hit Flores in the head.  Flores worked for Adonis Bronze's Alpine facility as a welder and metal chaser. 


Farm Incident Causes Women's Death

July 13, Pepperell, MA - 40 year old Leslie Spoth, a Brookside Farm employee, died after being trapped under an overturned hay wagon. There are no details as to how she became trapped under the wagon.  She died at an area hospital.

Pipeline Worker Killed by Dump Truck

July 14, Lairdsville, OH - 26 year old Timothy J. Wehr of Cambridge was killed when a dump truck that was backing up ran over him. Wehr, a pipeline worker for Integrity Pipeline Services, was helping to install water lines at a gaswell pad site when he was struck.

Teen Killed at Natural Gas Well

July 16, Jackson, OH - 19 year old Paul C. Sherman died in an explosion at an oil and natural gas well site. Sherman was doing touch up painting at the well site when the explosion occurred. Sherman's father, owner of MKE Producing, said his son, a full-time college student, was a subcontractor for the Canton-based company. 


Death of Man Working in Tree

July 16, Willow Springs, NC - 44 year old Johnny B. Blackman was found unconscious 100 feet up in a tree.  Blackman was either self employed or working for an unknown company and was hired to cut the tree down.  It is unclear how or why Blackman became unconscious and he died at the scene.

Man Dies From Industrial Incident

July 16, Davenport, IA - 45 year old Robert Leech of Wheatland, a general mechanic at Alcoa Davenport Works, was killed in an unnamed industrial incident. An investigation is ongoing.

Injured Plant Worker Dies of Burns

July 17, Tomahawk, WI - 47 year old Dennis Jay Gougeon of Onotonagon, MN died, and another worker injured, after being burned by fly ash while cleaning an ash system. Fly ash is aproduct typically produced during the burning of coal.  He was an employee for Packaging Corporation of America.

July 18, Sulphur, LA - 30 year old Gerald Wayne Fondel, Jr. of Lake Charles was killed when a large piece of sheet metal fell on him.  He was employed with Superior Supply and Steel on Cities Services Highway.



 Incident at Plant Kills Worker

July 18, Cedar Rapids, IA - 66 year old Kyriakos Korovilas died at the General Mills plant in Cedar Rapids. It was reported that a man was trapped between a piece of machinery and a safety post. He died at an area hospital. 

Utility Worker Struck by Falling Tree

July 19, Tupelo, MS - Brian Hill, an employee for Tombigbee Electric Power Association in Fulton, died after being hit by a falling tree. Hill was working with a crew to take down a rotting and splitting tree when a piece of it fell onto Hill.   

Man Crushed Under Crane

July 20, Excelsior Springs, KS - 55 year old Robert P. Walker was killed and a co-worker injured when a 3-ton suspended crane fell from an overhead beam, striking him and the co-worker, police said. Both men worked for Rexam, a plastics manufacturing facility. The co-worker suffered non life threatening injuries.

 Falling Beam Kills Worker

July 20, Santa Fe, NM - 29 year old Andrew Vasquez died after a large beam fell on his head while working a summer job dismantling a gas station. A crew was putting large I-beams on a flatbed trailer but one of the beams was not positioned correctly. While a forklift had the 60-foot beam in the air, it fell and landed on the worker's head.

Storage Building Collapses, Killing Man

July 21, Canton, NC - 29 year old Miguel Ortiz, who worked for Carolina Wholesale Bait and Ginseng Buyer, was working inside a storage building when, for an unknown reason, the building collapsed.  All of the workers except for Ortiz were able to get out of the resulting rubble of the building.  

Drunk Drivers Cause Workers Deaths

July 22, Torrance, CA - 56 year old Ramon Lopez of Chino Hills and 58 year old Ricardo Zamora of Winchester were killed when two drunken drivers collided causing the vehicles to strike Lopez and Zamora. The workers were employed by contractors to the California Department of Transportation.

Worker Killed While Hanging Cable


July 23, Huntsville, TN - 29 year old Bruce Lee Marion of Oneida was killed while he was hanging cable from a bucket truck. Authorities say a passing truck clipped the cable, causing it to strike and kill Marion, a lineman for J&R Cable.

 Man Crushed Under Overturned Forklift

July 23, Wallingford, CT - 60 year old Walter Sawallich, Jr. was killed in an incident involving a forklift. Sawallich operated A&W Hauling in town, was killed when the forklift he was operating at Tayco Corp. overturned, pinning him underneath it.  

Worker Killed by Machinery

July 24, Indianapolis, IN - 53 year old Ronald Davenport was killed when he became trapped in a piece of equipment while working at a job site for Greene Thumb Landscaping. Davenport had been standing on the side of the skid loader when the operator lowered the arms without realizing Davenport was there. When the arms were lowered, they pinned him between the arms and the frame of the machine. 

Worker Struck and Killed

July 24, Troy, WI - 66 year old Gerald Himebauch of Lyons was killed when he was struck by a dump truck who backed up over him. Himebauch was part of a crew doing sealing work on a highway in Troy. Himebauch died at the scene. 

Lumber Mill Worker Killed

July 24, Sonora, CA - 25 year old Ron Billings, a Sierra Pacific Industries worker was killed in an accident at the Sonora Lumber Mill in Standard. Family members of other employees say Billings died in an incident involving a piece of equipment. 

Man Killed in Work Related Incident

July 25, Auburn, IN - 56 year old Paul Graber was killed when he fell from the bucket of a cherry picker while working at Holiday Lakes. He and other Auburn Tree Service employees were attempting to raise a tree from a lake when possibly the weight of the tree caused something to break and the bucket to fall.

Factory Worker Dies in Horrific Incident

July 25, Addison, MI - 27 year old Hector Fermin Campos was killed at Next Specialty Resins Inc. Addison factory after he fell into an industrial grinding machine.  He was in the process of operating the grinder at the time of the incident. 

Man Electrocuted at Fabrication Shop

July 25, Vidor, TX - 40 year old Jason E. Reed of Orange was electrocuted while working in the fabrication shop at Southeast Texas Industries, Inc. It is not reported how or why he was electrocuted.

Man Suffers Fatal Injuries

July 26, Greensboro, NC - Michael Stimpson was killed when he got caught between a tractor and trailer during hookup at Evonik Stockhausen chemical plant.  A preliminary investigation has revealed that Stimpson walked into the path of the cab while another worker was backing up to connect the cab to the trailer.  Stimpson died at the scene.

Miner Killed

July 27, Charleston, WV - 35 year old Johnny Mack Bryant II of Lenore was crushed to death when he became pinned between a mine wall and the boom of a continuous mining machine. Bryant was part of a crew that was setting up the continuous mining machine for the upcoming day shift when the incident occurred.  Bryant worked at Coal River Mining LLC's Fork Creek No. 10 Mine.

Idaho Man Dies Working in California

 
July 27, Blocksburg, CA - 47 year old Curtis Wayne Zahourek of Mountain Home, ID died while working on an electrical structure as an employee of 3 Phase Construction in Blocksburg.  It is unclear how or why he died. 
 





Worker Dies in Fall From Ladder

July 27, Allen Park, MI - 55 year old Steny Wing Hoi Yu of Dearborn Heights died four days after he was found lying on the ground near a ladder inside the Postal Services’ Detroit Network Distribution Center in Allen Park, where bulk packages are sorted.


Oilfield Worker Dies in Drilling Incident

 
July 27, Reagan County, TX - 21 year old Chase Dillon of San Angelo died after falling from a rig while working at Robinson's Drilling Rig 12. He was transported to a local hospital where he died. No further details are available. 






Second Plant Worker Dies of Burns

July 28, Tomahawk, WI - 29 year old Dustin Dean Hale, who suffered burn injuries July 17th at Packaging Corporation of America, has died. His burns were caused by fly ash, which is a by-produce of burning coal. Dennis Gougeon (listed above) also died in the incident.

Tree Trimmer Electrocuted

July 30, Johnston, RI - 23 year old Leonardo Estrada of Providence, a lawn worker, died as a result of the injuries he sustained after he was electrocuted while trying to trim a tree in Johnston. Estrada was standing on an aluminum ladder and trying to trim a tree that was covering wires along the streets when his metal trimmer touched a wire. He was knocked off his ladder.

Man Dies in Fall From Grain Bin

July 30, Montgomery, IN - 56 year old Harry Stoll died of massive head trauma when he fell from a grain silo. Stoll had just finished loading a feed truck at Hillcrest Supply Inc. in Montgomery and stepped on a 9-inch-wide beam above the truck. The truck pulled away, and Stoll lost his balance and fell.

Another Miner Killed at Mine

July 31, Berkley, WV - 43 year old Gregory A. Byers died from injuries he received in an incident at Arch subsidiary ICG Beckley's Beckley Pocahontas Mine near EcclesByers was near a scoop at an underground charging station, when a second scoop hit the first, causing it to slide into Byers.  Byers was the 13th miner nationwide and the fourth in West Virginia to die on the job in 2012.

City Worker Struck by Truck

May 31, 2012 - Houston, TX - 75 year old Charles Williams Sr., was killed when he was struck by and run over by his truck while working for the Houston Public Works Department.  Williams and a co-worker were clearing storm drains when their work truck started rolling. He was killed as he attempted to stop the truck.


Waste Collection Worker Dies of Heat Stress

June 1, Trenton, NJ - 47 year old Mark Jefferson, a Labor Ready temp working for Waste Management picking up trash on a collection route died after becoming ill due to the heat.   Jefferson was transported to a local hospital where he died.



 Driver Killed by Runaway Truck

July 6, Des Moines, IA - 46 year old Carl Magee of Pekin, IL died when he reportedly tried to jump aboard his rig after it slipped out of gear and began to roll away from a loading dock. He lost his balance and was run over. He worked for the U.S. Postal Service and MCA Trucking of Little Rock, AR.

Railroad Worker Killed

August 1, Mason City, IA - 36 year old Georgiy Soloviyov of Stanhope, IA was killed when he was pinned between two rail cars. He was working on a car coupling when another car rolled into him, pinned him. He was employed by Union Pacific Railroad.

Worker Crushed Under Material

August 1, Fort Atkinson, WI - 51 year old Jack Graves, Jr., of Janesville died after being crushed under steel panels at Spacesaver Corp. He was reportedly using a hand truck to move the steel panels when they fell on him.

State Worker Drowns

August 1, Treynor, IA - 78 year old Bruce Jensen died when he drowned under an overturned mower.  Jensen, a Department of Natural Resources employee, was using a commercial mower to mow the grass along a steep embankment, when the machine rolled and landed on top of him. He drowned at the scene.

Man Suffers Fatal Fall From Ladder

August 5, Fort Worth, TX - 62 year old Fermin Chantaca died when he fell from a ladder while working at the construction site for the Kimball Art Museum.  He died of a closed head injury.


City Worker Dies in Tire Incident

August 2, Amarillo, TX - 57 year old Gary Duwayne Witt died of massive head and chest trauma while airing up a tire on a road grader.   Witt, an employee for the City of Amarillo, was locking the tire into the rim when a steel locking ring shot off and struck him.

Construction Worker Dies From Fall

August 2, West Palm Beach, FL - 38 year old Juan Arias fell to his death inside a vacant Sears store he was working with others to demolish.  Officials say Arias was near the ceiling removing tiles, and somehow slipped off a scissor lift and fell 15 to 20 feet to the floor below striking his head on the floor.

Worker Dies of Heat Related Illness

August 2, Logan, WV - 25 year old Albert Caleb Kiser died from a heat-related illness while cleaning up an abandoned coal-mine site. Kiser collapsed while cutting down a tree while working for Eastern Arrow Corp., which was reclaiming the Sugar Branch Burning Refuse Pile for the state Department of Environmental Protection's abandoned mine lands program.

Supervisor Shot During Meeting

August 2, Milwaukee, WI - 58 year old George Hites, a Trans-Coil International employee, was shot multiple times by a 23 year old disgruntled employee who had developed a grudge against Hites. Authorities did not disclose what motivation the employee had for what he did, but after shooting Hites, he killed himself.   

Two DOT Worker Struck and Killed

August 3, Murphy, SC - 60 year old Tony Leon Wright of Murphy and 56 year old Dannie Burrell of Marble were killed when a vehicle veered off the road and struck the workers. It is not clear why the 65 year old driver of the vehicle traveled off the roadway and into Wright and Burrell.  

Young Worker Electrocuted


August 3, Forked River, NJ - 20 year old Rich Grande, an employee for Jersey Central Power & Light was electrocuted from a high tension power line while working near the Rolling Thunder Horse Farm.  



Worker Dies in Fall From Man Lift

August 4, Alton, IL - 47 year old Jay VanBuskirk of Edwardsville fell to his death when he apparently lost his footing after riding a man lift and fell approximately 70 feet.  He was an employee for ConAgra in Alton.

Employee Found Unconscious at Steel Plant

August 4, Gary, IN - 61 year old Donald Graden of Portage was found unconscious at US Steel's Gary Works. He was transported to an area hospital where he died. There were no signs of trauma to Graden, but they do not know his cause of death at this time.

Park Worker Suffers Fatal Injuries

August 4, Mahwah, NJ - 23 year old Michael Storm Fischer of Ringwood died while working at Campgaw Mountain Reservation in Mahwah. Fischer was helping to close a set of large metal gates when one of them struck him in the torso. At the hospital it was found that he had suffered internal injuries far more severe than first thought and died during surgery.

Work Zone Crash Kills Worker


August 5, Topeka, KS - 22 year old Curtis James Harlan of Arma was killed while working on Interstate 470 on the west side of Topeka. Harlan, was an employee of R.A. Knapp Construction Inc. when he was struck by a vehicle in the work zone.



Paper Mill Worker Dies in Incident

August 6, Chillicothe, OH - 50 year old Rebecca Mitchell died while working at Glatfelter's Printing paper mill.  Authorities have not released the cause of her death.

Bouncer Killed in Other's Fight

August 6, West Orange, NJ - 32 year old Jacques Nelson was a bouncer at Misty's Restaurant when he was shot to death trying to break up a brawl.  No details are given but the report says he was trying to break up a fight in the parking lot when he was shot. Police are still looking for the shooter. 


 Wall Collapse Kills Two

August 6, Jacksonville, TX - 60 year old Donald Starling and 68 year old Hollis Morris died when a 40 year old retaining wall they were trying to reinforce collapsed onto them. Morris' son escaped without injury.  Workers said the wall seemed sturdy, so they rested near it as they waited for the steel reinforcements to arrive, but that is when the wall came crashing down.

Man Dies in Drilling Rig Incident

August 6, Dickinson, ND - 52 year old Terry Metcalf died after a fall at an oil rig. Metcalf was a derrick hand with Big Sky Well Service, Belfield, and was working in an elevated rig basket. It is unclear how or why, but Metcalf fell from the rig basket.


Worker Dies of Crushing Injuries

August 7, Belleville, MO - 43 year old James Gentilellor, Jr. died when he became pinned between an unmanned, automated vehicle that is used to haul and lift loaded pallets and a metal racking unit at Kraft Foods in Belleville. Officials say he oversaw the operation of unmanned vehicles in the warehouse.

Man Killed in Forklift Incident

August 7, Atlanta, GA - An unidentified 35 year old man was killed when the forklift he was operating overturned.  As it was tilting the man, who was not wearing a seat belt, fell off and the machine landed on top of him. The man was a temporary worker at Owens Corning.   

Worker Electrocuted After Touching Energized Truck

August 8, York, ME - 42 year old Christopher Dixon was electrocuted when he touched a dump truck that had come into contact with live electrical wires. Dixon was standing at the back of an Atlantic Paving truck when the dump truck's bed lifted, it hit Central Maine Power electrical wires, energizing the truck. The paving truck driver noticed Dixon on the ground and called for help.

Man Struck by Forklift

August 9, Las Vegas, NV - 54 year old Gustavo Sanchez died of blunt force trauma after he was struck by a forklift while working Rocky Mountain Recycling. No other details are available.  

Worker Electrocuted

August 9, Dothan, AL - 44 year old Roy Lawson Ford was fatally electrocuted while working on an air conditioning unit at a private home. After being found unconscious under the home, he was taken to an area hospital where he was pronounced dead. 

Teen Worker Dies in Fishing Incident

August 11, Bayou La Batre, AL - 15 year old Steven Branch died while working on the shrimp boat, Nettie-Q, when he was pulled into a winch.  Branch was wearing baggy shorts and they became tangled in the winch pulling him in.  It is unclear if he was operating the winch or standing near it. 

Oil Field Worker Dies of Injuries

August 12, Enfield, IL - 36 year old Shane Ferris has died of burn injuries he suffered on July 24, 2012. Farris and three other men were burned when an oil field tank that was being welded exploded. Authorities say the blast was caused by either someone grinding on the tank in preparation for welding, or the welding itself. The other workers are recovering.

Firefighter Dies Battling Forest Fire

August 12, Orofino, ID - 20 year old Anne Veseth of Moscow was killed when she was struck by a falling tree while fighting a forest fire near Orofino. She was a second year US Forest Service wild land fire fighter.   




Port Worker Dies in Trench Incident

August 14, Orange, TX - 65 year old James Keith Hudnall, an employee at the Total Port Arthur refinery was killed in a construction accident. Details are not clear but he became trapped in a trench at the construction site and died later at an area hospital.

Awning Collapse Kills Worker

August 14, Russellville, AR - 64 year old Bural Darter was killed when an awning collapsed onto him while working at Baker Refrigeration. Workers were reportedly attempting to remove a section of awning from the building when it collapsed suddenly and struck Darter.  It is not clear if he was an employee for Baker Refrigeration or a sub-contractor.

Road Worker Struck by Semi

August 15, Lowell, IN - 34 year old Chad Fleager died after being hit by a semi truck while working along the Indiana Toll Road. was outside his vehicle measuring part of the road when he was hit by the semitrailer. The driver of the semi took his eyes off the road looking for a truck parking lot when he veered off the road and in Fleager.  Fleager worked for Roadsafe Traffic Systems, of Crown Point.

Another Road Worker Struck by Vehicle

August 15, Pensacola, FL - 56 year old Frank Donson was struck and killed while working in the center lane of Eglin Parkway. It was completely coned off on both sides, but a truck coming from behind him drifted into the lane where he was working. Donson was working for APAC Mid-south.

Business Owner Electrocuted

August 15, Mussey, MI - 50 year old Henry DeBlouw, owner of Mike Pirrone Produce, died after being electrocuted at his business. DeBlouw  was adjusting the temperature controls for a refrigerated building when he touched a high-voltage element within the temperature control panel and was electrocuted.

Explosion at Oil and Gas Facility Kills One

August 15, Fort Lupton, CO - 60 year old Brian Wallis of Wyoming died and three others injured from an explosion at an Encana Corp. oil and natural gas facility north of Fort Lupton.  A Sheriff's spokesman said the explosion appeared to be related to excessive pressure at the well head where the men were working.

Ladder Collapses Causing Worker's Death

 
August 15, Frankfort, PA - 57 year old William "Bruce" Sweeney suffered fatal head injuries after falling from a ladder while working for the city's Office of Fleet Management.   Authorities say Sweeney was standing on a ladder, working, when for some reason it collapsed from under him sending him falling to the ground. He underwent two unsuccessful brain surgeries before he died.

Worker Dies of Toxic Chemical Exposure

June 25, Nitro, TN - the death of a still unidentified worker is being investigated by OSHA as to how a dangerous gas was apparently pumped into a man's respirator at the AC&S Chemical plant.  The man was rushed to the hospital after inhaling dangerous gases and then died from his injuries. The man was wearing a respirator and an incorrect gas was pumped into the apparatus' clean air supply, according to an initial report.

Mail Carrier Dies of Heat Illness

July 24, Kansas City, MO - 57 year old John Watzlawick died a day after complaining he wasn't feeling well. He wasn't allowed to end his shift early. The medical examiner this week confirmed that Watzlawick died of hyperthermia, or heat-related illness. After being found lying unconscious by a passerby, he was transported to an area hospital.

Two Deputies Murdered in Ambush

August 16, LaPlace, LA - 34 year old Brandon Nielson of Destrehan (left) and 27 year old Jeremy Triche of LaPlace, both deputies for St. John the Baptist Parish, were gunned down in an ambush after they responded to reports of a traffic officer being shot multiple times near an isolated parking lot for a Valero refinery. The officers were conducting an interview when someone came out of a trailer with an assault weapon and opened fire. Many arrests have been made in the incident.

Off Duty, But Still Working

August 16, New Harmony, UT - 27 year old Kevin Scott Smith of Cedar City, and a Cedar City firefighter, was killed in an off-duty industrial accident while working for his employer in New Harmony.  Smith and two other men were apparently working on a water pump when their boom truck came in contact with overhead power lines causing an electric shock that killed Smith and injured another.
 
Man Crushed Under Equipment

August 16, Jacksonville, FL - 21 year old Lawrence DaQuan Davis died when he became pinned underneath a piece of heavy equipment at the Bacardi Bottling Corp. plant. It is unclear how or what cause Davis to become trapped by the equipment.

Utility Worker Struck and Killed

August 17, Chester, PA - An unidentified 34 year old subcontractor for the Chester Water Authority was struck by a vehicle as he was exciting a manhole. The man worked for Metra Industries from Little Falls, New Jersey was replacing the waterline in a hole.

Incident at Quarry Kills Worker

August 17, Sloan, NV - 29 year old Anthony Holden of Las Vegas was suffered fatal crushing injuries at a quarry in Sloan. Holden was an employee for Southern Nevada Paving.  No other details are available.  

Inspector Killed in Roof Collapse

August 17, Medley, FL -  58year old Pierre Mezidor  died after the roof of a silo filled with powdery concrete mix collapsed, while he was standing on top of it. Mezidor, an inspector at Titan America in Medley, was about 200 feet up when the silo's roof collapsed underneath him. 



Worker Succumbs to Burn Injuries

August 19, Mount Prospect, IL - 29 year old Luis Macias, one of three workers injured August 9th in a chemical flash fire, has died. The men were working in the basement of an apartment complex when the explosion happened but there are no details as to what caused the explosion or who the men worked for.

Worker Killed After Being Knocked Out of Truck

August 20, Canarsie, Brooklyn, NY - 52 year old Thackoo Hargobin, an employee for Astoria-based Hephaistos Building Supply, died when he was hit by a school bus while loading his flat-bed truck at Super Tex Products in Queens. Hargobin died at the scene.

Falling Pipes Crush Worker

August 20, Theodore, AL - 55 year old Paul Edward Hodgon was killed when a two large pipes he was unloading fell on him.  Hodgon died at the scene after emergency responders arrived at Hertz Energy & Pump Services.

Plant Worker Falls to His Death

August 20, Heyworth, IL - 48 year old Michael Burleson has died after an injury suffered August 15 at the Cargill, Inc. soybean processing plant.  An official says Burleson was in the elevator area where the soybeans are stored and apparently fell onto a lower level, but it is unclear how it happened.

Carpenter Killed on Job Site

August 20, Washington, CT - 42 year old Christopher Dennis of Waterbury died after he fell from a scoffold at a job site. He was airlifted to a hospital where he died of his injuries. It is unknown who he was working for.

Road Worker Killed When Truck Flips

August 21, Milford, MI - An unidentified 58 year old Oakland County Road Commission employee was killed when the truck he was driving overturned. Police say the man was on a job, putting down material on the road when the accident occurred, but it’s not clear why or how the truck flipped over.

"Freak Accident" Kills Logger

August 21, Rowan County, KY - 30 year old Josh Ferguson of Elliotville was killed in what authorities are calling a freak accident.  "As he was cutting the timber, the tree that he cut fell against another tree, caught the top of that tree and basically lifted the bottom of that tree up. It pushed him over and caught him between the dozer blade and the tree, itself," described Sgt. Jim Bowling of the Kentucky State Police.

Another Construction Site Incident

August 21, Paso Robles, CA - 65 year old James Seim, a construction supervisor, died after apparently falling through the roof skylight of a commercial building his company was working on. It is unclear who the contractor was for the construction.   

Man Dies From Electrocution

August 21, Bedford, PA - 41 year old James Dick was electrocuted while relocating power lines. Dick was working from a bucket truck for J.R. Wyles relocating power lines for Adams Electric. Authorities say Dick was shocked by a 7,000 volt live wire.

Construction Worker Crushed by Equipment

August 23, Oklahoma City, OK - 43 year old Mario Mejia-Ramirez died after being pinned between a truck trailer and a piece of excavation equipment. He was working for Sherwood Construction Co. helping to dismantle theold Interstate 40 Crosstown.

Road Worker Struck by Out of Control Car

August 23, El Paso, TX - Jose Anguiano was struck and killed while doing road construction on Interstate 10. Authorities say a van hit a smaller car, causing it to go out of control, hitting Anguiano. He was pinned against a heavy piece of equipment.  

Worker Crushed Under Flipped Mower

August 24, Vine Grove, KY - 47 year old Thomas "Greg" Spence was crushed under a commercial lawn mower. He was mowing on a steep hill when the mower lost traction and rolled over onto Spence. Spence was employed by Whispering Hills Lawn Service.  

Worker Struck by Cement Truck

August 24, Berwick, ME - 55 year old Thomas Palermo died after being struck by a cement truck in the driveway of the Redimix Companies plant. There are no details on how or why Palermo was struck.  

Maintenance Employee Electrocuted 

August 25, San Antonio, TX - 49 year old Ricardo Corpus, a maintenance worker for Sea World San Antonio, died while working on an air conditioning unit at the park. Corpus and a co-worker turned off the machine to work on it but something went wrong and his co-worker found him with his arm inside the machine and him energized. The co-worker pulled Corpus out and called for help. 

Rail Worker Electrocuted

August 27, Queens, NY - 51 year old Ronald Sampson, a Metropolitan Transportation Authority worker, died after he was electrocuted near the Queens Village station of the Long Island Rail Road.  Sampson, a third railman, was electrocuted when he came in contact with the third rail while performing track maintenance work.

March 3, 2012, Flowery Branch, GA - 30 year old Adam Wingo died when he became caught in moving wire and was pulled into rotating rolls while working for Koswire. Koswire manufactures wires and cables. The linked article lists the violations the company received from OSHA. 

Man Struck and Killed by Truck

August 21, Upper Macungie, MA - 34 year old Fred Hossler of Northamption was killed after being struck by a tractor-trailer in a parking lot. He died of blunt force trauma to the pelvis and abdomen. Hossler was employed by Ehrlich Pest Control but no details are given as to how or why he was hit. 

Worker Strikes Head and Dies

August 23, Barre Town, VT - 61 year old Jerry Fisher of St. Johnsburg died of head injuries while on a trash collection route. It appears he fell off the truck and hit his head. He was employed with Myers Container Service Corp. 







EMT Dies in Crash

August 27, Staten Island, NY - 58 year old David Restuccio died when a car crashed into the ambulance he was driving.  He was a paramedic for Staten Island University Hospital. 






Logger Dies as Tree Falls 

August 27, Moscow, KY - 43 year old Rusty Jones of Springfield was killed when a tree fell onto him. He was logging with his father and brother when the tree fell, striking him.  


Construction Worker Dies on Site

August 28, Williston, ND - 23 year old Brandon Woodard of Mississippi died while working at a Weatherford Lodge construction site. No details are given as to how he died but says he was crushed under roof rafters. 

 
Worker Crushed Under Machinery

August 29, Loyalhanna, PA - 55 year old Robert Buskey Jr. was killed while working as a mechanic for Export Fuel Company. Buskey was a mechanic and was killed when he was repairing a paving roller that pinned him while he was working on the device.


Worker Crushed Under Heavy Hoist

August 29, McDuffie County, GA - 61 year old Edward Chamber Jr. was killed when a hoist fell on top of him. He was working in a workshop of Two-State Construction when the bridge crain hoist that was mounted on an I-beam near the ceiling of the workshop fell, crushing Chamber.  
 

Man Dies in Incident at Boat Dock

August 29, Bethel Island, CA - 59 year old Joshua Silva was crushed under a concrete staircase at a boat dock. Silva was a hired contractor who was working to fix the dock area when he became trapped near a levee.  


Bridge Worker Falls to Death

August 29, Springfield, MA - 46 year old Robert Parrott of Fitchburg died when he fell 60 feet through a gap on an interstate bridge while working. Parrott worked for Liddell Brothers, who provide traffic services and highway construction. 


Traffic Stop Proves Fatal

 
August 31, Winona, MN - 35 year old Corporal Theodore Foss was initiating a traffic stop on Interstate 90 when he was struck and killed by a semi truck collided with his vehicle and the van he had stopped, killing him instantly. 







Disgruntled Employee Murders Co-Workers

August 31, Old Bridge, NY - 24 year old Bryan Breen and 18 year old Christina LoBrutto were shot and killed by a disgruntled worker at the Pathmark Supermarket in Old Bridge. The gunman/employee left the market in the early morning hours and returned dressed in camo and opened fire on Breen and LoBrutto before killing himself. 


Worker Suffers Fatal Burns

August 31, Lake Elsinore, CA - 20 year old William Calabrese of Pacific Beach died from thermal burns after an incident involving fireworks at a baseball game at Lake Elsinore. He was employed part time for Fireworks America. 




Incident at Mine Causes Miner's Death

August 31, Elko, NV - 49 year old Allen Campbell of Spring Creek died while working at Newmont Mining Corp's Exodus Mine near Carlin. Reports state that he slipped and fell in the mine but no further details are given.

Air Show Pilot Dies in Crash

September 1, 2012 - Davenport, IA - 59 year old Glenn Smith of Texas was flying in formation with other members of the HopperFlight team at the Quad-City Air Show in Davenport when his plane nose dived and crashed into a field.  After taking an early retirement Smith spent his time restoring Soviet fighter jets and flying to exhibitions across the country.


 Worker May Have Been Electrocuted

September 1, Yukon, OK - 22 year old Ronnie "Chance" Hobson of Lexington, died while working for Oklahoma Gas and Electric.  Details are not clear but one report states he may have been electrocuted.

Carnival Workers Suffers Fatal Head Injury

September 2, Brentwood, NY - 22 year old Michael Austin of Mastic, died after being struck in the head by a spinning carnival ride on New York's Long Island.  Reports state Austin ducked under a ride to fix the music and as he stood up under the ride he was knocked to the ground. Austin was employed with Zorlenzan Amusements.

 Forklift Driver Dies of Head Injuries

September 3, Hanover, PA - 33 year old Charles Kratz of Plymouth died while operating a forklift at Core-Mark warehouse in Hanover Township. Kratz was found on the floor with a head injury.

Employee Caught in Machinery Dies

September 3, Hartsville, SC - 35 year old Jeffrey Scott Brown died as the result of an industrial incident at Sonoco's Paper Mill complex.  He was reportedly caught in a machine used to make paper. Three of Brown's family members also work at the plant. It is unclear how the incident happened. 

Worker Dies in Oil Rig Explosion

September 4, Williston, ND - 29 year old Jeffery Kinkaid of Nephi, UT, died of injuries suffered in an oil rig explosion in North Dakota. Kinkaid was transferring drilling fluid out of a 400-barrel tank when the blast happened. Kinkaid worked for Denver-based Frontier Drilling at the rig site owned by Kansas-based Slawson Exploration Co. Inc. 

Man Dies of Fall From Roof

September 4, Pittsburg, PA - 38 year old Melvin Kissinger of Greenville died when he fell from the roof of a building he was working on in Scott Township. He apparently tried to jump from one part of the roof to another when he fell about 30 feet. He was employed by a contractor out of Sandy Creek.

Station Attendant Shot 

September 4, Bronx, NY - 28 year old Lamin Sellah, a gas station attendant for only a week, was shot to death during a robbery attempt. Several customers and co-workers were at the station when two men attempted the robbery although no money was taken.

Father and Son Die From Toxic Exposure

September 4, Gettysburg, SD -  20 year old son Erik Hansen died when he and his father (see below) were overcome by methane gas at the West Whitlock Resort area. According to the sheriff, father and son were working on a lift station repair. The elder Hansen attempted to rescue his son before being overcome. Both men worked for the family business, Hansen Septic Service.


September 5, Gettysburg, SD - 50 year old Kirk Hansen died as he attempted to rescue his son Erik (above) while working on a lift station. Both men were overcome by a toxic exposure to methane gases.

  

Highway Construction Worker Killed

September 5, South Bethlehem, PA - 29 year old Chris Burch of West Berlin was killed while working on a highway expansion project through South Bethlehem. Reports say he was crushed by a 2,000 concrete barrier but no details as to how the incident happened.  He was an employee for HRI, Inc.

Power Company Worker Killed

September 5, Union, SC - 47 year old Thomas Brian Gibson was electrocuted while he was working on a power line. He worked for the local power company.

Two Men Electrocuted

September 7 -  37 year old Juan Bocanegra of Fort Myers, FL and 30 year old Dustin Manning of Bradenton, FL were in an elevated "cherry picker" basket when it struck power lines overhead.  The men were performing maintenance on a Fort Myers Beach Condo. Bocanegra was working for J Baller Construction; Manning was working for Cutting Edge Moldings.

Worker Struck by Equipment

September 9, Bismark, SD -  36 year old Lucas Voker of Sartell, MN was killed while working on a jobsite in Bismark.  Voker was working on an intake water project installing pipe when he was struck by the bucket of a backhoe. Voker worked for S and J Louis Construction out of Minnesota. 



Roof Collapse Kills One

September 10, Brooklyn, NY - 67 year old Winston Gillett of Bushwick was killed when the roof he was working on at the Carlton Mews Townhouses collapsed. It is not clear who Gillett worked for.




Toxic Exposure Kills Winery Worker

September 11, Sanger, CA - Robert Munoz of Sanger died at a central California winery when he, along with five other workers, were exposed to ammonia. Munoz and the other workers were employed by Gibson Wine Company. It is unclear how the ammonia was released.  Reports say the other workers tried to rescue Munoz but could not because of the fumes. 

Industrial Incident Kills One

September 12, Bossier City, LA - 55 year old Edward D. Bardwell of Keithville was killed when his shirt became caught in a lathe he was operating at River Cities Machine.

Vehicles Collide, Strike Worker

September 12, Avon, OH - 63 year old Peter Maglis of Campbell was struck and killed as he worked along I-90 near Avon. He was putting up tarps for structural painting when two vehicles collided sending one into a spin and into Maglis. Maglis was employed by Athos Painting.  

Husband & Father Killed During Robbery

September 13, Newark, NJ - 19 year old Jamil Bader, Jr. working behind the counter of his family's bodega, Family Deli, when two men, wearing masks, hoodies and armed with guns, charged into the store, threatening Bader and demanding money, police said. As Bader grabbed one of the suspects he was shot to death.

Worker Struck by Large Machinery

September 14, Belton, SC - 47 year old Gamaliel Enriquez-Salzador was crushed to death when he was unintentionally run over by the operator of a trackhoe while working at Belton Metal Company.  Belton Metal is a scrap metal plant.  

Wind Causes Fall From Roof

September 14, Cedar Bluffs, NE - 61 year old Charles Bridge of Colon died, and another worker injured, when they were essentially blown off the roof of a building they were working on. The men were handling a piece of sheet metal on a building's roof when a gust of wind caught the metal and caused them to fall off the roof. 

Worker Dies in Fall

September 17, Ottawa, OH -  49 year old Timothy Bockrath died after he feel from scaffolding while finishing drywall at an outbuilding in Ottawa. It is not clear how or why but he fell onto the concrete floor below him.  He was employed by Lauth Drywall of Ottawa.


Driver Ignores Warnings and Hits Flagger

September 18, Morton, IN - 27 year old Steven Overbay of Roachdale was killed when a motorist ignored all safety warnings and swerved into him.  Officials said there were five INDOT signs warning motorists of the work being done and that there was a flagger ahead. The driver didn’t stop where the flagger was and appeared to veer to avoid a rear-end crash causing him to struck the INDOT flagger Overbay.

Worker Dies of Crushing Injuries

September 18, Payallup, WA - 23 year old Ryan W. Veysey of Vancouver died of internal injuries while working for Roof Toppers, Inc.  Veysey and another worker were in a scissor lift doing work as subcontractors when he became pinned between the lift railing and building frame for several minutes.  


Technician Murdered in Robbery

September 19, Gladstone, MO - 58 year old Kevin Mashburn was murdered when he was struck in the head by a man wielding a crowbar who attempted to take his wallet.  Mashburn, an AT&T technician working on repairs to a tower when he was approached by an ex-con.  After being struck, Mashburn was able to get to his truck and call for help but died after being taken to a hospital.  The ex-con has been arrested for his murder.  

Two Road Workers Struck and Killed

September 19, Albuquerque, NM - Jonah Marioni and Hemelindo Autunez were struck and killed by the driver of an RV as they worked along I-40 near Gallup. Another worker was injured and released from the hospital.  The Valley Fence Co. employees were preparing to install fencing along the interstate when the driver swerved into the median and struck them.   

Employee Dies at Plant Loading Dock

September 19, Columbia, SC - 47 year old Susan Henley was found dead by the loading dock of the Otis Spunkmeyer bakery plant. A report states she went to the loading dock to unload a truck but was found beneath the loading dock plate. She died at the scene.  

Tree Service Employee Dies

September 20, Chaumont, NY - 39 year old Ronald Chance Watson of Gouverneur died while working to remove a tree stump in the Village of Chaumont. Watson was alone when grinding a stump and apparently became pinned between the grinder's protective shield and an adjacent building. He was an employee for Ryan's Tree Service. He died at the scene.

Equipment Incident Kills Worker


September 20, Hankinson, ND - 55 year old Dean Louis Marschel of Annandale, MN died when he became entangled in a piece of equipment.  He was found dead when the Sheriff's Office responded to a gravel pit area about three miles southwest of Hankinson.  He was employed with Bituminous Paving Company.


 Heavy Equipment Operator Killed in Incident

September 20, Elko, NV - 53 year old Pamela Hunt of Monticello, UT was killed while working on a construction project on Interstate 80 in Nevada.   She died when she was pinned between a rubber wheel roller and another metal roller.  Hunt was a heavy equipment operator for the Fisher Sand & Gravel Company on contract with the Nevada Department of Transportation's Silver Zone project. 


Man Dies at Work Site

September 20, Atlanta, IL - 53 year old James A. "Beamer" Davis suffered massive injuries while working at a road construction site near Washington. The coroner reports that Mr. Davis died as the result of being run over by an operated bulldozer. It is unclear how the incident happened.  Davis was employed by Stark Excavating.


MoDOT Worker Killed in the Line of Duty

September 20, Kansas City, MO - 50 year old Clifton J. Scott was killed when he was struck by a vehicle on I-70.  Scott had responded to a crash scene and was placing cones along the road when he was killed. Scott celebrated 15 years with MoDOT on the 17th and was the first employee to be killed after Missouri expanded its Move Over Law last month to include DOT employees and vehicles. 

  
Movie Set Crew Member Dies

September 21, California - 48 year old Mike A. Bridger died of a probable heart attack while working on the set of Disney's "Lone Ranger" movie.  Bridger was a welder/water safety expert who had worked on many movies including all of the "Pirates of the Caribbean" movies with Johnny Depp.  Reports state he may have suffered a heart attack while inside one of the large water tanks.

Bouncer Stabbed to Death at Night Club


September 23, Detroit, MI - 40 year old Lou Ferrin died after he sustained stab wounds at Club Pandemonium where he worked. He was attempting to eject a male patron from the club when he was stabbed in the neck by the man.


Two Killed in Construction Zone Incident

September 24, Chattanooga, TN- 28 year old Nicolas Feller of Shandon, OH (pictured) was killed when his truck was struck by a semi which then struck him as he was putting reflective lenses on pavement markers. Feller was working for Traffic Specialists, Inc contracted through TDOT. In the same incident, 53 year old Charles Morgan of Grainger Co, TN was driving the semi that struck the TDOT truck.  Reports say Morgan was not wearing a seatbelt at the time of the crash.

Utility Worker Dies in Fall

September 25, Las Vegas, NV - 29 year old Herbie Dempsey Goforth III died when he fell approximately 100 feet from a utility tower near Apex Industrial Park.  Goforth recently had completed his apprenticeship to become a journeyman lineman.  He would have celebrated his birthday on Sept. 26th and was planning to be married Oct. 6th. 

Equipment Failure Causes Death

September 26, Flomaton, FL -  55 year old Joseph Wayne Odom of Perdido, FL was crushed by a piece of equipment while working at Advanced Recycling Center in Flomaton.  Odom was working on a skid steer loader when hydraulic lines lost pressure causing the bucket to fall, pinning him underneath.

Miner Killed in Mine Ceiling Collapse

September 26, Tazewell, TN - 32 year old Jeremy Perkins of Wallins Creek, KY  was killed after part of the ceiling fell in on him inside a Kopper Glo Fuel mine in TN.  He was the only person involved in the incident in the company's deep mine.

Power Company Lineman Falls

September 20, Spokane, WA - 34 year old Matthew Karstetter  died after falling from a transmission tower near the Montana-Idaho border.  He was employed by Bonneville Power Administration.  According to his obituary, a few days before his death Karstetter was lowered to the wires by helicopter to rescue a crewman having a heart attack. 

Prison Worker Killed by Inmate

September 24, Ordway, CO - Sgt. Mary Ricard, a correctional officer at Arkansas Valley Correctional Facility, was killed in an assault that occurred while breakfast was being prepared.  



Man Crushed Under Steel Beam

September 27, 2012 - Lackawana, NY - 46 year old Sabahudin Ramulic was killed and a co-worker injured while working for Linita Design and Manufacturing when a 3-ton steel beam fell on them.  It is unclear what caused the beam to fall. 

Fired Employee Murders Five

September 27 - Minneapolis, MN - An employee who had been fired from Accent Signage Systems in Minneapolis responded by murdering three co-workers, a package delivery driver who was at the business as well as the owner of the sign business. The gunman then turned the gun on himself.  Below are the victims of this workplace shooting:

Energy Worker Killed in Blast

September 28, Pauls Valley, OK - 34 year old Billy Smith was killed in an explosion of a boiler unit while working for CVR Energy's Wynnewood Refinery. The cause of the blast, which seriously injured another worker, is unknown at this time. 


 Fuel Tank Explosion Kills Worker

October 1,  Lake City, FL - 30 year old Perry D. Pollard was killed when an explosion occurred in a tanker that contained a mixture of fuel. Pollard was measuring the fuel in the tanker truck when something ignited the vapors. He died at the scene.

Utility Worker Suffers Fatal Head Injury

October 2, Orleans, IN - 33 year old Brent "James" Minton of Paoli was killed when a utility pole struck him in the head. Minton, a journeyman lineman for Orange County REMC Electric Co-Op, was doing a routine task loading a pole with the assistance of two other employees when it fell lose from the cradle and struck him in the head and he was killed instantly.

Highway Worker Hit by Vehicle, Killed


October 2, Boone, NC - 55 year old David Cook of Salisbury, a North Carolina Dept. of Transportation employee, was struck and killed when he was trying to move a tree from a road. Cook died of head injuries at the scene.  


 Road Maintenance Worker Struck by Car

October 2, LaPorte, IN - 52 year old Daniel J. Miller, a Toll Road employee, was killed when he was struck by a speeding, out of control car driver.  Miller and another worker were in a work truck with warning signage alerting drivers to workers ahead. Miller, a passenger in the work pickup, had just exited the truck to get trimming equipment out of the back when the red passenger car hit Miller and the rest of the pickup. 

Arizona Border Patrol Agent Murdered

October 2, Naco, AZ - 30 year old Nicholas Ivie was shot to death and a second agent was wounded as they patrolled on foot an area near the international border near Naco.  The area is known to be a corridor for smuggling. Several suspects have been arrested by Mexican troops.



Highway Worker Killed by Dump Truck

October 2, Dorsey, IL - 62 year old Dennis G. Goebel was killed when he was struck by a Fort Russell Township Highway Department dump truck. Goebel was part of the township highway department crew performing culvert repair when the truck backed over him. Co-workers found him beneath the truck.

Man Dies After Being Run Over

October 2, Geneva, NE - 58 year old James Gable of Grand Island was killed when he was run over by a combine, a large piece of harvesting equipment. Gable worked for Allied Genetics in Grand Island and was measuring a test plot on the Elaine Kimbrough farm when the combine backed over him.  

Taxi Driver Gunned Down

October 3, Peekskill, NY - 53 year old Terry Camper was shot to death in what police are saying was an intentionally killing. Police would not discuss a possible motive behind Camper’s death, but said a suspect had been identified and is believed to have fled the area.  Camper's wife states that her husband worked privately in the area, driving local children to school and taking veterans to their medical appointments.

California Field Worker Crushed by Equipment

October 3, Lemoore, CA - 48 year old Victor Calderon Perez of Riverdale was killed when a trailer called a "cotton buggy" rolled over him.  He had been working in a cotton field when the incident happened.

Worker Dies of Electrocution

April 3, 2012 - Wichita Falls, TX - 39 year old Michael Glenn Tramell  was electrocuted when he attempted to restore power to the lease that had been knocked out earlier morning by storms. The company he worked for, Halcon Resources Corp. was cited for two willful and seven serious safety violations for this incident.


Truck Driver Dies in Crash

September 21 - Bonney Lake, WA - 60 year old Roger Adkins of Pacific died in a fiery truck crash after he left the roadway and his truck burst into flames. Adkins was driving a Fed Ex truck but was employed by MJM Trucking Inc. Reports state Adkins may have suffered a medical condition causing him to go off the road.

Heart Failure Causes Death

September 22 - Seattle, WA - 71 year old Felix Lagasca was a dishwasher at Gwinn Commons, Seattle Pacific University, for close to 25 years. Lagasca fell ill and was sent home only to collapse. He died of heart failure in the ambulance on the way to the hospital.

Worker Shot For Unknown Reason

September 26 - Campbellsville, KY - Wendell "Gleason" Pyles was shot to death at his place of employment, Tarter Gate Company in Dunneville, by individuals who have been arrested. It is unclear what caused the shooting.

Tow Truck Driver Struck

September 29 - Columbus, OH - 60 year old Bill Houck, a tow truck driver for Broad & James, was struck and killed by a suspected drunk driver while he was attempting to load a disabled vehicle. The suspected drunk driver hit the disabled vehicle which slammed into Houck, killing him.

Man Dies of Head Injuries

October 3 - Bakersfield, CA - 54 year old Barry Snelson was killed and another co-worker injured when a false bottom of a tank collapsed under them.  The men were testing a welding job by pressurizing the tank when the bottom fell, causing them to hit some pipes. Snelson struck his head and died of those injuries. The men worked for Brahma Group at Live Oak Cogan plant in Oildale.

Man Falls to Death at Warehouse

October 3 - Columbus, GA - 34 year old Gary Vielweber died after falling about 25 feet through the roof of a building he was working on. He was on a construction site in the Columbia Industrial Park and was an employee of Allstate Roofing Company.  

Elderly Harvest Worker Dies

October 3 - Bridgewater, ME - 80 year old Dorothy Rusby was struck and killed by a potato truck at Snow Road Farms where she worked. Police say she was working in the potato house and when a truck was leaving after a delivery, she was struck. The driver did not realize she was near his truck. Other employees say she had worked for the farm for a number of years and always loved the harvest season. 

Truck Driver Dies in Rollover

October 4 - Wichita, KS - 52 year old Abdaikarim Mohamed Haile of St. Cloud, MN was killed when the semi truck he was driving overturned on a curve near a toll booth of the Kansas Turnpike. Police say he was most likely traveling too fast into the curve. He suffered fatal head injuries. 

Workers Killed in Crash

October 4 - Duluth, MN - 25 year old Zachary M. Gamache of Esko and 29 year old Mitchell D. Lingren of Duluth died in a crash caused by a driver who was found to have drugs in her car. Gamache's truck, Lingren was a passenger, was struck after the other driver crossed the center line. That action sent Gamache's truck into the path of another vehicle. Both men worked for Carlton County Highway Works and were on their way to lay out directional lines for a scheduled culvert replacement in Wright.

State Trooper Struck and Killed

October 5 - Hanover, VA - 27 year old Andrew Fox of Tazewell, and a Virginia State Trooper, was struck by an SUV while directing traffic at an intersection in Hanover County. Fox was wearing the appropriate protective gear along with other troopers working with him. Traffic lights were flashing yellow for each direction yet the driver still crashed into him.

Worker Dies in Crane Incident

October 5 - Roanoke Rapids, NC -  Jimmy Denney of Roanoke Rapids died in what appears to involve a crane. He was employed by Tak Investments who was opening a recycling facility. A spokesman said either Denney was trapped in the crane or it fell on him.

Farm Incident Kills Man

October 5 - Durand, WI - 56 year old Eugene Hayden died while trying to harvest his crops on his farm near Lima. Reports do not specify what happened, but said a gravity box was involved in his death. 

Bouncer Killed by Patron

October 6 - Toledo, OH - 38 year old John R. White of Lake Township died of a skull fracture after being punched by a patron he was ejecting from the Deja Vu Club. White worked as security for the club. The suspect in the assault has been arrested.

Surface Miner Electrocuted

October 3, 2012 - Wishram, WA - 54 year old Richard Aldrich of White Salmon was electrocuted while working for Pacific Northwest Aggregates.  No details to how or why the incident happened are known. He was a General Superintendent of the mining operation.

Woman Murdered During Robbery

October 5 - Orange County, FL - 53 year old Ying Jie Sun of Pine Hills was shot and killed inside the China Wok restaurant where she was working. An armed and masked man came into the restaurant to rob it. When she didn't understand what he was saying, he shot and killed her.  

Office Dies During Training Exercize

October 7 - Austin, TX - 67 year old Sgt. Paul Hernandez suffered a fatal heart attack while training for physical readiness for the Texas Highway Patrol. Hernandez served as a law enforcement officer for 35 years.  



Worker Killed at Recycling Center

October 8 - New Castle, DE - 53 year old Robin Godfrey of Wilmington was killed when she was struck by a forklift while working for Eastern Metal Recycling in New Castle. She got off the machine to and it began to roll backwards. As she attempted to jump on the machine, she was pinned against a wall.

Vehicle Crash Kills Worker

October 8 - Dallas, TX - 26 year old Hakeem Mohammed was killed and his co-worker hurt when their Texas Department of Criminal Justice car ran into two horses standing on the road.   The crash happened at 3:00AM and it is not clear if it was work related or not.

Landscaper Killed by Felled Tree

October 9 - Hamilton County, OH - James Keller was killed when a tree fell onto him while working to remove a tree on a homeowner's property. His co-worker was not injured. 

One Worker Killed, One Injured at Plant

October 10 - Norfolk, NE - 58 year old Perry Hoemann, of Hoskins, was killed and a co-worker injured while working at the Norfolk Nucor Steel plant. They were dismantling a piece of equipment when a counterweight broke loose and struck the payloader machine they were standing in.

Wall Collapse Kills One, Injures Three

October 10 - Glasglow, KY - 63 year old Kenneth E. Decker of Bee Springs was killed when a nearly completed wall collapsed during a road construction project.   Officials believe Decker and the other workers were on top of the wall when it collapsed under them.

October 10 - Minneapolis, MN - Eric Rivers has died after being on life support for two weeks. He was working at Accent Signage System on September 27 when a disgruntled co-worker opened fire, killing five others, including a UPS delivery driver who was at the business.

Worker Dead at Food Plant

October 11 - Santa Fe Springs, CA - 62 year old Jose Melena of Whittier died when he became trapped in an active steam oven while working the overnight shift at Bumble Bee Foods tuna processing plant. He was discovered inside the oven by co-workers as they arrived for work in the morning. It is unclear how he came to be inside the active oven.


Road Worker Crushed

October 11 - Beverly Hills, CA - Adolfo Figueroa was crushed to death under a 3,000 pound steel beam while working on a road construction project. The beam was being lifted off the truck when it came loose and fell onto Figueroa. He was employed by Kiewit Resources.

Four Workers Crushed Under Falling Structure

October 12,15 - Doral, FL  - A five-story parking garage collapsed on the Miami Dade College West Campus in Doral, killing four workers and injuring several others. Following are the men who died in this incident:  48 year old Carlos Hurtado Demendoza; 60 year old Jose Calderon; 53 year old Samuel Perez and Robert Budhoo whose body was not recovered until the 15th. All the workers were employed by Ajax Building Group.

Worker Killed by Driver

October 12 - Canton, OH - 61 year old Franco Didomenico of Dearborn Heights was struck and killed by a driver who he was trying to help back out of his garage. Didomenico was a contractor working at a condominium when the incident occurred.

Musician Dies at Second Job

October 14 - Memphis, TN - 70 year oldBlake Baker Cunningham Jr., known as B.B., was shot and killed while working a second job as a security officer at a Memphis apartment complex. Cunningham came to the aide of a co-worker when he was killed in a shoot out at the complex.

Worker Crushed by Beam


May 25, 2012 - Fargo, ND - 30 year old Trevor J. Skarphol was killed while working for Scott's Electric on a school when he was crushed under a falling steel beam.





Man Presumed Drowned

October 7 - Longview, WA - 40 year old Charles "Bill" Wiley, Jr. of Vicksburg, MS is presumed drowned after falling from the Lewis and Clark Bridge into the Columbia River. Wiley was wearing a safety harness and it is unknown why it did not stop his fall. He was employed by Odyssey-Geronimo JV based in Houston, PA.

Employee Struck by Customer Truck

October 15 - East Conway, NH - 31 year old Fei Sun, and employee for E.C. Self Storage, was killed when a customer at the facility struck him with his truck.

Employee Suffers Stroke

October 15 - Portland, ME - 45 year old David Norton, a senior communications specialist for the University of Maine System, died at a hospital days after he was found by campus police in his Portland office. He was discovered after a co-worker reported Norton didn't show up for a meeting.

Worker Inmate Electrocuted

October 15 - Airway Heights, WA - 22 year old Danny R. Bergeson, an inmate from Airway Heights Correctional Center, was killed in a logging accident while working with a Department of Natural Resources work crew in Stevens County.The crew were thinning trees and Bergeson was electrocuted near power lines.

Boiler Explosion Causes Death

October 16 - Oklahoma City, OK - Russell Mann of Davis died after being injured September 27th when a boiler exploded. Mann was employed by CVR Energy's Wynnwood Refinery.


 Fall Causes Death

October 17 - Bridge City, LA - 44 year old Jeffrey Feucht of Kenner was killed and a co-worker injured while servicing a Cargill Industries grain elevator. Reports say Feucht and a co-worker were inside the elevator shaft when a cable broke, causing it to fall.  It is unclear who the worker's actual employer was.





Worker Dies When Harness Fails

October 17 - Jackson, MS - 43 year old Stacey Andrew Washington died after being injured in a fall October 12th. Washington, a subcontractor for Harrell Construction, fell five stories and his harness failed to stop his fall. It is unclear why. 

Postal Worker Dies of Dog Bite

October 17 - New Castle, DE - 55 year old Robert Rochester, a USPS postal carrier, died after being bitten by two dogs several days earlier. He was originally treated and released from a local hospital.  It is unclear what the exact cause of death was.

Man Dies at Oil Well Site

October 20 - Williston, ND - 59 year old Arthur B. Weyrauch of Wolf Point, MT died after becoming ill while working for K & R Roustabout's at an oil field owned by William Paul XEnergy in Killdeer.  

Three Murdered Inside Salon

October 21 - Racine, WI - 35 year old Cary Robuck of Racine, 42 year old Zina Haughton of Brown Deer, and 38 year old Maelyn M. Lind of Oconomowoc were all murdered by the estranged husband of Ms. Haughton.  They were all working inside the Azana Salon and Spa when Haughton's husband burst in the shop and started shooting. He later committed suicide. 

Bouncer Killed Breaking Up Fight

October 21 - Bronx, NY - 32 year old Christopher Braham was shot to death while attempting to break up a brawl between two warring groups at Pat's Palm Tree Club where he was working security. 

Worker Struck by Truck Dies

October 22 - Plymouth, PA - 66 year old Michael Sanfelice of West Norriton, PA was killed while working on a Pennsylvania Turnpike dump truck. Another vehicle slammed into the truck Sanfelice was working on, pushing it into him. He was an employee for the Pennsylvania Turnpike.

Death in the Oilfield

October 22 - Williston, ND - 60 year old Jesus Castillo of Santa Fe, NM  died after suffering burns while repairing a leak at an oil well site operated by William Paul XEnergy. Castillo worked for K & R Roustabout of Killdeer.

Longshoreman Dies

October 23 - Tampa, FL - 56 year old Hercules Gilmore of St. Petersburg was crushed under a bundle of steel pipes being prepared to offload from a ship at the Port of Tampa. As the pipes were being lifted, the strap broke sending the pipes onto Gilmore. Another worker was injured.  

Crane Operator Killed

October 23 - Kahului, Maui, HI - 62 year old Ronald Nakasone was killed when the crane he was operating tipped over, crushing him. Nakasone was moving equipment in the baseyard of Hawthorne Pacific Corporation when the crane began to tip over.

Plant Worker Dies

October 23 - Canton, OH - 37 year old Christopher Chin died while working at Pilgrim's Pride Chicken plant. Reports say Chin was trying to dislodge a piece of debris in a piece of machinery when he was killed. 

Backhoe Driver Killed

October 24 - Hastings, MN - 46 year old Jeffrey A. Tukua died of compression asphyxia after a backhoe he was operating tipped over, crushing him. Tukua was doing work for S.M. Hentges and Sons.

Worker Killed Working on Scissor Lift

October 24 - Bedford, IN - Brian Gilbert, who was injured on October 10th, has died of injuries involving a scissor lift at General Motors factory. Gilbert was a machine repairman and reports say the scissor lift collapsed onto him.  

Port Worker Dies

October 25 - Oakland, CA - 51 year old Mark Arnett, a Port America employee was killed while working at Port of Oakland. Arnett and another worker were working on a maintenance issue of a crane trolley and when it moved forward, Arnett was pinned between the trolley and a guardrail.

Worker Dies at Quarry

October 25 - Roanoke, VA - 70 year old Donald Reese was killed when he became trapped in a rock crusher. He was employed by Stanton Lime Company.  

Forklift Incident at Mine

October 25 - Ashland, NE - 52 year old Darold Abbott, Jr. was killed when a forklift he was operating overturned. The incident occurred at  Martin Marietta Aggregate mine near Weeping Water in Cass County.

Maintenance Worker Dies in Fall

October 26 - Lafayette, IN - 42 year old Jason Addler of Waynestown died when he fell three stories from a Marriott hotel in Lafayette. Investigators believe Addler was working in the attic when he slipped off a rafter and fell.

Mechanic Crushed Under Machine

October 27 - Deerfield, OH - 48 year old Dale Vacha was found pinned under a pneumatic lift while working at Xtreme Elements, a construction company. He was apparently working on a piece of equipment that was on the lift. 

Highway Construction Worker Killed

October 27 - Conroe, TX - 27 year old Joel Vega died when he was placing signs on along a roadway. Authorities say a vehicle struck a Williams Brothers Construction trailer which flipped, pinned Vega between the trailer and another truck. 

Texas Minister Murdered

October 29 - Forest Hill, TX - Danny Kirk, Sr. was murdered by someone who first drove their vehicle into the church building and then chased the pastor down and beat him to death. Kirk was the pastor of Greater Sweethome Missionary Baptist Church. It is unclear why the assailant sought to murder Kirk. 

Man Dies in Wall Collapse

October 29 - Honolulu/Kapolei, HI - 20 year old Alexander Tagaloa died at a hospital after being injured in a cinder block wall collapse. He was an employee with Nan, Inc. working at Schofield Barracks. 

Fireman Dies During Superstorm Sandy

October 29 - Easton, CT - 55 year old Russell Neary, a volunteer firefighter for Easton Fire Department was killed when he was struck by a falling tree limb while answering a fire call during Hurricane Sandy. Neary's full time job was as a vice president at General Re as an underwriter in the Global Ocean Marine division.
