We have in hand another list of our beloved workers who died last year and we do not want to forget them so we again back up to the end of 2012 to honor those we lost in our country.  Regardless of time past, our hearts go out to the families of these workers. 

September 21, 2012 - Fort Stockton, TX - 33 year old Hector Madrid died on the job while working for United Drilling, Inc when he was struck by an object. 

September 22 - Airmont, NY - 32 year old Celso Tebalan-Pixcar died while working when he was struck by a tree limb that fell onto him.  He was employed by Olmar Salazar who was not licensed. 

October 3, 2012 - Apollo Beach, FL - 23 year old Jose Ortega-Sunega died on the job when he fell from elevation while working for M.K.  Construction Group.

October 5 - Springfield, OH - 25 year old Matthew Clay died when his head was struck against a wall and stack of bricks.  His employer, Springfield Concrete Block, said he was leaving work at the time the incident happened.  

October 5 - Yukon, OK - 61 year old Roy Eugene Judd suffered a Cardiac/Respiratory incident while working for Anadarko Consultants, Inc.

October 6 - Granville, OH - 52 year old Michael Smith was killed on the job when he fell from an elevation.  He was employed with Par Golf Group, Inc.

October 8 - Houston, TX - 56 year old Frances "Frank" Vu died when he fell from an elevation at University of TX, MD Anderson.

October 10 - Doral, FL - 62 year old Jose Calderon Silva died on the job when a parking garage collapsed onto him while working at Miami-Dade College.  He was employed by Solar Erectors U.S., Inc.

October 11 - Watford City, ND - 45 year old Thomas McCarthy died on the job when he fell from an elevated area at a construction site.  He was employed with Pyramid Residential Construction.

October 22 - Tampa, FL - 23 year old Carlos Govea died on the job when he fell from an elevation while working for American Rice Growers.

October 22 - Garber, OK - 53 year old Ronald Harris was killed on the job when he was struck by an object.  He was employed with Garber Coop.

October 24 - Dublin, OH - 57 year old Keith Weisheimer of Hilliard died on the job after falling approximately eight feet from a ladder as he attempted to remove an item from a display.  He was working for Toys R Us. 

October 24 - LaBelle, MD - 48 year old Lanny Corbin died while working for Sharpe Holding, Inc. when he was struck by an unknown object.

October 26 - Selma, AL - 47 year old Marvin Bennett was killed on the job when he fell 16 feet from a fixed ladder while working at the Selma plant of Globe Metallurgical, Inc. 

October 26 - Fort Smith, AR - 52 year old Kevin Wisecarver died after he suffered an electric shock while working for Three Corners, LLC.

October 26 - Cleveland, AL - 69 year old George Allen was killed on the job when he was struck by an unknown object.  He was employed by Southeast Culvert, Inc.

October 28 - Prattville, AL - 58 year old Neil Taylor died of unknown injuries and an unknown cause while working for International Paper in Prattville.

October 29 - Miami, FL - 67 year old Jose Varela died after he fell from an elevation while working for Mila Services, Corp.

October 29 - New York, NY - 50 year old Anthony Nahr died on the job when he drowned as flood waters from Superstorm Sandy flooded the parking garage he was working in.  After the area was evacuated, his employer, Empire Parking Corp., required him to come in to watch over the few cars left in the garage.

October 30 - North Jackson, OH - 21 year old John Tomlin, Jr. of Niles, was killed on the job when he was pinned and crushed under a metal rack stacked with heavy aluminum.   Tomlin was employed by Extrudex Aluminum, Inc.

October 31 - Summerfield, TX - 21 year old Dakota Large died of injuries after being struck by an object in an unknown incident.  He was working for Bar G Feedyard.

October 31 - Dickinson, ND - 37 year old Chad Fawson of Lander, WY died as the result of a work related incident where he was caught in or between an object.  He was working for Western Oilfields Supply Co. in Dickinson.

November 1, 2012 - Darien, WI - 63 year old Thomas Warn died on the job from an unknown incident/unknown injuries while working for Badger Grain Co., Inc.

November 1 - San Diego, CA - 23 year old Lucas Hansen of Ramona, was killed on the job when he fell from elevation.  He was working for Asbury Steel Construction. 

November 2 - New Brunswick, NJ - 53 year old Rafael Reyes died after being exposed to carbon monoxide from a generator.  He was employed with Sajoma Fried Chicken.

November 3 - Zionsville, PA - 38 year old Jerome Tyson of New Tripoli died on the job when a tree fell and struck him.  He was employed by J&T Home Maintenance.

November 4 - New York, NY - 25 year old Luis Antonio Guaman died on the job when he fell through a skylight at a pier in Manhattan while storing storm supplies. The report states he was ending his shift and had unhooked his harness when he fell.  He was employed with Yukon Enterprises.

November 5 - Galveston, TX - 40 year old James Miller and 40 year old James May died on the job when they were caught in or between an unknown object.  They were employed by Askew & Workers Construction.

November 5 - Baytown, TX - 20 year old Antonio Torres was killed on the job when he suffered injuries after being struck by an unknown object.  He was employed with Armor Industrial Fabricator.

November 6 - Flemington, NJ - 66 year old William Hardenburg of Bethlehem Township, was struck and killed by a motorist as he was clearing damage from Superstorm Sandy in front of a home.  He was an emergency management coordinator for Bethlehem Township.  

November 6 - Naples, FL - 55 year old Frank Shepherd died on the job from an incident related to inhalation while working for APAC-Southeast, Inc.

November 6 - Leedey, OK - 27 year old Aaron Gish of Norman died on the job when he suffered Cardio/Respiratory event in an unknown work event.  He was employed with Nomac Drilling.

November 7 - Mazomanie, WI - 54 year old Marc Nondorf of Dodgeville was killed on the job when a load of trusses fell onto him as he used a forklift to secure the load.  He was employed by Wick Buildings, LLC in Mazomanie.  

November 9 - Oswego, NY - 37 year old Lateef Haskins of Clay, died on the job when he fell three stories while installing metal decking at State University College in Oswego.  He was employed by Solvay Iron Works, Inc. 

November 9 - Nacogdoches, TX - 39 year old John F. Goode of Louisburg, KS was killed on the job when he fell from a tree he was cutting down at Stephen F. Austin University Azalea Garden in Nacogdoches.  He owned J. Goode Environmental Service and was possibly subcontracting for Drewery Tree Service. 

November 11 - Worchester, MA - 45 year old Duane Race of Putnam, CT died on the job when his bread truck was struck from behind by another motorist, killing him.  He was employed by Interstate Brands Corp.

November 11 - Hamilton, NJ - 58 year old William Pryor of Monroe died on the job when he suffered injuries from a same level fall.  He was employed by Robert Wood Johnson Health Services in Hamilton.

November 12 - Denver City, TX - 66 year old Pablo Garcia died on the job of an unknown cause while working for R&C Backhoe Services.

November 13 - Malad, ID - 65 year old F. Dennis Thomas died on the job when the power lift he was operating got too close to the edge of a loading dock and fell off backwards, then he was struck by the tires of a trailer because the driver didn't know he had fallen.  He was owner of Thomas Marker, Inc.

November 16 - Saint James, NY - 36 year old David Freberg of York, SC was killed on the job when he was struck by a tree limb when he was attempting to trim a fallen tree.  He was employed by Patriot Tree Service.

November 20 - Miami, FL - 54 year old Eduardo Lopez died on the job when he fell from an elevation while working for Landshark Roofing, Inc.

November 21 - Feasterville-Trevose, PA - 32 year old David Kelble, Jr. of Glenside died December 2, 2012 after he suffered head injuries after being struck in the head by a tree branch.  He was employed by Monster Tree Service.  He was also a Volunteer firefighter for the Weldon Fire Dept.

November 21 - Englewood, FL - 20 year old Alfredo Rodriquez died on the job when he was struck against an unknown object while working for Suncoast Environmental Service. 

November 27 - Nanicoke, PA - 51 year old Michael Martin suffered a medical event and died on the job while training to become part of PPL Susquehanna, LLC fire brigade.  He was working at the PPL nuclear power plant in Salem Township.

November 27 - San Antonio, TX - 48 year old Martin Medellin of Huntsville was killed on the job when a trailer he was working under fell, crushing him.  He was welding legs on the underside of a semi trailer when a jack malfunctioned, causing the trailer to fall.  He was employed with Unique Semi Tractor Trailer.

November 28 - Port Washington, NY - 54 year old Ignacio Maldonado died on the job when he fell from an unsecured ladder while attempting to check out a (Sandy) storm-damaged chimney.  He was employed by J.F.T. Enterprises, Inc. 

November 28 - Colts Neck, NJ - 59 year old Gene Arace of Union Township was killed on the job when part of a tree he was cutting up flew back, striking him.  He was employed by Garden State Tree & Lawn, LLC.

November 29 - Wheeling, IL - 37 year old Bernardo Martinez of Cicero died on the job after he fell into a 6,000 gallon sized chemical tank and was overcome by the toxic chemical at the bottom of the tank.  He had been cleaning the tank near the top of it before falling.  He was employed by Phoenix Industrial Cleaning and was working at the Sunnyside Corporation in Wheeling. 

November 29 - Merrick, NY - 61 year old Amid Uddin died on the job when he fell from an elevation in an unknown incident.  He was working for A1 Construction.

November 30 - Pearl River, NY - 59 year old James Hoyt of Bogata, NJ suffered fatal crushing injuries when a large crate being moved between trucks came down onto him.  He was working for Specialized Trucking, Inc. through Labor Ready.

December 2, 2012 - Westbury, NY - 60 year old Jorge Posada died on the job while working for Jack Cooper Transport Co. when he fell from an elevation.

December 3 - Sperry, OK - 28 year old Andrew Valencia of Pawhuska was electrocuted on the job when a piece of equipment he was stabilizing at an oilfield was caught by the wind and touched a power line.  He was working for Lincoln Well Services, LLC.  

December 4 - Macomb, IL - 36 year old Anthony Williams of Tennessee, IL was killed when he fell from a ladder after a pole he was working on broke, causing the ladder to fall.  He was employed by Albert Cook Electric of Adair. 

December 4 - Miami, FL - 27 year old Jaime Rosales was killed on the job when he was struck against or by an unknown object while working for Centerline Directional Drilling.

December 5 - Houston, TX - 22 year old Spencer Symons was killed on the job when he was struck by an object in an unknown incident.  He was employed with JBC Plumbing.

December 5 - Mobile, AL - 49 year old Jeffery Hudson suffered fatal crushing injuries when a 1,000 pound generator fell, landing on him.  He was working for APM Terminals Mobile, LLC.  

December 5 - Timpson, TX - 58 year old Augustin Ramirez was killed on the job when he was struck by a broken band saw blade.  He was employed by Nix Forest Industries, Inc.

December 5 - Spring, TX - 66 year old Joyce Foster died on the job when she suffered a same-level fall while working for Kroger Company.

December 5 - Dawsonville, GA - 45 year old Jimmy Lassetter of Newnan died several days after being admitted to an area hospital's burn unit after suffering burns due to an explosion.  He and other workers were unloading a truck of tanks at Amicalola Propane when two tanks collided, shearing off the valve from one and the ensuing cloud of propane ignited.  He was employed with Lassetter Propane, LLC. 

December 6 - Henrietta, TX - 51 year old Evaristo Contreras-Mon.... (the incomplete name we were given) was killed on the job while working for Plaska Transmission Line Construction.

December 7 - Juno Beach, FL - 51 year old Ernest LaPlant died of a cardio/respiratory related medical issue while working for Ferrellgas, LP.

December 9 - Grapevine, TX - 50 year old Frank James Martinez died of a cardio/respiratory related medical issue while working for Wrico Stamping of TX.

December 9 - Guthrie, OK - 51 year old Rod Dotson was killed in an incident at an oilfield when he was struck by an object.  He was employed by Dan D. Drilling, Corp.

December 10 - Johnstown, PA - 57 year old George Adams III of Davidsville died on the job when he fell from an elevation while working at medical construction site.  The incident was un-witnessed.  He was employed with Nevin Stitzer Contracting, Inc. 

December 11 - Pilot Point, TX - 46 year old Martin Meraz died of a cardio/respiratory related medical issue while working for Whitmire Line Clearance, Inc.

December 12 - Pensacola, FL - 50 year old Matthew Bennett Sr. died on the job when he was struck by an object in an unspecified incident while working for Gulf-Atlantic Constructors, Inc.

December 12 - Commack, NY - 51 year old Curtis Mains Jr. of East Northport died of a traumatic brain injury when he fell from an elevation.  He was employed with The Home Depot Store #1202.

December 12 - Irvington, NJ - 66 year old Martin Thorn of West Orange died on the job when he suffered injuries from a same-level fall.  He was employed with Pocono Progressive, LLC.

December 12 - Emporia, KS - 26 year old Dewayne Dunn of Hartford died on the job from an unknown incident when he was struck by an object.  He was working for Summit Drilling Co. at the time of his death.

December 14 - Largo, FL - 35 year old Alfred Williamson of Brandon died on the job when he was electrocuted.  He was a painted and the ladder touched a live power line.  He was employed by AW Coatings. 

December 14 - New York, NY - 63 year old Luis Enriquez was killed on the job when he was struck by a moving truck while delivering 7-UP for Canada Dry Bottling Co. of NY.  

December 14 - Coram, NY - 25 year old Daniel Sturgis was killed on the job when he was struck by a payloader machine driven by another worker.  He was employed with Premium Mulch & Materials, Inc.

December 14 - Gloversville, NY - 41 year old James A. Vanderwerker, Sr. of Fort Johnson was killed on the job when he was struck by a tree he was cutting down with a chain saw at a logging site near Bleecker.  He was employed by American Forestry & Firewood. 

December 14 - Wichita, KS - 62 year old Lawrence Pirce died after he suffered a same level fall while working for the US Postal Services.

December 15 - Elmira, NY - 50 year old Michael Donahue died on the job when he was caught in or between while working to repair a hydraulic tabletop at Kennedy Valve foundry, a division of McWane, Inc.

December 16 - Bethany, WV - 44 year old Robert McDanell of West Monroe, LA was killed on the job when he was struck by an unknown object on a construction site.  He was working for Precision Pipeline, LLC.

December 18 - Crossville, AL - 55 year old Willey Glen Scott died on the job when he fell from an elevation while working for Ashley Masonry, Inc.

December 19 - North Lima, OH - 19 year old Alex Hrovatic of Salem died on the job after he fell approximately 25 feet through a roof at a construction site.  He was employed by Witmer's Inc.

December 19 - Palm Beach, FL - 65 year old Dave Groves of Juno died from an unknown incident or injuries while working for Groves Building & Design.

December 19 - New Orleans, LA - 45 year old Darrell Naquin of Saint Bernard was killed on the job when he was struck in the head and chest by falling debris as he was working a crane to demolish part of a building.  He was employed by Lou Gioe's Truck Service, Inc.

December 20 - Pompano Beach, FL - 38 year old Anthony Nunez of Boca Raton was fatally electrocuted while repairing an air conditioning unit at the site of Big Bear Storage and Packing.  He was employed with Konair, Inc.

December 20 - Yonkers, NY - 67 year old Frank Volpe died of a cardio/respiratory related medical issue while working for Hudson Plumbing & Heating Co.

December 22 - Ventnor City, NJ - 29 year old Davis Transue died suddenly at work from unknown circumstances or injuries while working for Wayne Transue. 

December 23 - Ozona, TX - 38 year old Nieves Cerda died at work when he fell from an elevation while working for Nabors Drilling USA.

December 26 - Saint Anne, IL - 21 year old Jackob C. Dluzak of Kankakee was killed on the job when he was caught in and trapped in the bars of a boom forklift.  He was working for St. Anne Area Farmers Consignment Auction.

December 26 - Corsicana, TX - 74 year old Kenneth Eugene Wheeler of Malakoff was killed on the job when he was caught in the clamshell mechanism on a trash truck as it was closing.  He had, for an unknown reason, reached into the back of the truck after it was off-loaded.  He was working for Clean Sweep Trash Service. 

December 28 - Brick, NJ - 69 year old Paul Friedauer of Long Branch was killed on the job after being struck by a motorist while attempting to stop traffic for a storm clean up operation.  He was employed with Glenside Equipment Company. 

December 28 - Geismar, LA - 55 year old Greg Price died on the job after falling from an elevation while working for James Construction Group.

December 28 - Greeley, CO - 73 year old Paul Kullberg died as the result of a same level fall while working for Quest Diagnostics.

December 28 - Elmont, NY - 46 year old Ahmad Luqman died on the job after falling from an elevation while working for RL Construction & Restoration.

December 30 - Snowmass Village, CO - 49 year old Patricia "Patsy" Hileman was killed in an avalanche related incident.  She apparently triggered a small avalanche and was swept over a cliff.  She was a 26 year veteran of Aspen Skiing Company.

December 31 - Phenix City, AL - 29 year old LeGary Antley, Jr. of Smith's Station suffered fatal crushing injuries while working for Strickland Tree Service.  He was working on adump truck with the body raised when the hydraulics failed, causing the body to fall onto him.

January 2, 2013 - Cleveland, OH - 22 year old Lauren Block of Richmond Heights died at work after falling from a second story balcony at Christie's Cabaret.  She was employed with Entertainment USA of Cleveland. 

January 2 - Conyers, GA - 46 year old David East was killed on the job when he was struck by an unknown object while working for Pactiv Corp.

January 2 - White Castle, LA - 49 year old Flemmie Finley was killed on the job when he fell from an elevation.  He was employed with Universal Cable Systems, Inc.

January 3 - Craig, CO - 49 year old Jonathan Herod was killed on the job due to an explosion at an Axia Energy oil rig near Craig. He was a truck driver attempting to offload water to another tank when it exploded due to a valve issue.  He was employed with Herod Industries, Inc.

January 3 - Madison, WI - 30 year old Rafael CaCahua died while working for CR Auto Plus after he inhaled a toxic substance.

January 4 - Boca Raton, FL - 32 year old Leonardo Perez-Castillo died of unknown circumstances while working for Epifanio Perez-Velasquez.

January 4 - Reydon, OK - 32 year old Eric Smith was killed on the job when he fell from an elevation while working for Latshaw Drilling & Exploration.

January 4 - Palm Beach, FL - 36 year old Joseph Miller died of unknown circumstances while working for ECS-Florida, LLC.

January 7 - Bensenville, IL - 57 year old Martin Sum was killed on the job after becoming pinned between his truck and a post while working for Vitran Express, Inc.  

January 7 - North Palm Beach, FL - 40 year old Ray Figueroa was killed on the job when he fell from an elevation while working for Stephens & Sons Paining, Inc.

January 9 - North Haven, CT - 73 year old Francis Almeida of Durham suffered fatal crushing injuries when he was struck while offloading a scrap car from his flatbed car carrier at Sims Metal Management recycling business.  He was employed by Almeida Trucking. 

January 11 - El Paso, TX - 27 year old Christopher Self died on the job after he was fatally struck by a motorist while working with a road clean up crew.  He was employed by Xceed Resources.

January 11 - Oklahoma City, OK - 44 year old Alvaro Ortiz-Luna died on the job when he fell from an elevation while working for Compos Roofing.

January 11 - Neward, NJ - 48 year old Luis Colorado died on the job when he was crushed between a truck chassis and a container wall inside a parking garage.  He was self employed.

January 13 - Brookwood, AL - 56 year old Terry Kimbrell was killed on the job of unknown circumstances while working for Abernate Auto Parts, where he was owner.

January 14 - Fairview, WV - 36 year old Jose Aguilar of Katy, TX was killed on the job when he was struck by an unknown object while working for Omni Energy & Seismic Service.

January 14 - Sardis, MS - 69 year old David Hollinger died on the job from unknown circumstances while working for Newt Davis, LLC.

January 14 - Texarkana, TX - 60 year old Robert Earl Gibson of New Boston died from a same level fall while working for Red River Army Depot - DLA.

January 15 - Bronx, NY - 54 year old Paul Schisler of Bridgewater, NJ died after falling from the High Bridge in the Bronx that he was working on.  He was employed by Schiavone Construction Co.

January 16 - Denver, CO - 60 year old Mike Wilson died on the job when he was struck by an unknown object while working for Beaver Builders & Excavation.

January 16 - Hastings, NE - 33 year old Jose Briseno-Carder was killed on the job when he was struck by an unknown object while working for Witt Plumbing, Inc.

January 16 - Atlanta, GA - 52 year old Eugene Williams died on the job when he fell from an elevation while working for Reliance Steel & Aluminum Co.

January 16 - Orlando, FL - 49 year old Randall Gustafson died on the job when he suffered injuries due to a same level fall while working for Gustafson Chipping Company.

January 17 - Krum, TX - 64 year old Jimmy Weber died on the job from unknown circumstances while working for Enterprise Crude Oil.

January 17 - LaMesa, TX - 57 year old Mario Mojica, Jr. died after inhaling a toxic substance while working for Hatchett Roustabout, Inc.

January 17 - Williams Bay, WI - 57 year old Lazaro Muniz Rueda was killed on the job after falling from an elevation.  He was working for Gilberto Becerra Chavez.

January 18 - Sheldon, ND - 49 year old Jon Bartholomay was killed on the job when he fell from an elevation while working for Plains Grain & Agronomy, LLC.

January 19 - Watford City, ND - 49 year old Michael Krajewski of Duluth, MN was killed on the job when he was struck in the head by a pipe while working at an oil well site.  He was employed by Halliburton, Inc.

January 21 - Utica, NY - 29 year old William Pope of Blossville was struck and killed by a garbage truck he was operating. He attempted to get into the truck, after having left it in gear, but fell and he was run over by it.  He was employed with Bliss Environmental Services.

January 21 - Port Jefferson, NY - Zhanbin Li died on the job when he fell from an elevation while working for Gao's Construction Corp.

January 23 - Cambridge, MA - 70 year old Wayne Rudkin of Whitman was killed on the job when he was caught in or between an unknown object while working for C.J. Mabardy, Inc.

January 23 - Denver, CO - Jeremiah Johnston died on the job from injuries he received in a same-level fall of unknown circumstances.  Employers listed for Johnston are: Rhein-O-Demolition; Cypress Investments, LLC; and Apex Construction Manager.

January 24 - Astoria, NY - 42 year old Ricardo Gonzales of Brentwood died of head injuries when he fell approximately 15 feet at a building construction site.  He was employed with Abacus Building Innovations, Inc.

January 24 - Belle Glade, FL - Pablo Leon was killed on the job when he was caught in or between an unknown object while employed by Donald Moss Corp.

January 25 - Kerhonkson, NY - 69 year old Doreen Richardson, owner of the Mountain Valley Resort, was killed when strong winds related to Superstorm Sandy sent a piece of a rental unit sailing through the air and crashing into the windshield of her vehicle as she was checking on her property.

January 25 - Horseshoe Beach, FL - 29 year old Jeffrey Hagan of Chipley died on the job when he fell from a water tower he was painting that was owned by Horseshoe Beach.  He was employed by Shores Construction, Inc.   

January 26 - Richmond, TX - 49 year old Feliciano Puerta of Houston was killed on the job when a barrel he was working on with a cutting torch exploded.  The barrels were marked as containing a cleanser. He was employed by SCR Construction Co.

January 28 - Frisco, TX - 31 year old Jose Carmen Nieves-Alvarez of Sherman died on the job when he was buried in a trench collapse at a construction site.  It appears pipes were being laid in the trench.  He was employed by Pittard Construction Co.

January 28 - San Antonio, TX - 52 year old Able Rodriguez was killed on the job due to head trauma while working at Takata Seat Belts factory.  He shut a machine down to oil it but it jump started and he was unable to exit the machine and his head was caught in it.  He was employed by TK Holdings, Inc.  

January 28 - Delphos, OH - 58 year old Billy Joe "Bill" Stephenson, Jr., of Spencerville died on the job when he suffered injuries due to a same-level fall in an unknown incident.  He was truck driver for P-D Midwest Transport.

January 29 - Talmage, NE - 68 year old Roger Teten was killed on the job when he was struck by a semi trailer that backed into him at a grain elevator.  He was employed by Farmers Cooperative Company. 

January 29 - Atlanta, GA - 26 year old Benjamin Klein died on the job in an unknown incident while while working for JAL Chemical Co., Inc.

January 30 - Rockford, IL - 53 year old Greg Stewart of Forreston died after being electrocuted while working in a false ceiling of a conference room.  He was a maintenance supervisor for Lowe's Home Centers, Inc. 

January 30 - Pharr, TX - 51 year old Natividad Hernandez died on the job after he fell from an elevation in an unknown incident.  He was working for Ricardo Roofing.

January 30 - Resaca, GA - 21 year old Leo James Pastrana died on injuries he received after falling from a piece of machinery approximately 15 feet to the ground.  He was employed with Resaca Sun Feeds, LLC.

March 13, 2013 - Avon, MA - 42 year old Michael Ledoux of Bridgewater was killed on the job when a co-worker ran over him with his semi truck. Ledoux had crawled under the truck but didn't inform the driver who then moved the truck.  He was employed with William H. Glancy & Sons, Inc./Glancy Crane Service & Rigging of Avon.  

May 11, 2013 - Issaquah, WA - 31 year old John Thomas Norman was killed on the job when he was involved in a tractor rollover.  He was employed with Goodwood Tree and Fire Wood Service.

May 20 - Moore, OK - Richard Jones died as the result of a tornado that ripped through Moore.  He was working in the Oklahoma City P&DC Moore post office branch when the tornado struck. 

May 24 - San Bernardino, CA - 47 year old Abel A. Aldrete of Colton was killed on the job when he was struck by a slow moving train in a San Bernardino rail yard.  He was an employee for  BNSF (Burlington North Santa Fe).

May 25 - San Bernardino, CA - 64 year old Maria Luisa Peinado of Colton died on the job after being crushed by a large metal gate that fell onto her.  She was attempting to put the gate back on it track after coming out when the incident happened.  She was a guard for Burrtec Industries.

May 28 - Corpus Christi, TX -  22 year old Christopher Cantu was killed on the job when he was crushed under a large cylinder full of sand blasting material as he was moving it at the Coastal Plating Company's plant. 

May 29 - College Station, TX - 49 year old Armando Gonzalez of Bryan died on the job when he was pinned under two 1,500 pound shoring panels while working on a construction site at the Texas A&M Health Science Center.  It is unclear who Gonzalez was working for. 

June 3, 2013 - Moody, TX - 62 year old Larry Goodwin, a Texas farmer, was killed after being attacked by a swarm of killer bees when he accidentally upset a hive.

June 5 - Watchung, NJ - 52 year old Hubert Humphrey Singley of Piscataway was injured while at work when he was struck with a stream of water under very high pressure that knocked him unconscious.  He was repairing a hydrant and a pipe burst.  He died on June 12th at an area hospital.  He was employed with New Jersey American Water Company. 

June 5 - Portland, OR - 17 year old Nikolay Kozhokar suffered fatal crushing injuries when a  semi-truck he was working under fell onto him after a hydraulic lift failed.  He was working for O & V Transport, LLC.  

June 6 - Manvel, ND - 58 year old Otis Gohman of Clear Lake, MN died on the job when he became trapped under a concrete grinder he was operating.  It is unclear who Gohman worked for.

June 6 - Fairmont, WV - 50 year old Robert Goff was killed on the job when he attempted to jump into a rolling semi truck.  As the truck rolled, Goff fell and was run over by the truck.  He was employed by Reclaim Company.

June 8 - Afghanistan - 54 year old Joseph Morabito of Hunter, NY was murdered while working for a private military contractor helping with security forces.  He was murdered by one of the soldiers he was there to train.  In the same incident, 40 year old Lt. Col. Todd Clark of Albany was also murdered.

June 10 - Champaign, IL - 54 year old Michael S. Williams of Humbolt died from injuries he received in a fall while working on a construction site at Beverly Cinemas.  It is unclear who he was employed with.

June 11 - Bothell, WA - 48 year old John Weyhing of Lake Stevens died of natural causes, one day before his 49th birthday, while he was at work operating a loader at UW Bothell Campus construction site.  He was employed with HOS Brothers Construction, Inc.

June 14 - Gesimar, LA - 29 year old Zacharay C. Green of Ponchatoula and 47 year old Scott Thrower of St. Amant were killed on the job after an explosion at the Williams Olefins Geismar chemical plant. Officials say 77 people were injured in the blast.

June 14 - Donaldsonville, LA - 55 year old Ronald "Rocky" Morris, Jr. of Belle Rose was killed and other employees injured in an explosion at the CF Industries chemical plant.  The incident involved a rupture in a small vessel as nitrogen was being off loaded from a tank truck.

June 15 - Carlsbad, NM - 30 year old Matthew Depew died on the job after being electrocuted while trying to fix a power outage.  He was an Excel Energy employee.

June 18 - Ontario, CA - 28 year old Winston Perez of Riverside died of internal injuries when he was pinned between a gate and fence as he was moving cattle.  He was employed by Dick Dykstra Dairy farm.

June 19 - Cloquet, MN - 62 year old Greg Blackburn was killed on the job when he was struck by a motorist driving a truck.  He was working maintenance for the Minnesota Dept. of Transportation's Mahtowa rest stop.  

June 19 - Big Spring, TX - 40 year old Felip Saiz died on the job when part of a rig collapsed while he was working for Earthco.  He was working in the Athlon Yard. 

June 19 - Mesa, AZ - 43 year old Steven Ariston of Peoria was killed on the job when a portion of a bridge he was working on collapsed.  Other co-workers were injured.  He was employed by Ralph L. Wadsworth Construction Company, LLC.   

June 20 - Big Spring, TX - 59 year old Juan Manuel Estrada-Duarte died on the job while working for Earthco, a day after another employee died in an unrelated incident.  Estrada-Duarte was working with others on a heater treater when it rolled, pinning him against another treater. He was working in the Athlon Yard.

June 20 - Portsmouth, NH - 43 year old Matthew Morasse died from injuries he received involving a stump grinder while working for Urban Tree Service.  

June 21 - St. Louis, MO - 42 year old Mensud Djelmo was killed on the job when he became trapped in a packing unit machine while working at Pepsi Cola Bottling Company. 

June 22 - Perry Township, PA - 52 year old Michael Eugene Over of New Bethlehem was fatally electrocuted while he was working to repair a power line.  He was employed by Central Electric of Parker. 

June 24 - Union Mills, IN - 67 year old James Swank died from injuries he received in a grain elevator explosion while working for Union Mills Co-op.  

June 24 - Manson, WA - 68 year old John Marker was killed when the farm tractor he was operating in his cherry orchard overturned down an embankment. 

June 25 - Pinellas Park, FL - 30 year old Eric Joseph Kanganis of Tarpon Springs was killed on the job when a large metal coil he was attempting to offload fell onto him, pinning him between it and the forklift he was using.  He was employed with USP Structural Connectors.  

June 27 - Port Arthur, TX - 40 year old Juan Antonio Garcia-Hernandez of Orange was killed on the job when a piece of iron fell from an elevated forklift and struck him on the ground.  He was working at a Hemi Systems site working on silos for Mason Construction.  

June 29 - Waterloo, IA - 54 year old Stan J. Delagardelle of Cedar Falls died after he suffered injuries when he was struck by a forklift at John Deere Service Parts in Waterloo.

June 29 & 30 - Tampa, FL - 24 year old Luis E. Gonzales died a day earlier than his father, 53 year old Ernesto Gonzales after the two entered an empty asphalt tanker and were overcome by toxic fumes due to lack of ventilation.  It is unclear who they may have worked for but were helping out a friend who owned the truck.  

June 30 - Bethel, ME - 60 year old Sam Chapman died on the job when a piece of metal on a wood chipper came loose and pierced his chest.  He was showing others how to use the piece of equipment.  He owned Chapman Tree Services.

June 30 - Phoenix, AZ - Nineteen firefighters of an elite Hotshot crew from Prescott, AZ were killed when they were caught in the Yarnell Hill wild fire.  Following are those team members who died:

January 26 - Hyden, KY - 52 year old Jerry A. Watts was killed when he was struck on the head by a piece of machinery while working for Watts Welding & Fabricating at T & T Energy's Begley Resources Mine #1.

February 26 - Blount, WV - 34 year old Brandon E. Townsend died after he was struck by a piece of equipment while working for Midland Trail Energy LLC's Five Mile Prep Pit & Refuse Impoundment.

February 7 - Sophia, WV - 43 year old Edward L. Finney was killed when he was crushed under a piece of equipment while working for Affinity Coal Co. LLC's Affinity Mine.

February 12 - Fairview, WV - 51 year old Glen Clutter was killed when he was struck by equipment while working for Consolidated Coal Company's Loveridge #22 Mine.

February 13 - Cutler, IL - 28 year old Timothy K. Chamness died when he was pinned by a piece of machinery while working for Knight Hawk Coal LLC's Prairie Eagle South Underground Mine.

February 19 - Sophia, WV - 44 year old John Myles was killed when he became pinned by a piece of equipment while working for Affinity Coal Company LLC's Affinity Mine.

March 13 - Comfort, WV - 63 year old Asa Fitzpatrick was killed when he was struck by falling rock while working for Newtown Energy Incorporation's Peerless Rachel Mine.

March 22 - Huntington, UT - 29 year old Elam Jones was killed when he was crushed under debris from a roof cave in while working for Castle Valley Mining LLC's C.V. Mine #4.

June 6 - Adger, AL  - 36 year old Christopher Brown died of injuries he received from falling from a catwalk and striking other machinery while working for Jim Walter Resources Inc.'s No. 7 Mine.

July 2 - Eldorado, IL - 35 year old Nathanial Clarida died of injuries after becoming pinned by machinery while working for Peabody Midwest Mining LLC's Wildcat Hills underground mine.

July 31 - Dubois, PA - 28 year old Steelyn G. Kanouff was killed when he was struck by machinery while working for AmeriKohl Mining Incorporation's AmeriKohl Strips.

January 7 - Smithland, KY - 49 year old Todd Sumlin was killed when he was struck by falling rock while working for LaFarge West Incorporation's Three Rivers Quarry.

January 21 - Las Vegas, NV - 54 year old Kenneth Korakis died from injuries he received involving machinery while working for Apex Quarry & Plant.

March 27 - Maryland Heights, MO - 61 year old William R. Sievert died when he was struck by debris from a blast while working for Fred Weber Incorporation's North Stone Mine.

April 4 - Yavapai, AZ - 30 year old Alvin H. Thomas died when he was struck by a pipe while working for TIC, The Industrial Co. / Freeport-McMoRan Bagdad Inc.

April 16 - Franklin, LA - 58 year old Michael Charles was killed after being struck by an object while working for North American Salt Company's Cote Blanch Mine.

April 27 - Shamrock, TX - 58 year old James D. Winegeart died after he became entangled in a conveyor while working for Allied Custom Gypsum's ACG #1 Shamrock.

May 17 - Questa, NM - 22 year old Issac A. Garcia died after becoming pinned between a wall and ore car while working for Chevron Mining Incorporation's Questa Mine & Mill.

June 2 - Carlin, NV - 43 year old Corey Vasquez was killed when the loader he was operating go close to an embankment and rolled down it while working for Newmont USA Ltd.'s Exodus Mine.

June 13 - Cupertino, CA - 50 year old Jose A. Gonzales was killed from injuries received in a haul truck crash while working for Stevens Creek Quarry.

February 20,2013 - Brunswick, MD - 39 year old Brian Snoots died on the job when an industrial sized tire he was inflating exploded.  He was employed by Brunswick Auto Repair. 

February 21 - Edinburg, TX - 21 year old Oscar Arellano died on the job from asphyxiation while working for Azteca Milling.  He was buried approximately 15 feet under corn grain inside a silo.  The company did not have confined space permits, allowing them to have anyone work inside the silo. 

February 22 - Woodbridge, NJ - 43 year old William Convey III of South Bound Brook, NJ was killed on the job when he was struck by a motorist as he stood at the back of his Fed Ex delivery truck, pinning him between the two vehicles. 

February 23 - Lincolnton, NC - 35 year old Marendonio Francisco Viruel Lopez died of traumatic injuries when he became caught in an angular saw while working at a cabinet making plant.  He was employed by RSI Home Product at plant #2.  

February 24 - Geismer, LA - 52 year old Nolan Cormier was killed on the job when he became entangled in a conveyor belt.  He was working for Impala Burnside Terminal plant. 

February 25 - Carrollton, OH - 28 year old Abdal L. Audeh of Dover was killed on the job when he fell backwards on an oil rig and was then struck by a piece of equipment.  Another worker was injured.  He was employed at R.E. Gas Development's Brace West drill pad. 

February 25 - Canaan, PA - 34 year old Eric Williams of Nanticoke, PA was killed on the job when he was stabbed to death by an inmate.  Williams was a guard at the Canaan penitentiary, working for US Bureau of Prisons. 

February 26 - Falls Township, NJ - 50 year old Janio Salinas Zalerda of Edgewater died on the job when he fell from a catwalk, into a sugar hopper while working for the CSC Sugar plant in Bucks County. 

February 27 - Alpharetta, GA - 49 year old Miguel Agirre of Duluth died on the job when he was struck and run over by a backhoe at a construction site.  He was an employee for D & H Construction doing work for Fulton County Water Services.

February 27 - Santa Cruz, CA - Sgt. Loren "Burch" Baker and Detective Elizabeth Butler were shot and killed by a suspect who they went to question about an incident.  Both were officers for the Santa Cruz County Sheriff's Office.

March 1, 2013 - White Pine, TN - 34 year old Chad Rogers was killed on the job after being struck by a semi while working on a road construction project.  Rogers was working on a TDOT project for Charles Blaylock and Sons when he went to retrieve his hard hat that had blown into traffic.

May 5, 2013 - New York, NY - 60 year old Oluwole Olofin was a taxi driver in December of 1992 when he was shot in the head by a customer who refused to pay his fair.  Olofin died May 5th, a little over 20 years later from complications resulting from the shooting such as post-traumatic seizure disorder.  The medical examiner has declared his death a homicide.

June 18, 2013 - Brooklyn Center, MN - 58 year old Leo Michael Brandt died of blunt force injuries when he was struck and pinned by a postal truck at the loading dock of the Brooklyn Center Post Office.  He was employed by the United States Postal Service.

May 28, 2013 - Copiah County, MS - 42 year old Michael Castelli of Baton Rouge, LA and 32 year old Johnny Martone of Taylorsville, MS were both killed on the job when they fell from a cell phone tower while working in Copiah County.  They were adding new antennas to the tower when it reports state there was a failure to a cable.  They were employed by Byrd Telecom.

July 5, 2013 - Medford, MA - 45 year old James Baldasarre collapsed on the job and died at hospital after suffering from heat exhaustion.  He was working for the US Postal Service from the Medford Post Office.  He complained to his wife earlier in the day about the heat.

July 5 - Auburn, WA - 56 year old Brian Love was killed on the job when his taxi was struck by an alleged drunk motorist driving a large van.  He was employed by Tim's Taxi.

July 5 - Chicago, IL - 36 year old Luis Mercado was killed after being struck by a Metra Electric Train while performing track work.  He was a trackman for Metra.

July 6 - Leisenring, PA - 53 year old Glenn Nicholson, Jr. was killed on the job when he was crushed under the bed of a dump truck he was working on.  It appears he was self employed.

July 9 - Bismark, ND - 42 year old Namon Smith of Chattanooga, TN and 25 year old Zachary Roberts of Lakeland, GA died on the job after falling from a cell phone tower.  The men were adding structural supports to the tower when one of the men fell, striking the other man, causing him to fall also.  Both workers were wearing safety harnesses and hard hats. The men worked for Monarch Towers, a subcontractor for Basin Electric Power.

July 9 - New Milton, WV - 45 year old Tommy Paxton of Walton, WV died after being burned in a Doddridge gas well explosion.  He was a subcontractor working for Antero Resources. 

July 9 - Omro, WI - Paul Olsen, a skydiving instructor for Skydive Adventure, was killed on the job during a tandem skydiving jump.  As the winds grew stronger the pair were blown off course and ended up in Lake Butte des Morts where Olsen drowned.  The student was able to swim to shore.

July 10 - Fife, WA - Craig Coelho died of natural causes while working at L & L Nursery Supply. 

July 11 - Redwood City, CA - 43 year old Mendie Udo died on the job when he was crushed inside a cardboard baler.  He was a janitor working for Grocery Outlet of Redwood City.

July 12 - Olympia, WA - 64 year old Marvin Komenda suffered a stroke while driving an empty school bus and later died.  He was employed by School District #3 Thurston County.

July 15 - Howard Lake, MN - 23 year old Brandon Scherping of Annandale died of crushing injuries when a piece of farm equipment he was working on suddenly fell onto him.  He was working for Midwest Machinery.  

July 15 - Dearborn, MI - 20 year old Brenna Machus was found dead in a field three days after being taken from the Dollar General store by someone who attempted to rob the store.  He co-worker, Joseph Orlando was found shot to death at the store by workers arriving to work the next morning.

July 15 - Zanesville, OH - 57 year old Gary L. Henry of Dennison was struck and killed while working along a highway construction area.  He was working behind a concrete barrier when a fully loaded dump truck backed into him.  He was employed by Shelly & Sands Inc. of Zanesville.

July 17 - Homer, AK - 26 year old Brian Mitvitnikoff of Anchorage died on the job from unknown circumstances but while he was walking between two drilling sites.  Co-workers heard a garbled call over the radio and he was found as they went to search for him.  He was employed by Crew 84 at a SAExploration drilling operation.

July 17 - Woodland, WA - 51 year old Timothy Ray Morris was killed on the job when a felled tree struck him.  He was owner of Five Star Logging.  

July 17 - Springfield, MO - Matthew Chwirka died from burns due to a combustible dust flash fire while working for Advanced Environmental Recycling Technologies.

July 18 - Winkler County, TX - Levis Awbry of Kermit died on the job when he was apparently overcome by Hydrogen Sulfide gasses while standing on top of a tank battery while working at an oilfield.   He told a co-worker that he smelled gas and then saw him fall off the tank.  He was employed by Bass Enterprises.

July 18 - Arkansas City, KS - 57 year old Richard Gates of Blackwell, OK died when he was injured inside a cattle pen at Creekstone Farms in Arkansas City while transferring the animals from one pen to another.  He was a truck driver for J. Butler Trucking Company.

July 19 - Lafayette, IN - 50 something Jay Summers was killed on the job when he was struck by a trailer on a construction site at Purdue University campus.  Summers was trying to pick up some concrete forms that had fallen off the trailer when the truck's driver, thinking Summers was out of the way, began to drive away.  He was employed by Fairfield Contractors.

July 20 - Fayetteville, GA - 21 year old Johnathan Siroky was killed on the job when a crane arm fell on him.  As the crane was being dismantled, a strap holding the arm broke and Siroky was pinned between the crane arm and a trailer door.  He was employed with Graphic Packaging International.  

July 22 - Kansas City, MO - 40 year old Donald Fenton died on the job after of toxic exposure in a confined space.  He was working alone repairing a pipe in a sewer when a temporary plug gave way and the space filled up with sewage.  He was an employee for Kansas City Water.

July 22 - Las Vegas, NV - 36 year old David Vanbuskirk died on the job when he fell into a canyon as he was helping to hoist a stranded hiker into a helicopter.  It is unclear how he became detached from the line he was on with the hiker.  He was a member of and employed by Las Vegas Metro's Search and Rescue and was soon to become a Metro police officer.

July 22 - Crenshaw, MS - 56 year old Fire Chief Hubert Joyner died on August 8, after being injured in a building collapse.  As he walked along next to a building after a storm, the roof collapsed and the wall bucked outward with debris striking Joyner.  Along with his injuries he suffered a heart attack while in the hospital. 

July 23 - Omaha, NE - 52 year old Walter Bequette died on the job while working on a roof in West Omaha.  His co-worker realized Bequette's hammer had stopped so went to investigate.  He found him unconscious.  It is unclear who he was employed by.

July 23 - Franklin, OH - 48 year old Terry Ray was killed on the job when he was caught in a moving piece of a pulp dryer while working for Cheney Pulp & Paper Company.

July 24 - Prineville, OR - 27 year old Joshua Lee Ketchem of Redman was killed on the job when a 1,500 pound metal press fell on him.  He was employed by Fontana Wood Products. 

July 25, Vienna, MD - 41 year old Michael Frontiero Cortes of Morovis, Puerto Rico died on the job when he fell approximately 25 feet from a radio-cell tower.  He was employed by Konet Corporation of Puerto Rico.

July 24 - Bridgeport, TX - 50 year old Jose Landeros of Chico was killed on the job from blunt force injuries.  He was welding on the back of an empty tanker truck when it exploded, throwing back several feet.  He was employed by Bridgeport Tank Trucks.

July 26 - Pasco, WA - Steven Matrin of Kennewick died of an unknown cause while working on a concrete finishing job in Pasco. He was employed by Jewell Darrell Edwin. 

July 27 - Bethlehem, PA - 42 year old Robert Rogers III of Orefield, PA was electrocuted while in a bucket truck working to clear trees from power lines.  He was a employed by Asplundh Tree Expert Co., a subcontractor for PPL Utilities. 

July 27 - Thompson Falls, MT - 35 year old Todd Mark Hanawalt of Bozeman was killed when the helicopter he was piloting went down near Thompson Falls.  Reports state he was surveying power lines in a private chopper when it crashed.  Two co-workes also in the chopper were injured.  He was employed with Summit Air Ambulance.

July 27 - Seattle, WA - 50 year old Damien Sluka died of natural causes while working a moving job for National Moving & Storage, Inc.

July 29 - Ripon, CA - 53 year old Joseph Carl Wagner died from injuries he received when he was run over by his farm tractor.  It seems he was caught on a tree branch, causing him to fall from the machine where he was then run over.  He was a self employed almond farmer.  

July 30 - Cleveland, OH - 48 year old Mohammed Ismail was shot to death during a robbery at the Biggie's gas/convenience store he owned.  He died at an area hospital after being shot in the back.

July 30 - Martinsville, VA - 53 year old James Manuel Carter, Jr. was killed on the job when he was struck by a felled tree.  According to the report, Carter was mostly pitching in to help others who had been hired to move a tree in a residential area.  He was, however, working we feel.

July 30 - LaCrosse, WI - 40 year old Eric Lecher died on the job when he some how fell into a cold furnace while performing routine maintenance for Torrance Casting foundry.  

July 30 - Columbus, GA - 53 year old Cpl. Thomas Keith Slay was killed his truck crashed along Veteran's Parkway en route to assist another officer.  Slay was an undercover officer for Metro Narcotics Task Force.

July 30 - Merced, CA - 43 year old Jose Jimenez of Lodi died on the job when he became pinned between a piece of heavy machinery and a concrete barrier while working along a highway construction area.  He was employed by Teichert Construction.

June 12, 2013 -  Swedesboro, NJ - 37 year old Jason Leffler, a well known Joe Gibbs Racing team driver, was killed in a horrific crash of his Sprint car at Bridgeport Speedway in New Jersey. 

August 1, 2013 - Dugout Lake, OR - 60 year old John "Big John" Hammack of Madras was killed when he was struck by the top of a tree which fell while he and another worker were trying to clear an area burned by a wildfire near Dugout Lake.  Hammack was a firefighter working for R & K Water Services of Bonney Lake, WA battling the Incident No. 398 fire in the remote Mt. Washington Wilderness Area.

August 2 - Evansville, IN - 31 year old Stephen Jennings died on the job after being electrocuted and then falling from a second story as he was doing roofing work.  It is unclear who he was employed by.

August 2 - Utica, NY - 56 year old Kelvin Sperry of Herkimer, NY was killed on the job when he was crushed between a dumpster and compactor while working for Oneida-Herkimer Solid Waste Authority.

August 2 - Kenley, NC - 59 year old Mike Sabiston of Troy was killed when he fell from a lift gate of New Day Motorsports hauler as he was helping to unload a car at the infield track before a race at Southern National Motorsports park.  He was a team member of New Day Motorsports.

August 3 - Lumberton, MS - 56 year old Jim Horak, Jr. of Pensacola, FL died when an equipment failure occurred during a tandem skydive near Lumberton with a student, who survived.  Horak was an instructor for Emerald Coast Skydiving Center in Elberta, AL. 

August 3 - Grand Prairie, TX - 59 year old James Anthony Long of Clarksville, TN died in a fiery crash when his semi truck crashed through a guardrail and plummeted to a lower section of the President George Bush Turnpike below.  It is unclear why the truck left the roadway or who his employer was.

August 4 - Abbottsville, PA- 63 year old Kramer Williamson of Palmyra, NY, a Sprint Car race driver, died from injuries he received from a crash during the United Racing Company 358/360 Sprint Car Challenge at Lincoln Speedway.  

August 4 - St. Charles Parish, LA - 41 year old Jeff Watson, an officer for the St. Charles Parish Sheriff's Office, was killed in a car crash as he was responding to a call for assistance from another officer. 

August 5 - Blue Springs, MO - 28 year old Gusteno A. Santamaria of Independence was killed on the job when a four-post vehicle lift tipped over and fell on him as he and other employees were trying to move it.  He was employed by Highline Auto Shop in Blue Springs.

August 5 - Lowville, NY - 55 year old Willard Moser of Deer River was killed on the job when he was struck in the head.  The MJL Crushing employee was trying to free an excavator that was stuck in a piece of machinery when it came loose, striking him in the head. 

August 5 - Stanton, TX - 25 year old David Edward Oliveira of Turlock, CA and 26 year old Christopher Geoffrey Breed of Moscow, ID were killed on the job when they fell from a cable suspended to a helicopter.  Both men, employees for Haverfield Aviation, were working on power lines when the helicopter pilot flew over a finished power line and the power line cut the cable that was holding Oliveira and Breed. 

August 5 - Hettick, IL - 66 year old Ronald D. Releford died at hospital from injuries he received when his farm tractor flipped, pinning him under it.  He lay under the tractor approximately 10 hours before being found by friends who went to find him. 

August 5 - Middletown, OH - 72 year old Charles Groh of Fairfield died of asphyxia when he was buried in grain at an unregulated family owned farm.  OSHA has no jurisdiction because the farm employed less than 10 workers.

August 5 - Casper, WY - 38 year old Jason Thompson of Gillette was killed on the job when he was struck by a dump truck while working at a City of Gillette construction site.  It is unclear who Thompson was working for.  The dump truck driver was arrested for driving impaired.

August 6 - Middletown, OH - 42 year old Tim Taylor of New Carlisle died of asphyxia inside a silo when he became buried under 8,000 pounds of fly ash (used in making cement).  He was employed by Central Ready Mix.   

August 6 - Portland, OR - 19 year old Jesse Trader was killed on the job when the water truck he was driving overturned on an embankment of a rural road.  He was a fire crew member working the Big Windy Complex of fires in Southwest Oregon.  It is unclear who he specifically worked for. 

August 6 - McCracken County, KY - 47 year old Deputy Chad Shaw collapsed and later died of a medical issue while working with school staff as they prepared for the new school year.  He was employed by McCracken County Sheriff's Office.

August 6 - Stamford, CT - 59 year old Francisco Dossantos of Naugatuck was killed on the job when he was struck by a front-end loader as he was walking.  He was employed by O & G Industries.

August 7 - Tyler, TX - Joel DeLeon of Houston died on the job when a Bobcat he was operating backed into a concrete pit full of damp sand.  The machine landed on top of him.  He was a contractor for D.H. Griffin working at Tyler Pipe.

August 7 - Evart, MI - 23 year old Zach Elden Adams was electrocuted as he worked to clear trees after a recent storm.  He was an employee for Trees Inc., contracted to Traverse City Light & Power when he came into contact with a high power electric line.  

August 7 - Waynesboro, VA - 37 year old Payman Biazarikari , only in the United States from Iran for two weeks, died on the job after he fell approximately 160 feet from a tower at nTelos Headquarters.  He was a private contractor working for Summit Tower Construction.

August 7 - Albany, NY - 61 year old Lance Gayton was injured and later died after being struck by a piece of heavy equipment at Youmans Flat garbage transfer station.  He was a New York Parks Department employee working at Bear Mountain State Park.  

August 7 - Milwaukee, WI - 44 year old William "Bill" Melendez, Jr. was shot and killed during a robbery attempt while working his shift at a Family Dollar store on the South side of Milwaukee.

August 8 - Athens, GA - 50 year old Lt. William Morgan collapsed and died of a probable medical event inside the Clarke County Sheriffs Office jail intake facility. 

August 8 - Pharr, TX - 51 year old Sgt. Jorge Luis "JL" Garcia died as the result of a two vehicle crash while en route to a work related meeting.  He was employed with the Pharr Police Department.

August 9 - Beulaville, NC - 54 year old Richard Maready died after being pinned under his farm tractor.  He was attempting to pull metal around a tree stump with a chain when the front end of the tractor came up and rolled over. 

August 9 - Buckeye, AZ - 48 year old Herme Escobar was the target of workplace violence when a co-worker shot and killed him on the farm they worked at.  The co-worker who shot Escobar was arrested on site.

August 11 - Midland, GA - 51 year old Glenn Nunley was volunteering to cut down trees at a local church when he was struck by a tree limb as it fell.

August 12 - Coates, NC - 49 year old John Dailey of Silvester, GA was killed on the job when he fell approximately 200 feet from a cell tower.  He was employed by Transmit PM LLC of Duxbury, MA. 

August 13 - Palatka, FL - 14 year old Blake Bryant was killed on the job when he fell approximately 50 feet from a tree.  Apparently he cut through his safety harness.  He was employed by John Wilkes Tree Service.

August 13 - Minden, MN - 55 year old Robert Honomichi died and a co-worker injured when they were overcome by toxic fumes while working inside a sewer confined space while trying to clear a blockage.  He was employed by Minden City Water & Sewer.

August 14 - Wolf Creek, OR - 60 year old Kathleen Roberta Wilson of Trail was killed on the job when she was struck by an arrow board trailer after the trailer of an 18 wheeler struck the board. She was working the road construction project for Knife River Corporation.  

August 14 - New Prague, MN - 28 year old Robert Fenstermacher of Oologah, OK was killed and a co-worker injured when they fell 50 feet into a hole being dug for a power pole.  The men were tethered to a strap suspending them when it somehow broke, causing the fall.  He was employed by Aldredge Electric of Libertyville, IL on the CAPX2020 transmission line construction project.

August 15 - West Lafayette, OH - 27 year old John E. Roberts of Caldwell, OH died on the job of head injuries after he slipped and fell nearly 50 feet from an oil rig.  The rig is owned by and Roberts was employed with FMS Drilling.

August 15 - Adamsville, TN - 44 year old Joseph S. Jones of Jackson was killed on the job when he lost control of his TN Dept. of Transportation dump truck and it rolled down an embankment and struck a tree.

August 12, 2013 - Thousand Oaks, CA - 51 year old Matthew McKnight, a Los Angeles Fire Department Captain, died on the job while working at the city's dispatch center. It is unclear why he collapsed.

August 16 - Orlando, FL - 63 year old Solange Blain was killed when the car she was driving was struck from behind by another vehicle.  She was a cast member for one of the shows at Walt Disney World.

August 18 - Gary, IN - 28 year old Mihid Abutemeh of Oak Lawn, was shot to death while working as a clerk at a family run convenience store during a robbery attempt. 

August 19 - New Castle, PA - 37 year old Brian Krauss was killed on the job when a fuel storage tank exploded and he suffered head/pelvis injuries as he was thrown from the site of the explosion.  Krauss was a welder and owned Krauss Portable Welding and was doing work for Brownies Oil Co., an operating division of Reed Oil.

August 19 - Missoula, MT - 44 year old Bruce Hall died from burns on August 20th when paint fumes ignited, causing a fire.  Hall was pouring lacquer from a barrel into a smaller container when static electricity caused the lacquer fumes to ignite.  He was employed by Rick's Auto Body.

August 19 - Erving, MA - 77 year old Kenneth Hubbard, Jr. died from injuries sustained in a farming accident.  Hubbard was using draft horses to hay a field, when something startled the horses causing him to fall. He was then dragged a short distance.

August 19 - Turner, ME - 57 year old Manuel Adame was shot while working at Moark Egg Farm.  Another worker was using a gun to shoot rodents and stray chickens to clear an egg house and one of his bullets hit Adame in the neck and chest.

August 19 - Severn, MD - 26 year old Manuel Armando Zuniga-Esquina of Silver Spring died on the job when he was buried to the neck in a trench collapse while working at a home construction project. It is unclear who he was employed by.

August 19 - Dandridge, TN - 31 year old Justin Randolph Bass of Knoxville died after being thrown from an elevated lift.  Bass, a self employed window cleaner, was in the elevated bucket of a lift truck when it became unstable and rolled down a hill, throwing him about 60 feet.

August 19 - Pasco, WA - Alfredo Madrigal collapsed and died on the job while picking beans in a field in Pasco.  He felt ill and stopped for lunch but was later found on the ground by co-workers.  Reports state he died of Cardio myopathy/edema.  He was employed by Willis Schinkelshoek. 

August 20 - McLean, VA - 36 year old Juan Francisco Cema Alfaro of Falls Church died on the job when the backhoe he was operating fell into a large hole and the dirt collapsed around him.  He was part of a four person crew working on a renovation project at a home.  His employer was not listed.

August 20 - Holliston, MA - 26 year old Ryan Hopkins died after he was electrocuted and burned while trimming trees.  He was in an elevated bucket truck when the chainsaw he was using touched a power line. He was a 10 year employee for Ahronian Landscaping and Design.

August 20 - Lometa, TX - 68 year old George William Reed was killed when the dump truck he was driving crossed the center line on a highway and was involved in a three vehicle crash.  He was an employee for Lampasas County.

August 21 - Southbury, CT - 83 year old David Dicks was killed while working on his family farm when he was crushed under his tractor while working in a hay field.

August 21 - Mesa, AZ - An unidentified man in his 50's died after being suffocated under hundreds of pounds of palm fronds while trimming a palm tree. The man had used a belt and spikes to climb the tree when he was caught under the fallen skirt of fronds.

August 22 - Buckeye, KY - 29 year old Bobby Maggard and 25 year old Scotty Elam were killed and three others injured when lightening struck a barn they were working in.  They were working in the barn hanging tobacco leaves to cure when the lightening struck.

August 22 - Plymouth, MN - 34 year old Ryan C. Mastracchio died of electrocution while hanging gutters on a multi-unit residence.  He was on a ladder installing the gutters when he somehow touched a live power line. 

August 23 - Shippenville, PA - 56 year old Kenneth Nichols of Emlenton died when he was crushed by a large piece of equipment at his job at Clarion Boards fiberboard plant.  He was performing maintenance when he became pinned between a roller and the frame of a roller.

August 23 - Eugene, OR - 31 year old David Huynh of Greshem died on the job when the crane he was working from toppled over.  He was employed by North American Power Line Construction Services repairing a cell tower and inside the bucket of the rented crane.

August 24 - Lake Butler, FL - 80 year old Marvin Pritchett, owner of Pritchett Trucking Company, and 28 year old Rolando Gonzales Delgato were shot to death by a former employee/co-worker for an unknown reason.  The victims were at different locations when they were shot.  The former employee also shot 44 year old David Griffis in the stomach at the company's headquarters and was hospitalized but he died on August 28th.  The man also shot another man who survived.  The shooter committed suicide after going on his shooting spree.

August 24 - Escondido, CA - 27 year old Matthew Courtright was killed when he was crushed under an overturned forklift.  He was operating the forklift when it tipped over.  He was employed by Stone Brewing Company.    

August 25 - Fort Worth, TX - 30 year old Sevastian Sanchez died after being struck by a falling chimney at a construction site on the Texas Christian University campus.  He was employed by Zamora Construction.

August 25 - Siskiyou County, CA - Oscar Montano-Garcia of Medford, OR collapsed while on a break with other crew members fighting the Nabob Fire in Northern California. He was a part of the pump operations crew said a spokesman for the U.S. Fire Administration.

August 26 - Columbus, GA - 50 year old James Jackson and 46 year old Allen Thomas died after being trapped in a trench collapse at an apartment complex.  The men worked for Allen Development Group. 

August 26 - Brooklyn, NY - 32 year old Mohammed Bilal died on the job when he was shot to death by a man who entered the New Attitude electronics store he was working at. After shooting Bilal, the man fled on his bike. 

August 26 - Woodbury, NJ - 40 year old Surinder Singh was killed on the job when he was shot to death by robbery suspects.  He was working at the Garden State Fuel gas station.

August 26 - Terre Haute, IN - 44 year old Kenneth Waller was found dead of massive head trauma in the basement of a building under construction.  His family found him after they did not hear from him for many hours after work.   He was last seen going to check windows above the place where he was found.

August 27 - Livermore, CA - 62 year old Oscar Marquez was struck and killed on the job by an asphalt truck that was laying new asphalt on a track at a middle school in North Berkley.  He was employed by Robert A. Bothman Construction, Inc. 

August 27 - Bakersfield, CA - 49 year old Alejandro Hernandez died on the job from electrocution while working at a Grimway Farms facility. He was shocked while working on a junction box in a cooler unit.  

August 28 - Burlington, NC - An unidentified utility worker was struck and killed and a co-worker injured while they worked along a highway near Wilmington.  The workers were installing fiber optic lines. 

August 28 - Lake Butler, FL - 44 year old David Griffis was shot on August 24 when an ex co-worker went on a shooting spree, killing two others, including the owner, of Pritchett Trucking.  He had been in critical condition until his death on the 28th.  The ex employee also shot another person who was not seriously injured. It remains unclear why the man sought out the individuals he did. 

August 28 - Cantonsville, MD - 36 year old Officer Jason Schneider was shot to death by a suspect during an early morning raid on a home in Cantonsville.  The alleged assailant was also killed.  Schneider was an officer for the Baltimore County Police Dept.

August 29 - Shady Grove, PA - Rickie Lee Grimsley died on the job when he was pinned between two cranes. He was standing between a parked crane and one that was being moved by another employee. The crane turned too sharply to the right and trapped him between the two cranes.  He was employed by Manitowoc Crane Group. 

August 29 - Jefferson County, OH - 31 year old Jesse Grimsley from Minnesota was killed when the piece of equipment he was driving rolled over and down an embankment. He attempted to jump off the vehicle but was caught on the track.  He was working to install a gas pipeline for Sheehan Pipeline Construction.

August 29 - Quapaw, OK - 48 year old James Carl Wivell of Longmont was killed on the job when his tractor trailer rig was involved in a highway crash.  The driver of a car was fleeing police after a bank robbery and attempted to pass Wivell's semi.  Due to another car coming head on, the car's driver tried to go in front of the semi but clipped the front of the truck causing the crash.

August 29 - Sleepy Eye, MN - 77 year old Donald George Richert died from burns related to a farming incident.  He was using a skid loader near a burn pit and when he got off the machine to move something the loader moved forward, pinning his leg and trapping him in the pit.

August 29 - Alliance, NE - 46 year old Juan Vasquez of Hemingford was killed on the job when he was crushed under a large truck axle.  He was working alone at the time and was discovered by his employer.  It is unknown who he was employed by.

August 29 - Cathedral City, CA - 18 year old Arthur Plata was killed and his co-worker injured when he was struck and pinned by a vehicle as they worked near a trailer attached to a truck.  A motorist struck the trailer which caused Plata's death. Plata was a landscaper but it is not clear who he was employed by.

August 30 - Andover, NY - 79 year old Douglas Wightman of Bolivar was killed on the job when oil rigging equipment fell onto him.  Workers were servicing a hydraulic well when pulleys and cables on its mast snapped and fell onto Wightman.  It is unclear who he worked for. 

August 30 - New Castle, PA - 34 year old Justin Callahan of New Wilmington died on the job of crushing injuries when the commercial mower he was operating tipped over and roll on top of him.  He was working for an unnamed landscape company.

August 31 - Irondale, AL - 24 year old Mike Isaac Alvarenga Carlos of Texas died on the job when he fell while painting a water tower.  Authorities say he was climbing a ladder inside the drained water tank when he fell.  His employer is not listed.

August 31 - Port Hudson, LA - 53 year old Joseph "Leon" Boudreaux was killed on the job after he apparently fell from a forklift he was operating.  He was working alone at the Georgia-Pacific Port Hudson Operations warehouse when he was found.  

August 31 - Belleville, IL - 80 year old Frank Antoine was a long time volunteer for Althoff Catholic High School.  He was part of the football team's "chain gang", a crew that uses a chain to measure downs on the field.  Mr. Antoine fell and struck his head while working with the crew at the Friday night football game.

September 2, 2013 - Newark, NJ - 20 year old Jesus Torres was murdered, shot to death, while making a pizza delivery for Pizza Hut.  His car then struck a fence. It is unclear what the motive for his murder was.

Sept 2 - Woodlawn, IL - 70 year old Alan Dempsey was killed while working on his farm.  He was mowing hay when he stopped the tractor on a hillside and attempted to get off the machine.  The tractor started to roll and as Dempsey tried to get back on it, he was struck by one of the rear tires.

Sept 3 - Marion, NC - 47 year old Rafael Magana was killed on the job when he was struck by a dump truck. He was sweeping the road on a construction site at West McDowell Junior High when the driver of the dump truck was backing down a hill when he struck Magana.  Both worked for Baker Grading and Landscaping.

Sept 3 - Greensboro, NC - 65 year old James "Lemmie" White, Jr. of Stoneville was killed while working when the loaded dump truck he was driving went off the roadway and overturned.  He was employed by Thomas Stanley Grading and Hauling, Inc.

Sept 2 - Clarkson, NE - 77 year old George Baumert who lived near Howells, was struck by and killed by a tractor while working on his farm.  He was working alone at the time and was later found by a neighbor.

Sept 4 - Bristow, OK - Kyle Polkingham, in his 20's and an oil field worker was killed and a co-worker injured when they were working to stop a leaking tank battery and an explosion and massive fire occurred. 

Sept 6 - Santa Fe, NM - 41 year old Token Adams was found dead after a lengthy search when he went missing while investigating a small wildfire.  An engine captain for the Jemez Ranger District, Adams left on an all terrain vehicle with two others but separated at some point.  He was found later by a Native American Search team.  Authorities say the ATV crashed.  

Sept 6 - Sandy, OR - 33 year old Dewayne Douglas Smith of Hood River was killed on the job, and several co-workers were injured, when wooden building trusses that were being set in place fell striking Smith in the head.  He and the others were employed with Stalcup Roofing and Construction.

Sept 8 - Winfield, KS - 58 year old Roger Chattam died on the job when the tandem semi grain truck he was driving overturned.  It is unclear the cause of the truck rollover or who he was employed with. 

Sept 9 - Swoyersville, PA - John Brdaric was killed on the job when part of a church that was being demolished crashed down onto the excavator he was operating.  Brdaric was the owner of Brdaric Excavating. 

Sept 9 - Mason County, MI - Michigan State Trooper Paul Butterfield was killed on the job when he was shot during a traffic stop.

Sept 9 - McVille, ND - 50 year old Ernest Brager of Cooperstown was killed on the job when the garbage truck he was riding in went off the roadway and rolled.  The vehicle was driven by a 19 year old driver.  It is not clear who the men worked for. 

Sept 9 - Shelton, CT - 58 year old Glenn Farr died on the job of asphyxia from chest compression when he was crushed underneath a snowplow truck.  It is unclear why he was under the piece of equipment.  Farr was the owner of GFW Welding Company. 

Sept 10 - St. Paul, MN - 61 year old Johnny Valek was killed on the job when a huge slab of concrete fell onto him as he operated heavy equipment during demolition at a construction site.  Valek worked for Rachel Contracting, a subcontractor for Ryan Companies. 

Sept 10 - Wendell, ID - 43 year old Jared Durfee died on the job when he was buried in a trench collapse.  He was inside a trench using a jackhammer when the walls gave way and collapsed around him.  He was employed by Wendell City Department of Public Works.

Sept 11 - Newman, CA - 49 year old Kenneth Canton was killed on the job when the farm equipment (resembling a forklift) he was driving went off the roadway, down an embankment and rolled.  It is unclear if he was a self employed farmer or worked for another. 

Sept 11 - Lyford, TX - 67 year old Jose Isais Leal died on the job when he apparently fell from a tractor he was operating and ran over by the machine.  He was a long time employee for Lyford Gin Association.

Sept 12 - Chicago, IL - 48 year old Martin Marino was killed on the job when he was trapped under a pavement grinding machine.  The equipment he was using was marked as belonging to G.A. Paving Co. of Bellwood and were doing work for Peoples Gas. 

Sept 15 - Chicago, IL - 47 year old Raul Lagunas died after being pinned between two vehicle while working on a construction site.  His employer is unnamed.

September 6, 2013 - Desert Hot Springs, CA - Victor Aguilar Gonzales of Compton was fatally electrocuted while working at Desert Springs Elementary School.  He was employed by Celco Construction.

Sept 7 - Auburn, WA - 40 year old Erica Meier died of a blood clot in her lung that dislodged after she had surgery for a work related broken ankle.  She was employed by College Success Foundation.

Sept 10 - Northville, ND - 48 year old Adan Sotelo Preciado of Gillette, WY died on the job after falling through a roof while working on a warehouse in Northville.  It is unclear who he was employed by.

Sept 15 - Pembroke, NY - 54 year old Kevin Doktor was working on his farm when he somehow became pinned under a skid steer bucket loader. 

Sept 15 - Chicago, IL - 47 year old Raul Lagunas was killed on the job when he was pinned between two construction vehicles.  It is unclear how the incident happened of who he worked for.

Sept 17 - Shenyang, China - Dave Riggs of Nevada was killed in a plane crash into a lake while practicing for a stunt for an air show in China.  It appears he was self employed.

Sept 18 - Avondale, IL - Gustavo Briceno died on the job when he drown and was swept away while working in a sewer pipe.  He was lining a sewer with styrene plastic, when storm water swept him away.  He was employed by Kenny Construction.

Sept 18 - Mount Vernon, IN - 33 year old Brandin Levi Almon died in a tower related incident while working for Design Build Technology.  

Sept 19 - Broomall, PA - 40 year old Jason McClay was shot to death during a robbery of the Rite Aid drug store where he was manager. 

Sept 20 - Ventura, CA - 62 year old Nicholas Rojas of Fillmore, CA died on the job when he fell from the tree he was trimming after a branch broke.  He was working at Golf n Stuff, its a miniature golf course in Ventura.

Sept 21 - Laguna Beach, CA - 42 year old Jon Coutchie, a Laguna Beach Police officer, died after his motorcycle collided with a truck as he was responding to a call about a speeding motorist. 

Sept 22 - Renville, MN - 58 year old Jeffrey H. Muetzel died in a farm related incident.  He was trapped under a corn head that fell off a combine when he was repairing it.

Sept 22 - St. Cloud, MN - 56 year old Lee Teske, a self employed window washer, was killed on the job when he fell 70 feet while working at the Kelly Inn hotel. 

Sept 23 - Danville, KY - Michael and Angela Hockensmith, owner of a downtown pawn shop, were victims of murder when they were shot by a robbery suspect.  A salesman for an unknown company, Daniel P. Smith of Richmond was also murdered.

Sept 23 - Bigfork, MT -  60 year old Peter Mahlum died while on a class project at Flathead Community College in Kalispell.  He fell backwards from a scaffold about eight feet, and into a 55 gallon barrel.  He was a student of the Building Trades department of the college.

Sept 23 - Long Island, NY - 45 year old Steven Giacobello was struck and killed while working a sanitation route in Oyster Bay.  He had just finished emptying trash into his truck's container when a motorist struck him.  He was employed by the Town of Oyster Bay. 

Sept 23 - Middletown, DE - Jerry Hurley of Port Deposit, MD died on the job when he was buried in a trench collapse.  He was operating an excavator in a trench for a sewer line.  He got off the machine and the ground collapsed under him.  He was employed by Austin & Bednash.

Sept 24 - Wentworth, SD - 58 year old David Feige was killed when he was crushed under a piece of farm machinery.  He was working on a corn harvester when the jacks failed and the machine fell on him.

Sept 24 - Sioux Rapids, IA - 64 year old Lawrence "Larry" Vaughn died of thermal burns after he fell into a burn pit on a farmstead and could not get out of it.

Sept 25 - Buckley, WA - Robin Wills died on the job after suffering a medical event that then caused the vehicle to go out of control and flip over an embankment while working for Derek Alan Muirhead dba Buckley Farm Fresh Produce.

Sept 25 - Beaver, WI - 58 year old Alice E. Fendryk was killed in an unknown incident while working on a farm. No other details are provided. 

Sept 25 - Cherokee, IA - 52 year old Marc Sarchet was killed when the crane truck he was driving crashed.  Sarchet lost control of the truck after a tire blew out, causing a rollover.

Sept 25 - Wilder, KY - 53 year old Robert Allgood of Leesburg, OH died while working when he lost his balance, falling about 25 feet while working on a construction site.  He was installing insulation for Metal Systems of Milford.

Sept 25 - Myerstown, PA - 17 year old David Swarey died in a tragic farming incident.  He was feeding corn into the chopper of a corn harvester machine when he was somehow pulled into the machine.  It is unclear if he was working his family farm or for another farmer. 

Sept 26 - Gaffney, NC - 24 year old Kenneth Robert Clary of Kings Mountain died on the job when he fell from an elevated lift.  He was operating a picker lift, retrieving merchandise, when his lift was struck by another lift operator causing him to fall.  He was employed by Automated Distribution Services - ADS - in Gaffney. 

Sept 26 - Seattle, WA - 59 year old Thomas Mordy was found unresponsive on the floor of the Washington State Convention Center Public FAC, where he worked.  It was determined that he died of a heart related medical event.

Sept 26 - Gerber, CA - 49 year old Danny Holman of Red Bluff, a Tehama County Public Works Department employee, suffered fatal injuries on the job when he was unloading equipment for road maintenance work. 

Sept 26 - Boise, ID - 63 year old Captain Henry Skillern of Humble, TX suffered a mid-flight heart attack while on a flight from Houston to Seattle.  The United Airlines plane made an emergency landing in Boise.  He later died at an area hospital.

Sept 26 - Tama, IA - 27 year old Christopher Wheeler was killed on the job when he was struck by a semi truck driver who did not slow sufficiently in a road construction work zone.  Wheeler was a flagger for the project but it is unknown who he was employed with.

Sept 26 - Poplar Bluff, MO - 29 year old Gregory "Keith" Robertson died after being pinned under water in a lake under heavy equipment.  Robertson owned Robertson Construction Company and was operating an excavator near the bank of Roxy Lake when the bank collapsed, causing the excavator to roll into the lake.

Sept 26 - South Holland, IL - 54 year old Richard Cowser was working as a security officer for Thornwood High School when he suffered injuries on the job.  Cowser and another guard were attempting to break up a fight on campus when he fell into a locker.  He had trouble breathing and was taken to hospital where he later died.  His exact cause of death is unknown.

Sept 26 - Coal Creek Canyon, CO - 46 year old Ubaldo Ruiz Marquez of Commerce City was killed when he was struck by his work vehicle.  He was on a work site doing flood repair work when his truck experienced an issue.  He exited the truck to check the problem, and it rolled backwards into him. It is unclear who he was working for.

Sept 27 - Indianapolis, IN - 25 year old Mitchell Ray Morgan was killed on the job when the crane he was using to lift a generator fell over due to the weight of the generator.  He was an employee for B-MAC Wireless.  (Second story on page)

Sept 27 - Austin, TX - 26 year old Jeremy Brugman died on the job when he fell while framing a home in the Villa Montana Community near Lake Travis.  It is unclear who he was working for.

Sept 27 - Lake Charles, LA - 53 year old Ronald J. Harris, Sr., Pastor of Tabernacle of Praise Worship Center was shot to death during a worship service.  A known suspect walked into the church during the service and shot Mr. Harris at close range.  The murder suspect was arrested.

Sept 27 - Boise, ID - 40 year old Mark Urban died in a parachute training exercise.  An Idaho smokejumper for the US Bureau of Land Management, Urban jumped out of a plane and something went very wrong.  It is unclear if his equipment malfunctioned or there was a weather issue. 

Sept 27 - Paragould, AR - 53 year old Donald Wayne Robertson died on the job when he was crushed under a double wide mobile home.  Robertson was attempting to set up the mobile home when it collapsed on top of him.  He was employed by Affordable Home Super Center of Blytheville.

Sept 27 - Bloomsburg, PA - 69 year old Carl R. Enlow, a helicopter pilot for Heritage Rotors of Port Orange, FL, was killed when he was struck in the head by the aircraft's rotor.  He was operating helicopter rides at the Bloomsburg Fair in Pennsylvania. 

Sept 29 - Santa Monica, CA - 63 year old Mark Benjamin and his 28 year old son Luke Benjamin died in the fiery crash of their corporate jet into a hanger.  The senior Benjamin was the CEO for Morley Construction Company.  It is not clear why the plane crashed or who was the pilot.

Sept 30 - Wilmington, DE - 43 year old Melvin E. Johnson, Jr. of Baltimore, MD was crushed under a large piece of equipment in Newcastle County.  He was attempting to load or unload a large piece of computer equipment when he lost his footing and the equipment fell onto him.  It is unclear who he was working for.

Sept 30 - Shakopee, MN - Jorge Ortega of Laredo, TX was working with a young horse on Sept. 12th at the Canterbury (horse) Park as a seasonal worker when the horse reared up, pinning him against a wall. He was hospitalized and died on Sept 30.

Sept 30 - Huntington Harbour, CA - 28 year old Brandon Orozco of Whittier was electrocuted as he worked inside an underground vault that suddenly became energized and then started on fire.  He was working with a crew for CAM Contractors hired by Southern California Edison.

October 1, 2013 - Huntley, IL - 59 year old Marco O. Aguilar of Steamwood was killed on the job as he worked for McHenry County Conservation District's Pleasant Valley Conservation Area.  He was working inside a maintenance building when he was struck by a vehicle being prepared to work on a concrete floor.

October 1 - South Bend, WA - Alfred Piercy died of natural causes related to the heart while he worked for J A Jack and Sons.  He was found unconscious inside the cab of his work truck.

October 1 - Syracuse, NY - 38 year old Shqair Shqair (pronounced shur-kahr) was shot during a robbery at Sabatino's Pizza restaurant that he owned along with his wife.   He died later at hospital.

October 2 - Lewistown, PA - 36 year old Randell A. Wert of Newville and 71 year old Kermit F. Wells of Newburg died on the job of head trauma when they were struck by a falling tree they were cutting.  Both were loggers but it is not clear who they were working for.

October 2 - Carrollton, TX - 48 year old Todd Edward Fenton was found shot to death while working at the bus barn for Carrollton-Farmers Branch Independent Schoot District (CRBISD) and was discovered by another employee.  He was supervisor of landscaping and maintenance for the district. It appears to be a case of workplace violence as police are seeking a co-worker for Fenton's murder. 

October 2 - Odessa, MO - 53 year old Michael D. Ray died on the job when the farm tractor he was driving rolled over and onto him.  The incident happened as he was driving across a dam embankment.

October 3 - Center Point, IA - 56 year old Roger Bach died while working in the feed lot of a farm.  The incident involved a skit loader machine but no details are given.  

October 3 - Newton, IA - 42 year old Thaddeus W. Hummel died after he was crushed under a piece of equipment at a salvage yard.  He was contracted by Car County Auto Salvage to work on a boom truck and somehow the arm of the boom fell onto him. 

October 3 - Solebury, PA - 64 year old Joseph Matejik suffered fatal crushing injuries when he was trapped between a forklift and tractor-trailer. He was using the forklift to unload a delivery on the tractor-trailer but after he got off the forklift, it moved forward, pinning him against the other.

October 4 - Brewer, ME - 50 year old Michael Campbell died on the job after he fell 50 feet from a pine tree he was working in.  His climbing line became entangled in the top portion of the tree that he had cut, and he was pulled down as the section fell. He was working for Campbell's Tree Service. 

October 4 - Collierville, TN - 42 year old John Shelley was working on a vacant home when he was robbed and shot death.  He was self employed.  This report states three teens have been arrested and charged with his murder. 

October 5 - Port Lavaca, TX - 21 year old Cody Cowan was killed when the wrecker truck he was driving went off the roadway after he lost control of the truck.  He was self employed with Cody's Wrecker Service. 

October 6 - Washington, DC - Melvin Lewis died on the job of asphyxiation after a toxic exposure to Freon gas while working in a mechanical room on an air conditioner at MedStar Washington Hospital Center in DC.  Four other workers were taken to the emergency room due to the same exposure. 

October 6 - Washington, DC - 41 year old Harold Ingram of Richmond, VA was killed on the job and other injured, when he was struck by a large piece of track rail that was dislodged after an explosion inside the Red Line tunnel near Union Station during maintenance of the track system.  He was an employee for the Illinois based Holland Company, who was contracted by the transit authority.

October 6 - Hansen, ID - 58 year old Salvador Centeno of Kimberly died at hospital after the farm tractor he was driving was struck from behind by the driver of a truck pulling a livestock trailer. 

October 6 - Paducah, TX - 34 year old Joshua Fitts of Hurricane, UT, a helicopter pilot, was killed when his chopper crashed after the tail rotor hit pulley equipment while trying to connect to a power line.  His employer is not named.

October 7 - Dania Beach, FL - 53 year old Joseph Frantz was killed on the job when the taxi he was driving collided with a pick up truck that caused a horrific accident.  He was employed by Friendly Cab. 

October 7 - Dallas, TX - 33 year old Antral Franklin, of Balch Springs and the manager for Williams Chicken eatery, was shot to death by an employee who he had earlier reprimanded for being late for his work shift.  The suspect, who has been arrested, left the restaurant, only to return later with a gun.

October 7 - Corona, CA - 34 year old Rolondo Anaya of Anaheim was killed on the job while working for RJ Noble Company in Corona.  He was caught in a conveyor belt and pulled into an asphalt grinder machine.

October 7 - Murraryville, IL - 69 year old Gary Clark died after an incident while working on a grain harvest.  He was working for an unknown trucking company and fell from the back of a trailer that was taking in grain from a combine.  After falling, he got up and walked around the truck before collapsing.  

October 7 - Harvey, ND - 21 year old Alex Rall was killed on the job when he was struck on the head by a falling piece of equipment.  He was working for Hope Electric at Prairie Towers Grain Elevator.  A crew was using a crane to lift a distributor unit when a piece of the metal grain chute fell, striking Rall as he was leaving the job site for the night. 

October 8 - Eastchester, NY - 42 year old Nicholas Farella suffered fatal crushing injuries he received on the job.  He was riding on the back of a Eastchester Department of Public Works garbage truck when the truck driver backed up striking a utility pole and Farella was pinned against it. 

October 8 - Gilman, IL - 47 year old Vaughn Borchers was killed while working on his farm when he was somehow pulled into the corn head of his combine machine. 

October 8 - Edinburg, NY - 69 year old Richard LaPort died after being struck and run over by a truck driven by his boss.  He was working along the shoulder of a recently paved roadway when his boss backed his truck up, striking LaPort.  He was employed by Saratogo County Town of Edinburg Highway Department.

October 8 - Florence, WI - 43 year old Dean G. Larsen was electrocuted while working on a power line in Fern, WI.  He was employed by Florence Utilities. 

October 8 - Alabaster, AL - 42 year old Samuel Edward Lowe was killed on the job when a backhoe he was operating tumbled down an embankment.  He was employed by a contractor working for Alabama Power and was using the backhoe to clear cut and push debris when it rolled down the embankment, crushing him.

October 8 - Millersburg, OH - 56 year old Paul Schweitzer of Massillon died after being involved in a work site incident involving a spring loading machine while working for Wayne Dalton's Mount Hope plant.  No other details are provided. 

October 9 - Washington Parish, LA - 55 year old Hillary Owens of Tickfaw was killed on the job when he was crushed under a heavy metal loading ramp.  It appears he was a logging, helping to unload logging equipment when the loading ramp fell onto him. 

October 9 - Andrews, SC - 24 year old Donald Scott has died after being injured weeks ago due to an explosion inside an underground tank of a gas station near Boston.  He and another man were re-lining the tank when something caused the explosion.  Scott and the coworker were employed by Tank Tech out of Missouri.

October 9 - Mt. Pleasant, TX - 53 year old Curtis Russell Guest of Pittsburg, TX was electrocuted while working on electrical equipment at Priefert Ranch Manufacturing plant in Mt. Pleasant.  

October 9 - Killdeer, ND - 25 year old Ryan Lee Provancher of Dickinson died of toxic gas exposure while working at an oil pumping site near Killdeer.  He and another man were working to replace pipes inside a building when one of the pipes broke loose sending out a cloud of hydrogen sulfide gas.  The co-worker escaped but realized Provancher had not followed him out of the building.  He died later at an area hospital. 

October 10 - Piedmont, MO - 38 year old Clinton Gary Million died after being crushed under a bobcat that overturned.  He was operating the bobcat while working for German's Recycling Center.  It is unclear how the machine overturned.

October 10 - Main Pass, LA - 47 year old Patrick Becnel of Palquemines died in the crash of a helicopter he was flying.  Becnel, working for Panther Helicopters, was transporting offshore oil rig workers who were off shift when it crashed on take off. Two of the rig workers were injured in the crash. Becnel was also owned Becnel's Citrus Company. 

October 11 - Springfield, MO - 62 year old John Bradford, an elephant handler for the Dickerson Park Zoo, was fatally injured by an elephant he was attempting to move into a chute.  The animal, a little skiddish, pushed into Bradford as it entered the chute, crushing him.  The zoo is not blaming Patience, the elephant, for Bradford's death.

October 11 - Humboldt, IA - 81 year old harold Henry Hinners died after his farm tractor was struck by a semi truck.  As Hinners was attempting to make a turn on a road, the semi driver attempted to pass him and the two vehicles collided.

October 11 - Mer Rouge, LA - Joseph Hill, of Port Allen died on the job after he fell into an auger at Big River Rice and Grain Plant in Mer Rouge. 

October 12 - Los Alamos, CA - 36 year old Roberto Garcia of Bakersfield died on the job when a large platform extension fell at an oil field site, although it is unclear how he was injured. He was a roughneck for Key Energy Services.

October 13 - Phoenix, AZ - 22 year old Richard Allen Bankston, a maintenance worker for Nueva Vista Apartments, was attacked with an object by a resident of the apartments.  The motive is not clear but the suspect and Bankston got into a physical argument and Bankston was found in the pool.  He later died of his injuries at hospital.  The suspect was later arrested.

October 14 - Santa Clara, CA - 60 year old Edward Erving Lake III of Vacaville was crushed under a bundle of rebar that was being unloaded from his truck.  He died later at hospital.  He was working for Gerdau Ameristeel's Napa Reinforcing Steel facility and was making the delivery to the under construction Levi Stadium. 

October 14 - Glenwood, MN - 37 year old Trevor Pederson died on the job when he was crushed under a large cargo dolly that he was building at Clyde's Machine's. 

October 15 - Moses Lake, WA - 34 year old Steve Weil of Quincy was killed on the job as he was working for his employer, Simplot Soilbuilders, to prepare soil at Friehe Farms.  He walked into a field and attempted to climb on board a tractor driven by another man when he fell and was run over by the rear tire of the machine.

October 15 - Las Vegas, NV - 64 year old James Calvin Perkins was killed on the job when the concrete truck he was driving crashed and burst into flames.  Perkins, employed by Nevada Ready Mix, ran into orange construction cones causing it to rolled onto concrete barriers and catch on fire.  He died of carbon monoxide intoxication.

October 15 - Thorp, WA - 37 year old Shawn Tolbert of Cle Elum was killed on the job when he was pinned between the backhoe he had been operating and its stabilizer arm. It appears he dismounted the machine, may have put a chunk of metal on the foot pedal of the machine and the boom arm swung to the side, pinning him.  He was employed by Level Best, Inc.

August 15, 2013 - Boston, MA - 61 year old Yogambigai Pasupathipillai, an immigrant from Sri Lanka, died on the job after being strangled by her apron as it was caught in a conveyor belt. She was working for Piantedosi Bakery Company. 

September 3, 2013 - Woodinville, WA - 49 year old Stewart Lacy of Monroe died when his work vehicle rolled while trying to negotiate a curve.  He was employed by Westfire Coastal Inc.

September 19 - Death Valley, CA - 64 year old Chuck Caha of Pahrump, NV died after suffering from heat exhaustion while working for Death Valley National Park.  In a series of events surrounding his work, he collapsed after attempting walk in the triple digit heat to return to his truck.

October 7, 2013 - Moses Lake, WA - 37 year old Robert Valdez, a truck driver, was killed in a collision between his potato truck and another semi truck.  He was employed by Benito M. Ozuna.

October 9 - Olympia, WA - Benjamin Lopez Santana, from Puerto Rico, was found unresponsive and may have died of natural causes while working for Factory Sales & Engineering Inc.

October 9 - North Andover, MA - 51 year old Carlos A. Amarel of Peabody died on the job when an explosion occurred in a lab at Dow Chemical in North Andover.  A chemical came into contact with air which caused the explosion.

October 14 - Herndon, MD - 38 year old Pablo Aquino of Hyattsville, MD, was killed on the job when he fell from the roof of a building in Herndon.  He was employed by Orndorff & Spaid, Inc., a roofing company.

October 16 - Kirkwood, PA - An unidentified 35 year old man died of asphyxia due to drowning while farming in Colerain Township.  Further attempts to locate his name were unsuccessful.

October 17 - Wapokoneta, OH - 39 year old John Austin of Limawas killed on the job when his dump truck struck an overpass on Interstate 75.  Reports state the empty bed of his truck was up and was what struck the overpass. His employer is unknown. 

October 17 - Draper, UT - 52 year old Camacho Guillermo Borcequin was killed on the job from crushing injuries when he was pinned between a loader machine and a trailer.  He was employed by Smith & Sons Meat Packing Company. 

October 17 - East Amwell Twp, PA - 35 year old James Schulte of Hellertown died of electrocution while working as a sub-contractor for Comcast.  He was employed by FCS Inc.

October 17 - Arlington, WA - Daniel Hall was killed on the job when he was struck in the head by a tree snag that fell onto him while working on the Springboard fire containment. He was working for Washington Department of Natural Resources.

October 17 - Cedar Springs, GA - 39 year old Shannon Jones of Bainbridge, died of unknown injuries while working for Georgia Pacific Cedar Springs mill.

October 18 - Butte, MT - 46 year old Jay F. Bickford died on the job from injuries due to an elevated lift that tipped over.  He was in the bucket of the lift as it was being loaded onto a flatbed trailer when it tipped. He was employed by Big Sky Rental.

October 19 - Oakland, CA - 58 year old Christopher Sheppard of Hayward was a Bay Area Rapid Transit Senior Track Manager and 66 year old Larry Daniels of Fair Oaks & Oakland, and a railroad consulting engineer were both killed when struck by a four car train while inspecting tracks.

October 19 - Land O' Lakes, FL - 62 year old Marc Curto of Tampa died when he fell from a plane on which he was wing-walking. After falling from the wing, his parachute failed to deploy.  It is unclear who he was working for or with.

October 19 - Detroit, MI - 37 year old Officer Patrick Hill was accidentally shot in the head by a fellow officer during a confrontation with a suspect on April 2 and passed away from those injuries on October 19. 

October 20 - Robersonville, NC - 27 year old Felix Hernandez Sanchez of Greenville was fatally electrocuted while working under a home. This report states he was not working for a specific employer but was a construction worker.

October 20 - New Orleans, LA - An unidentified man, a crew member for Carnival Cruise Lines, was doing exterior maintenance on Carnival's Conquest ship when a cherry picker type machine he was on pinned him between it and a platform holding a lifeboat. Attempts to identify the worker were unsuccessful.

October 21 - Sparks, NV - 45 year old Michael Landsberry died after being shot by a middle school child who brought a weapon to school and injured other students. Landsberry, a math teacher at Sparks Middle School, attempted to stop the child from shooting more but was instead shot.  The students survived.

October 21 - Maplewood, MN - 34 year old Cory Lewellin of Olgilvie died when the crane he was operating tipped over, trapping him in the cab.  He was operating the crane for Truck Crane Service of Eagan. 

October 21 - Aiken, SC - 34 year old Jorge Rodriguez was killed on the job when he was struck in the head by a piece of falling iron while working with steel beams.  He was employed by AGY plant in Aiken.

October 21 - Bellville, OH - 55 year old Clifford Smith suffered fatal electrocution while working inside a utility room on a redevelopment project.  He was employed by Romanoff Electric.

October 21 - Oxon Hill, MD - 27 year old Jesse Alexis Chavis of Fort Washington attempted to stop an armed robbery at the Clarion Hotel where he was restaurant manger and was shot by the suspect. Chavis heard a desk clerk scream as the suspect confronted her and tried to help when he was shot.

October 21 - Norfolk, NE - 53 year old Matthew Winkelbauer died of toxic exposure and suffocation when he was buried under a large pile of silage in an open silage pit owned by Fore-Quarters Feedlot. It is unclear how the incident took place.

October 21 - North Wilksboro, NC - 32 year old Luiz Alberto Corado died on the job of a head injury when he was struck by a falling brick.  He was with a crew from Martin Roofing working to replace a roof when a brick from an adjacent building fell as he was standing on the ground.

October 22 - San Antonio, TX - 54 year old Hector Castro, Jr. was suffered injuries due to a fall from a pole on October 1st and seemed to be recovering.  Unfortunately he died from his injuries on October 22.  He was employed by city owned CPS Energy as a lineman.

October 23 - Mount Pleasant, NC - 56 year old Jack Gordon Eller died on the job after being struck in the head by a dead tree that fell. He owned the unnamed logging company doing this particular work. 

October 23 - Cleveland, TX - 55 year old Oscar Alfaro was killed while working along railroad tracks for Bayou City Railroad. He was using a tractor to move debris and had exited the tractor to throw some debris into the tractor. Some of the debris hit the controls of the tractor causing the bucket boom to swing around, pinning his head between it and the tractor.

October 24 - Rolla, MO - 51 year old Rodney Allen "Rocky" Stumo died on the job while working for St. Louis Bridge Construction Co.  He was on a train trestle pier crossing the river on a johnboat when he did not see or hear a barge approaching.  The barge collided with the johnboat and was drowned.

October 24 - Lee's Summit, MO - 49 year old Brian D. Allen of Windsor died on the job when he was buried in a trench collapse. He was working for Strate Plumbing of Holden, digging the trench to connect a sewer line for home construction.

October 24 - Sanford, FL - 36 year old Christopher Putfark of Deltona was killed on the job when he was partially crushed under a gravel filled rail car that derailed while being moved in a rail yard. He was a spotter employed by Conrad Yelvington Distributors, Inc.

October 24 - Oklahoma City, OK - An unidentified worker at Arch Granite & Cabinetry was killed when a forklift tipped over, trapping the worker under the extended lift.

October 25 - Atlanta, GA - 53 year old Keith Fenton of Douglasville died of injuries sustained in a fall from a billboard. He owned the company making repairs to the sign and was carrying a piece of metal and tripped before he could tie off his fall protection.

October 25 - Cape Ann, ME - David Oaks, a commercial fisherman from South Thomaston, drowned after the boat he was on with another man began taking on water and they had to abandon the vessel.

October 25 - Ocala, FL - 50 year old Charles Howard died after falling through a metal roof while making roof repairs on a business.  He was a subcontractor for Central State Enterprises.

October 25 - Benton, LA - 46 year old Ronald Eugene Davis of Arcadia died of injuries he received in a 50' fall from a tree he was trimming.  He inadvertently cut the tree below where he was tied and when the top of the tree fell, he also fell with it.  He was employed by Asplundh.

October 28 - Belton, SC - 35 year old Robert Morgan of Six Mile died after being struck by a felled tree that bounced up, striking Morgan.  He was a long time logger employed by Southern Choppers Logging of Easley.  

October 28 - Belton, SC - 45 year old Sandriss Gilbert, Jr. of Starr died after being struck by a felled tree that first struck a smaller tree, bouncing back to strike Gilbert with the base. He was working with another logger for his uncle's company, Gilbert Tree & Landscape.

October 29 - Peabody, KS - 82 year old Eldon Andres was working on a farm tractor when it lurched forward, knocking him to the ground and rolling on top of him. 

October 29 - Wendover, NV - 53 year old Pita Asiata of Bountiful, UT was killed on the job when he was impaled by a drilling device after the tour bus he was driving ran into the back of a utility truck.  The utility truck was slowing along the highway to enter a median turnaround when the incident happened. Asiata was employed by Le Bus Transportation Co. and was heading from Wendover back to Salt Lake City.

October 29 - Pontiac, IL - 29 year old Officer Casey J. Kohlmeier, along with his K-9 partner Draco, were killed when their marked squad car was struck by another motorist as they were sitting in a median turnaround area.  They were employees for the Pontiac Police Department.

October 29 - Letohatchee, AL - 54 year old Charles Ming, an Alabama DOT employee, died on the job when he was struck by a motorist as he worked along a highway intersection. 

October 29 - Ashville, NC - 37 year old Robert Bingaman, an officer for the Ashville Police Department, died when his cruiser plunged off a highway bridge to the roadway below. It is unclear why his vehicle left the highway.

October 30 - Decatur, AL - 41 year old Michael Dewayne Atkins was crushed under a freight elevator that fell, trapping him underneath. He was using a sump pump to remove water under the elevator when the incident happened.  He was a plant operations employee for Decatur Morgan Hospital.

October 30 - Fox Point, WI - 38 year old Sean Held, a contractor from Slinger, was killed on the job when a garage he was working on collapsed onto him.  Wind braces were removed from the structure but before new ones could be installed to right the walls, the collapse happened. It is unclear who he was working for besides the homeowner.  

October 31 - Haddonfield, NJ - Cameron Lyon of Barrington, died from injuries due to falling approximately 40 feet from a tree he was trimming. He was working for Lyon & Son Tree Service.

October 31 - Rockmart, GA - 56 year old Elaine Davidson died of injuries related to an explosion at Meggitt Polymers & Composites. She was a quality assurance inspector for the company. Contents of a pressurized autoclave exploded inside a room, causing her injuries.

November 1, 2013 - Chicago, IL - 56 year old Alfred Valladares died after the dump truck he was driving was clipped by a car and he was thrown from the truck.  It is not clear who he worked for.

November 1 - Milpitas, CA - 34 year old Frankie Jimenez of Dixon was killed while picking up work zone cones when a motorist struck him, pinning him against his work truck and severing his legs.  He was employed by Caltrans. 

November 1 - Dallas, TX - An unidentified elevator maintenance worker from Mesquite died on the job after he fell about 20 feet into an open elevator shaft while working in a downtown parking garage. Attempts to local his name and employer have been unsuccessful. 

November 1 - Gainesville, TX - 44 year old Steven Flemming, Gainesville Police Chief, died after he fell from a ladder in the evidence building on October 25. 

November 1 - Los Angeles, CA - 39 year old Gerardo Hernandez was shot to death on the job while working as a TSA officer at the LAX Airport in Los Angeles. A man went into the airport with the intention of a killing rampage and Hernandez was shot.

November 1 - Bountiful, UT - 66 year old Richard Gillies was killed on the job when he was pinned against a building and the cherry picker crane bucket he was working from to replace a sign. He was working with his son but it is not clear who they were employed by.

November 2 - Atlanta, GA - 54 year old Darryl Simmons was killed on the job when the garbage truck he was a passenger in went out of control and crashed at a highway exit.  The driver was seriously injured.  They were employees of City of Atlanta Public Works Department.

November 2 - Ottawa, IL - 71 year old Roger W. Freeman of Oglesby died on the job when he was struck by a grain box truck. He was in a farm field near Grand Ridge when the incident occurred. 

November 2 - Henrico, VA - 24 year old Christian Martoni of Pittston, PA died from injuries he received while was working a salvage job in Henrico the day before.  He and another worker (see Romanczuk below) were using power tools to cut pipes in an old print shop when fumes ignited, causing an explosion & flash fire. He was employed with Hadley Construction of Pittston.

November 3 - Princeton, ME - 44 year old Grant Cilley died on the job after the cab of a tractor trailer truck he was working under fell off the jack, crushing him.  He was working in a yard owned by his family.

November 3 - Cheboygan, MI - 74 year old Bruce Spray was working on his farm when the tractor/mower he was operating flipped over an embankment and landing on him.  He also owned Spray Properties and was a long time employee for UPS. 

November 3 - Toms River, NJ - 39 year old Brennan Hughes, a tree service worker, was working on a ladder when he came into contact with an electrical wire and was electrocuted.  He then fell from the ladder he was standing on.  It is unclear who he was employed with.

November 3 - Everett, WA - 60 year old Joey Katter experienced shortness of breath as he was pushing a shopping cart in the parking lot of the KMart store he was manager of.  It was determined he died of natural causes.

November 4 - Merrillville, IN - 27 year old Andrew Fugate of Hobart was killed on the job when he was shot to death during an apparent robbery at his family business. He was found inside a Piatek Meats cooler.

November 4 - Oregon City, OR - 41 year old Robert Libke, an Oregon City Reserve police officer, was shot to death by a man who first started his home of fire and then opened fire, killing Libke.  Police determined that the man intentionally set the home on fire to lure police into an ambush.

November 4 - Minot, ND - 24 year old Lee Edward Helmers of Bismark died of head trauma after he was struck by a utility pole that fell, striking the ground, and bounced back hitting Helmers. He was employed by an unnamed Garrison utility contracting company.

November 4 - Vancouver, WA - 45 year old John Ballantyne was discovered unconscious in a men's restroom where he was working.  It appears he died of natural causes.  He was working for PeaceHealth Southwest Medical in Vancouver.

November 5 - Northeast NV - 53 year old Vicki Stenovich of Spring Creek, NV and 47 year old Arthur Lucero died and another worker injured when a motorist failed to slow down in a section of marked road construction, striking the workers.  All were employees with A-Core Construction.

November 5 - Reno, NV - An unidentified forklift company employee was killed on the job when he was crushed under a forklift that fell from the side of a flatbed truck at an Amazon.com warehouse.  Efforts to identify the worker has been unsuccessful. 

November 5 - McLeansboro, IL - 61 year old Alexis B. Anderson died on the job when his clothing became tangled and he was pulled into an oilfield pumping unit and crushed.  He was a self employed oilfield pumper, servicing an oil well.

November 5 - Everett, WA - Kapano-Pano Lopeti died on the job when he fell 40 feet from a tree he was harvesting.  He was employed by Dale Hofman who operates a logging business.  

November 6 - Henrico, VA - 37 year old Jason Romanczuk of Pittston, PA died of his injuries he received while working a salvage job in an old print shop in Henrico on November 1.  He and another worker (see Martoni above) were using power tools to cut pipes when print ink fumes ignited, causing an explosion & flash fire. He was employed with Hadley Construction of Pittston.

November 6 - Hutchinson, KS - 34 year old Jeff Enslinger was killed after being struck in the head by a malfunctioning steel gas cylinder while working at Hutchinson Airgas.  

November 6 - Mount Sterling, KY - 73 year old Carl Williams of Mt. Vernon, KY died after he fell from the roof of a local airport and another worker fell onto him. That worker was not seriously hurt.  The men were working at the Mt. Sterling-Montgomery County Airport but is unclear who they were employed by.

November 6 - Watertown, WI - 62 year old Randall Ninmann died on the job from an apparent fall from a ladder while working maintenance for Watertown High School.

November 6 - San Juan, TX - 64 year old Guadalupe "Lupe" Mendoza was fatally crushed under a 2,000 pound, 5 foot tall tombstone at Hillcrest Cemetery where he worked.  He was a supervisor at the cemetery and was trying the level the giant stone when it fell.

November 7 - Miami, FL - An unidentified worker died and five more were hurt in an explosion of a chemical storage tank that went airborne through the roof of American Vinyl Company of Opa-Locka.  Efforts to find the name of the deceased worker have been unsuccessful.

November 7 - Perryville, MD - 64 year old Capt. David R. Barr died after being struck two weeks ago by a motorist as he was directing traffic at an accident scene.  He was employed by Perryville Fire.

November 7 - Las Vegas, NV - 31 year old Arturo Mendez died a day after falling about 35 feet from a tree.  Mendez was a tree trimmer working for the MGM Mirage Hotel.  

November 7 - Washington DC - Julie Phillips was a long time employee for Marine Barracks Cafeteria.  She died after being struck by a large truck as she crossed the street near the cafeteria. 

November 9 - Sherwood, OR - 35 year old Renee Radziwon-Chapman was an eight year employee for Wildcat Haven Sanctuary as head (animal) keeper.  She was attacked and killed by a cougar as she was apparently cleaning out a cage alone.  Wildcat Haven owners said their people always worked in pairs for such a situation and don't know why she was alone.

November 10 - Maywood, IL - 44 year old Eric Terrell died after being crushed under falling boxes. He was inside a the back of a truck unloading boxed shelving units when he fell to the ground and then boxes fell onto him.  He was working at the Woodfield Mall but it is not clear who he worked for.

November 11 - Indianapolis, IN - 30 year old James Dozier was killed after he was pinned between a forklift and a guardrail. As he operated the machine he got off to move something by hand when the machine moved suddenly, pinning him.  He was employed by Recycle Force, a recycling plant. 

November 11 - West Columbia, SC - 35 year old Nelson E. Gutierrez of Miami, FL was killed when he was struck by a motorist in a lane closed to travel while working in a construction zone in SC.  His employer is not identified.

November 11 - Preston, WA - Rex Nichols died of heart related natural causes while he was working for Industrial Machinery, Inc.

November 12 - Atlanta, GA - 41 year old Jorge L. Pena died on the job when a city bus ran a red light, crashing into Pena's FedEx delivery truck and causing it to run into the side of a building. 

November 12 - La Mesa, CA - 41 year old Josh Allen Pudsey of Lakeside was killed when a tree branch fell onto him as he worked as a tree trimmer.  He and a co-worker were in an elevated bucket of a cherry picker trimming trees when one of the branches fell, striking Pudsey in the head.  It is unclear who he was employed by.

November 12 - Gibralter, MI - 30 year old Chelsea Ann Small was shot to death by a male suspect who attempted to rob the Advance America Cash Advance store she was working in. 

November 12 - Bradner, OH - 26 year old Paul Castle of Paintsville, KY was killed on the job when he was struck by a motorist who, attempting to evade the barricades, drove into a work area at a railroad crossing near Bradner.  He was a CSX Railroad employee.

November 13 - Laredo, TX - 46 year old Randy A. Thomas of Mississippi was killed on the job when the roller/compactor machine he was operating tipped over, crushing him.  It is unclear who he working for but he was working at property owned by United Independent School District. 

November 13 - Virginia Beach, VA - 49 year old Antonio Reid of Chesapeake died on the job after being struck by a piece of construction equipment.  He was working on a construction site operated by A & W Contractors but unclear who he was employed with.

November 13 - , East Chicago, IN - 39 year old Michael Samuelson of Valparaiso was killed on the job and co-workers injured as they worked at the Arcelor Mittal No. 3 steel making plant in Indiana Harbor West.  He died as the result of debris falling from a height onto the workers.

November 13 - Camp Pendleton, CA - Four Marines died from a mine explosion as they were performing a routine sweep of a range at Camp Pendleton to make the range safer for future training exercises. Those who lost their lives were 27 year old Miguel Ortiz of Vista, CA and was a Marine Sgt.; 28 year old Matthew R. Marsh of Long Beach, CA and was a Staff Sgt.; 31 year old J. Mullins of Bayou L'Ourse, LA and was a Gunnery Sgt.; and 32 year old Eric W. Summers of Poplar Bluff, MO and was a Staff Sgt. 

November 13 - Williston, ND - 35 year old William John Spint of Rapid City died when he was buried in a trench collapse while working near Williston.  Co-workers were able to get him out of the trench, but he died before help could arrive.  It is unclear who he was working for.

November 13 - Bellevue, WA - Margery Lee of Seattle died of heart related natural causes while working at New Dimensions Frame & Gallery LLC.  She collapsed on the job and also struck her head.

November 14 - Echo, UT - 67 year old Kenneth R. Groberg of Clinton, UT died as the result of the crash of his semi truck when it overturned on an off ramp curve.  The truck cab was crushed as the trailer portion flipped onto it.  His employer is not listed.

November 15, 2013 - North Las Vegas, NV - 49 year old Martin Martinez suffered fatal crushing injuries while working in a rock quarry south of Las Vegas.  It is unclear who he was working for.

November 15 - Pascagoula, MS - 46 year old Tonya Graddy of Semmes, AL died on the job when something caused an explosion in a furnace causing a fire at the Chevron Refinery in Pascagoula.

November 15 - West Jordan, UT - 55 year old Stan Riley was killed on the job when the empty school bus he was driving collided with a large truck, which then sent the school bus into a garbage truck.  He was employed by Jordan School District.

November 15 - Auburn, WA - Harold Frasier died on the job of natural causes while working for Worldpac, Inc.

November 16 - Boulder, CO - An unidentified man in his 50's died on the job after falling from the roof of a structure. It is unknown who he was employed by and attempts to find his identity failed.

November 16 - Adamsville, AL - An unidentified man from Georgia performing contracted tree removal work for the State of Alabama was struck and killed by a motorist in a blocked-off section of Highway 78.

November 17 - Lake View, IA - 36 year old Jeremy Schulte died of toxic exposure when he fell into and was trapped in a manure tank on a farm.

November 18 - Oakland, CA - An unidentified man died on the job when he fell from a crane onto a platform.  He was a crew member for the ship "Albert Oldondorff". 

November 18 - Turin, NY - 23 year old Kyle M. Brown died after a gust of wind caused him to fall from the roof of a building as he was carrying roofing materials.  He was employed by MTL Design working on the F.X. Caprara Construction site.

November 20 - Shawnee, KS - 62 year old Dwight L. Spencer of Gardner died of injuries he received after being pinned between a loading dock and a Fed Ex truck in Shawnee.  

November 20 - West Finley, PA - 46 year old John Wesley Pirl of Normalville was killed on the job when a tree fell onto him as he was cutting a different tree. Authorities say wind caused the tree to fall.  He was employed by Fayette Forest Products of Scottsdale. 

November 20 - Jackson, MI - 45 year old William Bradley died on the job while performing maintenance on a dump style pickup truck. He was crushed when the raised bed fell onto the truck.  He was working for Direct Asphalt Paving.

November 20 - Syracuse, NY - 44 year old Trooper Ross Riley died after falling during a high angle rescue training exercise at Letchworth State Park.  Riley was assigned to Field Command, Emergency Management in Albany.

November 21 - Notre Dame, IN - 22 year old Mark Ellsworth, Jr. of Mishawaka died of blunt force upper body trauma when he was struck by a tree that was being cut down while working for R&R Excavating on the Notre Dame campus. 

November 21 - Chicago, IL - 59 year old Gary Beno of New Lenox was working as an electrician at O'Hare International Airport's Terminal 5 when he fell to the ground level on his back.

November 21 - Muhlenberg, KY - Ronnie Walker died after falling from the top of a grain bin.  He was working for Chris McNally Painting and Seal Coating.

November 22 - Oakley, ID - 71 year old Wade Zollinger was killed on his horse breeding ranch when he became entangled in a PTO driven auger attached to a tractor. 

November 22 - Fort Mill, SC - 28 year old Fredy Noe Urraco died after falling from the roof of Doby's Bridge Elementary School which is under construction.  It is unclear who he was employed by.

November 23 - Cheverly, MD - 26 year old Tyson Jerome Barnette of Upper Marlboro was delivering mail for the United States Postal Service when he was shot to death by an unknown assailant. 

November 23 - St. Matthews, KY - 34 year old Jack Combs of Louisville was walking to his car after his shift at a Kroger Grocery Story when he was shot to death by an unknown assailant for an, as of yet, unknown reason. 

November 23 - Tampa, FL - 32 year old Michael Pavlak of Trinity died after he was struck and run over by a dump truck while working on a road construction project. It is unclear who he was working for.

November 23 - Bluffton, MN - Gail Svor of Carlos was fatally electrocuted while working on a project to replace highlines and power lines.  He was employed by Highline Construction Inc. of Payne, contracted to Rural Electric Cooperative.

November 25 - Brevard, FL - 52 year old Robert Sikorski was killed on the job when he was struck by a dump truck driver who was backing up as he walked in a closed section of roadway under construction.  It is not clear who he was working for.

November 25 - Phoenix, AZ - 26 year old Miguel "Mike" Angel Hernandez was attacked and stabbed to death by his co-worker inside the restaurant kitchen of the Tilted Kilt Pub and Eatery.  Police say there was no provocation for the attack by the co-worker who was arrested in the murder.

November 25 - Rio Vista, CA - 49 year old Richard Wayne Alexander died on the job when the excavator he was operating on a barge fell off the barge and into the Sacramento River.  He was inside the enclosed cab of the vehicle and it took dive teams to locate him.  It is unclear who he was working for.

November 26 - Longview, TX - Gail Sandidge, a nurse at Good Shepherd Ambulatory Surgical Center, was fatally stabbed inside the surgical center as she attempted to protect her patients against a young man who came into the center yelling not to hurt his mother. He wounded several others before he was arrested.

November 26 - Barrow, WI - 62 year old John Easterly of Wooster, OH died on the job after a large round hay bale rolled onto him as he attempted to unload it.  The name of the farm was not found. 

November 27 - Orlando, FL - 56 year old Rafael Duarte died after being struck by a motorist as he was returning to his work vehicle after a plumbing call.  His employer is not known. 

November 28 - Silver Spring, MD - An unidentified construction worker died on the job after he slipped on ice, falling and striking his head on the ground. Attempts to find his name and employer were unsuccessful. 

November 29 - Kirksville, MO - 43 year old Richard Dale Johnson was killed when the farm tractor he was driving lost traction and rolled over, pinning him under it. 

November 30 - Manhattan, NY - 71 year old Kalyanarat Ranasinghe, a uniformed NYPD Traffic Enforcement agent, was killed on the job when he was clipped by a large truck backing up causing him to roll under the trucks tire.

December 1 - Ferguson, NC - 72 year old Rob Roy Jones died on the job while working on his farm when an embankment he was driving his tractor near collapsed and the tractor overturned, crushing him. 

December 2 - Maybrook, NY - 53 year old Timothy Lang of Saugerties was killed on the job and two others injured when a retaining wall collapsed at a Halmar International construction site.

December 2 - Lorain, OH - 47 year old Steven DeSantis of Chardon was killed on the job when an industrial sized trash bin fell onto him. He was under the bin, partially on his truck, moving objects when the bin fell.  He was working at Mercy Regional Medical Center for Cotterman & Sons of Monteville.

December 2 - Roy, WA - 44 year old Steve Green died of suffocation after being buried in a corn grain silo collapse. He was employed by Wilcox Family Farm.  His body was not recovered until two days after.

December 3 - Glendale, AZ - 50 year old Douglas Elliot was murdered on the job when he was shot for an unknown reason while working as the maintenance manager for Glendale Commons apartments.

December 3 - College Station, TX - 25 year old Angel Garcia died on the job as the result of a fall from the fourth level of Kyle Field at A & M University. He was employed by Lindamood Demolition of Irving.

December 4 - Manhattan, KY - 63 year old Stephen Hetzler was killed on the job when he was struck by debris and rocks from an explosion at the Bayer Rock Quarry. Hetzler was standing very close to the detonation site.

December 4 - Independence, MO - 63 year old Manjit Nirwal, who is from Canada, was killed on the job in Independence when he was pinned between two semi trucks. He exited his tractor trailer rig it began to roll and as he tried to stop it, he was crushed.

December 4 - Conyers, GA - 31 year old Charles LeMasters of Covington died on the job when he was partially pulled into a conveyor belt. He was operating a machine that feeds wood chips onto a conveyor when his hand became caught and his arm was pulled into the system.  He unfortunately died of suffocation in the wood chips before he could be rescued. He was working for Pratt Industries.

December 4 - Avenell, NJ - 57 year old Ronald Smith of Irvington died when he was crushed between equipment at an Amazon.com sorting facility run by Genco. He was a temporary worker for Abacus. 

December 5 - Ramapo, NY - 56 year old Joseph Giarnella of Stoney Point was killed when his work vehicle crashed on a highway.  He was a contractor who owned J. Giarnella & Son.

December 5 - Old Town, ME - 32 year old Jon Atwood of Bangor died on the job when he fell from the roof of a construction project.  It is unclear who he was employed by.

December 6 - Negreet, LA - 66 year old William D. Branam of Many died while working on his farm. He was using a tractor moving hay bales when one rolled back onto the cab of the vehicle, crushing him.

December 6 - National Mine, MI - 61 year old Gary Hytinen of Ishpeming, a 39 year employee for Cliff's Natural Resources Tilden Mine, died on the job after falling from the top of a railroad tanker car.

December 7 - Jacksonville, FL - 56 year old Richard Ahlers, Sr. died after falling from the roof of a home he was working on for a private homeowner.

December 7 - South Jackson, WV - Charlie Pierson died of a heart attack as he was preparing to respond to a car crash call.  He was the South Jackson Volunteer Fire Chief. 

December 7 - Grass Valley, CA - 57 year old Robert "Farmer Bob" Keyser collapsed and died suddenly of a heart attack while loading a Christmas tree for a customer.  He was the owner of Farmer Bob's Produce.  

December 7 - Canton, OH - 51 year old Richard Wentling died on the job when he was crushed under a tour bus he was working under.  He was an employee with Alliance Tour Bus. 

December 9 - Fresno, CA - 52 year old Gilbert Acuna of Rialto died on the job when he was pinned between his tractor-trailer truck and a loading dock at an REI store at River Park. Authorities think the brakes may have failed after he exited the truck. 

December 9 - Schuyler, NY - 59 year old Wayne Johnson died after being pinned between the bucket of his tractor and scrap metal when the tractor rolled into him.  He was self employed.

December 9 - Boston, MA - 46 year old Marco Antonio Huezo Mancea, an immigrant from El Salvador, was killed on the job when he was struck and crushed by a falling 12,000 lb steel beam at Boston Bridge & Steel, Inc. in Boston. 

December 9 - Queens, NY - 23 year old Mauricio Edy Zambranovera died on the job while off-loading wood and other construction material from his truck at Sand & Gravel in Melville. He was found behind his City Recycling (of Brooklyn) truck and may have been struck by a large piece of the massive earth moving machines at the site.

December 9 - Lexington, KY - 51 year old Mukeshbhai Patel was shot to death during a robbery of the Marathon Gas Station he was working alone at.  The suspect has been arrested.

December 10 - Loudon, NH - 31 year old Ronald Schlegel of Hooksett was killed on the job when he was pinned between a scissor lift and an overhead door.  He was the logistics coordinator for Loudon's Pleasant View Gardens. 

December 10 - Cincinnati, OH - 82 year old Virgil Souder of Hebron, KY drowned after falling from a ferry into the Ohio River.  He was a deckhand for the unnamed ferry. 

December 11 - Athens, GA - 42 year old Michael Lancaster of Simpsonville, SC died on the job when he fell from the roof into a waster container at McLane, a wholesale grocery supplier.  He was discovered by another worker. It is unclear who Lancaster was employed by.

December 11 - Jarrettsville, MD - 32 year old Daniel Paul Dixon died after the roof of a barn collapsed onto him. He was working on the 2nd floor of the structure when the roof collapsed, trapping him in the rubble. It is unclear who he was working for.

December 12 - Abilene, TX - 57 year old Max D. Mitchell of Coleman died on the job after being struck by a garbage truck that backed into him.  He was employed by MCG Drilling and was struck by a City of Abilene vehicle.

December 13 - Randleman, NC - 61 year old Martin Soto Rodriguez was killed on the job when he became entangled and pulled into an extractor machine while working at Deep River Dyeing Company.

December 13 - Maybrook, NY - 50 year old Scott Winkler of Monroe has died after being injured December 2nd in a wall collapse while working for Halmar Construction.  

December 14 - Ashtabula, OH - 41 year old santos Medina de Jesus died of toxic suffocation and drowning when his tractor overturned into a manure lagoon on a dairy farm. 

December 17 - Moulton, TX - 45 year old Jose Torres of San Benito was killed on the job in an explosion of a tanker trailer at a GreenHunter Resources oil field site.  He was working inside the tanker trailer, holding a piece of metal for a welder on the outside of the trailer when the explosion occurred.  It is unclear who he was working for.

December 17 - Duanesburg, NY - 35 year old New York State Trooper David Cunniff was killed on the job when he was struck by a motorist. He was behind a vehicle he had pulled over when the other motorist struck his patrol vehicle from behind.

December 22 - Carpentersville, IL - 19 year old Stephanie L. Siers died on the job when she fell from the back of a work truck. She was employed by Tovar Snow Professionals of East Dundee.

December 23 - Billerica, MA - An unidentified worker was found dead inside an electrical closet at Nuvera Fuel Cells in Billerica.  Attempts to find the identity of the worker have been unsuccessful.

December 26 - Palos Park, IL - 25 year old Patrick J. O'Reilly was killed on the job when he was struck by falling concrete in a wall collapse. He was working for an unnamed company at a demolition site at the Westfield Hawthorne Mall.

December 26 - Orchard, TX - Eduardo Bravo of El Campo was killed on the job when he was pinned between a truck and well head at an Orchard well site. He was working with a crew preparing a rig when the truck lunged into him.  

December 27 - Spotsylvania, PA - 61 year old Mickey Wayne Jones was found shot to death behind the office building of his employer, Attkisson Truck and Equipment Sales. His death is being investigated as a homicide.

December 27 - Othello, WA - 57 year old Angel Pena Hernandez of Moses Lake died of natural causes while working for McCain Food USA, Inc. He was found slumped over the steering wheel of the potato truck he was in. 

December 28 - Columbus, OH - 76 year old Paul W. Stearns died after he fell from a ladder.  He was trimming trees when a branch fell onto him, knocking him off the ladder. He was working on his farm.

December 28 - Hamilton, IL - 53 year old James Drozdz died after the tractor trailer truck he was driving crashed at a highway ramp. He was driving truck for Dot Foods part time and was also a district attorney for Hancock County. 

December 29 - Mill Creek, WA - Salvador Ayala of Edmonds died of cardiac arrest a day after being taken to the hospital from his job.  He complained of dizziness and trouble breathing after using a chemical cleaning product and was seen and released from an area hospital.  He was employed by Los Compadres #6, Inc. dba Azteca Restaurant Mill Creek. 

December 30 - Silver Spring, MD - 47 year old Thomasine Maria Smith of Laurel died after she became pinned between her Metro bus and a wall.  Reports say she exited the bus to take a break when the bus began to roll. She apparently attempted to set the brake from outside the bus when she was pinned against the wall.

December 30 - Boston, MA - A 59 year old unidentified man from Melrose was killed on the tarmac of Logan International Airport after being hit by a truck that was traveling in reverse.  Attempts to locate the workers name or who he was employed with has been unsuccessful. 

December 30 - Princess Anne, MD - 73 year old Kenneth Jacob Beasley died when the dump truck he was operating was partially submerged in a ditch.  He was found by co-workers and flown to hospital where he died. He was working on private property but it is unclear who he was employed with.

December 31 - Normal, IL - 37 year old Darrin Condor of Bloomington died as the result of a second story fall at a construction site in Normal.  It is unclear who he was employed with. 

December 31 - Atlanta, GA - 59 year old AP Sports Photographer Dave Martin died of cardiac arrest while working the Chick-Fil-A Bowl at the Georgia Dome.  Martin was rushing onto the field to capture the celebration after the game when he collapsed on the field.

COAL MINE DEATHS 2013

July 3 - New Eagle, PA - 87 year old Samuel V. Vignoli held an unknown position for Robert E. Murray's Maple Creek Mining Inc., Maple Creek Preparation Plant at the time of his death. He was operating a skid steer type of machine at Maple Creek Preparation Plant's Ginger Hill #1 Pond when the machine traveled into the compound's waters as he was mowing an embankment.

August 6 - Holmes Mill, KY - 56 year old Lenny D. Gilliam was a Continuous Miner Operator for Arch Coal, Inc., Lone Mountain Processing, Inc., Huff Creek No. 1 mine at the time of his death.  He was killed when a coal/rock outburst occurred, causing the fall of a rock wall, fatally injuring him.

August 16 - Wright, WY - 24 year old Jacob R. Dowdy was a Utility Person for Arch Coal Inc., Thunder Basin Coal Company LLC, Black Thunder mine at the time of his death.  He was killed when an electric shovel lost propel capabilities, rolling backwards into a pickup truck he was operating.

October 4 - Cameron, WV - 62 year old Roger R. King was a Longwall Maintenance Coordinator for Consol Energy, Inc., McElroy Coal Company's McElroy Mine.  He was working with powered haulage when a series of events occurred which caused a piece of equipment to break loose, striking him in the back of the head.

October 5 - Carmi, IL - 47 year old Robert E. Smith was a general laborer driving a battery golf cart for Alliance Resources Partners LP, While County Coal, LLC, Pattiki mine as the time of his death.  He was operating the powered cart when it overturned on top of him. 

October 6 - Point of Rocks, WY - 44 year old Mark C. Stassinos was a heavy equipment operator for MidAmerican Energy Holdings Company; IDACOR, Pacific Minerals dba Bridger Coal Company's Jim Bridger Mine at the time of his death.  He died when the dozer he was operating went over the edge of a highwall he was building. 

October 11 - Petersburg, IN - 59 year old Larry Schwartz was a Shuttle Car Operator for Donald Blankenberger, Five Star Mining, Inc. Prosperity Mine at the time of his death.  He was killed when he was pinned between a shuttle car and a coal rib on the blind side of the shuttle car.

November 4 - Macedonia, IL - 36 year old Dallas D. Travelstead was a Longwall Chief for Coalfield Transport, Inc., M-Class Mining LLC's MC #1 Mine at the time of his death.  He was killed when a solid piece of coal and cap rock rolled into him as he was shoveling coal and loose rock.

November 23 - Beallsville, OH - 32 year old Ryan Lashley was a Shieldman for Robert E. Murray, American Energy Corp's Century Mine at the time of his death.  He was killed when he was struck by a high pressure hydraulic hose or fluid when it became stuck between a piece of equipment and the mine floor.

Metal Miner Deaths 2013

August 5 - Lowville, NY - 55 year old Willard Moser was a Plant Manager for Michael J. Lyndaker; MJL Crushing LLC at the time of his death.  He died when he was struck by a piece of an excavator bucket that dislodged and struck him.

September 16 - Huntsville, MO - 58 year old David A. Gully was a Truck Driver for Summit Materials, LLC; Con-Agg of MO, LLC at Huntsville Quarry at the time of his death.  He died when the truck he was driving traveled through a berm near the quarry and over a highwall.

September 18 - Albany, KY - 56 year old Lonnie E. Ferrill was a Front End Loader operator for Roy Beard; Gaddie-Shamrock LLC at Caldwell Quarry at the time of his death.  He was engulfed by material inside a hopper as he was attempting to remove a rock from the pug mill hopper. 

September 19 - Monroeton, PA - 31 year old Timothy Farr was a Laborer for Jan Tuttle/Charles Gold; T.G. Mountain Stone at Northrup Quarry at the time of his death.  He was operating a truck hauling water tanks on a steep roadway when the truck overturned.

October 17 - Lebec, CA - 52 year old Fernando Rivera was a Journeyman Electrician for Vicat S A; National Cement Co of CA Inc. at Lebec Cement Plant at the time of his death. He died after falling from a ladder as he was working to pull SO cable into a cable tray. The tray mounting bracket broke loose from the wall causing it to strike and knock over the ladder.

November 7 - Lithia Springs, GA - 46 year old Carl J. Clinton was an Equipment Operator for Vulcan Materials Company; Vulcan Construction Materials, L.P. at Lithia Springs Quarry at the time of his death. He was operating a haul truck that went off the road over an embankment and overturned.

November 17 - Ouray, CO - 59 year old Rick L. Williams was a Shift Supervisor for Rory Williams/James Williams; Star Mine Operations, LLC at Revenue Mine at the time of his death. He died of carbon monoxide exposure after entering a mine to look for another miner who did not return after entering the mine where an explosive had been previously detonated. 

November 17 - Ouray, CO - 33 year old Nicholas K. Cappanno was a Powderman Trainee for Rory Williams/James Williams; Star Mine Operations, LLC at Revenue Mine at the time of his death. He died of carbon monoxide exposure after entering a mine where an explosive had been previously detonated. 

November 22 - Salem, KY - 33 year old Bobby Crume was an Electrician Foreman for W. Scott McGeorge; Pine Bluff Sand & Gravel Company at Cumberland River Quarry at the time of his death. He was working in an electrical enclosure with cable for a new pump when co-workers saw him fall to the ground.  He was injured on November 18th and died later at hospital.

December 4 - Junction City, KS - 63 year old Stephen W. Hetzler was a Lead Man for Bayer Construction Company, Inc. at Kansas Falls Quarry at the time of his death. He was struck by fly rock after he detonated a blast at the quarry.

December 6 - Ishpeming, MI - 61 year old Gary A. Hytinen was a Reagent Handler for Ontario Tilden Company/Cliffs TIOP Inc.; Tilden Mining Comapny LC at Tilden Mine at the time of his death.  He died from a fall to the ground as he worked atop a railroad tanker car.

December 10 - Ledbetter, KY - 27 year old Dustin Burnham was a Deckhand for HMT Holdings, Inc; Hunter Sand & Gravel, LLC at Dredge IV (5) at the time of his death.  He drowned after falling into water from a barge as he was working on a dredge and stepped onto the barge.

December 13 - Junction City, GA - 53 year old Paul D. Barnes was a Utility Worker for Brown Brothers Sand Company at Wittichen Stephen Plants at the time of his death.  He died after being engulfed when the bank of a pit he was standing on collapsed under him.

October 9, 2013 - Port Angeles, WA - Benjamin Lopez Santana of Puerto Rico, died of natural causes while working for Factory Sales and Engineering. He was found unresponsive inside a port-a-potty. 

November 26, 2013 - Yuma, AZ - Ana Maria Barrera-Bogarin of San Luis, died on the job when she was run over by a tractor while harvesting on a farm in Yuma.

December 12, 2013 - Lyon County, KY - 44 year old Brian Lee Schuetz of Goreville, IL died on the job from a massive heart attack while driving his tractor trailer semi in KY.  The truck ran off the roadway and struck a tree.

December 17, 2013 - Seattle, WA - 61 year old Don Johnson, a truck driver for HMB Enterprises, was found dead of natural causes inside the truck he had been driving. 

December 18, 2013 - Fort Worth, TX - 32 year old Ruben Pugo Garcia of Denton was killed on the job when he was struck by a motorist while outside his truck and inspecting a manhole of a road in Fort Worth.  It is unclear who Garcia was working for at the time of his death.

December 18, 2013 - Williston, ND - 26 year old Daniel Okyere of Colorado was killed on the job when his jacket became lodged in a forklift and it somehow overturned onto him.  He was employed by the Canadian company, United Pulse Trading, based in Williston.  

December 25, 2013 - Everett, WA - Charles Eschbach died when the work van he was working on fell onto him, crushing him.  He was owner of Chuck's Home Remodel. 

December 30, 2013 - Bloomington, IL - 37 year old Darrin Conder died on the job after he fell from the roof of a home under construction. It is unclear who he was working for.

December 31, 2013 - Williston, ND - 30 year old Keith Munro of Harrisburg was killed on a Nabors Drilling Rig site near Williston when a piece of drilling rig pipe dislodged and struck him

January 2,2014- Falls Township, PA - 51 year old Gustav Propper of Mount Laurel died of ashyxiation on the job when he was buried under a 100 foot pile of rock salt. He was using an excavator to move the rock salt when the pile collapsed onto the vehicle. He was employed by International Salt. 

January 2 - Lincoln, NE - 61 year old Cheryl A. Graham collapsed and died of natural causes while working as a staff member at the University of Lincoln, NE. 

January 3 - Fort Worth, TX - 54 year old Curtis Anderson of Grapevine died on the job under unknown circumstances while delivering liquid food waste at Village Creek Water Reclamation Facility.  He was employed by Liquid Environmental Solutions. 

January 4 - Lovell, WY - 28 year old Anfesa Marie "Ky" Galaktionoff of Deaver and Lovell died on the job when she fell into a lift station that carries beets into the Western Sugar Cooperative factory.  She was a mechanic's helper there and often worked alone.

January 6 - Seattle, WA - 34 year old Aaron Adair died on the first day of his job with North Coast Iron Corp. when he fell from a scaffold after unclipping his safety gear in order to move to another place on the scaffold at the top of a building.

January 6 - Jeffersonville, IN - 38 year old Robert C. Bonee of Louisville died on the job after the taxi he was driving left the roadway and crashed.  He was employed by Best Cab Company of New Albany.  It is unclear what caused the crash.

January 6 - Louisville, KY - 50 year old Norman Kiper of Frankfort was killed on the job when he was struck by a vehicle as he sat at his desk.  An employee in the service department started to move a vehicle and it accelerated, striking Kiper from behind as he sat at his desk at Bill Collins Ford.

January 6 - Clinton, IA - 63 year old Charles M. Wilson, Sr. died after he fell from a ladder on January 1 while working in a storage room at a Clinton Walmart.

January 7 - San Antonio, TX - 62 year old Pedro Almeida was working with his son when he was struck and killed by a motorist who did not see him working to turn on equipment near the front of their Tiger Sanitation truck.

January 8 - Montgomery, AL - 63 year old David Lee White of Montgomery and 53 year old Lomax Phillips of Birmingham were killed on the job when the elevated manlift they were working from detached from the side of a bridge and fell about 90 feet to the ground. Both were employed with R.R. Dawson Bridge Company of Bessemer.

January 10 - Valparaiso, FL - 31 year old Jonathan "JD" Lord died on the job, and three others injured, when fire suppression foam was dispensed inside a hanger at Elgin Air Force Base.  He was a Defense Support Services contractor. 

January 10 - Maria Stein, OH - 43 year old Robert L. Meier died on the job when the bucket of a skid loader he was working on fell, pinning him between the bucket and the loader. He was working on a farm.

January 11 - Mingo Junction, OH - 34 year old John Devereaux of Weirton was killed on the job when his clothes became caught in a moving conveyor belt at the FEX facility.  He was employed by Tyvek.

January 13 - St. George, UT - 57 year old Eduardo Gayoso died on the job after falling from the roof of Dixie Middle School while unloading insulation.  He was employed by Skyline Roofing.

January 13 - West Jefferson, OH - 52 year old Nick Yurevich, Jr. of Bergholz was killed on the job when a co-worker, who did not see him, backed a truck over him as he unloaded materials at a power line installation site. He was employed by Moorehead Brothers.

January 13 - Madison, WI - 40 year old Robert Lund of Cambridge fell to his death while working at the Edgewater Hotel construction site.  The lead contractor is Findorff, but it is unclear who he was employed with.

January 15 - Riverdale, IL - 59 year old Bruce Camp was killed on the job when he fell into wood grinding equipment while working at Riverview Materials Plant.

December 22, 2013 - Longview, WA - 68 year old James Nels Dukart of Damascus, OR died on the job when a truck that was apparently left in reverse rolled back, striking him.  His company is not named but he was working at the Duke Construction & Excavating, Mint Farm Industrial Park construction site with his partner, son Steve Dukart.

December 23, 2013 - Olympia, WA - 37 year old Michael Webber of Hoquiam was killed on the job while driving his logging truck for Lutton Trucking, Inc.  He failed to negotiate a curve down a hill and went off the road. His truck struck a stump causing his load of logs to shift forward and into the cab of the truck.  

January 16, 2014 - Everett, WA - 52 year old Gregory Billen died as the result of natural causes while working on his recycling vehicle for Waste Management of Washington, Inc.

January 16 - Elkhart, IN - 20 year old Krystle Dikes was killed on the job when she was shot to death by a man who entered the store and opened fire on her and a customer.  She was employed by Martin's Supermarket.

January 16 - Manhattan, KS - 47 year old David Teske of Onaga died on the job when he was struck by a falling tree at a logging site near Manhattan.  His employer is unknown.

January 16 - St. Petersburg, FL - 60 year old Terry Lynn Flack of Brooksville died on the job after he fell about 30 feet from a catwalk on the Eckerd College Campus. He was an electrician employed by Universal Controls. 

January 17 - Seattle, WA - Teresita Acido was found unconscious and died of a heart related issue while working at Aramark Sports Safeco Field.

January 18 - New Bedford, MA - 35 year old Victor Gerena was killed on the job when he was crushed after becoming entangled in a shellfish shucking machine.  He was employed by New Bedford Sea Watch International. 

January 18 - Fairburn, GA - 21 year old Tim June of Roswell died on the job while working a tree trimming job for his company, Atlanta Tree Assurance.  In a series of events during cutting a tree he was struck by a limb and then became tangled in his safety line, which ended up causing him to become strangled. 

January 18 - Burlington, IA - 52 year old Jay Ehret died hours after falling from the roof of an apartment building.  He was reaching for his nail gun, when he slipped and fell.  It is unclear who he was employed with. 

January 19 - Bensenville, IL - 36 year old Hussein Saghir was shot to death during a robbery attempt as he and his brother, owner of Sam's Tobacco & Food Mart, were preparing to lock up the store and leave for the night. Saghir worked as a clerk for the store.

January 20 - Madill, OK - 23 year old Brody Davenport of Durant and 38 year old Gonzalo Martinez of Madill died of thermal injuries and smoke inhalation due to an explosion in a furnace that occurred inside Mid American Steel & Wire plant in Madill.

January 20 - Fall River, MA - 47 year old Allen L. Jarabek, Jr. of Swansea died after a commercial truck he was working on rolled over him.  He was owner of Ground Earth, Inc. and Dover Towing.

January 20 - Milan, NH - 42 year old Brian Young of Concord, VT was killed on the job when he was struck by a logging truck driven by his co-worker.  The co-worker believed Young was clear of the truck before moving forward and did not see him.  Both worked for Kelley Trucking of Milan. 

January 20 - Omaha, NE - 53 year old Keith Everett and 47 year old David Ball were killed, and ten others injured, when a blast caused the collapse of much of the International Nutrition plant in Omaha.

January 20 - Red Bud, IL - 44 year old Jeffrey Guebert died on the job after falling into a grain bin filled with corn.  He had not been seen by others for half an hour before being found in the bin. He was employed by Guebert Farms.

January 20 - Bronx, NY - An unidenfied driver of an 80,000 pound cement truck died after his truck veered off the roadway for an unknown reason, slamming into a apartment.  Repeated attempts to locate the driver's name or employer have been unsuccessful.

January 21 - Sully, IA - 26 year old Willie Mercer died of possible toxic exposure inside a tank trailer that may have contained a hazardous material.  He and two other workers passed out while inside the trailer.  All worked for Sully Truck Wash at the time of the incident.

January 21 - Dublin, CA - Sgt. Tom Smith, a Bay Area Rapid Transit officer from San Francisco, was shot and killed by friendly fire during a probation search of a suspect's home.  The other officer's weapon either accidentally discharged or he thought he saw someone else. The bullet passed through a gap in Smith's protective vest.

January 21 - Otay Mesa, CA - An unidentified man was killed when he became pinned between two tractor trailer rigs at Mex-Cal Truckline, Inc.  He was changing a tire on one of the rigs when it fell off the jack, pinning him against the other truck.

January 22 - Phoenix, AZ - Two unidentified employees for Riddell Painting Company were killed when the lift truck they were working in toppled over at the Lower Buckeye Jail facility.  The lift may have been parked on a slope.   

January 22 - Colorado Springs, CO - 41 year old Pedro Arrazola died on the job when he fell from a fire escape while inspecting a roof.  He was employed with Weathercraft Roofing.

January 23 - Raleigh, NC - 30 year old Gabriel Andrade died on the job when he fell from scaffolding at an apartment construction site. He was wearing a harness and lanyard but apparently unhooked it to lean outward to install a soffitt.  He was employed by Exterior Recovery, Inc. of Youngsville. 

January 23 - Davenport, WA - 54 year old Warren Wade Damschen died of toxic exposure while working inside a confined space septic tank.  He and other workers were cleaning out a septic tank when Damschen descended into the space to shovel excess waste to the hose. After calling for a ladder he did not appear and other workers used a hose to pull him out.  He was employed by Short Larry Harmon dba Short Septic Service & Contracting.

January 24 - Clarksville, AR - 33 year old Daniel Rice was killed in a tank explosion at his job at Quick Transports, Inc.  He was using a torch to thaw the tanker when it exploded and he was struck by a metal hatch plate blown off the back of the truck.

January 24 - White Pigeon, MI - 30 year old Eric Rodman of Elkhart, IN was killed on the job when he fell into one of the hopper railroad cars he was loading.  He was working for The Anderson's Grain and Storage facility in White Pigeon.  

January 24 - Mentone, IN - 42 year old David M. Seitner died from injuries he received when an air compressor system cover on a truck exploded in his face.  He was working for RVH Trucking at Cargill Nutrina Feed plant. 

January 24 - White City, OR - An unidentified 42 year old construction supervisor for Washington State based Global Pacific Environmental died on the job after being pinned to a wall by a dump truck. Global Pacific was working under Texas based Burton Construction.

January 24 - Salina, NY - 50 year old Rodney Dolittle died on the job when he suffered a heart attack while driving a taxi and crashed into a tree.  He worked for a company who uses the names Yellow Cab, Ace Taxi Service and City Taxi.

January 25 - Columbia, MD - 21 year old Brianna Benlolo of College Park and 25 year old Tyler Johnson of Mount Airy were both shot to death by an armed man who entered their Zumiez Skate Shop in The Mall of Columbia and opened fire on them.

January 25 - Gainsville, GA - 39 year old Jose Luiz Martinez was killed on the job when he fell from the elevated bucket he was working from. He was not wearing a safety harness and it appears part of a tree he cut bounced into the bucket, causing it to sway enough that he fell out. He was employed by MTZ Tree Service. 

January 26 - Toledo, OH - 42 year old Private Stephen A. Machcinski and 31 year old Private James A. Dickman died on the job of thermal burns and toxic exposure while fighting an apartment fire in Toledo.  Both were members of Toledo Fire & Rescue.

January 27 - Aurora, IL - Vincent Petrella, an Illinois Tollway employee, was killed on the job when he was struck by a semi truck as he was assisting a state trooper with another disabled semi truck on the Reagan Memorial Tollway.  The officer was injured.

January 27 - Silt, CO - A helicopter crash claimed the lives of three men in Colorado.  The men were inspecting power lines for Holy Cross Energy when the blade of the helicopter touch a power line, causing it to crash into a ravine.  Following are those who lost their lives in this incident:

40 year old Chris Gaskill of Aurora was a heat inspector for Hot Shot Infrared Inspections.
51 year old Larry Shaffer of Rifle was a crew foreman for Holy Cross Energy. 

January 28 - Trumann, AR - 53 year old Rick Winkles died on the job from an apparent heart attack while working at the scene of a fire.  He was Fire Chief of Trumann Fire Department.

January 28 - Waco, TX - 55 year old Jose Dario Suarez of Manor died on the job and his co-worker injured when the elevated boom, or manlift they were working in detached from a bridge they were working on and fell into the Brazos River. They working for Derr & Isbell Construction on a pedestrian bridge at Baylor University campus. 

January 28 - Cottage Grove, OR - 49 year old Santiago Giron of Phelan, CA was killed on the job when the semi truck he was driving rolled as he approached a stop sign.  It is unclear who he was employed by.

January 28, Westford, NY - 40 year old Daniel Banks died on the job when he became entangled in a mechanized corn chopper on a farm. The machine's power take off was frozen and then became engaged. Banks clothes were caught in the moving part. 

January 28, Pennington, PA - 33 year old Christopher Stiff of Newfoundland was killed on the job when he was crushed by a load of steel beams on his truck.  He apparently tried to stop too fast causing the load of beams to shift forward, moving into the cab of his truck.  His employer is unknown.

January 29 - Fries, VA - 55 year old Dennis Carlie Johnson died on the job after he was pulled into a conveyor belt.  He was attempting to clear lumber at C & B Lumber when his clothing was caught by the belt and he was pulled in. 

January 30 - Spanish Fork, UT - 44 year old Cory Blake Wride was killed on the job when he was ambushed and shot to death by a gunman as he was checking on what he thought was an abandoned vehicle.  He was a Sergeant for Utah County Sheriff Department.

January 30 - St. Mary's, PA - 52 year old Arwed Ralf Uecker of Boonton, NJ was killed on the job in an explosion of manufacturing equipment (an oven) at the Mersen USA plant in St. Mary's. Uecker was global director of research and development for Mersen based in Boonton.

January 31 - Woodlawn, MD - 54 year old John Kahl of Port Deposit was killed on the job when he was struck by a motorist while working along a section of roadway construction.  He worked for a subcontractor for State Highway Administration. 

January 31 - Indianapolis, IN - 30 year old Daniel Jaffke died after being shot by someone who then stole his vehicle.  Jaffke, a delivery driver for Papa John's Pizza, had made a delivery to a resident at an apartment complex and was confronted as he went back to his vehicle.
