Worker Dies When Catwalk Collapses

May 7, 2012 - Chattanooga, TN - 54 year old Larry Chubbs died when a section of catwalk came loose and caused him to fall into a piece of fast moving machinery. Chubbs worked for TAG Manufacturing which was sited by OSHA.

Clerk Dies During Robbery

July 27 - Burbank, IL - Baker Farhat was shot and killed by robbers while working at a west side gas station. No further details are provided.

Storm Damage Leads to Death

October 31 - Amberley, MD - 50 year old Santiago Lopez was killed during the removal of a tree that fell during Hurricane Sandy. Lopez worked for W.D. Olsen Tree Care for 10 years.

Worker Dies in Fall From Height

November 1 - Salt Lake City, UT - 51 year old Jose D. Parades died when he fell through a skylight while working on the roof of Harbison-Walker Refractories Company.

Miner Death

November 1 - Saugerties, NY - 30 year old Stephen Wickham of Greenville was killed when he became entangled in a drill auger while boring holes in the Northeast Solite Corp. shale mine in Saugerties.

Worker Suffers Fatal Injuries

November 2 - Auburn, ME - 60 year old David Brooks died while working for American Concrete. He had been stacking large blocks of concrete on a truck for transport. Reports say he was struck by a large piece of concrete.

Worker Dies in Fall

November 4 - Middlebury, CT - 53 year old Michael J. Pranulis of Naugatuck died after falling from a tree he was working to cut down. He was working to clear trees damaged during Hurricane Sandy.

Worker Killed Working with Bears

November 4 - Bozeman, MT - 24 year old Benjamin Cloutier died while cleaning out a pen that held two 500 pound trained brown bears. It is not clear if Cloutier was actually attacked by the bears as he had no defensive woundsnor had he used the bear repellant he was carrying. He may have fallen and struck his head before being mauled. He worked for Animals of Montana, Inc.

Construction Worker Dies in Fall

November 6 - Fort Collins, CO - 40 year old Joseph John Wiegand died when he fell from the interior roof to the floor while working on construction of a home.

Two Workers Murdered by Co-Worker

November 6 - Fresno, CA - 34 year old Manuel Verdin and 32 year old Salvador Diaz were murdered and two other employees received non fatal gunshot wounds when a co-worker went on a shooting rampage inside the Fresno Protein chicken processing plant. Reports show the gunman shot his victims deliberately. The gunman later shot himself.

Worker Struck by Car

November 7 - Bakersfield, CA - 25 year old James Chvalas Jr, an war veteran, died after being struck by a vehicle while working on a trench crew. Chvalas had crossed the highway to get a shovel and when he attempted to again cross the lanes, he was struck. It is unclear who his employer was.

Man Dies in Industrial Incident

November 7 - Dodgeville, WI - 54 year old Marc Nondorf died in an incident involving a forklift while working for Wick Homes. It appears a load he was moving with the lift collapsed and fell onto him.

Worker Dies of Unknown Causes

November 8 - Muskogee, OK - 33 year old Clint Woods was killed while working at the Georgia-Pacific paper plant in Muskogee. Woods worked as an electrical contractor who was subcontracted through Industrial Controls of Oklahoma. Details of the incident are not given.

Captain Dies in Fire

November 9 - Chicago, IL - 54 year old Herbert "Herbie" Johnson, a Chicago Fire Dept. Captain, died while battling a blaze on the south side. Fire officials say a possible 'flash over' killed Johnson while he was helping to fight a two-story house fire.

Worker Crushed Under Beam

November 10 - Sioux City, IA - 26 year old Cesar Balderama-Estrada was crushed under a heavy beam while working at the Sabre Industries site in Southbridge Business Park. It is unclear who his employer was.

Highway Worker Killed

November 11 - Rodanthe, NC - 36 year old Michael Brad Stevenson of Hertford was killed when the dumptruck he was driving rear ended the back of another dump truck, causing his truck to catch fire. Stevenson was a Dept. of Transportation employee working on a storm damaged section of highway.

Cell Tower Collapse Kills Worker

November 11 - Farmington, NM - 51 year old Sammy Pacheco was killed during construction of a cell phone tower. He was employed by L&B Telecommunication, LLC. Pacheco was on an already-placed section of the tower but was knocked off and to the ground after a crane failed to fully lift another piece of the tower over the other section.

Officer Struck byTruck

November 12 - Henry County, GA - 53 year old Officer Elgin L. Daniel was struck and killed while he was assisting a motorist. A roadside assistance worker was also struck by survived. Both were struck when another vehicle hit swerved into them.

Snapped Cable Causes Death

November 12 - Fontana, CA - Fernando Quezada died of blunt force trauma and another worker was injured while working for Oldcastle Precast when a cable snapped; striking them. The cable was being wrapped on an industrial sized spool to keep it taut.

Man Dies at Loading Dock

November 12 - Malad, ID - 65 year old F. Dennis Thomas died when he fell from a loading dock and then was hit by the wheels of a big rig trailer. Thomas was using a power lift loaded with food but fell off backwards when he got too close to the edge.

Worker Struck by Train

November 12 - Miami, FL - 48 year old George Andrews was killed while working on the tracks of a Miami-Dade Transit Metrorail train. It is unclear how or why Andrews was struck by the train.

Forklift Operator Killed

November 15 - Oakland, CA - 34 year old Eduardo Martinez Barajas suffered fatal head injuries when he was crushed under a forklift he was going to work on. Barajas worked for Super Link Plastics, a recycling company.

Dry Dock Worker Killed

November 15 - Hartford, CT - 23 year old Jason T. Sagez of Hardin was killed while working at National Maintenance & Repair in Hartford. He was a welder and maintenance worker. He died after a chain that was holding a 4-ton tugboat propeller broke under the tension and struck him, knocking him into the Mississippi River.

Helicopter Pilot Dies in Crash

November 15 - Corning, NY - 24 year old Mackenzie Bleth of Grand Ronde was killed when the helicopter he was piloting became entangled in wires and crashed. He was employed by Haverfield International inspecting transmission lines for New York State Electric & Gas Corp in upstate NY.

Tree Crushes Worker

November 16 - California City, CA - 55 year old Richard Dobrozdravic of Tehachapi was killed when a tree fell on him at the Hyundai Test track in California City. Dobrozdravic was employed by Kett Engineering of Van Nuys and it appears the tree fell as it was being moved or carried by a backhoe operator.

Deadly Oil Rig Explosion and Fire

November 16 - Grand Isle, LA - An oil rig explosion and fire on the Houston based Black Elk Energy platform killed two, injured many and one is still missing. According to this report, 42 year old Ellroy Corporal was killed, 28 year old Jerome Malagapo is missing and 49 year old Avelino L. Tajonera was among the injured but died on November 23rd.

Mine Worker Crushed

November 17 - Equality, IL - 30 year old Chad Wayne Meyers of Goreville was killed when he was pinned by machinery he was operating and the coal rib. He was working for Peabody Energy's Willow Lake Mine near Equality.

Trench Collapse Kills Worker

November 20 - Raleigh, NC - 39 year old Luis Javier Martinez was killed when a trench he was working in collapsed. Martinez was working for J F Wilkerson Contracting installing a water line for the city. He was buried in about three feet of dirt.

October 2, 2011 - Louisville, KY - 60 year old Edward Reinke died during a fall while photographing an Indycar Race in Sparta. He was a photography for The Associated Press.

October 5 - Zionsville, IN - 50 year old Kenneth L. McCulloug died on the job while working for The Power Source, LLC.

October 11 - Albany, NY - 56 year old Edmund F. Kelly Jr., died on the job while working for Boces Neric of Albany.

October 13 - Lebanon, TN - 37 year old Shawn Reddick died on the job while working for Walden General Contractors, Inc.

October 19 - Berkeley, CA - 51 year old Pedro Maldonado died on the job while working for Performance Abatement Service.

October 24 - Monroe, MI - 38 year old Ryan Charles died on the job while working for Spartan Steel Coating, LLC.

October 26 - Anaheim, CA - 49 year old Gary Wheelwright died on the job while working for the City of Anaheim.

October 28 - Fort Worth, TX - 66 year old James Locklear died on the job while working for Harris Quality, Inc.

November 3, 2011 - Okeechobee, FL - 60 year old Elton Hutson died on the job while working for Lykes Bros, Inc.

November 6 - Moorestown, NJ - 53 year old Stephen G. Pollock died on the job while working for American Biltrite, Inc.

November 6 - Seattle, WA - 77 year old Thomas Spencer died on the job while working for Dick's Drive In Restaurants.

November 7 - Mayaguez, Puerto Rico - 37 year old Pablo Montalvo died on the job while working for Oas Rigging Crane Corp.

November 9 - Wilson, NC - 49 year old Mark Raper died on the job while working for the Town of Lucama.

November 15 - Massapequa, NY - 32 year old Benjamin Canales died on the job while working for a company DBA Giannini Landscaping.

November 16 - Marks, MS - 25 year old Sulema Marisol Barre died on the job while working for Producers Gin of Belen, Inc.

May 2 - Indianapolis, IN - 51 year old Paul Profrock died when he was pulled into machinery while working for E&B Paving.

May 3 - Colusa, CA - 59 year old Daniel Kesterson died when a wall he was salvaging fell onto him while working for Ken's Firewood.

May 8 - Indianapolis, IN - 54 year old Bradley D. Arthur of Spencer died after being struck by a drunk driving while working for Milestone Contractors LP.

May 9 - Sacramento, CA - 62 year old Abdul Saladuhuddin died of unknown injuries while working for Abdullam Salahuddin.

May 25 - Smithfield, VA - 53 year old Adile Langley died of unknown injuries while working for Smithfield Packing Company, Inc.

May 29 - Asheville, NC - 49 year old Charles James Baker of Greeneville, TN died of massive blunt force trauma after being struck by a vehicle while working for Southern Concrete Materials in Asheville, NC.

June 1, 2012 - Nashville, IN - 20 year old Chelsea Ann Lovett of Granger died of blunt force trauma when a tree fell onto her while leading a horse-guided tour for Brown County Guided Trail Ride.

June 5 - Bloomington, MN - 37 year old Shawn Sprinkles of Harris died of head trauma injuries while working for Veit Companies, LLC.

June 5 - Oroville, CA - 45 year old Peter Scott Monti died in a logging incident of a fracture injury while working for Bamford Timber.

June 7 - Watsonville, CA - 37 year old Alberto Reyes died of unknown injuries while working for Vision Farms.

June 12 - Henderson, NV - 54 year old Daniel Emerson died of unknown causes while working for Harber Company, Inc.

June 13 - Buffalo, NY - 33 year old Jacqueline (Jackie) Wisniewski was murdered by her doctor ex-boyfriend while working at Erie County Medical Center.

June 15 - Albany, NY - 55 year old Michael Cook of Selkirk died suddenly while working at the Albany City Schools Main Office.

June 18 - Parsonsburg, MD - 34 year old Shawn William Malone of Salisbury, was crushed under several hundred pounds of drywall sheets while working on a home for Cooper Road LLC.

June 21 - Brentwood, CA - 69 year old Jim D. Odom of Antioch, died in a construction site incident while working for Jimmy A. Odom General Building.

June 26 - Norfolk, VA - 58 year old John Frederisy died of unknown causes or injuries while working for Crown Corr, Inc.

July 2,2012 - Oakland, CA - 47 year old Randy Wayne Sisler died of head injuries when his harness broke and he fell to the ground while cutting a tree. He was owner of Oakland Timberjax, LLC.

July 13 - Irvine, CA - 59 year old Soledad Randon died of unknown injuries or cause while working for Dupont Residential Care.

July 16 - Quincy, CA - 47 year old Todd Raymond Walker of Red Bluff died after he was struck by logs rolling off his logging truck while working at Sierra Pacific Industries lumber. He was owner of Todd Walker dba Todd Walker Trucking.

July 27 - Normal, IL - 50 year old Paul Collins died of unknown injuries or cause while working for IL State University.

Worker Dies of Natural Causes

March 14, 2012 - Ashburn, VA - 74 year old Nicolas Giampietro died of natural causes while on his lunch break. He did sustain a few minor injuries during his fall and died on the 18th at a hospital. He was working for L-3 Unmanned Systems, Inc.

Man Suffers Electrocution

March 21 - Goldsboro, NC - 58 year old Roland Harlow died on the job while working for SPX Transformer Solutions, Inc. He was working on a transformer when he touched an object and he became the path to ground and was electrocuted. He died on June 1, 2012.

Woman Suffers Fatal Fall

March 22 - Woodbridge, NJ - 73 year old Jean Donato died from injuries sustained in a fall while participating in the mayor's exercise event. She fell down several concrete steps and hit her head. She was working for Woodbridge Township Board of Education.

Worker Dies of Toxic Ingestion

March 30 - Nanjemoy, MD - 35 year old Roger Townsend died under tragic circumstances while working for Melwood Horticultural Training. Having a history of seizures he was disoriented and instead of drinking water he mistakenly swallowed a gas/oil mixture that was contained in a plastic container.

Fall From Height Causes Workers Death

April 2, 2012 - Doral, FL - 55 year old Edelberto Escalera died on the job when he fell approximately 35 feet when he fell through a roof skylight at the end of his work day. He owned Edelesca Service, Inc.

April 2 - Gulfport, MS - 43 year old Shannon P. Reeves died of head trauma after falling from a yacht to the ground below. He was employed by Progressive Products & Service, a subcontractor for Premium Windows & Siding.

Crushing Injuries Causes Worker Death

April 6 - Sumrall, MS - 23 year old Thomas White died after being crushed under a falling tree he was helping cut. He was employed by Looks Great Services of MS, Inc.

Tow Truck Driver Struck by Motorist

April 7 - Florence, SC - 24 year old James Coffey of Timmonsville died on the job when he was struck by a motorist while assisting another tow truck driver. He was employed by Millers Foreign Car.

Worker Suffers Crushing Injuries

April 9 - Winchester, VA - 54 year old Jack Taylor was killed on the job while working for Rubbermaid Commercial Products. He was operating a stand-up forklift and struck a horizontal beam. He was ejected from the lift and the 8,500 lb beam fell onto him.

April 9 - San Juan, Puerto Rico - 60 year old Gaspar Hernandez died on the job when he was struck by a client's vehicle at a shipping location. He was employed by Maderas 3C, Inc.

Possible Stroke Causes Man's Death

April 10 - Irvine, CA - 55 year old Steve Miller died while he was working for Dino's Masonry, Inc. Miller felt dizzy and he died of a probable stroke.

Sanitation Worker Dies From Fall

April 13 - Peekskill, NY - 40 year old John Fischer II, a maintenance worker, died April 23rd of head injuries he sustained while working for Peekskill City DPW. He was working on a garbage truck when he was ejected from the truck, falling head first onto the ground.

Man on Mower Struck by Motorist

April 17 - Collinsville, VA - 75 year old George Lee Marlowe of Martinsville was killed on the job while working for Patrick Henry Community College. He was operating a riding lawn mower when it collided with a vehicle and he was knocked off.

Dump Truck Backs Over Worker

April 17 - Deerfield Beach, FL - 53 year old Ricardo Corrales of Hialeah suffered fatal head trauma when he was struck and run over by a dump truck driver who was backing up the truck.

Transportation Worker Killed

April 24 - Red Oak, IA - 54 year old Wallace G. Lerebvre of Callaway died on April 26 while working in Omaha, NE. He suffered fatal spine and head trauma when he fell from the top of a tanker trailer.

Utility Worker Electrocuted

April 25 - Middletown Springs, VT - 53 year old Russell Callahan suffered a fatal electric shock while working for Adirondack Utility Construction. Callahan was installing internet service when the part of his bucket truck came into contact with hit a power line.

Worker Dies of Unknown Cause

April 26 - Chicago, IL - 62 year old Thomas Osborne died June 12 of unknown injuries while working for the City of Chicago. He had just inspected a building and was struck by a car while walking across the street.

USP Driver Struck by Vehicle

April 27 - Lower Manhattan, NY - Michael Rogalle was killed while working for United Parcel Service when an SUV driver jumped the curb striking Rogalle. He died at an area hospital on April 29.

Worker Dies From Fall Injuries

May 5, 2012 - Tacoma, WA - 59 year old Robert Brown died May 18 after he suffered injuries due to a fall while working for Americold Logistics, LLC. After becoming stranded on a boom liftanother worker attempted to use his forklift with pallets to help him down. Unfortunately the pallets shifted under Brown and he fell 35 feet.

Employee Collapses While Working

April 8 - Virginia Beach, VA - 60 year old Nenita Aquino died of probable natural causes when she collapsed while sweeping floors. She was employee by WalMart.

Heart Attack Claims Worker

May 18 - Wilton, CA - 62 year old Fidel Ramirez died May 22 after suffering a heart attack as he was leaving work after trimming in a vineyard. He was employed with Martin H. Aldana.

Marine Worker Dies of Crushing Injuries

May 17 - Seattle, WA - 61 year old Paul Stewart died of injuries received while working at SSA Marine Inc. Reports state he was caught between the mast of a forklift and overhead protection. He apparently climbed over the steering wheel and inadvertently activated the controls with his foot tilting the mast back onto his body.

Medical Issue Causes Death

May 19 - Livermore, CA - 35 year old Colin Jude Klausner of Castro Valley died of a heart attack as he was leaving an event he was working as caterer. He was employed by Wente Bros/Wente Family Estates.

Worker Dead of Head Trauma

May 21 - Bardstown, KY - 37 year old Benito Cruz Moreno of Bloomfield died of head trauma due to a fall while working for Raisor Construction, LLC. He fell from a walkboard to the ground.

Man Dies in Fall From Grain Elevator

May 30 - Crawfordsville, IN - 72 year old Richard "Dick" Lowe of Darlington died in a fall while working on a grain bin. He was self employed with Lowe Construction and was using a crane to remove a filter box. The box swung one direction but as it swung back it struck Mr. Lowe, causing him to fall to the ground.

June 5 - Otsego, MI - 28 year old Matt McKee was killed on the job when he became caught in the running belt of a conveyor. He was employed with Renewed Earth, Inc.

June 5 - San Pedro, CA - 52 year old William Ojers III died of a medical event while working for Abhe & Svoboda.

June 7 - Catano, Puerto Rico - 30 year old Manuel A. Fernandez was killed on the job when the driver of a garbage truck backed over Fernandez causing head trauma. He was employed by Andres Reyes Burgos, Inc.

June 7 - Virginia Beach, VA - 30 year old Jose Bustamante died when he fell into an elevator shaft while at lunch. He was employed by Carlos Ramirez.

June 13 - Raleigh, NC - 50 year old Randy Wilson died when he was struck from behind by a car while he was testing fuel at a gas station manhole. He was employed by Clean Fuels of Indiana, Inc.

June 13 - Memphis, TN - 40 year old Dionicio Beltran died when he was caught by a loading arm of a garbage truck while working on the packing mechanism. He was employed by Scruggs Equipment Co, Inc.

June 15 - Olive Hill, KY - 55 year old Paul Michael Walters died on the job of heat exhaustion while working on the roof of a local school. He was employed by Fairfax, Inc.

June 15 - Tenmile, OR - 30 year old Daniel Garcia was killed when a choked log broke loose and flew directly at him while working for Millicoma Logging, LLC.

June 18 - Nashville, TN - 47 year old Jay Luther died of asphyxia when he became trapped inside a cooler unit that lost power when the door closed behind him which had a missing lock release mechanism. Workers were using dry ice to keep the unit cold which he was exposed to. He was found the following day. He was employed by Food Business, Inc.

June 19 - Leesburg, VA - 65 year old Sanford H. Gulick died when he was struck on the head by an excavator bucket that he was working on. He was employed by Gulick Excavating, Inc.

June 19 - Tucson, AZ - 44 year old Mark Geise died of heat exhaustion while working for Seese Construction & Management.

June 20 - Deming, NM - 29 year old Adam Cabrera died when he fell through a hole while repairing an air conditioner unit. He was employed by South Plains Implement, Ltd.

June 21 - Lake Orion, MI - 62 year old David Mastny was killed on the job while working as a refueling porter for Powers Distributing Co. He was found crushed under multiple kegs that had fallen from the back of a semi truck.

June 22 - Central City, IL - 55 year old Michael Fontaine was killed on the job when the limb of tree that was on fire fell onto him. He was awaiting the arrival of a fire truck. He was employed with Village of Central City.

June 22 - Hollister, CA - 35 year old Jose Gonzales died while working in a field after he passed out. He was employed by Swank Farms.

June 27 - Sheridan, NY - 64 year old Ronald J. Keddie died of a heart attack after responding to his station for a call. He was employed by Sheridan Fire Department.

June 27 - Dandridge, TN - 74 year old Hulet Lee Ducker was killed when he fell from a mezzanine in a storage barn while working for Dandridge Golf and Country Club.

June 29 - Baxter, KY - 42 year old Bobby A. King, Jr. died of crushing injuries when the forklift he was operating overturned onto him. He was employed by Bluegrass Materials Co.

June 30 - Fresno, CA - 47 year old Jesus Carrillo died after he was found unresponsive inside a walk-in refrigerator stocking shelves. He was employed by Vallarta Food Enterprises, Inc.

July 2 - Jessup, MD - 50 year old Kevin Diggs died of crushing injuries when he reached through the space between the top of a forklift mast and edge of cab and was pinned. He was employed by Elite Spice, Inc. (recycling)

July 9 - Watsonville, CA - 45 year old Liborio Garcia collapsed and died while picking fruit due to an aneurysm that was not work or heat related. He was employed by Oasis Berry Farms, Inc.

July 10 - Buffalo, NY - 61 year old Carl Yost died when he struck a concrete barrier while driving a work vehicle. He may have had a heart attack. He was employed with the Buffalo City DPW Office.

July 10 - Canby, OR - 48 year old Gregory Priest was killed while performing road construction when he was struck by the driver of a vehicle. He was employed with Knife River Corp.

July 11 - Fresno, CA - 24 year old Regan Johnson died when he was struck by a vehicle while working on a highway resurfacing project for Windsor Fuel Co.

July 13 - Albany, NY - 77 year old Esterina Trichilio fell while cleaning and broke his leg. He died of surgery complications. He was employed with NYS Office of General Services.

July 19 - Germantown, TN - 52 year old Jose Guadalupe Hernandez was killed when he fell from a roof that he was helping clean off old materials, throwing them to the ground. He was working for Rafael Esparza Lopez.

July 23 - Locust Valley, NY - 45 year old Jorge Guevara was killed when the over sized cement mixer he was driving struck an bridge overpass, causing it to go out of control and strike another vehicle. He was working for Locust Valley CSD.

July 30 - Galt, CA - 69 year old William Portillo was unloading a rack of baked goods from the rear of a delivery truck when the rack tipped over and pinned him under it. He was employed by Bimbo Bakeries USA, Inc.

AUGUST 2012

Aug 1 - Indianapolis, IN - 58 year old Anthony Graves died on the job of blunt force head trauma while working for Henry Schein, Inc.

Aug 1 - Henderson, NV - 49 year old Lou Lucido died of injuries he sustained from being cut by pressurized spewing fluid from a cracked hydraulic cylinder. He was employed by ROC Equipment, LLC.

Aug 1 - Malone, NY - 61 year old George C. Joyal died as the result of a tragic set of circumstances while working for NYSDOCCS Franklin Correctional Facility. It appears Mr. Joyal slipped from a chair and as he hit the floor it caused a bone spur in his neck to dislodge. It caused paralysis and when told he would be permanently paralyzed he asked to be removed from life support.

Aug 5 - Las Vegas, NV - 57 year old Steve Mason died of heat exhaustion while working at a stadium event for the University of Nevada, Las Vegas.

Aug 6 - Pasco, WA - 63 year old Jose Jara Enriquez died from unknown causes after collapsing at work. He was employed by Twin City Foods.

Aug 8 - Buffalo, NY - 48 year old Christopher Taczak was found unresponsive at his desk and later died of unknown causes. He was employed with Erie County Medical Center (ECMCC).

Aug 21 - Beverly Hills, MI - 58 year old Kevin Patterson died of positional asphyxia when he was trapped inside his overturned dump truck. He was employed with Oakland County Road Commission.

Aug 23 - Charleston, IL - 59 year old Doug Lawhead died of heart attack while in a meeting on the Eastern IL University-College campus. He was a photojournalist instructor.

Aug 25 - Louisville, KY - 39 year old Sergio Perez "Poncho" De La Cruz died on the job when he fell from a ladder, and then the ladder fell onto his head. He was employed with Jorge Vargas Martinez.

Sept 13 - Manteca, CA - 54 year old Calvin Pitto died while on the job when he was struck by a truck driven by a co-worker. He was employed by Delicato Winery.

Sept 15 - Modesto, CA - 52 year old Clemente C. Galan died on Sept 17 after being removed from lift support. He was working in a guard shack when he collapsed while working for Panella Trucking.

Sept 21 - Bayamon, Puerto Rico - 70 year old Ramon Vazquez died when he fainting walking down stairs while working on school construction for Bermudez Longo Diaz Masso Se. It was determined he suffered a stroke.

Sept 24 - Indianapolis, IN - 55 year old Ken Wiltsie died of a gunshot wound to the chest under unknown circumstances. He was working for Indiana Women's Prison.

April 2 - Houston, TX - 34 year old Carlos Guillen died of an unknown cause. He was found by others laying on the floor deceased. He was employed by Metroclean Commercial Building Services.

April 2 - 35 year old Daniel Yonashiro Jr., of Cushing, died on the job when he came into contact with an energized wire and was electrocuted. He was employed by Bloom Oil Field Electric Services.

April 2 - Pembroke Pines, FL - 28 year old Victor Sanchez died of thermal burns when the fuel being poured into a riding mower ignited/exploded. He was employed by MDL Property Management, Inc.

April 2 - Manning, ND - 58 year old Hiroshiki Soto died of blunt force trauma when he was struck by pipes that slid off a truck, causing him to fall off and under the pipes. He was employed by A&R Construction.

April 3 - Brooklyn, NY - 22 year old Giovanni Santos Garcia died on the job while working for J.D. Construction and Contracting Corp. when the second story of a home collapsed, crushing him.

April 3 - San Antonio, TX - 55 year old Roy Garcia died while working when he fell from a ladder while painting. He was employed by Spectrum Painting.

April 4 - Douglas, MA - 53 year old Martin "Marty" Gamache died when he came into contact with an energized wire and was electrocuted while working on machinery. He was employed with Classic Envelope Inc.

April 4 - Gonzales, TX - 33 year old Jose Adrian Govea did on the job while working for Express Energy Services Operating, LP. He was a derrick hand. He slipped, lost his footing and fell approximately 50 feet to the steel rig floor.

April 5 - Florham Park, NJ - 57 year old Frank Nobile Jr. died at work of a heart attack. He was employed with Latierno & Son Corp.

April 6 - Simsboro, LA - 54 year old Carolyn Durbin was killed at work when she was struck by a forklift. She was employed by St Gobain Containers, Inc. dba Verallia Containers.

April 9 - Prudhow Bay, AK - 53 year old David "Davey" James of Fairbanks, was killed on the job when he became caught between a pipe elevator and wall on a Doyon Drilling, Inc. rig.

April 10 - Kansas City, KS - 62 year old Ivica Malic was killed on the job when he was crushed under a tractor axle that fell from an overhead hanger. He was employed by Harlan Global Manufacturing.

April 11 - Big Spring, TX - 57 year old Maria Patricia Cardenas was killed when the truck she was driving for One Source Industrial was struck by a train. The incident occurred at the Alon Refinery in Big Spring.

April 11 - Andalusia, AL - 49 year old Melinda Ward, of Rose Hill, died while at work when she suffered an aneurism while cleaning. She was employed with Environmental Service Systems, LLC.

April 13 - Newark, DE - 31 year old Nicholas "Nick" Njenga of Middletown died on the job when he became pinned between a hydraulic liftgate and truck. He was employed by Savanna Auto Care, LLC.

April 14 - Leonia, NJ - 19 year old Denis Duarte was killed on the job while working for D. Barro Landscaping & Construction, LLC. He was working to smooth a section of ground under a large slab of concrete when the chain holding the slab broke and crushed Duarte.

April 16 - Fort Rucker, AL - 41 year old Anthony Brock was killed on the job when steel beams collapsed onto him. He was employed by Precision Erection Company, Inc.

April 16 - Hammondsport, NY - 51 year old Daniel Martin Sr., of Bath, died when he became trapped under an overturned tractor in a Keuka Lake Vineyards, Ltd.

April 16 - Mandeville, LA - Benito Cerna died while working when he was struck by lightning while walking to his work truck. He was employed by Marcello Juarez dba Marcello Juarez Concrete.

April 17 - Dallas, TX - 64 year old Ozel Dismuke was killed on the job while working for Trinity Industries, Inc. He fell from a tractor and was run over by the rear axle.

April 17 - Alva, OK - 25 year old Gaylon Taylor of Granbury, was killed on the job while working for Caprock Energy Services, LLC. He was struck in the head by a flailing high pressure water hose and fell face down in water, drowning.

April 18 - Millersview, TX - 77 year old Richard Stovall of Midland, drowned while working underwater on a concrete tower pumping station. Details as to how he became trapped underwater is not clear. He was owner of Stovall's Scuba & Travel in Midland.

April 18 - Galveston, TX - 19 year old Bryan Hunt of Houston, was killed on the job while working for WM Contractor. He was power washing a roof when he stepped on the roof skylight and fell to the ground. He was wearing a harness but had not attached the lanyard.

April 18 - Catarina, TX - 34 year old Cory Glen Zablosky died on the job when he was struck in the head by an injector leg of a machine. He was employed by Thru Tubing Solutions.

April 19 - Angleton, TX - Carlos Cuellar was killed on the job when he fell 22 feet through an unprotected roof skylight. He was employed with Juan Alajandro Alvarez, dba Alex's Remodeling.

April 20 - De Pere, WI - 58 year old Raymond Aschenbrenner was killed on the job when he was crushed under a falling boom crane while unloading equipment from a trailer. He was employed by Lunda Construction Company.

April 21 - New Orleans, LA - 44 year old Dan Gordon was killed on the job when he was crushed under a collapsed concrete wall of a business adjacent to his workplace. He was employed by Dan Gordon.

April 21 - Capron, OK - Martin Kern was killed on the job when he fell from a derrick board. He was not tied off. He was employed by Dan D. Drilling, Inc.

April 23 - Oklahoma City, OK - 46 year old Peter Cleeland was killed on the job while working for Horizon Energy Services, LLC. He was pinned between a hoist platform and substructure while attempting to pin the hoist in place.

April 23 - Omaha, NE - 83 year old Robert Karasek died on the job when he was struck by a metal door of a roll-off container that was being offloaded from a truck. He was employed by Teamwork, Inc.

April 24 - Portsmouth, NH - 26 year old Ben Harris, of Newington, was killed on the job when an over-pressurized keg ruptured into pieces, striking him in the head and chest. He was employed by Craft Brew Alliance, Inc. dba Redhook Ale Brewery.

April 24 - Princeville, IL - 51 year old Harvey Smith of Chillicothe, died of injuries to his legs when he was struck and run over by his runaway garbage truck. He was employed by Wigand Disposal Company.

April 24 - Pueblo, CO - 58 year old Raymond "Pete" Duran was killed on the job when he was struck by and run over by a five-ton industrial forklift. He was employed by Evraz Rocky Mountain Steel mill.

April 25 - Liberty Corner, NJ - 25 year old Nick Rutkowski was killed on the job when he was struck in the head by a falling tree branch. He was employed by Tamke Tree Experts, Inc.

April 26 - Cuyahoga Falls, OH - 46 year old Win Tun died of a heart attack while working at a window finishing machine. He was employed by Associated Materials LLC dba Alside.

April 28 - Magnolia, TX - Elroy Vargas-Contreras died while working when he fell from a height. He was standing on an unstable ladder on a scaffold. The ladder slipped, causing the ladder, scaffold and worker to fall.

April 28 - Washington DC - 32 year old Valdimir Polejaev was killed on the job while working for DSNK Corporation. He leaned against a temporary balcony guardrail which gave way, causing him to fall 21 feet.

April 28 - San Francisco, CA - 25 year old Richard Din died of a deadly bacteria, the same type of meningitis strain that he had been researching at the VAMC hospital. He was employed by U.S. Dept. of Veteran's Affairs.

April 30 - Willoughby, OH - 47 year old Drago Glavas of Mentor, was killed on the job while working for Glavas LLC. He was working in a basket 20 feet from the ground when he touched a live wire. That caused him to be thrown back in the basket which then came off the forks and fell to the ground. He struck his head on a metal table as he fell.

April 30 - Borger, TX - 50 year old Walter Dale Richardson of Dumas, was killed on the job when he was struck by a load-out chute when a securing pin failed. He was employed by ConocoPhillips.

April 30 - Larose, LA - 51 year old Hector Hernandez died while working when he was found unconscious at the bottom of stairs leading to an employee housing unit. He was employed by North American Shipbuilding, LLC.

April 30 - Philadelphia, PA - 33 year old Andres Cano was electrocuted when a bucket he was hoisting up made an electrical contact. He was then knocked from the scaffolding. He was employed by Eight Fairmount Builders, LLC.

May 3 - Honolulu, HI - Tran Nam Phuc died while working for David Vo Fishing, LLC. He suffered a fatal heart attack.

May 4 - Perkins, OK - 49 year old Francisco Cerda died on the job when he fell from an elevated platform, striking his head against a metal table. He was employed with Ralph's Packing Co.

May 5 - Bronx, NY - 52 year old Edison Oseguera was killed on the job when he fell through a roof skylight. He was working for ALR Environmental Corp.

May 7 - Asheville, NC - 63 year old Dana Bruce was killed on the job when the riding lawn mower he was operating slid down an embankmentat a Blue Ridge Parkway overlook. He was employed by U.S. Department of Interior/National Park Service.

May 7 - Roswell, GA - 64 year old John Clarke was killed on the job when he fell off the roof of a building as he was taking measurements for a pre-construction job. He was employed by Malon D. Mimms Co., LLC.

May 7 - Jackson, MO - 29 year old Matthew Pooker was killed on the job when he became pinned between the tracks of a mobile crane and railroad timbers while helping to change a track on the crane. He was an engineer for Penzel Construction Company.

May 8 - Boulder, CO - 39 year old Eduardo Castro was killed on the job when he was crushed under a steel canopy that collapsed as it was being erected. Two other workers were injured. He was employed with Wyatt Construction Co, Inc. a sub for High Ball Erectors, LLC and Steelstar Corporation.

May 9 - Edgeworth, PA - 47 year old Chester Sipes of Aliquippa, was killed on the job when a pole saw he was using in an elevated bucket contacted an overhead power line. He was owner of DLS Services.

May 9 - Lake Sherwood, MO - David Martin, Jr. was killed on the job when he came into contact with energized power lines while working for C. Bennett Building Supply, Inc. dba Contractors Roofing and Supply Co.

May 10 - Keatchie, LA - 22 year old Zachary "Ty" Stewart was killed on the job when he fell 90 feet on an oil rig. He was wearing fall protection but the cable was severed.

May 12 - Portland, CT - 63 year old Andrew B. Sloan of Colchester, died on injuries he suffered in an incident May 10th. He was found under a powered industrial truck that rolled down an embankment. He was working for Fairways Fivesome LLC dba Portland Golf Club.

May 12 - Cleveland, OH - 43 year old Timothy Sherman of Garfield Heights, was killed on the job when he became tangled around the neck with an airline hose that caught on a cement truck auger. He was employed by Sherman Trucking, cleaning out a cement truck at Cuyahoga Cement Co.

May 13 - Hawley, TX - 48 year old Barry Gordon was electrocuted as employees were raising a 25 foot antenna which came into contact with an overhead power line. He was employed by Downtown Design Services, Inc.

May 14 - Kingwood, WV - 63 year old Clifford Likens, of Tunnelton, died of a broken neck when he was struck by an elevator car while working in an elevator shaft at Preston Memorial Hospital where he worked maintenance.

May 15 - Albion, ME - 52 year old Robert Rodrigues of Augusta, was killed and another worker injured when a structure fell onto them. The men were working to replace a support beam when rain soaked earth caused automotive bottle jacks to fail under the cottage causing it to collapsed on the workers. They were employed by Kennebec Home Improvements.

May 15 - Houston, TX - Bradley Felchak died of electric shock and subsequent fall from a ladder while working for Hightower Electric Services, Ltd.

May 15 - Kane, PA - 53 year old Dale Ostrander suffered fatal blunt force trauma when an unoccupied truck rolled into him while he was working at Kane Community Hospital.

May 16 - Odessa, TX - 42 year old Humberto Villanueva suffered a fatal heart attack while working for J M Carriers.

May 16 - Harvey, LA - 47 year old Randy Harris drowned while working as he attempted to retrieve a boat that came loose from a dock. Co-workers threw him a life ring but he was unable to reach it. He was employed by Total Marine Services of Jefferson, Inc.

May 16 - Beaumont, TX - 36 year old Robert Dustin Whittaker of Nederland, died of electrocution when he was working in an electrical cabinet. He was employed by Gulf Coast Machine & Supply Company.

May 16 - Joplin, MO - 40 year old Robert W. Clark of Baxter Springs, KS, died on the job when a power saw he was operating kicked back and struck him in the neck. He was employed by Missouri American Water.

May 17 - New York, NY - Adrian Zamora died on the job when he fell two stories from a scaffold onto a ground level shed. He was not wearing fall protection. He was employed with Bras-Al Construction Corp.

May 17 - Seattle, WA - 26 year old Francisco Montex-Lopez died while working when he was overcome by toxic gases produced by a gas engine of a pressure washer he was using in an enclosed space. He was employed by Karma Coating & Maintenance at Clipper Seafoods, Ltd.

May 17 - Big Spring, TX - 45 year old Kenneth Caddell was killed on the job when he fell approximately 80 feet from a derrick board. His fall device failed. He was employed by ACME Energy Services, Inc., dba Big Dog Drilling.

May 19 - Philadelphia, PA - 44 year old Gregory Morgan was killed on the job when he was struck by a motorist while painting a fence. He was employed by Sandmeyer Steel Company.

May 20 - Natchez, MS - 24 year old Catlain Carithers of Meadville died on the job of blunt force head trauma when he was attacked by inmates during a prison riot. He was employed with Corrections Corporation of America dba Adams County Correctional Center.

May 20 - Hartley, TX - Jeremy Tunnell died of electrocution while working alone on an irrigation electrical system for 15 Black, LLC.

May 21 - Northvale, NJ - 48 year old Gregory B. Barber was crushed to death under a wood chipped when a hydraulic jack under it failed. He was employed by Ken's Tree Care, Inc.

May 22 - Fairview Heights, IL - 48 year old Dennis Beard died and three others were injured when they were struck by an erratic driver while working on a road construction project. They were employed by Briteway Striping Services, Inc.

May 22 - Mesquite, TX - 52 year old Jose Monsirais-Garcia died at an area hospital after feeling ill during a cleaning process. He was employed by Transportation Services, Inc.

May 22 - Pittsburg, PA - 26 year old Joshua Oswalt died of electrocution after coming into contact with live wires. He was working for American Contracting Enterprises, Inc.

May 22 - Arlington Heights, IL - 55 year old Neil Nicholson of Itasca, died on the job from blunt force trauma caused by an explosion of equipment at Arens Controls Company, LLC.

May 24 - New York, NY - 44 year old Antonio U. Dardenas was killed on the job when he fell about 30 feet through a window opening to the ground. He was employed by Stephen Gamble, Inc.

May 24 - Bridgeville, DE - 47 year old Richard White of Milford, died on the job when he was buried in an 8 foot trench that collapsed onto him. He was employed by Walter H. Sharp & Son, Inc.

May 24 - Clewiston, FL - 20 year old Richard Ramsey of Okeechobee, was killed when an airboat he was operating collided with another airboat. He was working for Applied Aquatic Management, Inc.

May 25 - Garland, TX - 76 year old Nancy Harris died of thermal burns she suffered May 20th when she was set on fire by a robbery suspect while working as a clerk at Fina Whip-It (owned by TMT, Inc.) Her attacker was arrested.

May 25 - Quincy, MA - 42 year old Freddie Clay was killed at work when he was pinned between his truck and a gate as he attempted to stop the rolling truck. He was employed by Devaney Energy, Inc.

May 25 - Cincinnati, OH - 35 year old Miguel Nicanor-paz was killed on the job when he slipped and fell while working on a sloped roof with no fall protection. He was employed by Esponiza Construction.

May 25 - East Greenbush, NY - 48 year old Daniel Tobin of Petersburg, died of electrocution when he came into contact with a high power line. He was employed by DelSignore Blacktop Paving, Inc.

May 25 - Kearny, NJ - 55 year old David Kornas was killed on the job when a machine he was operating ejected a piece of aluminum, smashing through the viewing window, striking him in the head and neck. He was employed by Hummel Machine & Tool Co.

May 25 - Kansas City, MO - 18 year old Brandon Ford was killed on the job when he lost balance and fell from the back of a rear-loading refuse truck and as the driver was reversing, Ford was struck by the rear wheels.

June 1 - Birmingham, AL - 51 year old Thomas Williams was killed on the job when a bundle of cross ties fell from a forklift, striking him. He was employed by Beasley Timber Co, Inc.

June 1 - Olmsted, IL - 54 year old Robert "Bob" Billingsley of Vienna was killed on the job when he fell in between a sand barge and towboat as the barge was being pushed into place. He was working Olmsted Locks and Dams for URS Washington Division.

June 1 - Burlington, CO - Kenneth Neve was killed on the job when a tire and wheel he was working on exploded sending it into the roof and striking Neve in the head. He was working for Eastern Colorado Tire & Auto, LLC.

June 2 - Houston, TX - 48 year old Javier Delgado was killed on the job while working for L-Con, Inc. He was welding when crude oil remnants ignited and a plumber's plug blew out of the end of the pipe, striking Delgado.

June 4 - Worthington, WV - 31 year old Jason Scot Wiles of Terra Alta died on the job when he was struck by a falling tree branch from a felled tree. He was working for Backwoods Logging, LLC.

June 4 - Blossburg, PA - 26 year old Dustin Dobson of Mansfield, was killed on the job when he was crushed between a fixed post and a piece of machinery. He was employed by Ward Manufacturing, LLC.

June 4 - San Antonio, TX - 36 year old Victor Trevino died while working for Williams Brothers Construction Co., Inc. He was working on a bridge frame when he fell through a hole, approximately 50 feet, in the highway deck.

June 4 - Houma, LA - 52 year old Timothy James Morvant was killed on the job when a metal plate collapsed onto him when an attachment clip gave way. He was working for Oil States Skagit Smatco, LLC.

June 4 - Houston, TX - Manuel Antonio Funez was killed at work when he was struck by pipes that were being loaded onto his truck via a forklift. He was employed by Richway Transportation Services, Inc.

June 5 - Caputa, SD - 68 year old Robert Gilland of Rapid City was killed on the job when a paving roller machine he was operating rolled over, crushing him. He was working for Simon Contractors.

June 5 - Ligonier, PA - 53 year old Cian Yates of Conemaugh Township, died on the job when he slid off the roof of a building. He was working on a wet metal roof, unhooked his rope grab to climb down but instead slid off the roof.

June 6 - Mobile, AL - 46 year old Kenneth Lundy was killed on the job when he was crushed under falling pile driving equipment. He was working for Jordan Pile Driving, Inc.

June 6 - Fort Leonard Wood, MO - 45 year old Louis C. Stallings of Atoka, OK, died on the job when he was struck by a rolling rail car while inside a track mounted vehicle. He was employed by McAlester Army Ammunition Plant performing maintenance.

June 6 - Salem, OH - 72 year old Frank Cisneros of Rootstown, died on the job after being injured June 4th when he fell from an unsecured extension ladder that was falling. He was employed by Tele-Solutions, Inc. at Boc Water Hydraulics, Inc.

June 10 - Neenah, WI - 58 year old Paul J. Merkley died at work as the result of a mechanical failure during maintenance. He was struck in the head by a hydraulic check-valve part that exploded. He was employed by Neenah Foundry dba Neenah Enterprises, Inc.

June 11 - Cameron, LA - 25 year old Pablo Trejo of Dallas, TX, was killed on the job when he was struck in the head by a suspended load when a forklift carriage tilted, striking him.

June 12 - Tampa, FL - 54 year old Joseph Wilson died while working when he fell approximately 60 feet through an unsecured roof opening. He was working at and for Dynamic Concrete Solutions, LLC / Miller Bros. Construction, Inc.

June 12 - New Carlisle, OH - 68 year old John Gearhardt of Covington, died on the job when he was pinned by forklift forks when a cotter pin failed, causing it to collapse on him. He and a crew were working on a pipe project for Covington Plumbing, Inc.

June 12 - Chicago, IL - 50 year old Andrew Felske was killed on the job while working for Central Steel & Wire Company. He was walking backward, directing a load, when he tripped over materials and fell onto the concrete floor.

June 13 - Newbury, NH - Brian Knox died of electrocution when the chipper of a crane outrigger touched an overhead power line. His hands were on the controls of the chipper. He was employed by Guillemette Tree Service.

June 14 - DeLand, FL - 56 year old Pedro "Pete" Ayala, Jr. was killed on the job when he fell approximately 32 feet through a roof skylight to the floor. He was working for Ball Fabrics, Inc.

June 14 - Ridgewood, NJ - 71 year old Carl Taylor of Sussex, was injured June 14th and died on June 23rd, when he was struck in the head by flying wood debris during home deconstruction. He was working for Joy Construction.

June 14 - Fort Worth, TX - 47 year old Billy Rockmore died of heart attack while working at Fort Worth Country Day School, Inc. He was found unconscious by others on a golf cart.

June 14 - Hays, KS - 27 year old Jeremy Johnson was working under a farm truck, fully loaded with grain, when the jacks failed and the truck crushed him. He was employed with Kansas Land Tire Co, Inc. in Hays.

June 14 - Houston, TX - Ciro Lagunas died of unknown injuries while working for Advanced Drilling Systems.

June 14 - South Bay, FL - 39 year old Juan M. Ruiz of Belle Glade, died after his tractor rolled into a canal as he was spraying weeds. He was employed by Sugar Farms Co-Op.

June 14 - Yorktown, TX - 42 year old Francisco Martinez was killed on the job when he was struck by a pipeline pressure manifold object. He was working for Camino Agave, Inc.

June 15 - Katy, TX - Jenry Sorto was killed while working when he fell from a boom truck's bucket due to mechanical failure of the turntable. He was employed with The Davey Tree Surgery Company.

June 15 - Cumming, GA - 39 year old Jose Rodriguez-Duarte was killed on the job when he was crushed under a 2,640 pound "pill press" machine that fell onto him. It is unclear how it fell. He was an employee for Lakewood 400 Antiques Market, LLC.

June 15 - Austin, TX - 58 year old Jose Barahona died while on the job possibly due to heat stress. He was employed by Webber, LLC.

June 15 - Stuart, FL - 38 year old Jonathan Sheaffer of Coconut Creek, was crushed to death when a hydraulic jack being used under a mobile home failed and fell on him. Two others were injured. He was working for Sheaffer Services, Inc. dba Sheaffer Mobile Home Installation.

June 16 - Staten Island, NY - 64 year old Ken Danziger of Archville, Scarborough, was killed while working when he fell from the top of a moving elevator car. He fell between the shaft wall and moving car. He was the owner of The Elevator Man, Inc.

June 18 - Norman, OK - 24 year old Colby Shaw of Oklahoma City, died on the job when he slid from the roof of a building, approximately 48 feet. His lanyard was not attached to an anchor. He was employed with Universal Roofing & Sheet Metal, Inc.

June 18 - Faith, SD - 58 year old Steven Bootz of Lemmon, was killed while working when a packer machine he was operating became unstable on an incline, rolling over and pinned him. He was employed by Zandstra Construction out of Rapid City.

June 18 - Vienna, GA - Roy Harris was killed on the job when a combustible liquid caught fire while he was working on an engine. He was employed by Vienna Cotton Co., Inc.

June 19 - Adel, GA - 26 year old Juventino Mejia Cinto was killed on the job when he was run over by a bus used to transport workers to a farm field. He had fallen asleep on break under the bus. He was employed by Mario Munquia dba Munguia Harvesting.

June 20 - Saint Louis, MO - Arthur E. Koch III died on the job when he fell from a ladder while climbing it with a laptop in one hand. He was employed by Kaiser Electric, Inc.

June 20 - Malakoff, TX - 32 year old Felipe Wilbanks of Log Cabin died while working when he was crushed by a felled tree. He was employed by Long Cove Development, Inc.

June 21 - Staten Island, NY - Salvador Rodriguez died on the job when he struck his head on blacktop when a trench he was working in collapsed against his body. He was employed by Reliable Septic Systems dba Reliable Enterprises, Inc.

June 21 - Blythville, AR - 57 year old Tommy Westmorlan collapsed and died while working for Rosiek Construction Co., Inc. Exact cause of death unknown.

June 24 - Bennet, NE - 47 year old Vincenta Contreas was killed on the job while working for T.H. Construction Company. He was using a pipe wrench attempting to remove an end cap, when stored up energy caused the wrench to spin striking Contreas in the head.

June 24 - Abbeville, LA - Roderick Fitzgerald was killed when he was struck by a Marine Portable Tank (MPT) at Grand Isle Shipyard, Inc. that was being moved with a forklift. He was employed by Trussco, Inc. Parent company Omni Energy Services.

June 24 - Austin, TX - 41 year old Dominic Lozano died on the job while working for Samsung Austin Semiconductor, LLC. He was found laying on a stairway landing by co-workers. Cause of death is not clear.

June 25 - Factoryville, PA - 26 year old Lucas "Luke" Mitchell of Hop Bottom was killed on the job when a drum he was using a cutting torch on exploded. Flammable vapors (methanol) caused the drum to explode. He was employed by Clark's Garage.

June 25 - Bayfield, CO - 42 year old Randy Mathews of Mancos was killed on the job when a "pig launcher" from a pump truck ruptured, striking Mathews first before flying approximately 900 feet away. He was employed by Elkhorn Construction Inc. at BP’s Piñon Compression Station.

June 26 - Brookhaven, MS - 32 year old Joseph Woodward Jr. of Edwards was killed on the job when he was struck by a motorist in a highway work zone. He was employed by Lane Line, LLC.

June 26 - Benton, FL - 39 year old Larry Smithhart of West Melbourne, and 41 year old Teresa de Jesus Camargo Perez were killed in a boat collision between 2 BRP boats while on a demonstration run. Smithhart was employed with BRP US, Inc. and Camargo was employed with Jason Inc, dba Milsco de Mexico, A Unit, Jason Inc.

June 27 - Fairfield, OH - Fred Cook died while working for Hav-Tech, Inc. when he fell from a ladder. He may have suffered a heart attack which caused his fall.

June 27 - Englewood, FL - 62 year old Allan Roberts was killed on the job when the compactor he was operating went into a ditch, pinning him. He was employed by Florida Natives Nursery, Inc.

June 27 - Gooding, ID - 43 year old Higinio Ledesma was killed on the job when he was crushed between a loaded semi trailer and front end wheel loader with bucket removed. He was employed by Edward Rosa dba R&R Holsteins.

June 27 - Dawson, TX - Robert Lehew died of natural causes while working for Ballard Tree Service, Inc.

June 28 - West Mifflin, PA - 52 year old John Hazel, Jr. of Finleyville died at work of asphyxiation due to neck compression when he was pinned between an aerial lift and the ceiling. He was employed by R&B Mechanical, Inc. performing maintenance at GE Energy plant.

June 28 - Conroe, TX - 56 year old Louie Ruiz of Porter, was killed while working for Big 4 Erectors when the operator's cab of a crane toppled over, crushing him inside.

June 28 - Arboles, CO - 44 year old Adleberto Pena died of fall injuries when, while working on top of a mast, lost his balance and fell to the ground. He was employed with Key Energy Services, LLC.

June 29 - Solomon, KS - 42 year old Vincente Castaneda was killed on the job when he was crushed under a 9,000 pound electrical transformer tipped onto him. He was employed with Solomon Corp.

June 30 - Plains, GA - 31 year old John O. "Jay" Williams Jr., was killed when he stepped on and fell through a fiberglass skylight panel. He was employed at his family business, Williams Warehouse, Inc.

June 30 - Houston, TX - 33 year old John-Henry Janse Van Rensburg of Angleton died when he was struck by lightening while working for North Houston Pole Line.

July 1 - Nitro, WV - 45 year old Rex Michael Wilcoxen, of Fraizers Bottom, who on June 25th was exposed to toxic fumes died of asphyxiation. According to reports, a helper hooked up what he thought to be an air line for Mr. Wilcoxen but it instead was to a nitrogen line. When Wilcoxen put on the supplied air hood, he lost consciousness from the nitrogen. He was employed by AC&S, Inc. a Nitro chemical manufacturer.

July 1 - Andrews, TX - 20 year old DeAndre Ross died on the job while working for Thornton Drilling Co. He was electrocuted while power washing.

July 2 - Lucedale, MS - 15 year old Thomas Harlan Jr. was killed while working when he was struck in the head by a pole moving on a tram. He was employed by Tri-State Pole & Piling, Inc.

July 2 - Houston, TX - 61 year old Alfredo Ruiz was killed on the job when he sustained head injuries after being struck by a forklift and knocked to the ground. He was employed by CHEP dba Catalyst & Chemical Containers.

July 2 - Anchorage, AK - 42 year old Joseph Renteria, of Eagle River, died of drowning when his security truck drove off the end of a pier and into Knik Arm at the Port of Anchorage. It is unclear the cause for the truck driving into the water. He was port security guard for Doyon Universal Services, Inc.

July 2 - Austin, TX - 42 year old Pedro Hernandez Gomez was electrocuted when the aluminum extension ladder he was using touched power lines. He was an arborist for The Tree Tender.

July 2 - Solon, OH - 60 year old Lewis A. Bissell of Mentor, sustained injuries in an un-witnessed event and he was found on the floor with his forklift nearby in-gear and running. He was employed with Superior Beverage Group, Ltd.

July 3 - Jefferson, GA - 40 year old Scott Parr died on the job after falling into an energized switchgear (electrical box) while attempting to remove a stripped screw from the cover. He was employed by Escoe Industrial Contractors, Inc.

July 3 - Random Lake, WI - 39 year old Karl Bramschreiber of Horicon, was killed on the job when he was caught between moving machinery and a building structure. He was employed by Laning's Dairy Supply.

July 3 - Lake Geneva, WI - 45 year old Keith Krause was killed on the job when he fell approximately 25 feet through an opening in a roof at the Lake Geneva Yacht Club. He was self employed with Krause Building, LLC.

July 3 - Teague, TX - 52 year old Charles Matthew died on the job when he fell from an elevated bucket while trimming trees. He was employed by Rutherford Tree Service.

July 5 - Chireno, TX - 34 year old Jeremy Keith Neely was killed on the job while working for Platinum Energy Solutions, Inc. He was struck by an end cap that separated from a high pressure pumping unit at a gas well site.

July 6 - Tuscaloosa, LA - 47 year old Mike Singley of Axis, was killed on the job while working for West Alabama Mechanical, Inc. He was struck by a crane gear box/pump on the boom as he attempted to jump off the crane as it was tipping over.

July 6 - Fort Hancock, TX - 29 year old Leopoldo "Leo" Cavazos, Jr., a Border Patrol agent for Customs and Border Protection, was killed while working when he was crushed under his ATV vehicle as it flipped backwards on a slippery and steep embankment.

July 6 - Atlanta, GA - 23 year old Jose S. Coronado-Flores was electrocuted when the aluminum extension ladder he was using came into contact with overhead live wires. He was employed by Alma Delia Gonzalez.

July 7 - Machesney Park, IL - 47 year old Erwin Guzman, of Round Lake Heights, was killed on the job when he was pinned between a rolling delivery truck and a semi trailer at the Lowe's store. He was employed by Jose Gaitan dba Gaitan Deliveries.

July 9 - Grand Prairie, TX - 65 year old Hipolito Suarez died while working when he lost his balance while carrying a leaf blower up a ladder, and fell with the ladder. He was working for Saddle Creek Corp.

July 10 - Orlando, FL - 63 year old Gregory Bloomstorm died of drowning when he was trapped under a riding mower he was operating that flipped into a canal. He was employed by Pioneers International, Inc. dba Pioneers, Inc.

July 10 - Forest Hills, NY - 63 year old Frank Avino was struck and killed by a motorist while setting up cones along Grand Central Parkway. He was employed by Welsbach Electric Corp.

July 10 - Mansfield, PA - 57 year old Robert Hannigan of Hunlock Creek, was electrocuted when a aerial lift truck made contact with a power line. He was employed by Lecce Electric, Inc.

July 10 - Pedricktown, NJ - 25 year old Isael R. Salguero was killed on the job while working for Leonard Chavarria dba Leonard Chavarria. He fell from the second story of a roof edge.

July 10 - Arvada, CO - 37 year old Luis Duran was killed on the job while working for Tecta America Co. He was pulling a cutter backwards on a flat roof when he fell about 12 feet to a lower surface.

July 10 - Wichita, KS - Jesus Esparza died while working for Case & Associates when he fell from a moving vehicle, striking his head on the pavement.

July 11 - Mentone, TX - 19 year old Ryan W. White, of Carlsbad, NM,, was killed on the job when he was struck in the head and caught between the digger bucket and outrigger. He was working for Joyce Fished, LP dba Fisher Construction.

July 11 - Cambridge, MA - 51 year old Mark D. Buckminster of Malden, died on the job when he fell from a ladder while working for Far East Construction, Inc.

July 11 - Blanco, TX - 24 year old Martin Surez of San Marcos was killed when he was knocked from the tailgate of a moving truck while working for C&B White Services. The truck was also carrying signs and as one blew out, it knocked the worker from the truck.

July 11 - Overland, MO - 22 year old Roman Vasquez was killed when he tripped and fell through an intact skylight while moving a roller on the roof of a building. He was employed by L & C Exteriors.

July 12 - Breckenridge, TX - 62 year old Danny Howk was killed on the job while servicing a pump jack at an oil well site. He was caught be the pump jack counter weight. He was employed by Breck Operating Corp.

July 12 - Triplett, MO - 50 year old Jimmie Bruce was killed on the job while working for Hampton Alternative Energy Products. He was crushed under a heavy steel pipe top rail that fell off the stanchions, pinning him to the ground.

July 12 - Stamford, CT - 40 year old Juan Carlos Elisea Orozco was electrocuted while working for Welding Services, Inc. He was pulling a steel beam over a suspended electrical metal conduit when his hand touched pre-existing exposed live wires.

July 13 - Pearl, MS - 34 year old Michael E. Evans, Jr., of West, was killed on the job when he lost his balance and fell through a skylight 20 feet above the floor. He was employed with Dean Construction.

July 13 - Washington DC - 49 year old Clifton Besey died on the job when the ladder he was using slipped, causing him to fall about 40 feet. He was working for Thompson Paint.

July 13 - Brownsville, TX - 57 year old Jose A. Badillo was killed on the job while working for RM Walsdorf, Inc. at Port of Brownsville. Report states he was working on a hydraulic lift platform that suddenly fell, striking his head.

July 13 - Elm Mott, TX - 56 year old Javier Herrera died of unknown causes when he collapsed while shoveling asphalt. He was employed with Williams Brothers Construction.

July 13 - Williamsburg, PA - 46 year old Ray McCabe was killed on the job when he was crushed under a tree he had just felled. He was employed by Baker Logging.

July 13 - Parsons, WV - 28 year old Joseph Kyle Helmick, of Keyser, WV, was killed on the job when he was struck by a tree limb. He was employed by Nichols Construction LLC.

July 16 - Reserve, LA - 28 year old Billy J. Keller, Jr. of LaPlace, was killed on the job while working for Associated Terminals, LLC. He was working alongside a barge when a tension rope broke, striking Keller, knocking him into the water.

July 16 - Gruver, TX - 28 year old Oscar Cruz was killed on the job while working for Sunray Co-Op Gin. Report states he dropped a crank handle from the truck into a pit and when he went into it to retrieve the handle he was engulfed in 450 bushels of wheat. He was not attached to a life line.

July 16 - Florence, AL - 57 year old Robert Salter was electrocuted while working on an air conditioning unit. He was sitting on metal duct work when he came into contact with a gas line that had been energized. He was employed by J.C. Hamm & Sons, Inc.

July 16 - Beaver Dams, NY - 74 year old Charles L. McFall Sr. of Corning, died on the job while working for McFall's 414 Service. He was crushed between a 4-yard rear loaded dumpster and a garbage truck.

July 16 - Newark, NJ - 45 year old Raymond DeSontro died of injuries sustained in a fall while working for Chimento Construction Company. He was working from an aerial lift, not wearing a safety harness.

July 17 - North Bay Village, FL - 51 year old Kenneth Hayden died on the job when he fell from a tree while using a manual pole trimmer. He was employed by Jim's Trim & Landscape Inc.

July 17 - New Castle, PA - 29 year old Allan Greenwood of Belle Vernon, was killed on the job when he was crushed by a telephone pole that he was working with. It appears a motorist hit the pole, causing it to hit Greenwood. He was employed with Bruce and Merrilees Electric Company.

July 17 - Harrisonville, MO - 39 year old Steve Hillman died when he stepped off a ladder and into an unprotected hole, falling through. He was self employed for Steve Hillman Construction.

July 17 - Saint Augustine, FL - 55 year old Mark Sowards of Elkton, was killed on the job when the tractor he was operating was struck by a motorist. He was ejected when the tractor flipped on its side. He was employed with Bone Dry Enterprises, Inc.

July 19 - Amarillo, TX - Huy Pham died of crushing injuries when granite slabs fell onto him after the restraints were removed. He was employed with Panhandle Quartz Fabricators, LLC.

July 19 - Mullin, TX - 52 year old Jesus Govea of Corsicana,died after falling from a communications tower. He was attempting to loosen a nut on a brace but his body weight caused the brace to pull away from the tower. His lanyard then slipped off the brace causing his fall. He was employed by Chapman Construction Co.

July 20 - Channelview, TX - 54 year old Silvino Hernandez died on the job when he received an electric shock while working under a mobile home. He was employed by Affordable Mobile Home Movers.

July 20 - Garden City, TX - 58 year old Archie Lee Collins died while working for Sendero Drilling when he was struck by a pipe while working with a crew to lay down drill pipe.

July 24 - Paragould, AR - 30 year old Andrew "Andy" Wayne Myers was killed on the job while working for Dixieland Gin Repair, LLC. He was working with a lathe when he was caught by the cylinder teeth and pulled into the machine.

July 25 - Saint Joseph, MO - 28 year old Jayson W. "Yogi" Gilbert died of electrocution when he came into contact with an overhead power line with an extended pole chain saw. He was employed with Trees Cut Cheaper.

July 25 - San Antonio, TX - 62 year old Juan Robles died on the job while working for Elias Ovalle-Gonzales. He fell approximately six feet from a mobile scaffold while preparing walls for paint.

July 26 - Waco, TX - 32 year old Pedro Garcia III died of electrocution when he inadvertently touched live wires while working under a residence on a gas line. He was employed by Cornerstone Plumbing.

July 26 - New York, NY - 41 year old Anastassios Intzeyiannis died after suffering injuries July 16 in an elevator shaft. He was working on a wood plank when he tripped and fell 75 feet into an elevator shaft.

July 26 - Danville, IL - 51 year old Brett Hauk was killed on the job while working the Pepsico Quaker Plant. He fell approximately 500 feel from a man lift. He was employed by Quaker Manufacturing, LLC.

July 26 - Boca Raton, FL - Alberto Justo Osorio was killed on the job when he was ejected from a boom lift basket while performing painting work. He was not attached to a harness. He was working for Colors Construction, Inc.

July 26 - Mount Laurel, NJ - 49 year old Mark Teufel, believed to be from Warminster, PA, died while working when he received an electric shock. He was owner of Mark Teufel Electric Construction.

July 26 - Saint Cloud, FL - 31 year old Chris Snetzko was killed on the job while working for Beyers Portable Welding, Inc. He was working on a trailer and, wet from perspiration, suffered an electric shock as he touched a live wire.

July 27 - Rock Falls, IL - 23 year old Cody Bragg of Sterling died on the job when he was crushed under steel plates that fell while being moved. He was employed by The IFH Group, Inc.

July 28 - Cotulla, TX - Alfredo Zepeda was killed on the job when he was crushed between a mobile crane and a truck trailer. He was employed by UV Logistics, LLC.

July 29 - Grover, PA - 21 year old Nathan McCandless of Butler, was electrocuted when an aluminum pole he was using touched a power line. He was collecting wildlife date for Wildlife Specialists, LLC.

July 29 - Brookfield, WI - 21 year old Jaja Jumoke Davis died while working when he suffered a fatal asthma attack while working as a dish washer. He was employed by Surg Restaurant Group, LLC dba 8-Twelve MVP Bar and Grill.

July 30 - Petal, MS - 60 year old Reese Gore was killed on the job when he was crushed inside the cab of his truck when the loaded trailer overturned onto the cab. He was employed with Warran Paving Inc.

July 30 - Chicago, IL - 59 year old Raul Tabares was killed in an industrial incident while working for Soudan Metals Company, Inc. He was struck by and caught between a roll of steel.

July 30 - Three Rivers, TX - 58 year old Wayne Moehring died of unknown causes after being found on the ground next to a trailer he had been loading sand into. He was working on a drilling site for 3 Way Trucking, Inc.

July 30 - Lubbock, TX - 58 year old Larry Castro died of an unknown cause while performing maintenance at an apartment complex he was working at. He was employed with Case & Associates Properties, Inc.

July 31 - Bernville, PA - 49 year old Martin D. Reusing of West Reading, was killed on the job when he was struck by a tree limb while trimming trees. He was employed by Manny's Tree Service.

July 31 - Westcliffe, CO - 20 year old Jeffery Mosher of Clear Creek was killed when he was struck by a semi truck during a construction zone flagging operation. He was working for Mountain Barricade, Inc. and Tezak Heavy Equipment Co., Inc.

July 31 - Westcliffe, CO - 57 year old Robert White of Temple, TX was killed when he was ejected from his semi truck/trailer after being involved in an accident in Colorado.

July 31 - Hialeah Gardens, FL - 41 year old Dalia Monteaguro was killed on the job when he was crushed under a load of materials falling from a forklift. He was employed by Brian's Door, Inc.

July 31 - Dallas, TX - 44 year old Cruz Delgado Sanchez died on the job of heat stroke/heat stress while working for Ramirez General Construction Company.

July 31 - Sweetwater, TX - 46 year old Maxwell Holsey died of an unknown cause while working for Texas TST, Inc. He was handling hot aluminum ingots on a stacking transfer table when he collapsed in front of one of the cooling units.

July 31 - Toledo, OH - 63 year old Richard Wilhelm Jr., of Northwood, died of unknown causes after a same-level fall and hitting his head on a wooden table. He was employed with Libbey Glass Inc.

August 1 - Miamisburg, OH - 61 year old Mark Rainey of Dayton died of unknown causes while on his first day of work for A.H. Sturgill Roofing, Inc. He was working on a roof when he began to act lethargic, loopy, slumped over and became unconscious.

August 1 - Marshall, TX - 41 year old Michael Noel died of electrocution while in an aerial bucket trimming trees. He was employed by Fason Tree Service.

August 2 - Fort Walton Beach, FL - 56 year old Ronnie Blackmon died on the job while working for D & B Builders, Inc. He was taking measurements when he lost his balance, falling approximately 16 feet.

August 3 - Liberty, MO - Edward Robets (Roberts?) died of injuries he sustained while working for RR Donnelly Company. He fell while attempting to work on an existing air line. Other details not known.

August 3 - Dayton, OH - 58 year old William "Bill" Hounshell died after being injured on July 27 while working for ATW Automation, Inc. He was struck by/caught between two conveyors and suffered a broken spine.

August 4 - New Albany, OH - 29 year old Fermin Hernandez died of fall injuries while working for Horacio Trejo. He was reaching for materials when he slipped and fell from the roof of a building.

August 7 - Keithville, LA - 28 year old Derek Sonnier died of reported pulmonary congestion while working for Elite Coil Tubing Solutions, LLC.

August 7 - Midland, TX - 20 year old Daniel Sauls died when an ATV machine he was operating flipped on top of him while he was trimming weeds. He was employed by AAW Services.

August 7 - Fond Du Lac, WI - 63 year old David Miller, Sr. suffered crushing injuries while working for ACH Foam Technologies, LLC. He was crushed by a pneumatic door on a steam expander silo.

August 7 - Broken Arrow, OK - 40 year old Francisco G. Matehuala died of heat related stress while working for Ron Davis. Other details are unknown.

August 8 - Chicago, IL - 61 year old Joseph Malinowski died on the job while working for Ogden Avenue Materials, Inc. He slipped on a conveyor frame, fell onto a scaffold and then to the ground.

August 8 - Palmetto, GA - 35 year old Leo L. Robinson was killed on the job while working for Exel, Inc. He was pinned under an overturned forklift he was operating.

August 9 - Pembroke Pines, FL - 33 year old Marvin Vasquez died after becoming ill while working as a landscaper for National Core Services Corp., dba Grounds Group.

August 9 - Atlanta, GA - 35 year old Antonio Guzman died after being struck by lightening after picking up his lunch box from his work truck. He was employed by AAA, National USA, Inc.

August 10 - Conley, GA - 31 year old Jeremy Sisler was killed on the job when the elevated cab of a semi truck fell down pinning him to the frame of the truck. He was working for DJS Tractor & Trailer Service, Inc. and Qualawash Holding, LLC.

August 11 - York, PA - 59 year old Bernard "Bernie" Lenhardt Jr. died of blunt force injuries when brakes failed on a vehicle he was operating. He was employed by BAE Systems.

August 11 - Commerce City, CO - 28 year old Juan Garcia died while working for MG Transportation. He was working on the back of a truck when another worker started the truck. The truck then lurched and knocked him to the ground.

August 13 - Manchester, OK - 55 year old Byron Little Coyote of Enid died while at work for Atlas Drilling Company when he fell from a height at an oil rig.

August 14 - Brentwood, NY - 46 year old Patricia Selegna-Maqueda was killed when a house exploded. She was employed with State Farm Insurance and was at the home regarding a flooding claim when a propane explosion occurred. A small child was also killed.

August 14 - Dalhart, TX - 50 year old Louis Kelly was electrocuted when copper ground wire he was wrapping around a vehicle fell across high voltage lines. He was employed by Complete Power Pole Inspection, Inc.

August 14 - Fort Worth, TX - 34 year old Keith Cook died of blunt force injuries when he fell from a ladder that became unstable. He was employed by Frontline Property Management, Inc., dba Candle Chase Apartments.

August 15 - Aspermont, TX - 58 year old James Green died of a heart attack while working for GEM Drilling.

August 15 - Williston, ND - 39 year old Jason Pinasco of Higden, AZ died of blunt force injuries when he was struck on foot by a vehicle that was being moved during an oil rig blow-out. He was working for Zavanna LLC.

August 15 - Pangburn, AR - 40 year old Ronnie Youngblood of Adona died while working for Millsap Construction, Inc. when he fell ill working in a remote location.

August 16 - Wooster, AR - 21 year old Justin Robinson was electrocuted when he inadvertently touched a live wire while stripping wires on a job site. He was employed by Hammett Electric, Inc.

August 16 - Atlanta, TX - 19 year old Joshua Alexander died on the job when he was buried in an unprotected trench collapsed. At the time he was working for John Hathcock Construction.

August 16 - Houston, TX - 59 year old Simon Coverson III died while working for Andrews Transport, LP. He died of a probable, unwitnessed, fall from a trailer.

August 16 - Ridgewood, NY - 43 year old Anthony Consalvo died of blunt force injuries when the ladder he was using on a warehouse rack became unstable and as it fell, caused Consalvo to fall backwards. He was employed by M&V Provisions Co., Inc.

August 17 - Deer Park, TX - 45 year old Brian Johns was injuredon July 16 when he was sprayed with a hot condensate during equipment operation while working for Dow Chemical, Texas, Inc. He died on August 17 from his injuries.

August 17 - Loxahatchee, FL - 50 year old Charles Sears of Riviera Beach was killed when, as a passenger in a garbage truck, was crushed under the truck as it overturned. He was employed with Waste Pro USA, Inc.

August 19 - Hockley, TX - 36 year old Arturo Pineda was killed on the job when he was caught in a dust explosion caused by a cutting torch. He was working for Jordan General Contractors, Inc.

August 19 - Hockley, TX - 27 year old Alejandro Limon was killed on the job when he was caught in a dust explosion caused by a cutting torch. He was working for Jordan General Contractors, Inc.

August 20 - Theodore, AL - 55 year old Paul Hodgson was killed on the job when he was struck by falling pipes as he loosened the straps on a flatbed trailer. He was employed with Hertz Energy & Pump Services, Inc.

August 20 - Harvey, LA - 46 year old Olen R. Duplessis Sr. died almost two weeks after being struck by a motorist while working in a work zone. He was employed by Fleming Construction Company.

August 21 - Allentown, PA - 34 year old Frederick Hossler died of crushing injuries when he was trapped by a tractor-trailer truck against a loading dock. He was employed with J.C. Ehrlich Co., Inc., dba Ehrlich Pest Control. Also listed is Amcor Rigid Plastics USA, Inc. and TransVantage Transportation, Inc.

August 22 - San Antonio, TX - 31 year old Victor Diaz died on the job when he fell 20 feet to the floor of a giant smokestack on his first day of work at the Pearl Brewery. His employer, Celestino Cervantes, head roofer, cannot explain why Diaz was at or near the area where he fell. His body was discovered several days later.

August 22 - Peoria, IL - 54 year old Stanley Musgrave Jr., of Norwood was killed on the job when pressurized hydraulic fluid was released, striking Musgrave. The high-powered stream cut off his arm and sliced into his abdomen. He was working for Komatsu America Corp.

August 22 - El Campo, TX - 21 year old Abraham "Fat Boy" Bustamante suffered fatal blunt force head trauma when he was struck in the head by Gin-Poles while working for Pioneer Energy Services, Inc.

August 23 - Philadelphia, PA - 22 year old Keith Amos died of crushing injuries while at work for Trico Lift. He was laying behind the right rear wheel of a flatbed truck when for some reason the truck went into reverse, rolling onto Amos.

August 24 - Durant, OK - 57 year old Sammy Clay Schultz of Kenefic died while at work when an explosion of plasma cutting heads threw metal parts into him. He was employed by Roll Offs USA, Inc.

August 24 - Harvey, LA - 37 year old Wali Muhammad died in an explosion and fire while working for Golden Leaf Energy, Inc. It is reported that he was using an inappropriate tool from an ungrounded cage tote which caused a chemical explosion causing a fire.

August 24 - Philadelphia, PA - 39 year old Joseph Curcio died while working after he fell approximately 114 feet while working for Palmer Waterproofing, Inc.

August 25 - Cocoa, FL - 63 year old Thomas Pesaturo was killed on the job when a 1,000 pound slab of granite fell from a truck and onto him. He was employed by Stone and Surface Designers, Inc.

August 25 - Jonesboro, AR - 34 year old Javier Valverde died of unknown causes while working for David Inman Construction. Reports state he had just finished cutting a joist when he stood up, stiffened and fell 11.5 feet to the ground. Another employer listed is J. Cook Enterprises, Inc.

August 26 - Fort Lauderdale, FL - 31 year old Jorge Carrera-Zarate died of electrocution when he touched a live wire while trimming trees for All Florida Tree & Landscape, Inc.

August 27 - Dover, NJ - 52 year old Wayne Mong of Egg Harbor Township was killed at work when a top heavy forklift he was operating began to overturn. When he attempted to exit the machine, it tipped over, crushing him. He was employed by AWG Painting/National Maintenance & Build Out Design.

August 27 - Medley, FL - 19 year old Alexander Lubian suffered fatal head injuries when he struck his head on the floor after losing his balance on a conveyor and falling approximately 3 feet. He was employed by Forward Air Solutions, Inc.

August 27 - Loyal, OK - 32 year old Martin Melecio-Israel Banuelos Nevarez died of crushing injuries when a 20 foot tall heater fell onto him. He was employed with Southwest Oilfield Construction Company.

August 27 - San Juan, Puerto Rico - 55 year old Tomas Pacheco-Rodriguez was killed while working when he was struck by and became trapped between two trucks while working for Crowley Puerto Rico Services, Inc.

August 27 - Kooskia, ID - 52 year old Vernie R. Sinclair of Weippe suffered fatal blunt force injuries when he was struck by a tree that had been felled by another logger. He was employed by Finke Logging Company, Inc.

August 29 - Honolulu, HI - 74 year old Chang Hwan Kim drowned while working under a floating dock. He was employed with Kuapa Isle Association of Apartment Owners.

August 30 - Thomson, GA - 61 year old Edward Chambers, Jr. suffered massive blunt force trauma when a five-ton overhead crane came off the beams it sat on, falling to the ground and striking Chambers. He was employed by Two State Construction Company, Inc.

August 30 - Rye, NY - 34 year old Ceasar Magana died of fall injuries while working for Werkheiser Roofing, Inc. He apparently disconnected his fall protection harness and began to descend the ladder. His foot got caught and he fell approximately 20 feet to the ground.

August 30 - Watertown, NY - 24 year old Travis E. Fuller was killed on the job when he was struck by a motorist as he was performing his flagman duties. He was employed with Jefferson Concrete Corp.

August 30 - Lithia Springs, GA - 40 year old Calvin Carter suffered fatal crushing injuries when a forklift he was working under fell onto him. He was employed with Toyota Forklifts of Atlanta.

August 31 - Stonewall, TX - 30 year old Billy Walker died on the job when he fell approximately 200 feet. He was employed with Ransor, Inc. No other details are known.

August 31 - Milton, MA - 28 year old Mark MacDougall died of possible electrocution while working for MacDougall Plumbing & Mechanical LLC. No other details are known.

August 31 - Corpus Christi, TX - 67 year old Ricardo Alegria Guzman died after receiving thermal burns while working for Sam Kane Beef Processors, Inc. Details are unknown.

Sept 2 - Lincoln, AL - 22 year old Richard Kenda of Pell City died of crushing injuries when a jack being used on a trailer he was working under failed. He was employed by AJ's Transport, LLC.

Sept 4 - Carnegie, PA - 39 year old Melvin Kinsinger of Greenville died when he fell from an upper roof area to a lower portion of a building he was working on. He was employed by Green Tek Roofing.

Sept 4 - Cut and Shoot, TX - 44 year old Ranulfo Velasquez Perez was killed while working when he was crushed under a falling 2,000 pound flange housing while working for Rangeline Tapping Services, Inc.

Sept 4 - Kyle, TX - 37 year old Christopher Carlos Reyes died of electrocution while standing on a ladder. He was employed with Mullen Electric Co.

Sept 4 - Flushing, NY - 31 year old Ezra Black was fatally stabbed in the neck during a verbal altercation with a co-worker. He was working for the New York Dept. of Parks & Recreation Al Oreter Recreation Center.

Sept 5 - Houston, TX - 54 year old Jaime G. Silva died of blunt force injuries when he was crushed by the bucket of an excavator. His shirt became entangled with the machine's controls while attempting to raise the front windshield which then activated the lowering of the bucket. He was employed with Industrial TX Corp.

Sept 5 - Double Springs, AL - 34 year old Christopher Barton of Haleyville suffered fatal crushing injuries when a truck he was working under fell off a jack. He was employed by Tidco Transportation, Inc.

Sept 6 - Swedesboro, NJ - 36 year old Steven Leonard of Coatesville died while working for Garden State Foundation Waterproofing, Inc. He was inside a permit required confined space spraying waterproofing material when he was overcome.

Sept 6 - Plain City, OH - 24 year old William "Logan" Krebehenne of Richwood died when he was crushed by a bull against a steel gate. He was employed by Select Sires, Inc.

Sept 6 - Gleason, TN - 34 year old Isaac Meeks suffered fatal fall injuries when he fell from a cell phone tower that he was attempting to climb for practice. He was wearing fall protection. He was working for Fuse Tech Wireless, LLC.

Sept 7 - Ringgold, PA - 51 year old Edward Geist of Punxutawney died from injuries sustained in an explosion and fire. As fluid was being pumped out of a tank, there was an explosion and flash fire. Geist was thrown approximately 10 feet and his clothes caught on fire.

Sept 7 - Alvin, TX - 26 year old Ezequiel Martinez died while at work for Ameriwaste, Inc. He was riding a trash truck when it rolled pinning him. The handle on the truck pierced his heart.

Sept 9 - Moselle, MS - 39 year old Ronnie Duffy died on the job when he fell into a screw conveyor while working for Southern Hens, Inc.

Sept 10 - Cedar Grove, WI - 38 year old Lemuel Aguirre of Sheboygan suffered fatal blunt force injuries when he was caught in a shell core molding machine. He was working for Willman Industries, Inc.

Sept 10 - Mt. Vernon, IA - 31 year old Justin Steenhard of Elma was killed at work when the backhoe he was operating rolled off a roadway, pinning him under it. He was employed by Concrete Foundations, Inc.

Sept 10 - Pittston, PA - 38 year old Shailesh Patel of Duryea suffered massive body trauma when he was crushed under a machine when it fell on him at a plastics and paper cup plant. He was employed by Letica Corporation.

Sept 10 - Amarillo, TX - 74 year old Eugene "Gene" D. Henry died of blunt force head and body trauma when he was truck by a steer. He was hit in the ribs with sufficient force to throw him into a fence post where he struck his head. He was working for K & S Parrott, LLC dba Amarillo Livestock Auction.

Sept 10 - San Antonio, TX - 72 year old Eudaldo Martinez died on the job while working for Stagg, LLC. He was mopping a floor when he slipped and fell to the floor.

Sept 10 - Brooklyn, NY - 67 year old Winston Gillette suffered fatal crushing injuries as he worked under a floor of a construction site that was overloaded with masonry materials and collapsed onto him. He was employed by S&B Masonry Corp.

Sept 10 - Wichita, KS - 26 year old Matt Lyons died on the job when he fell approximately 12 feet from an upper level into a pit. He was employed by Spirit Aerosystems, Inc. He died the next day.

Sept 10 - Houston, TX - 22 year old Jamine Perez died of fall injuries when he lost his balance and fell from an elevation. He was working for Pizzitola Guttering Co.

Sept 11 - Watkinsville, GA - 47 year old Mario A. Carranza died when the tractor he was using for mowing overturned on uneven ground. He was working for Southeastern Growers, Inc.

Sept 11 - Las Vegas, NV - 50 year old Cayentano Solis aka Luis Lopez-Solis and Calletano Lopez drowned when a front end loader he was operating was overcome by flood waters which then swept him away. He was employed by Par 3 Landscape & Maintenance.

Sept 12 - Avon, OH - 63 year old Pangelli Makglis died of blunt force trauma when he was struck by a motorist in a road work zone. He was employed by Athos Contracting, LLC and Mosser Construction, Inc.

Sept 12 - Tillamook, OR - 73 year old Robert Reinke of Florence suffered fatal crushing injuries when he was thrown from a roller machine as it rolled down a steep section of road. He was employed by Farwest Portable Crushing, Inc.

Sept 12 - Milan, OH - 67 year old Leon Smith of Norwalk died of blunt force injuries when he was struck by a rolling farm wagon. He was employed with Growers Chemical Corporation dba Growers Mineral Solutions.

Sept 12 - Lynnfield, MA - 48 year old Josais Neves died of fall injuries when a ladder he was standing on shifted against the side of a house causing him to fall from it. He was employed by Yes! Painting dba David Crockett Carpentry.

Sept 13 - Englewood, TN - 37 year old Dale Gourley died at work while working for Nathaniel Wright dba Done Wright. Gourley was working from a personnel platform that was elevated on the forks of a boom, when the machine tipped over.

Sept 14 - Westerville, OH - 61 year old Larry Kostelnik of Blacklick died of crushing injuries when he became caught between two 24 ton steel coils while working for Worthington Cylinder Corporation.

Sept 14 - Jacksonville, FL - 39 year old Corey Baker died of blunt force injuries when he was crushed under the raised hopper of the machine he was operating. He was employed with Econosweep & Maintenance Service, Inc.

Sept 14 - Mercersburg, PA - 46 year old Robert Brian Thomas of Fort Littleton was a paving construction worker when he was fatally struck by a motorist in a work zone. He was employed with Robert D. McCulloh Excavating and Paving, Inc.

Sept 15 - Hebronville, TX - 25 year old Jeffery Ramirez of Laredo died after becoming ill due to heat stress several weeks earlier. He was a Border Patrol Agent for the Dept. of Homeland Security, Customs and Border Protection.

Sept 15 - Dalton, GA - 38 year old Brian Scotty Williams of Chatsworth was found unconscious inside the well of a tanker truck. Details are unknown. He was working for New South Distribution, Inc.

Sept 17 - Menomonie, WI - 35 year old Joseph C. Janisch of Ellsworth died of electrocution when highway equipment he was working with made contact with overhead power lines. He was employed by Highway Technologies, Inc.

Sept 17 - Peshastin, WA - 61 year old Christopher Seelye of Darby, MT was working on a wild fire in Washington state when he fell ill. Exact cause of death is unknown. He was employed with U.S. Timber Cutters.

Sept 18 - Idalou, TX - 21 year old German Murguia of Amarillo died after he was struck by a truck that was transporting a trailer on a construction site. He was struck as it passed by him. He was employed by J. Lee Milligan, Inc.

Sept 18 - Elm Mott, TX - 43 year old Manuel Rivas died of blunt force injuries when he was struck by a semi truck while installing a road sign. He was employed by Williams Brothers Construction Co., Inc. (see towards end of linked article)

Sept 18 - Houston, TX - 52 year old Juvencio Perez died of fall injuries when he fell from a tree branch that he was tied off to. Details are unknown. He was employed by Moss Landscaping, Inc.

Sept 18 - Cotulla, TX - 42 year old Undre Deon Jones died of blunt force injuries when he was struck by a valve and other hardware from a fracking manifold system that he was working on. He was employed by FTS International, Inc. His hometown is unknown.

Sept 19 - Steger, IL - 32 year old Angela Popolla died after suffering an apparent seizure while working at the Bloom Township High School District 20.

Sept 20 - Ardmore, OK - 41 year old Gregory Lornes died while working when he was crushed under the overturned forklift he was operating. He was employed by Atlantic Corporation of Wilmington, Inc.

Sept 21 - Channelview, TX - 41 year old Paulino Garay died of crushing injuries when the trench compactor he was operating caused him to be caught between the roller and a house wall, forcefully pinning him by the machine. He was employed by G.R. Birdwell Construction, LP.

Sept 21 - Soledad, CA - 40 year old Miguel Angeles died at work after suffering from a subdural hematoma (a form of traumatic brain injury) while hand-digging a trench. He was employed with Constellations Wines US.

Sept 21 - Issaquah, WA - Roger Adkins died when the Fed Ex truck he was driving drove off the roadway and crashed into a tree, then catching on fire. A probably medical condition may have caused the crash. He was employed by MJM Trucking, Inc. of Bonney Lake, WA.

Sept 21 - Waldport, OR - 69 year old John Thomas "Tom" Vanderbeek of Kirkland, AZ and Waldport died of blunt force injuries when he was struck by a boat. He was operating a boat in a circle when the boat hit its own wake, ejecting Vanderbeek. The boat came back around, striking him. He was working for Alsea Bay Products.

Sept 22 - Thomasville, GA - 42 year old Brent C. Coleman died when a ladder he was standing on became unstable, touching a power line, electrocuting him. He was employed by Brian Cromer dba Brian Cormer.

Sept 22 - San Francisco, CA - 60 year old U Zaw Aung of Daly City died of blunt force injuries when, for an unknown reason, drove his service truck off the roadway and into a parked private jet at San Francisco International Airport. He was employed by Prime Flight Aviation Services.

Sept 23 - Boys Ranch, TX - 47 year old David Greenlee (hometown unknown) died when he was struck by shrapnel while working for Archer Underbalanced, LLC. He was working with a piece of equipment when its booster exploded, sending shrapnel outward striking Greenlee who was standing in front of the equipment.

Sept 24 - Sugar Hill, GA - 28 year old Miguel Sanchez died on the job when he fell from an unsecured metal deck that shifted, allowing him to fall. His lanyard was not secured. He was employed by Kayco Erector, Inc.

Sept 24 - Rayne, LA - 36 year old Johnny Gammage died while working for SITI, LLC. He was lying on the ground reaching into a storm water drain in order to trowel cement when he was run over by a truck driven by another employee.

Sept 24 - Windsor Locks, CT - 57 year old Joel Dehm of Somers suffered fatal head trauma when a wooden stake on a dump truck broke, causing the tailgate to fall, hitting Dehm. He was employed by Galasso Materials, LLC.

Sept 24 - Minot, ND - 50 year old Scott Staeck of WI died of fall injuries while working for S&S Flooring and Kinney Construction. Supports for stairs had been removed and collapsed with Staeck stepped on them. He fell approximately 21 feet.

Sept 24 - Houston, TX - 31 year old Louis Padilla died of electrocution when he made contact with energized wiring and fell from a ladder. He was working for Justin & Hennifer Solares.

Sept 25 - Pointe a la Hache, LA - 50 year old James Reid died while working when he apparently suffered a heart attack while operating a bulldozer. The dozer then went into a pond and he drowned. He was employed by Holliday Construction Company, LLC.

Sept 25 - Brooklyn, NY - 46 year old Mario Arana suffered fatal fall injuries when he fell approximately 50 feet while attempting to hoist materials to a roof. He was not wearing fall protection. He was employed by New Haven Maintenance, LLC.

Sept 25 - Vestal, NY - 47 year old Darryl Vavra died of fall injuries when he stepped off the edge while walking backward. He was working for Dailey Enterprises, Inc. dba CM Restoration.

Sept 27 - Flora, MS - 51 year old Limmie Tramble suffered fatal fall injuries when he fell from an unknown location, striking his head on the concrete floor. Tramble was employed by Southland Container, Inc.

Sept 27 - Rankin, TX - 30 year old Jimmy Campos died at work while working for Viking Drilling. He was using an ATV to move materials when the load contacted a power line that was strung between a generator and drilling rig.

Sept 28 - Englewood, NJ - 36 year old Javier Santos suffered a fatal head injury while working for Popcorn, Indiana. He had a seizure while operating a forklift. He fell from the machine and struck his head.

Sept 29 - Warren, MI - 41 year old Jason Thorpe of Marysville suffered fatal injuries while working for DiPonio Contracting, LLC. Thorpe had extended a pipe into a roadway when a bus struck the pipe, propelling Thorpe into a crane's outrigger.

Sept 29 - Prattville, AL - 59 year old Antonio Ramirez is reported to have fallen from a scaffold and then suffered a heart attack. He was employed with NBC Acoustics, Inc.

Sept 29 - Chicago, IL - 53 year old Grzegorz Rarzonko died of fall injuries when he fell from a roof having no fall protection in place. He was working for Exterior Building Solutions, Inc.

Sept 30 - Pittsburg, PA - 35 year old Steven Mehaffey died after falling approximately 20 feet while working for Bradley's Roofing & Home Improvement, Inc.

Sept 30 - Pittsburg, PA - 49 year old Robert Evans died while at work when he was found unresponsive next to the open valve of a Sour Gas line that he was tasked to manually drain. He was employed by Shenango Inc.

Oct 2 - Chicago, IL - 58 year old Earlene Lewis-Madison died of an unknown cause/reason when she fell to the floor from her desk while working at the City of Chicago, Dept. of Public Works.

Oct 2 - Granger, IN - 51 year old Daniel Miller died when he was struck by a motorist while working at the back of his truck cleaning weeds. He was employed by ITR Concession Company, LLC where he was a maintenance employee.

Oct 2 - Arbuckle, CA - 36 year old Jaime Paez Gomez was killed when the farm wagon he was operating pulled into a public traffic lane and was struck by a 19 year old motorist, who also died in the crash. Gomez was employed by Tom Charter & Perry Charter dba T&P Farms.

Oct 2 - Seattle, WA - 55 year old Lee Morris, a Captain Pilot for Alaska Airlines, was the victim of a possible homicide in Los Angeles, CA. Police were notified when he did not report for a flight and his body was found near a freeway off-ramp. No further details are given.

Oct 3 - Salinas, Puerto Rico - 32 year old Edwin Lopez Menendez died of electrocution when he touched a high voltage power line while attempting to remove a cable from a truck. He was employed with Consolidated Waste Corp.

Oct 4 - Bedford, IN - 60 year old Kerry Meadows was found unconscious/unresponsive after working on the ground with another lift. He was a journeyman lineman for Duke Energy Indiana, Inc.

Oct 5 - Vancouver, WA - 82 year old Prebble Dudley died November 2, 2012 after she was injured on Oct. 5th while working at Crossroads Community Church, Inc. She apparently tripped on a blanket or pillow while working with children, hitting her head on the floor. She did not immediately seek medical attention until Oct. 9.

Oct 5 - Charlotte, NC - 38 year old James Halstead of Ocean Springs, MS, drowned while attempting to retrieve his keys that fell under a dock. He worked as a white water rafting tour guide for U.S. National Whitewater Center.

Oct 9 - Spokane, WA - 50 year old Michael E. Farrington died of natural causes while performing his normal work duties on a roof with a co-worker. He was on his first day of work for Holliday Enterprises, Inc.

Oct 9 - Indianapolis, IN - 38 year old Christopher Buster died of a vehicle crash when he lost control of a street sweeper which flipped and ejected him. He was employed with Robert Pitman Jr. dba Envirosweep.

Oct 10 - Virginia Beach, VA - 60 year old James Price died of an unknown cause while observing panel readings on the 3rd floor of the HRSD-Chesapeake-Elizabeth plant (Virginia Beach plant).

Oct 10 - Bennettsville, SC - 44 year old Jeffery P. Cheek of Rock Hill was electrocuted while working on a back-up generator at Marlboro County High School. Wires had been installed improperly by another company. He was employed by Nixon Power Services.

Oct 12 - Prather, CA - 57 year old Dennis Cowley was working as a handyman for a homeowner when he was killed in an 8 foot trench collapse. He was working for Fred Dow.

Oct 13 - Wolcottville, IN - 56 year old Terry "Chopper" Keck died of blunt force trauma while working on a semi-truck that was on a jack. The semi was struck which caused Keck to be pinned between the tractor and cement floor. He was employed by Lakeside Transport, LLC.

Oct 18 - Elm Mott, TX - 42 year old Prek Gjokaj of Sterling Heights, MI died in a fiery crash when a semi truck in front of Gjokaj slowed for road construction and he drove into the back of that semi, causing his truck to burst into flames. It is unclear who Gjokaj worked for.

Oct 23 - Alameda, CA - 38 year old Corwin Hardham died of apparent natural causes while in his office of Makani Power, Inc., a company he co-founded.

Oct 25 - Hillsboro, OR - 55 year old David Huttula of West Linn, died after falling approximately 16 feet from an extension ladder while installing new siding on a home. He was employed with Fairway Construction, LLC.

Oct 25 - Dallas, TX - 42 year old Josh McGowan was killed when he was struck by a construction truck that was pushed into him after it was struck by a tractor-trailer rig. He was working for Brooklyn Sweeper, a highway clean up company.

Oct 29 - Easton, CT - 54 year old Russell F. Neary died in the line of duty when he was struck by a falling tree while cleaning debris after Hurricane Sandy. He was President of the Easton Volunteer Fire Company.

Oct 30 - Eugene, OR - 60 year old Richard Wigney died of head trauma when he fell from a short ladder, striking his head, while working for Rainbow Valley Design & Construction. Specifics are unknown.

NOVEMBER 2012

Nov 2 - Portola, CA - 55 year old Jose Noriega, possibly of Anaheim, drowned when an excavator he was operating tipped and rolled into Lake Davis. He was employed with Creative Home Inc., dba Creative Home Interiors (CHI) Construction.

Nov 6 - Fresno, CA - 32 year old Salvador Diaz and 34 year old Manuel Verdin were murdered by a co-worker who went on a shooting rampage at Valley Protein where they worked. Two other co-workers were also shot, but survived, the shooting by a recently discharged parolee who worked at the plant a little over a year. It appears the shooter had no reason to open fire on his fellow workers, before taking his own life.

Nov 7 - Madera, CA - 25 year old James S. Chavala of Bakersfield, was struck and killed by a motorist when he was attempting to cross a roadway to continue his work. He was employed by Pavletich Electric & Communications.

Nov 13 - Newton, IA - 71 year old Clarence "Hoot" De Hoedt died of head and neck injuries when he fell from the back of a flat bed trailer while helping to unload it at a construction site. He was a driver for Bisom Trucking.

Nov 14 - Mooresville, NC - 59 year old Frank Carl Booth was working as a crossing guard, directing traffic for Iredell-Statesville Schools, when he was struck by a motorist.

Nov 16 - Harrisburg, IL - 53 year old Captain David Underwood died of presumed natural causes after being found on a restroom floor of the Saline County Sheriff's Department. He was the jail administrator.

Nov 16 - Indianapolis, IN - 45 year old Anthony Gerard Hicks was shot to death by another employee in the break room of Conagra Foods Packaged Foods. No further details are known.

Nov 19 - Jamul, CA - 69 year old Mateo Murillo died of blunt force head and chest trauma when he was crushed under a felled tree. He was working for Altanil Corp., dba Rancho El Cielo (a private ranch).

Nov 21 - Fresno, CA - 40 year old Arturo Ortiz suffered a fatal injury to his upper chest when a car fell off a jack stand and onto him. He was self employed with Arturo's General Auto Repair.

Nov 28 - La Habra, CA - 58 year old David Gonzales died of natural causes according to authorities. He was working in a CVS Pharmacy store.

Nov 30 - Redmond, WA - 50 year old Michael Little died of blunt force injuries due to a 35 foot fall from a roof. He was employed by Unique Roofing Concepts Co.

Dec 1 - Burns Harbor, IN - 35 year old Jason C. Scott died of an unknown cause while working for Arcelormittal Burns Harbor Division. He was found unresponsive, laying on the locker room floor.

Dec 3 - Salinas, Puerto Rico - 67 year old Alfredo Cartagena died while working when he was hit and run over by an operator of a loader machine who didn't notice him. He was working for Empresas Diaz Betterecycling.

Dec 4 - Newton, IA - 63 year old James Harder of Omaha, suffered crushing injuries when he was trapped between a semi truck and container trailer while working for TSL Transportation, Inc. The incident happened at Jacobson Warehouse.

Dec 11 - Moses Lake, WA - 31 year old Michael Huntley suffered fatal head trauma when he was crushed between a paint cart and mast tube while working for Genie Industries, Inc.

Dec 11 - Davenport, IA - 57 year old Thomas McLachlan was engaged in grading work on a hill when his dozer rolled near an edge, rolled and pinned him to the ground. He was employed by Hawkeye Paving Corporation.

Dec 12 - Tacoma, WA - 64 year old Roger Warren of Orting died when the tractor trailer rig he was driving traversed a barrier and rolled over. He was employed with Nichols Trucking Co, Inc. of Tacoma.

Dec 13 - Seattle, WA - Andrew Charleston of Tacoma died while working when he fell from an elevated work platform called a Stacker. He was priming an airplane for painting. He was an employee for Boeing Services, Inc.

Dec 31 - Fort Wayne, IN - 58 year old J. Robert Shimer II was beaten to death while working inside his booth at the Antiques on Broadway shop by someone attempting to rob the store.

January 3 - Marysville, WA - 67 year old Gary Anderson of Monroe died of natural causes when he collapsed while working at a warehouse for Whitley Evergreen, Inc.

January 4 - Mount Vernon, WA - 32 year old Michael Rongey from Maple Valley, WA died when he fell approximately 80 feet from a cell tower. He was wearing fall protection but there was a failure at the anchor point. He was employed by W.S. Consulting & Construction, Inc.

January 10 - Tacoma, WA - Lane Loop died while working at the Tacoma Dome when he was seen falling down several rows of seats. Authorities believe he died of a possible heart attack.

January 11 - Connell, WA - 32 year old Yan Garayquipialaya was killed when he was struck from behind while riding an off-road vehicle on Highway 260. He was employed with C & L Lockers, a sheep/goat farming business. Washington State Patrol is investigating.

January 16 - Arlington, WA - 58 year old Lamont Killian of Concrete, WA was found deceased inside his assigned vehicle when he could not be located to start his shift. It appears his death was from natural causes. He was employed with Smokey Point Distributing, Inc.

January 28 - Everett, WA - 48 year old John Satko of Edmonds collapsed while preparing food while working as a chef for Wapo Investments LLC dba Holiday Inn Hotel, Everett.

February 1 - Pasco, WA - 38 year old Brent Peterson died of natural causes while delivery of newspapers on his route. He was discovered inside his vehicle that was stopped just off the edge of the roadway. He was employed by McClatchy Newspapers, Inc. dba Tri City Herald.

MARCH 2013

March 1 - Spokane, WA - 40 year old Max Talley of Medical Lake died of asphyxia when toxic fumes built up inside a trailer he was sleeping in. He did fashion a ventilation system but it failed. He was returning from an auction after making a purchase. He was employed by GB Auctions, Inc. dba DAA Northwest.

March 6 - Silverdale, WA - Paul Stewart was found unresponsive at his desk and died of a probably heart attack. He was Volunteer Coordinator Training Officer for Central Kitsap Fire & Rescue.

March 12 - Tacoma, WA - 46 year old Jeff Surber of Bonney Lake, WA was killed on the job when he was crushed between parts of a Container Gantry Crane. He was working at Pierce County Terminal for Pacific Crane Maintenance Company.

March 25 - Tacoma, WA - 57 year old Dana Gorham of East Spanaway died of natural causes while working on reefer units of a shipping container. He was employed by Pacific Crane Maintenance Co.

September 5, 2012 - Grayson, KY - 47 year old Dru Gay of Olive Hill was killed on the job when he suffered an apparent electric shock while working for Smithfield Foods in Grayson.

October 27, 2012 - Buffalo Lake, MN - 28 year old Andrew "Brooski" Burnett of New Ulm was killed on the job when he fell approximately 60 feet from a grain elevator. He was employed by Valley View Electric, a sub-contractor to the grain elevator

November 23, 2012 - Fairhope, AL - 47 year old Scott Ward was shot to death while on duty as a deputy for Baldwin County Sheriff. Ward responded to a call and when he arrived at a trailer, the man inside opened fire on him.

November 27, 2012 - Sarasota Springs, UT - 31 year old Nicholas Blatnick was killed on the job while working on a construction project as a subcontractor for Questar Gas. His crew was laying new asphalt on top of a new gas line when he was pulled into an asphalt zipper. It is unclear who he worked for.

November 28, 2012 - Las Vegas, NV - 26 year old Michael Angelo Garcia was killed on the job while working along Interstate 15. He was helping to load heavy guardrails onto a truck when they fell onto him, crushing him. He was a sub contracted employee for Nevada Dept. of Transportation. It is unclear who his employer was.

November 29, 2012 - Hope, AR - 40 year old Tracey Tyrone Harris died at work when he became pinned between a gate/railings and a cow. He was working for Hope Livestock Auction.

November 30, 2012 - New Bedford, MA - 55 year old Carlos Tabares was killed on the job when he was pinned by a Bobcat bucket when the hydraulics failed. He was employed by the city of New Bedford.

November 30, 2012 - Delphi, IN - 30 year old Arthur Kerns died on the job when an excavation trench he was standing in collapsed around him. He was employed by T.J. Lambrecht Construction doing work on a Hoosier Heartland Highway bridge.

November 30, 2012 - Pocahontas, WV - 27 year old Steven O'Dell of Mt. Nebo, was killed on the job while working for Alex Energy's Pocahontas Mine. He died when he became pinned between a scoop and a continuous miner (equipment).

December 2, 2012 - Santa Cruz Island, CA - 34 year old Terrell Horne III, of Redondo Beach and a Coast Guard Chief Petty Officer, died of head trauma when his inflatable boat was rammed by suspected smugglers near Santa Cruz Island.

December 3, 2012 - Lexington, SC - 21 year old Nicholas Johnson was killed on the job when he was struck by a motorist. He was employed with SCDOT and was struck when he was walking along a section of Broad River Bridge when he was struck by a drunk motorist.

December 3, 2012 - Memphis, TN - 50 year old Daniel Tittle died on the job and another worker was injured when the sight glass on a piece of equipment ruptured, causing a release of hazardous chemicals inside the Valero Refinery in Memphis. He was employed by Valero.

December 4, 2012 - Bronx, NY - 35 year old Michael Mananghaya was killed instantly while working for Agrekko, an energy serving company, at Bronx Lebanon Hospital after a chain on a crane snapped which sent an air conditioning unit crashing onto Mananghaya. The a/c unit was temporary and was being removed from the hospital.

December 5, 2012 - Lower Providence, PA - 31 year old Jason Schmidt of Jacobus was killed on the job and a co-worker injured while working at an Audubon Water water tower for Corrosion Control Corporation. An equipment malfunction occurred and Schmidt fell to the bottom of the empty water tank.

December 10, 2012 - Rochelle, IL - 65 year old Andy Olesen, pilot; 40 year old Jim Dillow, RN and 48 year old Karen Hollis, RN, were all killed from a helicopter crash while flying between two Illinois hospitals. All three worked for Rockford Memorial Hospital. (photos in link)

December 11 - Kansas City, MO - 33 year old Kevin Edmondson died on the job when he was struck by a semi truck due to miscommunication between the driver and spotter. He was working at the YRC Freight operation.

December 11, 2012 - Augusta, ME - 64 year old John Pearl died on the job when he apparently suffered a medical condition, causing his state vehicle to strike a pole. He was employed by Department of Administrative and Financial Services Bureau of General Services.

December 11, 2012 - Kiryas Joel, NY - Anastacio Vasquez of Spring Valley was killed on the job when he was struck by a hit & run motorist while picking up trash for Kiryas Joel Sanitation.

December 14, 2012 - Memphis, TN - Martovia Lang was killed on the job and her partner injured, when she was shot to death in an exchange of gunfire with a 21 year old suspect when attempting to serve a drug warrant. She was an officer for the Memphis Police Department.

December 15, 2012 - Mineral Point, MO - 31 year old Deputy Christopher Parsons was killed on the job when he was shot by a man who ambushed him. The Deputy had just helped the local EMS load a patient into an ambulance when the suspect came up and shot Parsons. Parsons worked for the Washington County Sheriff's Office.

December 16, 2012 - Topeka, KS - 50 year old Cpl. David Gogian and 29 year old Officer Jeff Atherly were shot to death by an armed suspect while investigating possible drug activity in a grocery parking lot. The suspect was later shot to death by police.

December 18, 2012 - Fort Payne, AL - 57 year old Glen Scott was killed on the job when he fell after a scaffold collapsed. He was working at the construction site for the new Crossville Elementary School.

December 24, 2012 - Hampton, NH - 32 year old Merle R. Wilbur IV died on the job when he fell to the ground from a tree he was working in and a branch fell onto him. He was employed by Plante's Tree Removal Take-down.

December 24, 2012 - Webster, NY - 43 year old Lt. Michael Chiapperini and 19 year old Tomasz Kaczowka were shot to death when they responded to a house fire. The occupant of the house, and responsible for setting the fire, opened fire onto the men responding. Chiapperini was a volunteer firefighter and police office and Kaczowka was a 911 dispatcher.

December 24, 2012 - Wauwatosa, WI - 30 year old Jennifer Lynn Sebena, an officer for the Wauwatosa Police Department, was found shot to death outside of a fire station after other officers could not get a response from her. Officers later arrested her husband for her murder. He had been stalking her for days.

December 27, 2012 - Westfield, NY - 49 year old Billy Johnson II of Crown City, OH was killed on the job and a co-worker injured, when he was struck by a motorist. Johnson and his co-worker were standing next to their work truck when the motorist drifted off the road during a storm and struck the men. Both worked for CTS Telecommunications of Orville, OH.

December 22, 2012 - Rushville, IN - 30 year old Eric W. McKinney was found dead at his place of employment, Intat Precision, Inc. He was first reported missing when he did not come home after work. Upon a search of the facility, he was found. His death involved machinery but no other information is found.

December 27, 2012 - Alexandria, MN - 50 year old Tim Anderson of Evansville was killed on the job while working for Pope/Douglas Solid Waste. He was crushed by a piece of machinery.

December 28, 2012 - Rockford, IL - 60 year old Terry L. Fogel suffered a fatal heart attack when he was returning to the Rockford, IL DOT maintenance facility. He was a highway maintainer.

December 30, 2012 - Hermiston, OR - 69 year old Freeman Thomason of Post Falls, ID was killed on the job when the vehicle he was a passenger in went out of control in icy road conditions and rolled three times. Thomason suffered fatal head injuries. He and the driver were employed with WalMart Associates, Inc.

January 2, 2013 -Gage Park, IL - 57 year old Michael Kozel was shot to death during a robbery at his muffler shop. He owner the Independent Muffler Shop for two decades.

January 2, 2013 - Charlotte, NC - 31 year old Anthony Crowder died on the job when he was electrocuted while performing demolition work. He was employed by Arc Electric Company of Indiana.

January 2, 2013 - Jacksboro, TN - 40 year old Richard L. Haga died on the job from fall injuries when he fell from a mobile scaffold to the concrete floor. He was employed by Huling Construction and Erections, Inc.

January 3, 2013 - Indianapolis, IN - 57 year old Michael J. Higgs died of natural causes while working for United Parcel Service. The particulars of how he died are not listed.

January 3, 2013 - Mason City, IA - A medical helicopter crash near Mason City killed three people. They were on their way to another location to pick up a patient. Those killed in the incident were:
44 year old Shelly Lair-Langenbau of Hanlontown. She was a nurse for Mercy Medical Center North Iowa. 48 year old Russell Phiel of Forest City was a paramedic, and 53 year old Gene Grell, home town unknown, worked for Med Trans Air Medical Transport.

January 3, 2013 - Columbia Tusculum, OH - 20 year old Edward Griffin of Hamilton was killed on the job when he fell from a height while working on a Columbia Tusculum construction site. It is not clear who he was employed by.

January 4, 2013 - Hebron, KY - 67 year old Jessie "Tom" Lane of Burlington died on the job when he fell approximately 13 feet while installing stairs from a 2nd story landing. He was kneeling and fell head first to the ground. He was self employed with Tom Lane Contracting.

January 4, 2013 - Asheville, NC - 64 year old Phillip Allen Pace was killed on the job while working for American Cable & Rigging Suppliers. He was using a core drilling machine to drill holes in precast concrete when the machine malfunctioned, causing the machine to fall off an 8 foot wall.

January 7, 2013 - Smithville, TX - 38 year old Thomas Blain Stewart of Buffalo Gap, was killed on the job when a tank being loaded onto a truck slid off, striking him. He was an employee with Smith Pipe Co.

January 8, 2013 - Memphis, TN - 69 year old Gaylon Karnes was killed on the job when he was struck by and run over by a John Deere front end loader after he had exited his truck. He was waiting for his truck to be loaded when he was struck. He was employed with Johnston Trucking, Inc.

January 9, 2013 - North East, MD - 42 year old Luis E. Tadeo of Bowie, MD and 41 year old Victor D. Jackson of Washington DC were killed on the job when they were struck by a motorist while working in a highway work zone that was clearly marked with orange cones. The men worked for Pessoa Construction Company.

January 9, 2013 - Kokomo, IN - 54 year old Rex D. Markham died on the job of cardiac causes after he collapsed and hit his head on a piece of office equipment. He worked in the Advance Manufacturing Engineers building of Chrysler Group.

January 9, 2013 - Bridgeland, UT - 62 year old David Broome of Vernal was killed on the job while working for RJ Taylor-Wyatt LLC. He was using a pipe bending machine and it began to slip on an icy surface. He tried to prevent the machine from moving but it fell into a trench, as did Broome, and he was struck on the head by the pipe.

January 10, 2013 - Wayne, NJ - 35 year old Heidi Cullere of Paterson was killed on the job when she was struck by a raised, asphalt filled, dump-truck bed that malfunctioned and collapsed. She was employed by H. Williams Paving in Wayne.

January 17, 2013 - North Charleston, SC - 71 year old Howell Jeffcoat died of a coronary blockage after he suffered trauma while at work for County of Orangeburg Public Works. He was working in a small hut at a recycling facility when a strong storm with rain blew open the door and as it was slamming shut it struck Mr. Jeffcoat. He was knocked out of the door, suffered broken bones and was discovered on the ground by citizens. He was taken to the hospital where he died.

January 19, 2013 - Chester, VA - 29 year old Jeffrey Lockhart, Jr. suffered a possible heart attack while working for Integrity Staffing Solutions when he was found on the floor unresponsive. There was no sign of injury to Mr. Lockart.

January 21, 2013 - Chicago, IL - 57 year old Theresa Burns was killed on the job, and a co-worker injured, when they were struck by a hit and run motorist while attempting to remove a student from the roadway. Burns, who was employed by Northern Illinois Academy, went after an unstable teen who sat down in the middle of the road. When she and a co-worker attempted to remove the student, they were struck.

January 21, 2013 - Markham, IL - 55 year old Norman Wallace died of a probably heart attack while working for the Illinois Department of Transportation. He was feeling sick and laid his head on his desk. He was later found unresponsive.

January 21, 2013 - Apex, NV - 54 year old Kenneth Kurakis of North Las Vegas was killed in a mining incident while working at the Lhoist North America quarry. It is unclear how he died.

January 22, 2013 - Pittsburg, PA - 59 year old Harold E. Taylor, Jr. was killed on the job when he was electrocuted while working at Carnegie Mellon University for Eaton Corporation.

January 22, 2013 - Cawtaba, SC - 39 year old Samir Storey died, and his co-worker injured, when they were overcome by noxious fumes while cleaning inside a tank at Resolute Forest Products. The co-worker, Storey's cousin, said he questioned the safety training they received before performing the work. They were contract workers for Tradesmen International.

January 22, 2013 - San Francisco, CA - 45 year old Youjun Wang of Milbrae was killed on the job when he was pinned between the limousine he had been driving and another vehicle. The limo was apparently not in park and began to roll. Wang was pinned as he was attempting to stop the vehicle. He was employed with Sherwin Limo Service, Inc.

January 23, 2013 - Canton, GA - 21 year old Josh Roberts of Woodstock was killed on the job when the upright mast of a forklift he was working on collapsed onto him. He was employed by Mauldin's Body Shop at Little River Marina.

January 25, 2013 - Memphis, TN - 49 year old Hong Dao died on the job while working for Quality Trailer Services, LLC. He was a mechanic changing a tire of a car on a trailer. The truck the trailer was attached to somehow moved and Dao was run over by the tires of the trailer.

January 27, 2013 - Smyrna, TN - 50 year old Martin O'Connell of Murfreesboro was killed on the job when he was pinned by his truck while working at the Nissan Plant in Smyrna.

January 29, 2013 - Fort Wayne, IN - 61 year old Leroy J. Hammond of New Haven was killed on the job when he fell through an unguarded opening and fell approximately 7 feet to the ground. He was employed with Rea Magnet Wire.

January 31, 2013 - Renville, MN - 30 year old Erv Knuston of Sacred Heart was killed on the job when he was struck and run over by a payloader while working for Southern Minnesota Beet Sugar Cooperative.

February 2, 2013 - Malibu, CA - 45 year old Ronald Carver of Newbury was struck and killed by a drunk motorist as he was he hooking up a disabled car to his tow truck. The driver who struck Carver was arrested on felony hit and run plus drunk driving.

Feb 4 - Stoddard, NH - 39 year old Shannon Sarcione died on the job when he was fatally struck by a falling tree while working with others to cut trees. His employer is unclear.

Feb 12 - Chowchilla, CA - 69 year old Natividad Castro died of a possible heart attack while installing wiring in a grape vineyard in Chowchilla. He was employed by Westside Ag, Inc.

Feb 15 - Flemington, WV - Brian Hopkins was killed on the job when a natural gas well pad he was working on exploded at EQT Corp. He was employed by Central Environmental Service of Washington, WV.

Feb 16 - Fort Wayne, IN - 55 year old Frederick Biancardi died of a probably heart attack while working to clean restrooms for All American Foods. He was found unresponsive by a co-worker.

Feb 21 - Coamo, Puerto Rico - 50 year old Hernan Reyes Cordero died when he fell 14 feet down a portable set of stairs to the concrete floor below. He was self employed.

Feb 23 - Lincolnton, NC - 35 year old Marendonio Francisco Viruel Lopez was killed on the job when he was caught in an angular saw while working for RSI Home Products, Inc.

March 4, 2013 - Greenville, NC - 58 year old David Harris was found unresponsive and died of natural causes while he was cleaning a carding machine at Mohawk Industries, Inc.

March 7 - Phoenix, AZ - 67 year old Ronald Koons died on the job when he fell approximately 15 feet from a fixed ladder. He was carrying empty buckets when he lost his balance and his grip on the ladder. He was employed by Roof USA Service Center.

March 8 - Jericho, NY - 60 year old George A. Turner, Jr. suffered a fatal heart attack while responding to a vehicle crash. He was employed with Jericho Fire Department Headquarters.

March 8 - Indianapolis, IN - 49 year old Jerry Gipson died on the job of natural causes. He was found at his desk, not breathing, by a co-worker. He was working for Compucom.

March 14 - Pacific Palisades, CA - 50 year old Gilbert Vargas died on the job, and a co-worker injured, when he was was buried in a trench collapse. Both men were employed by Los Angeles Engineering, Inc.

March 16 - Murfreesboro, TN - John Boyd of Rockvale was killed on the job when the rock truck he was driving overturned off a 60 foot embankment. He was employed by Hoover Rock Quarry of Murfreesboro.

March 18 - Logonier, IN - 55 year old Mark A. Werbiansky died on the job when he was electrocuted whole working alone on a scissor lift 8 foot off the floor. He was installing a light fixture when he touched a metal part. He was employed by Parker Integrated Sealing Systems.

March 18 - Stanley, ND - 31 year old Joshua Hoiland of Minot was killed on the job while working at an oil field drilling site for First Choice Energy Services. He was cleaning a truck when he was struck by a piece of equipment.

March 21 - Indianapolis, IN - 65 year old Richard Lane died on unknown circumstances while employed by Harriman Investment Corporation.

March 22 - North Charleston, SC - 38 year old David Priester died on the job when he was fell, striking his head, from a mobile platform while working for Boeing Services.

March 22 - Newport News, VA - 76 year old Jerry Prescott died while on the job when he fell approximately 8 feet from a roof. It is unclear if he suffered a heart attack or fell, but he was not wearing fall protection. He was employed by CEC, Inc.

March 23 - Hopkinton, CT - 41 year old Charles Austin, a Verizon lineman, was electrocuted while working from a bucket for National Grid.

March 25 - Kingsville, MD - 57 year old Philip Wayne Higgins died on the job when he was struck by the truck he was using for snow removal in a parking lot of a county owned building. He was employed Baltimore Country Property Management Division.

March 31 - Russellville, AR - 24 year old Wade Walters was killed and eight others injured when a 500 piece of equipment fell on top of him when a temporary lift device failed. He was employed by Precision Surveillance Corp. working at Entergy Arkansas, operator of the Nuclear One plant.

April 1, 2013 - Mesa, AZ - 51 year old John C. Stephens was included on an official worker death list as being found dead on the premises of Crackalackin, LLC. His manner of death, however, is not listed and an online search was unsuccessful.

April 3 - Mankato, MN - 83 year old Reuben Reimers suffered a fatal head injury as he was carrying a projector down a staircase while facilitating a funeral at a church in Mankato. He was employed by Samuelson Funeral Services, Inc.

April 8 - Indianapolis, IN - 45 year old Thomas Welch died on the job when the garbage truck he was riding on dipped into a hole, bumping Welch off the truck. He was employed by the Indianapolis Department of Public Works.

April 10 - Burns Harbor, IN - 62 year old Andrew Turnbull of Valparaiso suffered a fatal heart attack while he was working at Arcelor Mittal Burns Harbor.

April 12 - Big Cottonwood Canyon, UT - 34 year old Craig Patterson of Park City was killed on the job when he was buried under an avalanche while on duty as a back country skier for the Utah DOT.

April 16 - Towson, MD - 39 year old Noe Rodriquez Alverto of Middle River was killed on the job when he was struck by a motorist while performing cleanup duties with a backpack style blower. He was wearing all protective gear to be seen. He was employed by Akehurst Landscaping, Inc.

April 19 - Milton, WI - 27 year old Jerod Guell was killed on the job when he was buried under thousands of bushels of corn as he tried to clear clogged chutes at the bottom of a grain bin. He was employed by United Ethanol's Milton plant.

April 22 - Columbia, TN - 48 year old David Haynes was killed on the job when the John Deere zero turn mower he was operating on an embankment overturned onto him. He was found by a passerby. He was an employee for Maury County Building Maintenance.

April 23 - Long Beach, CA - 60 year old Vincent Martinez of Downey was found dead in a water canal intake at the Haynes Generating Station in Long Beach. He was employed with Los Angeles Department of Water and Power. His death is currently under investigation.

April 24 - Las Vegas, NV - 60 year old William Paul Little died on the job when he was crushed under a large drainage pipe that was being installed on a road construction project. It is unclear who Little worked for but other sources say he was a truck driver.

April 25 - Talleyville, DE - 60 year old Luis Benjamin Sanchez of Upper Darby, PA died while at work when he was struck by a steel cable that was shot out of a chipper machine as it went through it. Another worker was also struck. It is unclear who the men worked for.

April 26 - Salinas, CA - 30 year old Jose Juan Serrano was killed on the job when he was struck by a counter weight while covering a pallet of strawberries. He was working at Grower's Street Cooling.

April 26 - West Bottoms, MO - 68 year old William A. Prater Kansas City died on the job when he was crushed in a building collapse that was under demolition. It is unclear who Prater worked for but two of his son's also worked there and were on site.

April 26 - Clakamas, OR - 41 year old Hugo Avalos-Chanon of Portland died on the job while working for DCS Sanitation Management. Avalos-Chanon and several other workers were cleaning at Interstate Meat Distributors when he somehow fell into a running machine and became entangled in it.

March 4, 2013 - Springfield, MO - 35 year old Jason Taylor was killed on the job when he was struck by an earth mover while working for All Metal Recycling.

April 15, 2013 - Waco, KY - 39 year old Kevin Turpin was killed on the job in a tree cutting incident. It appears he was working for his brother Matthew's company, name unknown.

April 22, 2013 - Kettle Falls, WA - 48 year old Carl Barnfield, of Grand Forks, British Columbia, died while working on the job for DCT Chambers Trucking Ltd in Kettle Falls. He was struck and run over by a vehicle while attempting to disconnect a semi from its trailer for unloading purposes.

April 23, 2013 - SeaTac, WA - Ayenachew Taddese was killed on the job after fell from and was run over by a baggage tug he had been operating. He was employed by Primeflight Aviation Services, Inc.

April 29, 2013 - Yakima, WA - 25 year old Jose Alcazar Ochoa of Cowiche was killed on the job when he fell from a tractor and was run over by it. He was employed by Sundquist Fruit & Cold Storage, Inc.

May 1, 2013 - Albuquerque, NM - 47 year old Glenn "Bo" Martin was found dead on the job while working for Albuquerque Bernalillo County Water Utility Authority. Authorities say an apparent vehicle mishap occurred while performing routine maintenance on a waste water lift station.

May 2, 2013 - Jefferson City, MO - 51 year old Alan J. Spaulding was killed on the job when the grain truck he was driving was sideswiped by another truck, sending his truck over an embankment and over turning. His employer is not named.

May 3, 2013 - Brooklyn, NY - 40 year old Inunion Tursunov was killed from a fatal fall while working at the Brooklyn Navy Yard. The elevator he was riding in stopped mid-ride and although rescuers were en route to free him, he pried the door open and apparently lost his footing and fell in the dark shaft.

May 4, 2013 - Canton, OH - 50 year old Mark R. Tovissi of Magnolia and 38 year old Brian Black of Clinton were killed on the job while working at Timkim's Faircrest plant. The men were preparing to lift a load with a crane when the cable snapped, causing the load and boom to fall onto the men. They were employed with Beaver Excavating.

May 5, 2013 - Fayetteville, AR - 20 year old Brannon Rhine of Springdale died on the job due to a trench collapse. He was working at a Parkcrest Builder construction site for the Vue Apartments in Fayetteville when equipment struck a sewer line. He jumped in to clear debris and the hole collapsed around him. It is not clear who he was employed by.

May 6, 2013 - Magalia, CA - 43 year old Abraham Cuevas of Live Oak was killed on the job when he was struck by a piece of a falling tree. He was in the air, using a safety harness when part of the tree fell. It is unclear who his employer was.

May 6, 2013 - Karnes Co, TX - 25 year old James Dean "Jimmy" Burnett of Goliad suffered fatal head injuries while working on a BHP Billiton Petroleum construction site in Eagle Ford Shale. He died when a large pipe fell, striking him on the head and neck. He was an employee for Jet Maintenance.

May 7, 2013 - Jacksonville, TX - 42 year old James Anthony Lucas died on the job when he fell inside a silo-type structure, hitting his head. He was employed by Longview Asphalt, Inc.

May 8, 2013 - Myton, UT - 28 year old Tyson Boren of Neola, UT was killed on the job when an oil tank exploded during routine maintenance at the Newfield Exploration Company's wellsite near Myton. Two other workers were injured in the explosion. Boren was a welder for S. Boren Services contracted to Newfield.

May 8, 2013 - Rothbury, MI - 43 year old Kevin W. Dilts of Muskegon was killed on the job when he fell 30 feet to the ground. He was employed with Shoreline Metal Fabricators and was taking measurements at Rothbury Steel plant when he tripped and fell through a skylight.

May 9, 2013 - Fayetteville, AR - 44 year old Jess Wilson of Jolin, AR was fatally electrocuted when his head touched a high voltage power line while working at Parkcrest Builders construction site of the Vue Apartments. His employer is not named.

May 10, 2013 - Chattanooga, TN - 61 year old Rick Beck suffered fatal head trauma while working when the lower half of a silo he was attempting to attach to the upper portion shifted, striking him in the head. He was an employee for Astec.

May 11, 2013 - North Charleston, SC - 53 year old Kenneth Drayton of Hollywood, SC was killed on the job of traumatic asphyxia when he was buried inside a trench/hole that had been excavated. He was a temporary worker and his employer is not listed.

May 13, 2013 - Miami, FL - 29 year old Corence Lawrence Gumbs died on the job when he fell from the seventh floor of a building at the BrickellHouse construction site. It is unclear if he worked for BrickellHouse or was a subcontractor.

May 13, 2013 - Lapaz, IN - 61 year old Rick Pertic died on the job when his clothing became entangled in irrigation equipment as he was tending to his crop of blueberries during cold weather. He was owner of Pertic's Blueberry Plantation.

May 15, 2013 - Columbus, MS - 24 year old James Meyers died on the job while working on a runway project at the Columbus Air Force Base. He was cleaning a finishing concrete machine when his vest became caught and he was pulled into the machine and crushed. He was employed by RC Construction Co.

May 15, 2013 - Florissant, MO - 55 year old Mach V. Nauyen died of probably electrocution when he was found slumped over an electrical box, unconscious and not breathing. He was employed by St. Louis Cold Drawn Inc.

May 15, 2013 - Baggs, WY - 32 year old Don "Carl" Jordan of Worland, WY died on the job when he was struck by equipment on a drilling rig. It appears he worked for Winchester Well Service, Inc.

May 16, 2013 - Plainfield, WI - 26 year old Robert Losey of Adams was killed on the job when he apparently fell from a semi truck he was working on and ended up under the trailer of another semi truck that was backing out next to him. He was working at an unnamed potato farm.

May 16, 2013 - Dyer, IL - 57 year old Michael Kerr was killed on the job when a crane knocked a sixth floor beam out of place causing it to strike Kerr in the head and chest. He was employed by Power Construction Co., LLC.

May 17, 2013 - Moundsville, WV - an unidentified sanitation worker for the city of Moundsville was killed on the job when a tree limb was dislodged by the sanitation truck he was riding on and struck him in the head.

May 20, 2013 - Owensboro, KY - 35 year old Nett Gonzales of Houston, TX died on the job as the result of electrocution while working on Blue Bridge in Owensboro. He was found unconscious in the safety netting. It is unclear who he was employed by.

May 20, 2013 - Virginia Beach, VA - 41 year old Christopher Lorek and 40 year old Stephen Shawdied on the job of unknown causes. Both were part of the FBI CIRG's Hostage Rescue Team.

May 20, 2013 - Cougar, WA - 28 year old Kyle Schlesser of Rochester was killed on the job when he was struck by a falling tree. Another logger felled a tree, which hit another tree causing it to fall onto Schlesser. He was employed by Jim Schlesser, Inc.

May 21, 2013 - Bath, PA - 28 year old Paul Daniel Mast of Columbia, PA died from fall injuries while working to erect a barn of the property of Seiple Farms in Bath. He was employed by JDS Construction of Manheim, PA.

May 21, 2013 - Phoenix, AZ - 23 year old Brad Harper, a firefighter, was killed on the job when he was pinned between an ambulance and fire truck that were trying to pass each other on a narrow road. He was a Phoenix Firefighter.

May 22, 2013 - Pocatello, ID - 23 year old Derrick E. Martinez of Garden City, KS died of toxic chemical exposure while working at a Simplot fertilizer plant near Pocatello. He was found unconscious inside a mobile testing trailer. He was employed with Blick's Phosphate Conversion.

May 22, 2013 - Chattanooga, TN - Allen Phillips was killed on the job when the taxi he was driving was struck by another vehicle. Phillips was employed by Nation Taxi.

May 25, 2013 - Bardstown, KY - 33 year old Jason Ellis of Chaplin was a member of the Bardstown Police Department when he was killed by someone who ambushed him as he left work in a department vehicle.

May 28, 2013 - Mexico, MO - 30 year old Kenny Baker of Auxvasse was killed on the job when a loading dock plate he was repairing fell onto him. He was employed by Home Decorators, a subsidiary of Home Depot.

May 28, 2013 - Hanover, PA - 22 year old Trevor DePugh died of severe head injuries when he was struck in the head by a hydraulic cylinder that fell from an earth mover. He was employed with AKM Machinery.

May 29, 2013 - LaGrange, AL - 42 year old Teresa Weaver Pickard of Wadley, AL died on the job from heat related stress while working on a project weld line for Sewon America in LaGrange. Sewon supplies auto parts for Kia.

May 31, 2013 - Houston, TX - 35 year old Captain Matthew Renaud, 41 year old Engine Operator Robert Bebee, 29 year old Firefighter Robert Garner and 24 year old probationary Firefighter Anne Sullivan all died during a massive fire at a Southwest Inn which collapsed. All were members of Houston's Fire Department.

May 31, 2013 - Everett, WA - 38 year old Washington State Patrol Trooper Sean O'Connell of Marysville was killed on the job when his motorcycle collided with a box truck
