January 29, 2014 - Detroit, MI - 23 year old Nurzhigit Kaparbekov died on the job when he became pinned between two tractor trailer rigs while working for Logistics Insights, Inc.

January 31 - Little Rock, AR - Jake Harrell, a pilot for the Arkansas Forestry Commission, was searching the area for any fires when his plane crashed. The plane was not found until February 11th.

January 31 - Shelby, NC - An unidentified worker was killed on the job when he or she was struck in the head by a pipe that exploded. He was employed by United Mechanical Corp. working at KSM Castings installing heat and air conditioning units with the incident happened.

February 1, 2014 - Clarkburg, WV - 32 year old Kyle Kirkpatrick of Hulbert, OK and 27 year old Terry Lee Richard, Jr. of Bokoshe, OK were killed on the job when a tower they were repairing and removing old structure supports collapsed, causing them to fall approximately 70 feet. Other workers were injured when a second tower, weakened by the collapse of the first, also collapsed. They were employed by S and S Communications out of OK, having been subcontracted by SBA Communications.

February 1 - Clarkburg, WV - 28 year old Michael Dale Garrett died on the job when he was caught in the collapse of a tower while attempting to help communications workers who fell when another tower collapsed. The collapse of the first tower compromised the second and it came down on Garrett and others. He was a member of the Nutter Fort Volunteer Fire Department.

February 1 - Cedar City, UT - 56 year old Mark Rowley died on the job when he became trapped in a conveyor belt while working for CML Metals at the Comstock/Mountain Lion Iron Ore mine in Iron County.

February 2 - Twin Cities, MN - An 81 year old unidentified farmer was killed when he became pinned under his tractor. How the incident happened is not clear.

February 3 - Clarkfield, MN - 77 year old Richard Rosetter died after he was buried in grain while he and others were working to empty the grain bin.

February 4 - Brownsville, NE - 66 year old Ronald Nurney of Smithfield, VA was found unresponsive on the floor of a reactor building at Nebraska Public Power District's Cooper Nuclear Station. He was an employee of Bartlett Nuclear, Inc. working in NE. It is unclear how he died.

February 4 - Otay Mesa, CA - 68 year old Salvador Gonzales-Rincon of Tijuana, MX was killed on the job when he was struck and run over by a forklift while working at Central Auto Parts in Otay Mesa.

February 4 - Natick, MA - 48 year old Michael McDaniel, Jr. was killed and another worker injured when a backhoe moved forward, striking the workers. They were fixing a water main break and worked for Natick Department of Public Works.

February 5 - Barrington, IL - 51 year old Diane Shogren died after being struck by a snow plow in the parking lot of Good Sherhard Hospital where she was a nurse.

February 5 - Holualoa, HI - 38 year old Donny Loando died on the job when he was thrown from a piece of construction equipment. He was operating a single-drum roller to build a private driveway when the vehicle rolled backward down a steep slope and crashed into the carport of a neighboring residence. It is unclear who he was employed with.

February 6 - Boring, OR - 41 year old Grady Waxenfelter of Estacada was shot to death by a man who Waxenfelter had pulled over just to tell him he was missing a license plate on a trailer. He was a weightmaster for Clackamas County.

February 6 - Carrollton, VA - 56 year old Ronald Allen Swarts of Virginia Beach was killed on the job when he lost control of a dump truck which struck an embankment and rolled. He was employed by T.A. Sheets, Inc., a general contractor.

February 7 - San Francisco, CA - 46 year old Hector Vazquez of Oakland and 53 year old Philip Marich of South San Francisco both died after they were crushed under huge slabs of granite as they were attempting to unload them from inside a truck. The men worked for Galaxy Granite, Inc. of Bernal Heights.

February 7 - Fairbanks, AK - 64 year old William T. Evans died after he fell from a road grader he was operating to clear snow at a highway maintenance camp. He was an Alaska Department of Transportation employee.

February 7 - Lovington, NM - 28 year old Alegandro Garcia Perez was killed on the job when a hoist he was in touched overhead power lines and he suffered head injuries and was electrocuted. He was working in an oilfield for Patron Services.

February 7 - Houston, TX - A 32 year old unidentified worker died on the job when he buried inside a trench that collapsed onto him. The trench was not properly shored. He was employed by Interfacing Company of Texas (ICTX).

February 8 - St. Petersburg, FL - An unidentified worker was electrocuted while trimming trees.

February 9 - Middleton, DE - 46 year old Sonja Morris of Hartly was leaving her job at Amazon in Middleton when she suffered a heart attack standing next to her car.

February 10 - Idaho Falls, ID - 40 year old Jeffrey Jaggar was killed on the job when a large piece of electrical gear fell onto him. He was employed by Wheeler Electric, Inc. working at Longfellow Elementary School.

February 10 - Napoleon, ND - 39 year old Charles Sperle died after he was buried under corn grain. He was vacuuming corn from a bin when some of it became stuck. The grain suddenly broke loose and spilled through the door of the bin, burying him.

February 11 - Dallas, TX - Firefighter William Tanksley died on the job after falling from an icy highway overpass while crews were responding to one of several major accidents around the area. He was employed by Dallas Fire Department.

February 11 - Blytheville, AR - 48 year old Thomas Dazey, a locomotive operator for Nucor Steel Arkansas, was killed when a train car he was riding in derailed and he was pinned between two rail cars.

February 11 - Woonsocket, RI - 52 year old George Nasuti died on head trauma when he fell, striking his head. He was the Woonsocket High School Athletic Director and was officiating a basketball game for the school when he fell.

February 11 - Heyburn, ID - 54 year old Antonio Ponce of Burley was killed on the job when he slipped and became caught in farm equipment used to mix cattle feed. He was employed by Anderson Farming, Inc.

February 12 - Highland, CA - 39 year old Nelly Kantun was shot to death inside the Domino's Pizza restaurant where she was manager. Reports say it was a domestic dispute and her boyfriend was arrested.

February 12 - Harrison, NJ - 65 year old Raymond McDonough, beloved Mayor of Harrison died of a heart attack in his office after returning from lunch.

February 13 - Ashburn, VA - 32 year old Lovo Guevara Geovany Arnoldo of Vienna was struck and killed by a dump truck as he stood near his truck. He was a contract worker for Virginia Department of Transportation in Ashburn.

February 13 - San Leandro, CA - 48 year old John Jacini of Castro Valley was killed at work when he was crushed under equipment that fell onto him. He was working under a front wheel loader when it slipped off its supports, falling on Jacini. He was employed by Volvo Construction & Equipment Services.

February 14 - Scotts Bluff, NE - 24 year old Amanda Baker died on the job when she was strangled to death by a 15 year old inmate she was checking on. She was employed by Scotts Bluff County Detention Center.

September 14, 2013 - Canastota, NY - 58 year old Richard C. 'Rick' Whitney, Jr. of Bernhards Bay was killed on the job when an explosion occurred as he was welding in a methane gas dome at the Canastota Wastewater Treatment plant. He was employed by Joy Process Mechanical.

October 21, 2013 - Chattanooga, TN - 34 year old Mandie Rachael Creel Chitwood died after the ladder she was working from was knocked out from under her by a forklift operator sending her into a large pipe at Wrigley (candy) Manufacturing.

January 9,2014 - Bowling Green, FL - An unidentified truck driver of a fully loaded fuel tanker died when the truck crashed, and exploded into a fire ball. Efforts to find his identify were unsuccessful. He was employed by B Line Transport.

January 13 - Chicago, IL - 22 year old Marco Antonio Huicochea Gonzales was killed when an aerial steel acrobatic ring he was using for dance practice broke free, striking him in the head while he was on a break at Danceworks Chicago.

January 14 - North Highlands, CA - 22 year old Marc Zhuchenko was trying to help his co-workers when a robbery suspect was herding them into a back room at O'Reilly Auto Parts when he stabbed the suspect. That act angered the suspect who then shot Mr. Zhuchenko to death.

January 16 - Kitzmiller, MD - 20 year old Daniel Lambka died on the job when he became pinned between a frame of a feeder machine and a rib wall at Mettiki's Mountain View Mine.

January 22 - Bracken County, KY - Tom Stephenson was killed after he was knocked down and then trampled by a bull as he worked at David Emmons Farm.

February 3, 2013 - Vancouver, WA - 45 year old Ryan Momeny of Camas was shot to death by an employee for an unknown reason. Momeny was General Manager for Benjamin Moore & Co.

February 6 - Tacoma, WA - David Pierson died on the job of a cardiac event while working for Larson Automotive Holdings Inc. dba Larson Hyundai.

February 10 - Morton, WA - 21 year old Tyler Bryan of Toledo, WA was killed on the job after being struck by a spinning log dangling from a skyline logging cable. He was employed by Brindle Technical Logging, Inc. dba Brintech.

February 13, 2013 - Alexandria, VA - 34 year old Travis Jackson was working on road construction when he was struck by a light trailer. He was standing near the trailer when a motorist struck a vehicle that the trailer was hooked to, causing it to swing around striking Jackson.

February 15 - Gillette, WY - 26 year old Nathan James Scott died after he was struck by a welding truck while working at the Houston based EOG Resources, Inc. Cyclone Rig 33. He was an employee for Cyclone Drilling, Inc.

February 17 - El Cajon, CA - An unidentified worker was killed on the job when he became pinned between a forklift and large equipment while working at GKN Aerospace.

February 17 - Fresno, CA - 33 year old Juan Gonzales and 34 year old Brian Law, both California Highway Patrol troopers, were both killed on the job after their cruiser crashed and rolled as they were responding to a call in Kingsburg, CA.

February 17 - Monroeville, OH- 29 year old Andrew Robert Bishoff died after being injured on January 11th when he fell from a ladder he was climbing on the outside of a grain silo. He was employed by Triple H Farms.

February 18 - Belridge, CA - 53 year old James Dennis Robbins died on the job after falling from a production oil rig near Belridge. It is unclear who he was employed by.

February 18 - Long Beach, CA - 50 year old Efren Sanchez of La Puente was killed on the job after he fell from scaffolding while working at Port of Long Beach. It is unclear who he worked for.

February 18 - Columbus, NE - 53 year old Denise Jean Wieser of Richland was killed on the job in an incident possibly involving a forklift. She was working for BD Medical manufacturing plant when the incident occurred.

February 18 - Dallas, NC - 63 year old Joe Herman Ratchford died on the job when he was struck by a branch from a large tree he was attempting to cut a limb. He was owner of Apple Creek Golf Course where the incident took place.

February 19 - South Windsor, CT - 22 year old Connor Betts and 55 year old Lloyd Folsom, Jr. were both electrocuted when the truck they were in became energized after the raised bed of a dump truck Betts was operating touched electrical lines. He exited the truck and was electrocuted when he touched the truck. Mr. Folsom then exited the truck to help Betts and was also electrocuted. Folsom owned Folsom Construction.

February 20 - Acadia, LA - 19 year old Taylor Morvant died at hospital after receiving injuries at an unidentified oil field. Morvant and other worker were pumping crude from a vacuum truck when a hatch flew open, striking him in the head and chest.

February 20 - Lexington, KY - 24 year old Isaias Cabrera Ramos of North Carolina died of traumatic head injuries while working at a University of Kentucky campus construction site. He was working on the ground when plywood blew off the roof of a building, striking him. He was employed by Messer Construction Company.

February 20 - Benton, MO - 54 year old Terry Meek of Grant City suffered crushing injuries when an unloading belt fell onto him while working at Helena Chemical Company in Benton.

February 20 - Doctortown, GA - 27 year old Sarah Elizabeth Jones of Atlanta was killed on the job when she was struck by a freight train while working as a second camera assistant on a film. The film crew were working on a train track and while waiting for two scheduled trains, they were surprised by an unexpected train moving very fast. It appears she was working for the Georgia-based Meddin Studios, along with several other companies involved in the film production.

February 21 - Louisville, KY - 37 year old Felipe Mata Fiscaya of Alabama died on the job when he fell about four stories down an elevator shaft. He was contract employee working at Kosmos Cement Plant.

February 21 - Grundy, VA - 24 year old Arthur David Gelenster was killed on the job when he became pinned by a coal extracting machine. He was working for SunCoke Energy's Dominion Coal Corporation Mine #30.

February 21 - Woodbridge, VA - 21 year old Glenda Marisol Coca-Romero was shot to death and her 42 year old unidentified co-worker critically injured during a robbery attempt at Plantanillos Grocery and Jewelry.

February 21 - West Haven, VT - 59 year old Darnell Hannah of Millstone, NJ was killed on the job when the tour bus he was operating was involving in a crash, on icy roads, with a tractor trailer semi truck and he was trapped in the bus. He was employed by Triple D Travel out of New Jersey.

February 21 - Bovina, WI - 63 year old Michael Calusen of Clintonville died on the job when he was pinned against a barn by a truck that slid on the ice and into him. He was working on a farm in Bovina.

February 21 - Los Angeles, CA - 51 year old Cesar Valenzuela died while working as a baggage handler at the Los Angeles airport LAX. He fell from a baggage tug and authorities are investigating a possible heart attack that caused Valenzuela to fall.

February 21 - Wichita, KS - 44 year old Efren Antonio Villarreal-Alvarado was found inside his Santana Concrete work truck, shot in the chest. He later died at hospital. Police say robbery was the motive and two men have been charged with his murder.

February 21 - Houston, TX - 52 year old Donalvon ODell Rhyne was shot to death by robbery suspects while driving his United Cab vehicle. All four suspects have been arrested for his murder.

February 22 - Columbia, MO - Bruce Britt, a Lieutenant for the Columbia Fire Department was killed on the job when a concrete walkway collapsed and he was buried under some of the debris. He was responding to a call about the partial collapse of the walkway.

February 22 - Chicago, IL - 28 year old Javan Boyd was driving his own vehicle as a livery (cab) driver when he was shot to death. It was unclear if robbery was a motive. He was working for Pershing Livery.

February 24 - Toone, TN - 41 year old Michael Chism died after suffering burns on February 22nd over 90% of his body in an explosion inside a mixing room at Kilgore Flares.

February 24 - Corpus Christi, TX - 53 year old Jeffery Babzien was killed on the job when a 2,000 pound pipe fell from an overhead crane, crushing him. He worked for Southern Minerals at Trafigura EF-90 storage terminal complex.

February 24 - Houston, TX - Two unidentified workers died after being electrocuted when a crane touched power lines. It is unclear who they were employed by but were reportedly oilfield workers.

February 25 - Elmwood, NE - 51 year old John P. Bruns was killed on the job when the propane truck he was driving was struck by a train. There were no gates to warn drivers of oncoming trains; only signs. He was employed by Drake Propane Services.

February 26 - El Cajon, CA - 49 year old Bernardo Teran died on the job when a palm tree he was cutting down became unstable and fell onto him. He was the owner of the company working at a private residence.

February 26 - East Peoria, IL - 35 year old Ryan Michael Regan was electrocuted while working on a power pole behind a private residence. He was employed by J.F. Electric, Inc. of Edwardsville.

February 26 - Peculiar, MO - 29 year old Joshua Anderson of Adrianwas crushed to death by a tractor trailer semi rig that struck him as he was helping the driver back up. He was employed by Worldwide Steel Buildings in Peculiar.

February 27 - Union Township, OH - Keith Jester was killed and a co-worker injured when they fell from a utility pole, which then fell onto them while working for Duke Energy.

February 27 - Oakfield, WI - 42 year old Anthony J. Shields of West Bend, a member of the maintenance staff for Seneca Foods, died on the job when his clothing became entangled in a lathe on a piece of machinery.

February 27 - Prescott, AZ - 64 year old Derk Janssen collapsed and died during one of his English classes while working at Prescott Unified School District's Prescott High School.

February 28 - Spokane, WA - 52 year old Paul M. Inc died on the job from head trauma. After receiving an electric shock he was knocked off a ladder causing him to strike his head on the ground. He was a long time employee for Nordstrom, Inc.

February 28 - Portage, WI - 29 year old Michale Kittle was killed on the job when he was crushed under a concrete form that from equipment being used to lift it. He was employed by Wieser Concrete Products, Inc.

February 28 - New Haven, IN - 50 year old Todd Fritcha was crushed to death under the rear wheels of a dump truck. He was Vice President for Fritcha Excavating.

February 28 - Riverhead, NY - 50 year old Declan Boland died when he was buried in an avalanche of mulch and soil that collapsed underneath him. He was attempting to put out a mulch fire at the open pit sand mine owned by East Coast Mines and Minerals.

February 3, 2014 - Kingston, NY - 33 year old Aron J. Thomas of Olivebridge was shot to death by a co-worker while working at their Department of Environmental Protection office. No motive was given for his murder but his co-worker has been arrested and charged.

February 21- Roy, WA - Lonnie Olsen was killed on the job when the he was struck by his truck. After parking the truck, he walked behind it and it rolled back into him. He was employed by Tacoma Moving and Storage.

February 26 - Kent, WA - Robert Rodrick died of a heart related medical issue while working for Domino's National Commissary Corp / Domino's Pizza.

February 28 - Clayton, NC - 43 year old Phillip S. Olive was killed on the job when a tree fell on him while clearing land. It is unknown who he was employed by.

March 2, 2014 - Pendleton, OR - Mario Santana of Sunnyside, OR died near Pendleton when the milk truck he was driving crashed. He was employed by LTI Inc dba Milky Way.

March 3 - Phoenix, AZ - 43 year old John Hobbs, a member of the Phoenix Police Department, was killed on the job when he was shot during a gun battle with an ex convict. Another officer was injured.

March 3 - East Wanatchee, WA - Jason Herrin was killed on the job when the truck he was driving was involved in a head-on crash as he tried to pass another vehicle. He was employed by Confluence Health.

March 3 - Kensington, MN - 72 year old Paul Stark died on the job when he became trapped or pinned between the bucket of his farm tractor and a metal cattle gate.

March 5 - Detroit, MI - 23 year old Erik Deighton was killed on the job when he was crushed inside a press machine. He was attempting to clear an obstruction when the machine cycled and crushed him. He was employed by Colonial Plastics.

March 5 - Belton, SC - 47 year old Mike Cowert died after being exposed to toxic fumes while working at Mike's Sodablasting and Sandblasting. He was sandblasting a vehicle when a generator exploded and he inhaled toxic carbon monoxide that came through the mask that was hooked to the generator.

March 6 - Hastings, MI - 42 year old Alicia Avalos Rebolledo was fatally injured when she was attacked by a bull while working at a Barry Bounty farm.

March 6 - Seagraves, TX - 43 year old James Croft of Tyler was killed on the job when a silo tank he was attempting to take down fell down prematurely. He was using an excavator and the Venture Chemicals tank fell down on top of him. He was employed by K & L Oil Field Demolition.

March 7 - Tonawanda, NY - 56 year old Steve Pawlowski, owner of Steve's Auto Service, died after a fuel pump he was working on exploded and caught on fire. He was severely burned.

March 7 - South Plainfield, NJ - 65 year old Robert Donohue of Glen Gardner suffered fatal crushing injuries when a stack of steel beams fell over, pinning him against a tractor trailer semi. It is believed he worked for Bushwick Metals.

March 7 - Beverly Hills, CA - 40 year old Nicholas Lee, a Los Angeles Police officer with the Hollywood Division, was killed when his patrol car was struck by a dump truck as he and his partner were responding to a police call. The dump truck driver, who lost control of the vehicle, and the other officer were injured.

March 8 - Greenwood, SC - 46 year old Todd Hendrix died on the job when he became trapped in a piece of machinery at the meat processing plant Louis Rich in Newberry.

March 9 - St. James, LA - 36 year old Marcos Rodriguez living in Campbell, OH but originally from Brazil, died on the job when he fell 100 feet from scaffolding at the Sunshine Bridge in Louisiana. He was working alongside his brother as painters for North Star Painting of Youngstown, OH.

March 10 - Upper Manhattan, NY - 58 year old James Romansoff of Yonkers was struck and killed by an elevated train. He was working with a crew who thought train traffic had been suspended in the area. He was an employee for Metro North's Power Department.

March 10 - Beloit, IL - 39 year old Alfredo Arrendondo was killed on the job when he became trapped in a piece of machinery while working at Behr Iron and Metal, a metal recycling plant.

March 10 - Gibsonia, PA - 64 year old Bradley Oberg of Cranberry died after falling from a scaffold. He was a volunteer helping to set up for a musical being held at Pine-Richland High School in Gibsonia.

March 10 - Rotterdam, NY - 39 year old John Dean of Albany died after being pinned by a tow truck that rolled into him. It was his first day on the job for AC Body Works.

March 10 - Seward, NE - 36 year old Jeramie Schoepf died in hospital a week after being struck in the head by a tree branch as he was clearing trees for a city recreational trail expansion. He was working for Seward Street Maintenance Department.

March 10 - Glendale, AZ - 26 year old Scott White was fatally shot to death by a customer who was attempting to steal gas from the AMPM Convenience store he was working at.

March 11 - Russellville, KY - Genaro A. Quevedo from Mexico was killed on the job when he was caught in a piece of machinery. He was working for Stark Truss and not being seen by others he was crushed between a press and roller of the machine.

March 11 - Enterprise, OR - 62 year old Harry McNall was killed on the job while working to erect a silo for Wallowa County Grain Growers. He was on a section of scaffold that tipped, falling into the structure and knocking him to the ground.

March 11 - Edgar, NE - 66 year old Alan Watson of rural Fairfield died on the job while working in a grain silo on a farm near Edgar. He was using an auger inside the silo moving grain from it to a truck. When others noticed corn grain wasn't coming out they discovered him inside. He may have died of natural causes.

March 12 - Little Rock, AR - 51 year old James Taylor of Cabot died on the job after falling from the roof of an industrial building. He and a co-worker were measuring the size of the roof when Taylor from it. It us unknown who his employer was.

March 12 - Annapolis, MD - 41 year old Benjamin Delano Kirby, Jr. was assaulted and shot while driving his personal vehicle for ABC Green Taxi.

March 13 - North Bend, OR - 65 year old Phillip "Phil" N. Bowman was fatally injured when he was struck by the gate of a dump truck. He was a North Bend Street Department employee.

March 13 - Orwigsburg, PA - 66 year old Terry Lee Warner of Schuylkill Haven was killed on the job after being struck by a semi truck backing into a loading dock. He was employed by Classtex Mills of Orwigsburg.

March 14 - Cedar Rapids, IA - 28 year old Jacob "Jake" B. Harper of East Moline, IL suffered fatal head injuries after steel pipes fell off a truck, striking him. He was unstrapping the pipes at Cedar Rapids Country Club when they fell from the truck. He was employed by Michels Pipeline Corporation, a subcontractor for MidAmerican Energy Company.

March 14, 2014 - Tucson, AZ - 59 year old Thomas Robert Quesnel, a Tucson Fire Department arson investigator died of work related leukemia.

March 17 - Troy Mills, IA - 75 year old Donald Reilly of Coggon died on the job after he fell from the top of a grain trailer that he was walking along. He was working on a farm near Troy Mills.

March 18 - Seattle, WA - 62 year old William Strothman of Bothell and 59 year old Gary Pfitzner of Issaquah were both killed on the job when their KOMO News helicopter crashed upon take-off below the helipad on Broad Street in Seattle. Strothman was a news photographer and Pfitzner was the pilot of the station helicopter.

March 19 - Fort Bragg, CA - 48 year old Ricky Del Fiorentino, a deputy for the Mendicino County Sheriff's Office was shot to death while he was still in his patrol car when he confronted a suspect.

March 19 - Pasadena, MD - 21 year old Chad Weller of Stevensville, was killed on the job when he fell approximately 200 feet from telecommunications equipment that was on top of a water tower. He was working for a subcontractor for the telecommunications company but the name of either is unknown.

March 20 - Onondaga, NY - 41 year old Christopher Spicer of Marietta, died on the job after he suffered fatal crushing injuries. He was working on Volles Dairy Farm after he accidentally hit a lever on a hydraulic lift of a grain truck, and the box lowered on top of his chest.

March 20 - Dickinson, ND - 32 year old Kristopher Barnes of Minot died on the job when he was struck by a 'traveling block', a large piece of equipment used at drilling rig sites. He was employed by Ensign U.S. Drilling. Petro-Hunt out of Dallas owns the rig site.

March 20 - Molalla, OR - 57 year old David Moore of Woodburn died on the job when he was struck by a forklift as he was securing a load on his semi truck at RSG Forest Products lumber yard. He worked for Blue Pacific Logistics Inc.

March 21 - Outlook, WA - 48 year old Richard Piroli died after falling ill while changing a tire at George Deruyter and Son Dairy Farm. He took a rest and was talking to co-workers but became unresponsive and died at the scene.

March 21 - East Chicago, IL - 62 year old Robert "Lane" Watts of Westville died after he suffered thermal burn injuries on February 8th. It is unclear how, but he fell into scalding water at ArcelorMittal Indiana Harbor in East Chicago.

March 21 - Catoosa, OK - 48 year old Eri Hernandez of Edmond was killed on the job when she was struck and run over by a loaded dump truck while working near the interstate. She was a contractor with Mack's S&S in Oklahoma City.

March 21 - Boston, MA - An unidentified man, who is reportedly a Ugandan immigrant, was killed when he fell in an elevator shaft. He was an elevator operator and the freight elevator became stuck between floors. As he attempted to jump to safety, he missed a platform and fell to the sub-basement.

March 21 - Liberty, PA - 35 year old Andrew L. Graham was killed in a logging incident. He was working with his father and as a tree was cut, another tree that was leaning up against the first tree fell and struck Graham.

March 21 - Sinton, TX - 60 year old Marciano Martinez-Garcia died on the job after being struck by a tractor trailer in a construction work zone. The semi driver saw Martinez-Garcia attempt to cross a lane and attempted to avoid hitting him, but the trailer struck him.

March 22 - Oso, WA - 55 year old Stephen Neal of Darrington, was at a private home in Oso installing a water heater when he was buried in the massive mudslide that occurred in Oso. He was employed by R&R Plumbing.

March 22 - Windermere, FL - 31 year old Robert German, an officer for Windermere Police Department, was shot to death by two teenagers that he had stopped while on foot patrol. He called for back-up but when they arrived, they found him dead. The teens committed suicide after the shooting.

March 22 - Oso, WA - 66 year old Bill Welsh of Arlington, was an electrician for Whitley Electric and was meeting Stephen Neal (see above) at the same home when the massive mudslide happened in Oso and he was buried in it.

March 22 - Youngwood, PA - 57 year old Edwin "Jeff" Wentzel was killed on the job when he was struck by a train under a bridge. He had been searching for a missing woman. Wentzel was a volunteer firefighter for Youngwood Fire Department.

March 24 - Norfolk, VA - 24 year old Petty Officer Mark Mayo was shot to death as he was attempting to protect a sailor from a man with a gun. A civilian truck driver was able to get on board a pierside destroyer and had taken away the gun from a female sailor when Mayo went to help. As he pushed the sailor out of the way of the gunman, he was shot.

March 25 - Princeton, IN - 41 year old Timothy W. Memmer died of injuries received while performing maintenance on an underground piece of mining equipment. He was employed by Gibson County Coal LLC.

March 25 - Leray, NY - 25 year old Jesse M. Chase of Evans Mills died after he was pinned by a feed trailer on a farm. He was at the rear of the trailer checking a tire when the trailer loaded with feed tipped backwards, pinning him between it and the ground.

March 25 - Carmel, IN - 59 year old Michael Davis of Noblesville died after he was electrocuted and was on fire at the Carmel substation where he was a substation maintenance supervisor.

March 26 - Boston, MA - 43 year old Lt. Ed Walsh and 33 year old Firefighter Michael Kennedy died when they were trapped in the basement of a brownstone that was called a 9 alarm fire. Both were Boston Fire Department firefighters.

March 26 - Blaine, KS - 38 year old Martin Powers of St. Charles and 25 year old Seth Garner of St. Peters died after they suffered injuries due to a fall. Both were working to remove old equipment from an older tower when it and another tower collapsed. The men were working for Wireless Horizon.

March 26 - Sale City, MN - 62 year old Jackie McCook was found dead inside a grain bin while working for Snipes Farms. He was discovered with his heart medication nearby and authorities think he may have had a heart attack.

March 27 - Thomasville, GA - 31 year old Joshua Fulghum died on the job when he slipped into an auger while sweeping seeds inside a grain silo. He was employed by Southwest Georgia Ethanol plant.

March 27 - Houston, TX - 53 year old Rueben Portillo was shot to death on the job during a robbery attempt at the Lucky Star Game Room where he was a security guard.

March 28 - Salkum, WA - 69 year old John B. Leonard of Chehalis died from injuries he received at a logging site. As Leonard made a partial cut on a tree in order for the company owner to push it over with equipment, the tree brushed against another causing a large limb to snap off and fall onto Leonard.

March 29 - Tonganoxie, KS - 43 year old Sergiy Angelchev of Aurora, CO died from injuries he received when the tractor trailer semi he was driving struck a Kansas Turnpike Tollbooth at highway speed. His passenger was injured.

March 30 - Towson, MD - 24 year old Alejandro Morales Silver of Bladensburg died after he was struck by a car while directing traffic on March 21st. He was an employee for Olney Masonry Corporation.

March 30 - West Carrollton, OH - 48 year old James Mitchell of Vandalia was found dead a day after he apparently died while working at Certified Heat Treating in West Carrollton. OSHA reports that he was caught in a door while working with a furnace.

March 31 - Johnson City, NY - 43 year old David Smith, an officer for the Johnson City Police Department, was shot and killed by an assailant who wrestled away his service revolver during a struggle as the officer still sat in his patrol car.

February 17, 2013 - Sayreville, NJ - 34 year old Michael McQuade of Clementon was killed when he was pinned by a barge under a bridge. He was responsible for welding and pouring concrete under water for Hydro-Marine Construction.

March 11, 2013 - New Jersey - 50 year old William Montgomery died on the job when he fell from a roof. No other information is available.

March 27, 2013 - New Jersey - 30 year old Jose Barralaya was killed on the job when he fell from a roof. No other information is available.

March 24 - Newark, NJ - 30 year old Mohammed Sy, also known as Papa Khaly Ndiaye, was shot to death as he tried to break up an altercation between patrons at an IHOP Restaurant in Newark.

March 28, 2013 - Clinton, NJ - 35 year old Jeffrey Scheurer of Flemington died after he was struck by a vehicle during a controlled burn in Hunterdon County. He was a firefighter, employed by New Jersey Forest Fire Service.

April 5, 2013 - Kearney, NJ - 59 year old Horacio Fonseca suffered a fatal heart attack while at work. He was owner of Portugal in America Construction Company. No other information is available.

April 18, 2013 - New Jersey - 57 year old John Tiedeman died on the job after he was struck by an industrial vehicle. No other information is available.

April 29, 2013 - Ramsey, IN - 52 year old Kenneth Downey was killed on the job when his utility truck was struck by a train. He apparently did not see or hear it approaching. He was employed by Ramsey Water Company, Inc.

April 30, 2013 - Silver Bay, MN - 58 year old Lee Roy Jacobson died on the job when the excavator he was operating flipped upside down into a river. He was using the tracked excavator to break up ice clogging road culverts. He was employed by Lake County Highway Department/Silver Bay Street Department.

May 1, 2013 - New Jersey - 70 year old William Schmidt, Jr. died after he tripped and fell while working. No other information is available.

May 3, 2013 - Madison, WI - 57 year old Scott Schultz died on the job after he fell on May 1st from an elevation while working for Scott Schultz Roofing and Painting.

May 6, 2013 - Miami, FL - 54 year old Silvio de Jesus Tapasco died on the job after he fell from an elevation while working for SD Restoration, Inc.

May 7, 2013 - Orono, ME - 56 year old Richard Thibodeau of Carmel was struck and killed by a motorist as he worked along a road way. He was employed by Harvey Paving & Sealcoating.

May 7, 2013 - Chamblee, GA - 40 year old Vasile Mois was killed on the job after be was caught in or between an unknown object. He suffered asphyxia. He was employed by Southern Mechanical Contractors, Inc.

May 7, 2013 - Hayward, CA - 38 year old Christopher Cole collapsed due to natural causes while working for Bohler-Uddeholm Corporation.

May 8, 2013 - Westland, MI - 29 year old Brian Woehlke died on the job of smoke inhalation after his air hose became kinked and he was not able to find the exit of the building in time. He was firefighter for Wayne Westland Fire Department.

May 9, 2013 - Ferndale, MI - 73 year old David Burnette died on the job when he fell from the roof of a building. He was an insurance adjuster and used a ladder that was not fully extended above the roof line. He missed the top rung, causing him to fall. He was employed by Eagle Adjusting Services, Inc.

May 9, 2013 - Elizabeth, NJ - 51 year old Otto Benke of Summit was crushed to death when a car he was working under fell off a lift. He was employed by J&L Auto Body Parts.

May 12, 2013 - Middletown, CA - 65 year old Vincente Valencia died after he became ill at work. He died of natural causes while employed by Engineering/Remediation Resources.

May 14, 2013 - Commerce City, CO - 49 year old Luciano Seranno Parades died on the job after he fell from an elevation while working for Garay Construction, Inc. No other information is available.

May 14, 2013 - Gas City, IN - 45 year old Andrew Powell was found unresponsive by his coworkers and had died of heart failure. He was employed by American Woodmark Corporation.

May 16, 2013 - Daphne, AL - 51 year old Roberto Campos Leco was killed on the job after he fell from an elevation while working for Olmedo Construction, Inc. No other information is available.

May 16, 2013 - Scottsdale, AZ - 49 year old James William Brown was killed on the job when he was struck by a tree just as an excavator operator was dropping a large tree on an 'end dump'. He was employed by M&P Venture Partners, LLC.

May 17, 2013 - Merrillville, IN - 34 year old Stephen Lewis of Rolling Prairie died on the job of electrocution. He was guiding an iron pipe connected to an excavator attached to a rope sling into a trench. The boom of the excavator came into contact with a power line and became energized. He was employed by H & G Underground Utilities.

May 17, 2013 - Camden, NJ - 42 year old Keith Rainer died on the job after a concrete barrier wall was pushed over by front end loader and he was crushed under debris. He was an employee at Harris Camden Terminal.

May 18, 2013 - Gary, IN - 54 year old Lamar Beach collapsed of a heart attack while talking to other workers and died at hospital. He was employed by Multi-Wall Packaging Corporation.

May 20, 2013 - Casper, WY - 62 year old Douglas J. Franklin died at hospital after suffering injuries when his clothing was caught in a vertical metal lathe. He was employed by N. C. Machine LLC.

May 21, 2013 - Bee Cave, TX - 42 year old David Maldanado died on the job after he was caught in or between an unknown object while working for NPL Construction Company. No other information is available.

May 21, 2013 - Brooklyn, NY - 30 year old Juan DeLaCruz was killed after he fell from an elevation while working for Gerardo Ramirez. No other information is available.

May 21, 2013 - Chico, CA - 42 year old Deacon Murphy of Los Molinos died after being struck by a log that rolled from a truck as he attempted to remove the last remaining log-binding cable. He was working for himself. No other information is available.

May 23, 2013 - Salt Lake City, UT - 31 year old Nathan T. Jackson was killed on the job when he was crushed under a heavy container that became unstable and slid off onto him. He was employed by Mountain Peak Vinyl.

June 1, 2013 - South San Francisco, CA - 66 year old Larry Bailey was killed on the job when he fell from an elevated platform. He was painting from a pallet that was elevated on the forks of a fork truck when he fell. He was employed by K. Wilson & Co., Inc.

June 3, 2013 - Carlsbad, NM - 23 year old David L. Duran died on the job as he was attempting to disassemble and move a crane. A chain on the rig began to fall and as he reached to stop it from falling, he fell off the rig. He was harnessed but not tied off. He was employed by Wilbanks Trucking, Inc.

June 3, 2013 - Spirit Lake, IA - 40 year old Cory Ritzer died at hospital after he was struck in the head. He was directing a skid steer loader to the edge of a trench to fill when the edge collapsed sending the machine into Ritzer. He was employed by Tom's Plumbing & Heating, LLC.

June 4, 2013 - Westport, CT - 34 year old Walter Pani Chimbo of died after he slipped and fell from a roof while working for G & G Enterprises LLC.

June 4, 2013 - Phoenix, AZ - 56 year old John Edgar Coolidge died after falling from a 12 foot ladder when he lost his balance. He was an independent cameraman working in the film industry.

June 4, 2013 - Radford, VA - 39 year old Steven Anthony Cox was killed on the job when he was struck by a tractor trailer semi truck. He was performing routine maintenance along a roadway for Infrastructure Corporation.

June 5, 2013 - West Monroe, LA - 38 year old Alfredo Zavala died after he fell through a hole at the Graphic Packaging Paper Mill. He was employed by Industrial Roofing and Construction LLC.

June 6, 2013 - East Ridge, TN - 43 year old Roger Lee was killed on the job when he fell from the roof of a building. He was using a pulley system to pull a compressor to the roof's edge but it hung up on the gutter causing the rope to break. He and the machine fell from the elevation. He was working for Jake Marshall Service, Inc.

June 10, 2013 - Bridgewater, NJ - 37 year old Michael Spencer died on the job after he was struck by a pipe wrench. He was employed by Kmetz, Inc.

June 10, 2013 - Butte, MT - 37 year old Jason St. Pierre died on the job when he fell from an unknown elevation, suffering a concussion and abrasions. He was working for Mike Rasnick.

June 11, 2013 - Oswego, NY - 44 year old John Halstead, Jr. of Fulton was killed on the job when a trailer he was helping to dismantle collapsed on top of him. He was working for Gerald Shepard.

June 11, 2013 - Port of Camden, NJ - 56 year old John Mahoney of Sicklerville died after he was struck by a forklift, receiving several traumatic injuries. He was a member of the International Longshoreman's Association working at the Beckett Street Terminal.

June 11, 2013 - Laurel, MD - 40 year old Erik Meekins of Baltimore suffered fatal head trauma after being struck by a motorist during road construction. He was employed by Road Safety, LLC.

June 16, 2013 - Woodland, NJ - 40 year old Jose Delcid was killed on the job when he was struck by a tree limb that was cut by another worker who was higher in the tree. He was working for Greenway Lawncare and Landscaping.

June 17, 2013 - Corning, NY - 52 year old William Schaeffer was electrocuted and then suffered a cardiac event while working at the Town of Corning rail yard for JMG Construction Corp.

June 18, 2013 - Altoona, WI - 65 year old James Bridges of Eau Claire died on the job after falling from an elevation at a residential construction site. He was employed by C & E Wurzer Builders LLC.

June 18, 2013 - Frisco, TX - 36 year old Victor Hugo died on the job after falling from an elevation under unknown circumstances while working for JAA Construction.

June 18, 2013 - Ponce, Puerto Rico - 68 year old Ismael Perez Baez was killed on the job when he was buried in a trench collapse while placing formwork. He was then struck by an excavation machine. He was working for Optima Contratista General.

June 19, 2013 - Des Moines, IA - 37 year old Michael Grant was killed on the job when he was pinned between a garbage truck and a parked semi truck as he was directing a wrecker to back up to the disabled garbage truck. He was working for Recycling, Inc.

June 19, 2013 - Desert Center, CA - 49 year old Art Morales became ill from the heat while working for E Light Electric Services, Inc. He refused medical treatment at the time but collapsed later while talking with a coworker.

June 20, 2013 - Indianapolis, IN - 71 year old Richard Gene Bray died on the job after he fell through a roof skylight as he tried to remove fasteners from the skylight. He was employed by Bray-Independent Contractor.

June 22, 2013 - Hughson, CA - 43 year old Marco A. Flores fell ill and died at hospital while working as an irrigator for Duarte Nursery, Inc.

June 23, 2013 - Las Cruses, NM - 38 year old Raymond Duran was injured on June 11th when he was struck in the chest by a reciprocating saw a coworker was using. When that worker lowed the saw, it slung back striking Duran in the chest and puncturing his heart. He was employed by Outlaw Construction Company.

June 24, 2013 - Rye, NH - 57 year old David French of Franklin died after falling from a roof height ladder, then striking a deck railing before landing on the ground. He was working for Peggy Arend.

June 24, 2013 - Waxahachie, TX - 53 year old Burl Dean Blevins died on the job after he fell from an unknown elevation, causing head injuries. He was employed by KBS Enterprises, LLC.

June 24, 2013 - Vieques, Puerto Rico - 73 year old Elliot Guzman was killed on the job when he was struck by a concrete beam that was knocked over by a truck he was directing to back up. He was employed by Municipio De Vieques Saneamien.

June 24, 2013 - Cantril, IA - 28 year old Jerome Horst died of an injury related to a forklift while working for Cantril Feed and Grain.

June 24, 2013 - Muscatine, IA - 65 year old Lyle Wayne Haury died of head trauma as a tornado swept through the area he was working in. He and others sought shelter but he was found, after it passed, with head injuries. He was working at Krieger Collision Center.

June 24, 2013 - Knoxville, TN - 28 year old Dr. Zackary Rose was found unconscious in a breakroom of the University of TN where he was an anesthesiology resident.

June 24, 2013 - Seeley, CA - 56 year old Richard Jones died of probably natural causes at a solar facility. He was found unresponsive next to his work truck. He was employed by CLP, a skilled labor staffing agency.

June 25, 2013 - Chicago, IL - 36 year old Ronald H. Guyton, Jr. of Aurora died from heat stroke while working for Aldridge Electric, Inc.

June 25, 2013 - Monticello, KY - 29 year old Kevin Miller was owner of Townsend Tree Service and using an elevated bucket truck to lower branches he was cutting. The branches became too heavy for the bucket and toppled the bucket.

June 26, 2013 - San Jose, CA - 52 year old Guadalupe Arteaga died on the job from electrocution when a climbing rope made contact with an overhead power line. He was employed by Lupe's Tree Service & Landscape.

June 27, 2013 - Boca Raton, FL - 22 year old Michael Sosa was killed on the job from electrocution in unknown circumstances while working for Tim Graboski Roofing, Inc.

June 27, 2013 - Columbia, IL - 60 year old Dennis Fassel died on the job of natural causes when he suddenly slumped over the steering wheel while driving his City of Columbia Public Works truck. His passenger was able to stop the truck.

June 28, 2013 - Saraland, AL - 47 year old Walter Pitts died after he was struck by an unknown object while working for Hosea O. Weaver and Sons, Inc.

July 3, 2013 - Ordway, CO - 32 year old Nicholas W. Wilson was killed on the job when he was struck by an unknown object while working for Midwest Mole, Inc.

July 3, 2013 - Groveland, CA - 21 year old Annais Rittenberg died on the job when she was struck by a falling tree. She was sitting around an eating area and campfire circle when an oak tree spontaneously broke and fell. She was near Yosemite, CA at Camp Tawonga / United Jewish Community.

July 5, 2013 - New York, NY - 55 year old Abbas Bahnam-Bakhsh of Staten Island died on the job after being struck by pipes falling when harness used to hoist them from the truck broke. He was working on Governor's Island for Bedford-Carp Construction JV, Inc.

July 5, 2013 - Miramonte, CA - 55 year old Bruce Michigan was a maintenance supervisor for Sequoia Lakes Conference Center and was working with others on a plumbing install when he fell ill and collapsed.

July 5, 2013 - Bluffton, SC - 44 year old John Trerotola was killed on the job by electrocution when he touched a live wire in the attic of a home he was working in for Dedicated Electric, Inc.

July 5, 2013 - Honolulu, HI - 73 year old Marvlee Naukana-Gild suffered fatal head trauma after falling down a concrete stairway to the landing while working for UHM-School of Hawaiian Knowledge.

July 5, 2013 - Waterloo, IA - 35 year old Deea Lauritzen died on the job when she was buried in a trench collapse while repairing a sewer line for Strickfort Construction Corporation.

July 8, 2013 - Caruthers, CA - 30 year old Manuel Sanzon was killed on the job when he was struck by a work truck. As he was walking to his truck, it began to move. The passenger attempted to stop the truck but instead stepped on the gas, running the truck into Sanzon. He was employed by Watson Ag Chemicals, Inc.

July 8, 2013 - Portland, OR - 50 year old Jon Wright died on the job after he lost control of his vacuum truck and crashed into a creek bank. He was employed with Lovett, Inc.

July 9, 2013 - Nowata, OK - 38 year old Jesus Valencia Martinez of Bartlesville died on the job when he fell from the roof of an historic hotel being restored in Nowata. No one witnessed the incident. He was working for the hotel's owner, James Mitchell.

July 9, 2013 - El Paso, TX - 22 year old Cheyenne Rhodes died of electrocution while working at the El Paso International Airport for Johnson Controls, Inc.

July 9, 2013 - Lexington, KY - 25 year old Christopher Holt suffered a fatal heart attack while standing on a fixed ladder. He then fell backwards, striking his head on the floor. He was working for The Fresh Market, Inc.

July 10, 2013 - Pago Pago, American Samoa - 39 year old Fa'tateleupu Nakiso was killed on the job when he was electrocuted while working for McConnell Dowell Constructors.

July 10, 2013 - Medina, OH - 30 year old Michael Krebs suffered a fatal electric shock inside a trench containing water at Fox Meadows Country Club. Reports state he may have fallen in and tried to grap a cut wire to pull himself out and it was live. He was employed by Fore Golf Management, LLC dba Fox Meadows Country Club.

July 12, 2013 - Portage, IN - 53 year old Paul Grott died of a cardiac event while working at a drug store making a delivery for Coca-Cola Refreshments.

July 15, 2013 - Quinton, VA - 46 year old David R. Black died on the job when he fell down a stairway from a second floor new home construction. He was working for Crosby Plumbing & Water Treatment.

July 16, 2013 - Oakland, CA - 45 year old Joy Daniels suffered a seizure while operating a powered industrial truck for Total Terminals International.

July 16, 2013 - Atlantic Mine, MI - 38 year old James Spagnotti died of electrocution and a fall from scaffolding while working as a volunteer at a private home for Amazing Grace Ministries.

July 17, 2013 - Batesville, IN - 56 year old Steven W. Bouldin was installing a flat screen TV in a funeral home when he collapsed and was found by a coworker. He died at hospital of natural causes. He was working for The Premier Group.

July 17, 2013 - Greenfield, IN - 48 year old Jerry Moon died of hyperthermia (heat stress) while taking a break in the shade. He was a residential roofer for Blunk's Building and Remodeling.

July 18, 2013 - Redding, CA - 51 year old Ralph Stewart suffered a fatal cardiac event after becoming too hot while working in an non air conditioned environment. He was employed by Shasta County Health & Human Services.

July 19, 2013 - Hoboken, NJ - 47 year old James Green was killed on the job from electrocution in an unknown incident while working for Logram-Eastern Electric Corporation.

July 19, 2013 - LaVergne, TN - 51 year old John Meyer died on the job after being injured on July 12th when he was pinned inside a tire assembly machine and suffered head and chest trauma. He was employed by Bridgestone America's Tire.

July 19, 2013 - Florham Park, NJ - 52 year old Alfredo Rodrigues of Union died on the job of electrocution while working for PCS Wireless, LLC. Circumstances unknown.

July 19, 2013 - W. Lafayette, IN - 53 year old Jay E. Summers was killed on the job when he was struck by a trailer being pulled by a truck in a work zone. He was employed by Fairfield Contractors.

July 20, 2013 - Danbury, CT - 61 year old Kevin Moroney died on the job from a natural cardiac event while working for Custom Exterior Systems, Inc.

July 22, 2013 - Robersonville, NC - 25 year old Juan Carlos Perez died during surgery after his arm was caught in, and he was pulled into, a tobacco harvester machine while working for Paul Harris Farms, Inc.

July 22, 2013 - New Castle, IN - 55 year old Richard Rawles died on the job after the tandem semi he was driving crashed down an embankment as he attempted to avoid an accident. He was employed by United Parcel Service, Inc.

July 22, 2013 - Fullerton, CA - 45 year old Jose Barra died on the job of possible electrocution or cardiac event while working to install an exhaust fan in the attic of a private residence. He was working for Jose Luis Barragan.

July 23, 2013 - Bristol, RI - 46 year old David Dupee of Bozrah, CT was killed on the job when he fell from the Mount Hope Bridge in Bristol, RI. He was performing steel painting and repair work on the bridge. He was employed by TDA Construction, Inc.

July 23, 2013 - Fort Wayne, IN - 25 year old Tyrek Murphy died of blunt force trauma and leg amputation when a tractor trailer semi driver crashed into his truck as it was being serviced on the roadway on July 11th. He was working for Celadon Trucking Services, Inc.

July 23, 2013 - Plymouth, MA - 26 year old Jason Faria of Fall River was killed on the job when a concrete form he was harnessed to came loose and fell. He was working for Diaz Construction Co, Inc.

July 23, 2013 - Fort Wayne, IN - 44 year old Rafael Torres-Diaz was killed on the job when he fell from a tree. He was attached to a safety line working in the tree, but as the tree was rotted it broke, causing him to fall. He was working for Tree 3 Rivers Landscaping.

July 23, 2013 - Dallas, TX - 23 year old Acosta Gaston died after falling from an unknown elevation and suffering head injuries while working for Sergio A. Gaytan.

July 23, 2013 - Aberdeen, SD - 66 year old Larry Huffman died after being struck by a semi "pup" trailer while working on a construction site for Lien Transportation Company.

July 24, 2013 - Charleston, SC - 55 year old Ernest Paterchak died on the job when he became pinned between a moving concrete truck axle and a forklift while working for MB Kahn Construction Company.

July 25, 2013 - Evanston, IL - 44 year old Francisco Mondragon Acosta was killed on the job when he was struck by an unknown object while working for J&M Hauling Co., Inc.

July 25, 2013 - Hobart, IN - 59 year old Cvetko Gjorseski collapsed during a lunch break while working for Airtek, Inc. and later died at hospital.

July 25, 2013 - Wheeling, IL - 33 year old Shamekia Goodwin-Badger died of natural causes after becoming ill on her job as an evidence technician in training for Village of Wheeling Police Dept.

July 26, 2013 - Ames, IA - 39 year old Justyn Jarnagin was spraying foam insulation for Kinzler Construction Services on July 23rd when he suffered an asthma attack and then a heart attack.

July 26, 2013 - Lostine, OR - 51 year old Shane Cosgriff died after losing control of an unloaded log truck. He was working for Burke L. Lathrop.

July 27, 2013 - Racine, WI - 31 year old Christopher Schroeder died of heat exhaustion in unknown circumstances while working for Carlson Racine Roofing & Sheet Metal, Inc.

July 28, 2013 - Madison, CA - 24 year old Christopher Gladden was killed on the job when he was pinned between two vehicles. He was a tow truck driver for Capitol City Automotive, Inc. at the rear of a disabled vehicle when it was struck from behind.

July 29, 2013 - Medford, OK - 45 year old Maclovio Salas died on the job after being struck by an unknown object suffering head injuries. He was employed by Static Oilfield Services, Inc.

July 30, 2013 - Telford, PA - 45 year old Brett Moyer of Silverdale was killed when he was struck by and pinned under a street sweeper machine. He was working for P.K. Moyer & Sons, Inc.

July 30, 2013 - Gold Beach, OR - 35 year old Benjamin Cool of Grangeville, ID died on the job after he made contact with a high voltage power line and was electrocuted. He was employed by Wilson Construction Co.

March 10, 2014 - Colorado City, AZ - 14 year old Rulon Barlow Jessup died while working for his father when the forklift he was operating went off a bridge embankment. Another boy, who was also riding on the machine, was able to jump off before it went over the side.

April 1, 2014 - Plymouth, MA - 43 year old Greg Maloney died when his Plymouth Police Department motorcycle was involved in a crash. Details are unknown.

April 1 - Grayville, IL - 66 year old Vic Hawkins was killed on the job as he worked to repair a skid loader machine at a construction site. He was pinned between the boom and bucket when, for some reason, the hydraulics became activated. His employer is unknown.

April 1 - Vevay, IN - 57 year old Richard E. Hunter of Osgood was killed on the job when he was struck and pinned by a tree that fell when he cut another tree. He was employed with Rodney Sloan Logging Company of Batesville, IN according to his obituary.

April 1 - Fernley, NV - Christopher Smith died on the job when a pipe fell off a flatbed trailer, crushing him. He was working for BV Nevada Corporation, which dismantles trucks.

April 1 - Bagley, MN - 41 year old Mike Larson of Fosston died on the job when his jacket was caught on an automatic 40 foot wide, hanger-type door as it began to go up. He was employed by Bagley Hardwood Products.

April 2 - Auburn, WA - Greg Foss died of natural causes/cardiac disease when he was at his KME Diving Suits, Inc. shop. The co-owner of the shop found Foss unresponsive when he arrived for work.

April 2 - Manhattan, NY - An as of yet unidentified man in his 30's died when he fell from one level to a scaffold while working at the Dream New York hotel. Efforts to locate his name have been unsuccessful.

April 2 - Vassar, MI - 26 year old Travis Bielat of Taylor was killed on the job when the tanker truck carrying ethanol he was driving exploded in a fiery crash. He was a driver for Petroleum Transport, Inc.

April 2 - Fort Hood, TX - The soldiers were killed by a gunman who entered the base and opened fire at random. Thirty-nine year old Sgt. 1st Class Danny Ferguson of Tampa, FL; 38 year old Staff Sgt. Carlos A. Lazaney-Rodriguez of Aguadilla, Puerto Rico, and 37 year old Sgt. Timothy Owens of Effingham, IL were all killed at the Fort Hood base during the shooting rampage.

April 2 - White Sulphur, KY - 75 year old Charles Morgan "Hoot" Gibson died when his farm tractor flipped over as he worked to unroll hay, crushing him.

April 3 - Aspen, CO - 47 year old Jeffrey Beadle, a lift operator at Snowmass Ski Area, was found by a skier with severe chest trauma. He was alone at the time of the incident. Circumstances are unknown.

April 4 - Ecorse, MI - 36 year old Chris Castro of Warren died on the job when the crane he was operating tipped over. He was ejected as it tipped. He was employed by Connelly Crane, working at U.S. Steel Great Lakes Works faciltiy.

April 4 - Haverhill, OH - Jim Morrison was an employee for Haverhill Chemical plant and was working in an area not usual for him. He was helping to fix a pipe that burst, killing him.

April 5 - Victoria, TX - 43 year old David Zamora died on the job when he was crushed by a piece of heavy equipment while working for Lee Specialties Ltd at Rice Energy's Big Foot pad.

April 6 - Summerville, SC - 34 year old Heath Sutherland of Phil Campbell was struck and killed by a motorist as he ran into a roadway because he was trying to avoid a falling tree. He was in South Carolina to help clear trees and debris from the median of Interstate 26.

April 9 - Bellevue, WA - 50 year old Erik Wolff of Gold Bar died of natural causes while working for Pacific Coca-Cola Bottling Co. He was making a delivery when he was found unconscious.

April 9 - Bronx, NY - 38 year old Dennis Guerra, a New York Police Department Housing Officer, died after being overcome by smoke from a fire in a building. A resident started the fire and Guerra went in to help but was overcome and pulled out of the building unconscious.

April 9 - Camp Lejuene, NC - 21 year old Lance Cpl Mark N. Boterf of El Paso, TX died of a single gunshot wound to the chest as he was on duty at the main entrance to the base. It appears another Marine accidentally discharged his weapon in the guard shack.

April 9 - Madison, NC - 29 year old Walter E. Murray, Jr., thought to be from Eden, was killed on the job when he fell about 80 feet inside a water tower he and his family were painting. He was working for Boss Enterprises of Eden.

April 10 - Orland, CA - 32 year old Tim Evans of Elk Grove/Sacramento was killed on the job in a fiery crash of his FedEx truck that crashed into a tour bus. For an unknown reason, his FedEx semi crossed the median and crashed into the bus, carrying students and others. In all, 10 people were killed.

April 10 - Orland, CA - 53 year old Talalelei Lealao-Taiao of Sacramento was killed on the job in a fiery crash when a FedEx semi truck crossed the median of Highway 5 and crashed into the Silverado Stages bus she was driving. She was transporting students and others when the incident happened. She and 9 others were killed in the crash.

April 10, Florence, MT - A 51 year old unidentified man from Bonner was killed when he fell from the roof of a home in Florence as he cleared shingles. Repeated attempts to locate his name were unsuccessful. He was working for Missoula based Jared Langley Enterprises.

April 11 - St. Genevieve, MO - 53 year old John Hahl of Farmington and 29 year old Chris Rawson of Hillsboro were killed on the job when they fell from an elevated boom lift. Both men in the basket of the boom lift when a big rock dislodged and crashed into the boom, detaching it. They worked for Mississippi Lime Co. based in St. Louis.

April 12 - Buzzards Bay, MA - 34 year old Joseph Boyd III of Fall River and 34 year old John Loughran of Quincy died on the job when the boom truck there were working from tipped backwards. They were in an elevated boom lift working on electrical lines when for an unknown reason the truck tipped backwards, cab up. They were employed by Mass Bay Electrical doing maintenance for Utility Nstar.

April 12 - Dalton, GA - A 48 year old unidentified employee for McMinnville, TN based Porter Roofing Contractors died on the job when he fell through an unstable portion of a roof at Shaw Industries. He fell about 30-35 feet to the ground. He may have been from Tennessee.

April 12 - Kettle Falls, WA - Melissa Moon of Evans, WA died of a massive heart attack while working at Boise Building Solutions Manufacturing LLC in Kettle Falls.

April 14 - Manhattan, NY - An unidentified worker was killed on the job when he fell onto scaffolding while working at Dream New York Hotel. Repeated attempts to locate his name or employer have been unsuccessful.

April 14 - Polk County, WI - 50 year old Michael J. Seversen of St. Croix Falls was a member of the Polk County Sheriff Department when, in April of 1991, he was shot in the line of duty. He was paralyzed from the neck down and although he was paralyzed he remained active in the sheriff's office using his wheelchair. Recently he had some severe medical issues which contributed to his death this month.

July 13, 2013 - Marlton, NJ - 35 year old Christopher Consiglo of Hazlet was injured on July 5th when he fell from a lift. No further information is known.

July 26, 2013 - Hasbrouck Heights, NJ - 68 year old Shin Sam Lee died of a cardiac event that was exacerbated by heat. No further information is known.

August 2013

August 1, 2013 - Mt. Vernon, IA - 29 year old Winfred Davis of Clinton died on the job when he fell off the side of a tower he, and others, were painting. He was employed by Arndt Enterprises, Inc.

August 1, 2013 - Sunrise, FL - 49 year old James Mollica died after being struck by an unknown object while working for T & R Painting Specialist, Inc.

August 2, 2013 - Gretna, LA - 27 year old Ross Breaux died on the job when he fell through a skylight as he was making repairs to a roof. He was employed by Breaux Unlimited, LLC.

August 2, 2013 - Alexander, AR - 58 year old George "Dusty" Horn was killed on the job when he fell from an unknown elevation while working for Thuston Construction Co., Inc.

August 4, 2013 - Escondido, CA - 38 year old Juan Ceballos was injured on July 30 when a sliver of metal teeth from an excavator went into his cheek. The metal traveled, causing a laceration to his carotid artery. He was working for Schilling Paradise Corporation.

August 6, 2013 - Gaithersburg, MD - 18 year old Nicholas Lembesis of Washington Grove was killed on the job when he was struck by a tree that he was cutting down at a private residence. He was a member of the Town of Washington Grove's maintenance team.

August 6, 2013 - Salinas, CA - 56 year old Javier Aramburo suffered a stroke while driving a vehicle as he was supervising a field harvest crew. He was employed by Vista Hermosa Harvesting, Inc.

August 7, 2013 - Jal, NM - 20 year old Briar Scruggs died on the job when he was caught between a substructure being removed from a truck and the truck as it was being unloaded. He was a flagger for 5J Oilfield Services, LLC.

August 7, 2013 - North Las Vegas, NV - 54 year old Russell Williams died of head trauma after falling while working for Sharp Communications. He was attempting to remove electrical wiring and climbed a 12 foot ladder, but then climbed an additional 4 feet away from the ladder and lost his grip. He suffered head trauma.

August 8, 2013 - Corpus Christi, TX - 41 year old Martin Naranjo was killed on the job when he fell from an unknown elevation while working for Marcos Salgado Construction.

August 10, 2013 - Marlton area, NJ - 57 year old Anthony Pills died on the job after falling from a flatbed truck in unknown circumstances. No further information is available.

August 11, 2013 - Kansas City, MO - 43 year old Hector Nonez was killed on the job when he fell from an unknown elevation while working for All Season Roofing and Restoration.

August 12, 2013 - Meyersville, TX - 24 year old Jeffrey Wilder of Clearlake, CA was electrocuted after contact with a high voltage line. He was employed by Techline Corporation, Inc. and was replacing power lines for Victoria Electric Cooperative.

August 12, 2013 - Andrews, SC - 22 year old Kenneth Long died on the job after being struck by a motorist. He was a flagger for Jim Young Construction Co, Inc. and one motorist struck a vehicle in a work zone which then struck him.

August 15, 2013 - McCall, ID - 61 year old John Steven Kutzner died on the job when he fell from an unknown elevation and suffered head trauma. He was employed by Steve's Drywall, Inc.

August 16, 2013 - Logansport, IN - 55 year old Carl D. Gordon collapsed while operating a forklift while working at Tyson Fresh Meats in Logansport. He was rescued from the machine by a coworker but died later at hospital.

August 17, 2013 - Ashtabula, OH - 60 year old Bennie Threatt died on the job after falling from an unknown elevation while working for North American Roofing, LLC.

August 17, 2013 - North Salem, NY - 34 year old Claudenei Martin died on the job after falling from an unknown elevation while working for Golden Touch Painting.

August 17, 2013 - Dandridge, TN - 31 year old Justin Bass of Knoxville was killed on the job when he was thrown from a lift basket. He was washing windows in a material lift basket when the lift rolled down the slope of a yard, over a retaining wall, and he struck rocks after being thrown about 60 feet. He was self employed for Proworx.

August 17, 2013 - Fort Wayne, IN - 52 year old Bruce D. Crist died of natural causes while working inside his classroom for the Fort Wayne Community School District. He was found after he did not return home after work.

August 21, 2013 - Kingsville, TX - 27 year old Timothy Quinn died from heat exhaustion under unknown circumstances while working for Koch Construction Company, Inc.

August 21, 2013 - Midland, MI - 34 year old Casey Moe of Beaverton was injured on August 19 when he fell from an elevated lift while painting for Lake Painting, Inc. While painting, the lift was moved but a rear tire fell into a storm drain grate which caused the lift to tip over.

August 22, 2013 - Queen Creek, AZ - 58 year old Maria Figueroa was operating a utility cart on a side walk and failed to negotiate a turn and the cart impacted railing of a barrier structure. She was employed by Chandler Unified School District.

August 22, 2013 - Ely, NV - 26 year old Steven Pickering was a farm labor ranch hand for Spring Valley Associates, LLC and was operating an ATV vehicle while working. Not wearing a helmet, he crashed into a wheel-line sprinkler causing head injury on August 10th.

August 23, 2013 - Porter, TX - 52 year old Ronald Newman was killed on the job when he was struck by an unknown object while working for Crystal Springs Water Co., Inc.

August 27, 2013 - Dixon, CA - 84 year old Miguel Ramos was found dead by coworkers. He had returned to his irrigation work for Ferrara Ranches, Ltd. when he collapsed.

August 29, 2013 - Houston, TX - 31 year old Rejinaldo Valdez died after falling from an unknown elevation while employed by United Forming, Inc.

August 29, 2013 - Medicine Lodge, KS - 21 year old Dakotah Holt was electrocuted when a power pole he was working to set up was blown into a nearby power line causing the pole to be energized. He was employed by Track Utilities LLC.

August 29, 2013 - Columbus, TX - 68 year old Tommy Blanchard was killed on the job when he was struck by an unknown object while working for Auger Services, Inc.

August 30, 2013 - Valparaiso, IN - 67 year old Terry Malmquist died after falling from a ladder while painting brick on Diocese of Gary - St. Teresa of Avila Church.

August 30, 2013 - Evansville, IN - 56 year old Paul Miller suffered a fatal heart attack while driving and his truck crossed the median and went into a culvert. He was employed by Professional Transportation, Inc.

August 30, 2013 - Tucson, AZ - 58 year old Jeffrey S. Steele died on the job when he fell from a ladder and struck his abdomen in unknown circumstances. He was working for Swift Transportation Co.

August 30, 2013 - Monroeville, NJ - 35 year old Dwayne Fennal died after being struck by a telephone under unknown circumstances. He was employed by Atlantic City Electric.

September 4, 2013 - Mathis, TX - 58 year old Arnoaldo Guido Barrera, Jr. of Larado died on the job after falling while working on a rail bridge on a rail line. He and a coworker were on a hydraulic lift. He was employed by Union Pacific Railroad Company.

September 4, 2013 - Skippack, PA - 49 year old Herman Mescher died of electrocution in unknown circumstances while working for Greentech Energy Services.

September 5, 2013 - Elkhart, IN - 55 year old Jagtar Bhatti and 20 year old Pawan Singh were shot and killed during a robbery at the JV Food Mart Inc. store they were working at.

September 5, 2013 - Grand Haven, MI - 25 year old Robert Jay Greving of Holland was killed on the job when he was struck by a loader machine while working on a road project for Dan Hoe Excavating.

September 5, 2013 - Nelson, CA - 48 year old Jose Huizar was injured on September 2 when he suffered a non-injury brain bleed while operating an ATV and fell off. He was working for Bianchi Partnership.

September 8, 2013 - Piscataway, NJ - 59 year old Amrick Singh of Deptford was killed on the job when he attempted to start a bulldozer that was in gear. The machine jerked, throwing Singh off and then running over him. It is unclear who he was employed by.

September 8, 2013 - Shasta, CA - 63 year old Kenneth Smith of Redding drowned while working to help the Shasta County Sheriff's Office recover a drowning victim. He was self employed by Shasta Marine Services.

September 8, 2013 - Yerington, NV - 70 year old William A. Pechtel was sitting in an onion hauling truck at the edge of an onion field when he was found dead inside the truck. He was employed by Peri and Sons Onion Company.

September 11, 2013 - Hasbrouck Heights, NJ - 56 year old Henry Van Dorian Campbell died after he was exposed to a toxic level of Toluene. No other information is available.

September 11, 2013 - Lake Oswego, OR - 38 year old Mauricio Zavalo suffered a medical event while climbing a tree to attach rigging in order to fell the tree. He was caught by his harness from falling to the ground. He may have suffered a seizure or from the heat. He was working for Westcoast Tree Care, Inc.

September 13, 2013 - Clinton, UT - 68 year old Robert DiMarco was injured on September 9th when he fell from a 4 foot tall ladder he was using in a stock room at Wal Mart. He suffered head trauma.

September 16, 2013 - Pacific Grove, CA - 63 year old Rodney Herndon suffered a fatal heart attack while performing ground maintenance work for Pacific Grove Unified School District.

September 16, 2013 - Miami, FL - 37 year old Clayton Bailey of Fort Lauderdale was fatally crushed by construction material in an unknown circumstance. He was a pipelayer for Ric-Man Construction Florida, Inc.

September 17, 2013 - Franklin, IN - 57 year old Stephen E. Johnson suffered a fatal heart attack while operating a weed trimmer outside the company office. He was working for Brown's Tree Service.

September 19, 2013 - Fort Payne, AL - 39 year old Clayton Woods suffered fatal head trauma after falling from an unknown elevation while working for R&J Construction, Inc.

September 19, 2013 - Death Valley, CA - 64 year old Charles Saha suffered a cardiac event due to heat exhaustion while working for the National Park Service at Death Valley National Park.

Septempber 23, 2013 - Waynesville, MO - 25 year old Justin Guinn was killed on the job after falling from an unknown elevation while working for D&K Roofing, LLC.

September 25, 2013 - Blackwell, OK - 26 year old James Eudaily died on the job after falling from the roof of LA Boiler Works in the Blackwell Industrial Park. He was employed by Terry's Construction.

September 25, 2013 - Fort Wayne, IN - 42 year old Mark Allen Fisher suffered a fatal heart attack while working at his computer for Pet Smart.

September 26, 2013 - Tupman, CA - 37 year old Donald Drewry was found dead inside his sleeping quarters with no signs of trauma while working on an oil field site for Geo Drilling Fluids, Inc.

September 27, 2013 - Austin, TX - 27 year old Jeremy Burugan died after falling from an unknown elevation and suffering head trauma. He was employed by Robert Morey.

September 29, 2013 - Daphne, AL - 25 year old Federico Ruiz died after falling from an unknown elevation while working for Mega Construction Co, Inc.

September 30, 2013 - Holbrook, AZ - 33 year old Ace Blacksheep suffered fatal head trauma after falling under unknown circumstances. He worked for Skanska USA Civil West, Rocky Mountain District, Inc.

September 30, 2013 - Elizabeth, NJ - 59 year old Juan E. Regalon-Alena died on the job after being struck by a truck in Port Newark. He was a truck driver for E-Trans.

This is our final blog remembering those workers who died in 2013 that we were previously unaware of. It saddens us to know that so many workers die, and yet nothing is covered in the news. Families need to know their loved one doesn't just become a statistic somewhere, but that others know what happened and that, for most, the incident was preventable.

October 10, 2013 - Trinidad, CO - 58 year old William Martinez died on the job after being struck by an unknown object while working for Sun Construction, Inc.

October 11, 2013 - Houston, TX - 29 year old Oscar Chavez died on the job after falling from an unknown elevation while working for Texas Stud Welding.

October 19, 2013 - Athens, AL - 20 year old Jesus Teran (aka Cesar Vallejo) was killed on the job when he was struck by an unknown object while working for Christopher Professional Enterprises, Inc.

October 19, 2013 - Minnetonka, MN - 57 year old John Stopski died under unknown circumstances when he was found next to a short ladder while remodeling a business. Others said he did not look well before he was found. He was owner of Stopski Construction, Inc.

October 22, 2013 - Franklin, LA - 55 year old Bart Barrilleaux died on the job after falling from an unknown elevation while working for DDA Construction, Inc.

October 25, 2013 - Kennebunk, ME - 57 year old Stephen Gardner was killed after he fell from the roof of a porch he was working on. He was employed by DJS Building & Restoration, LLC.

October 28, 2013 - Bunnell, FL - 43 year old Roberto Martinez died on the job after slipping and falling from the roof of a building while unloading roofing materials. He was employed by Wildkatz, Inc.

October 28, 2013 - Tonopah, NV - 38 year old Melfred Parrish was found dead inside a porta-potty after complaining of chest pains and heartburn. He was working for Brahma Group, Inc.

October 28, 2013 - Pagosa Springs, CO - 52 year old Kenneth Wilson died of head injuries after falling when a scaffold collapsed while working on a home. He was a self employed contractor.

October 29, 2013 - Bass River, NJ - 46 year old Donald Tomaio of Bayville died on the job after being struck by a dump truck while working for an unknown subcontractor on the Garden State Parkway.

October 31, 2013 - Haddonfield, NJ - 36 year old Cameron Lyons of Barrington was killed on the job when he fell from a tree while working for his company, Lyon and Son Tree Service.

November 2013

November 1, 2013 - Laredo, TX - 53 year old Darrell Austin was killed on the job after he was struck in the head by a large metal drainage pipe that was being offloaded from a truck. A forklift was being used to remove a top pipe, but the forks caused the other to fall and strike Austin. He was employed by Ronny Little Trucking, Inc.

November 6, 2013 - Yorktown, TX - 75 year old Dennis Webster, Sr. was killed on the job after he was struck by a tar truck driver who did not see him as he backed up. Webster, a co-owner of Cummings Paving & Recycling, LLC was working on a paving project.

November 8, 2013 - Newnan, GA - 42 year old Jaimes Tobias died on the job when he fell from an unknown elevation while working for RJE Services.

November 14, 2013 - Stockbridge, MA - 51 year old Francis Bona of South Lee was killed after suffering a fatal head injury during a fall. He was employed by Fairview Contractors, Inc.

November 15, 2013 - New York, NY - 57 year old Jaime Sillart of Bergenfield, NJ died after he fell about 70 feet from scaffolding while working on a construction site at NYU. He was employed by BDB Masonry Corporation.

November 16, 2013 - Boulder, CO - 51 year old Jesus Chavez Rodrigues was killed on the job when he fell from an unknown elevation while working for Twin Peak Erectors Company.

November 23, 2013 - Winslow, NJ - 58 year old Edward Heilman died on the job after falling from a ladder in an unknown event at work. No further information was found.

November 23, 2013 - Menomonee Falls, WI - 29 year old Joseph Barth died after being injured the day before when he suffered asphyxia from being struck by an unknown object while working for Applied Flooring Solutions, LLC.

Novemberr 23, 2013 - Tampa, FL - 32 year old Michael G. Pavlack died after being struck by a dump truck while working with a crew on the Crosstown Expressway. He was employed by Blackrock Milling, LLC.

November 26, 2013 - Marlton area, NJ - 49 year old Robert Freyginber was killed on the job when he fell through an opening in a roof. His employer is unknown.

December 2013

December 3, 2013 - Cleveland, OH - 55 year old Jacob Tholl died after receiving an electric shock on November 15th when the elevated bucket he was working from touched an electrical power line. He was employed by Donley's Inc.

December 3, 2013 - DePere, WI - 31 year old Adam Tilkens of Appleton was injured on November 21st when he was caught between an excavator bucket and 30 inch pipe. He was employed by Advance Construction, Inc.

December 5, 2013 - Las Vegas, NV - 70 year old Ernest L. Wilson suffered a fatal cardiac event while stocking shelves using a step stool at a Target store and fell from the stool. He was employed by Mondelez Global LLC dba Nabisco.

December 5, 2013 - Boston, MA - 37 year old Paul Devine was killed on the job when he was caught in a scissor lift and was crushed. He was employed by East Coast Fireproofing Co., Inc.

December 12, 2013 - Livingston, NJ - 67 year old Gregory Johnston was struck and killed by a Livingston Public Works vehicle at their complex. He was an employee for Fairfield Maintenance where he serviced diesel and gas pumps.

December 13, 2013 - New Jersey - 49 year old Segundo Pinto, a New Jersey Public employee, died on the job after slipped on ice under unknown circumstances.

December 14, 2013 - New York, NY - 36 year old Margarito Hernandez-Moreno was killed on the job when he was struck by an unknown object while working for Accurate Elevator Fire Door Corporation.

December 15, 2013 - Corpus Christi, TX - 19 year old Colter Wane Huff from Temple and 57 year old Andres R. Olivarez from Mathis were killed on the job while working at Corpus Christi's Trifigura plant when an oil storage tank collapsed. Both men worked for Tarsco, Inc., a California based company.

December 20, 2013 - Houston, TX - 50 year old Cruz Martinez died on the job after falling from an unknown elevation while working for C R Structural.

December 23, 2013 - Mission, TX - 47 year old Juan Gonzalez was killed on the job when he fell from an unknown elevation while working for Texas H&H Contractors, LLC.

April 15, 2014 - Madison, SD - 60 year old Gerald Ottoson was killed on the job when the dump truck he was driving had a tire blow-out, causing the vehicle to crash. He was working for Helena Inc. as a truck driver.

April 16 - McEwen, TN - 23 year old Rodney Edwards was killed on the job and three others injured when an explosion occurred at the Rio Ammunitions plant in McEwen where he was employed. The explosion occurred during a cartridge loading operation.

April 17 - Issaquah, WA - Gary Jones was found unresponsive in his Lakeside Industries, Inc. truck in a parking lot. He died of a heart related medical event.

April 17 - Seattle, WA - 30 year old Dr. Christen Adams died of an unknown cause while working as an anesthesiologist at the University of Washington. His cause of death is pending and no other information was found.

April 17 - Puyallup, WA - 41 year old Chris Kerns died after being struck by an impaired motorist while walking back to the dealership he was working for. He was employed by Korum Automotive Group, Inc. dba Korum Automotive.

April 17 - Ruth, MI - 58 year old Michael Rempp of Elwell was killed on the job when he fell from an opening in a grain tower owned by Cooperative Elevator Company. He was employed by an out of county subcontractor working to dismantle a feed mill leg when he fell.

April 17 - Brooklyn, NY - 52 year old Swadh Maged was shot to death behind the counter of 427 Central Avenue Deli and Grocery, where he worked, by an individual who he had been arguing with before he went to work.

April 18 - Houston, TX - 51 year old Rene Almeida was shot to death while working as a security guard for Leawood Condominiums. He got into a scuffle with a person who was trying to get into the gated community and that person was able to get Almeida's weapon and shot him.

April 19 - Keene, NH - 46 year old Nate DeMond of Fitzwilliam was fatally electrocuted by a hanging power line near Keene State College Athletic Complex. He was a ground's keeper for Keene State College.

April 20 - Kill Devil Hills, NC - 53 year old Mark Harvey suffered severe head trauma on April 13 when his Atlantic Cab collided with a parked jeep. He was found inside his cab in a private home's driveway.

April 20 - Midland, TX - An unidentified worker from Boerne, TX died from an unknown incident at an oil rig owned by Sidewinder Drilling. Attempts to locate the employee's name and other information has not been successful.

April 21 - Roscoe, SD - 79 year old Arnold Schurr died in a farm related incident of which details are unknown.

April 21 - Baton Rouge, LA - 22 year old Will E. Wright, Jr. of Hammond died on the job when a gravel filled dump truck he was driving veered off the road and into a ditch. It is not known who he was employed by.

April 22 - Edgewood, MD - 61 year old Willie Stevenson died after being injured in a 'grass cutting' incident while riding a lawn mower at Aberdeen Proving Grounds where he was an Army civilian employee.

April 22 - Damon, TX - 52 year old Hugh Ferguson died after collapsing while fighting a house fire. He was Damon Fire Chief. It appears he collapsed from a medical event.

April 22 - Garfield, KS - 67 year old Don Cates of Claflin died on the job when the crop dusting plane he was piloting hit a power line and crashed into a ditch. He was a pilot for Pratt's Farmers Spraying Service.

April 22 - Rankin, TX - An unidentified worker died on the job in an equipment related incident at a drilling rig near Rankin. Attempts to locate more information have been unsuccessful.

April 22 - Port Lavaca, TX - 26 year old Christopher Adames was killed on the job when he fell about 25 feet while performing routine maintenance at Formosa Plastics Corp. plant. He worked for an unnamed subcontractor.

April 25 - Parker, SD - 21 year old Brady DeStigter of Lennox died on the job when he was pinned by a skid steer machine he was operating. He was working for Turner County Dairy in Parker.

April 25 - Dubuque, IA - 19 year old Colin Jackson of Bellevue was killed on the job when be became trapped in a piece of industrial equipment while working for Jackson Concrete Pumping in Dubuque.

April 26 - Commiskey, IN - 74 year old Jerry L. Spicer was killed on the job when he was struck by a 1,000 pound log while making a delivery to Baxter Lumber Mill. He was the owner/operator of Spicer Logging and was found on the ground next to the huge log.

April 26 - Maxwell, NE - 52 year old Heidi Craig died after becoming pinned under a large round hay bale while working on her family farm.

April 27 - Houston, TX - 26 year old Elmer Oscar Barrera died on the job when he fell into a fortune cookie dough mixer machine while working for Wonton Food Corporation.

April 27 - Surf City, NC - 53 year old Larry Kinkade of Holly Ridgewas killed on the job when an excavator rolled and pinned him in a pond on a construction site. His employer is unknown.

April 28 - Williston, ND - 20 year old Zach Buckles of Glasgow, MT died after being found unresponsive over an open crude oil tank cover. It is expected that he died of exposure to toxic fumes being emitted from the crude. He was employed by Black Gold Testers at Continental Resources well site.

April 28 - Elko, NV - 53 year old Richard C. Otto died on the job when his clothing became tangled in the drill steel of a jackleg drill he was operating. He was employed at the Midas Mine, owned by Klondex Mines, Ltd.

April 29 - Gretna, LA - 55 year old Blake J. Helmer was a cab driver for Glenn's Cab Company when he was shot to death by two passengers who attempted to rob him. Both passengers have been arrested for his murder.

April 30 - Blue Ash, OH - 65 year old Donald J. Stevenson of Lima died on the job when he was pinned between a wall and metal loading ramp while working at SpartanNash Warehouse in Blue Ash.

April 30 - Orla, TX - 46 year old Amos B. Ortega of Artesia, NM and Roberto A. Mandela of El Paso, TX were killed on the job when a pressure explosion occurred in a well head at RKI oil site in Orla. It is unclear who was their employer.

Awareness is the key in accomplishing better work place safety. We have heard many say they just didn't know how many of our workers die on the job - until it happened to their family. Our hearts and thoughts go out to all families who have or are suffering the loss of a loved one.

April 7, 2014 - Berkley, CA - 45 year old Damon Frick died after he fell from a motorized lift that tipped over. He was a custodian cleaning windows at University of California's International House.

April 29, 2014 - San Antonio, TX - 30 year old Aaron Turner of Virginia Beach, VA died as the result of being stung by a huge swarm of bees. He was working for Lohmann Fencing of Houston in Grey Forest when the hive was disturbed, sending the bees swarming.

May 1, 2014 - Tanana, AK - Alaska State Troopers Gabriel "Gabe" Rich (left) from Anchorage and Patrick "Scott" Johnson (right) were shot to death during an arrest attempt of an unruly person who also caused problems the day before. Two people were arrested for the trooper's murders.

May 1, 2014 - Caldwell, ID - 53 year old Roy Fraiser of Nampa was killed on the job when he was struck by a heavy steel beam. He was using a forklift to move the beam when it shifted, falling from the lift. He was working for Rule Steel in Caldwell.

May 2, 2014 - Orlando, FL - 26 year old Brandon Bristol died on the job when he was run over by the garbage truck he was riding on. It is unclear how he came to be struck by truck. He was employed by the City of Orlando.

May 2, 2014 - Winston-Salem, NC - 37 year old Christopher "Chris" Jones was fatally electrocuted when the elevated bucket he was in came into contact with a power line. He was am employee for Time Warner Cable.

May 3, 2014 - Ocala, FL - 30 year old Chelsea Richard, a Florida Highway Patrol Trooper, was killed on the job when she was struck by a vehicle as she was assisting in a two vehicle crash. She was speaking to a tow truck driver when another crash occurred on the opposite side of the highway, sending a vehicle crashing into her and the tow truck driver (below).

May 3, 2013 - Ocala, FL - 57 year old John Duggan died on the job when he was struck by a vehicle as he stood near the site of an accident he had responded to. Duggan was owner/operator of S.M. Duggan Towing Company and was speaking with an FHP Trooper when an multi-vehicle accident occurred on the opposite side of the highway sending a vehicle crashing into them.

May 3, 2014 - Lynchburg, VA - 63 year old Donnie Lee "DL" Bryant, Sr. of Evington died after he fell three stories when a railing he was leaning on gave way. He was the owner of DL Paint Contracting working on a Jamerson-Lewis Construction work site.

May 4, 2014 - Fairfield, CA - 77 year old Eddie Andreini of Half Moon Bay died when his stunt plane crashed during an air show at Travis Air Force Base. He was performing his Stearman Superbatics performance, flying upside down and close to the ground, when something went wrong and he crashed. He was owner of Eddie Andreini Airshows.

May 4, 2014 - San Antonio, TX - 29 year old Julian Pesina was shot to death when he was ambushed as he was closing up and left his Notorious Ink tattoo shop. He was also a Balcones Heights Police Officer.

May 5, 2014 - Plymouth, NC - 56 year old Floyd Greene of Old Town, FL was killed on the job and another worker injured while working at Domtar Pulp Plant in Plymouth. He was standing in front of a hydraulic line that pressurized and suddenly burst. He was contracted through CBT Services, Inc. of Chattanooga, TN.

May 5, 2014 - Genoa, IL - 72 year old Leon Madey was found dead inside a grain silo after his family realized he was missing. He was helping to dislodge grain on the family farm when be was buried.

May 6, 2014 - Shawnee, KS - 21 year old Nicholas Huston of Edwardsville died from injuries he received after falling out of a golf cart at the FedEx facility in Shawnee where he was working. It is unclear why he fell from the cart.

May 6, 2014 - Corona, CA - Samuel Enciso of Santa Fe Springs, was killed when an explosion occurred as he welding on a petroleum tanker truck. A co-worker was injured. The blast occurred at National Distribution Services in Corona.

May 6, 2014 - Ardmore, OK - 31 year old Christopher DeWayne Cryer of Lone Grove died after falling approximately 15 feet from a plant in Ardmore. He suffered fatal head trauma. He was employed by Atlas Roofing.

May 7, 2014 - Orlando, FL - 29 year old Keith Bramblet died after falling or being thrown from an elevated cage attached to a crane. He was working from the cage to install an electronic billboard. A witness states the cage was swaying so much that it also came apart from the crane but the reason is unknown at this time.

May 8, 2014 - Delmar, IA - 75 year old Donald Cain was killed on the job when he was pinned in farm machinery in rural Delmar, near Maquoketa.

May 9, 2014 - Scottsbluff, NE - 67 year old Llewellyn Mattern died after falling about 11 feet down into a ditch on fire. He and his Pathfinder Irrigation District crew were burning weeds in the ditch when he fell in and was unable to get out.

May 9, 2014 - Indianapolis, IN - 49 year old Kenneth Duerson, Jr. of Indianapolis and 24 year old Coty Demoss of Noblesville, we both killed on the job when they were struck by a motorist while removing cones during road construction. They worked for Rieth Riley Construction Co., on an Indiana Dept. of Transportation "Operation Indy Commute" project.

May 9, 2014 - Howard, SD - 66 year old James Henry Rentschler died after he fell from a skid steer he was operating and was pinned between the cab and arm of the machine on his farm.

May 9, 2014 - Arlington, WA - 66 year old Donald Monroe Long died after suffering a fatal heart attack while working from a patient's home. He was employed by Fred and Debbie Inc. dba Visiting Angels Living Assist.

May 10, 2014 - Zeeland, MI - 49 year old Ricardo Ramos Grand Rapids was killed on the job when he was caught in a piece of machinery. He was working at the meat packing plant Hillshire Brands, performing maintenance, when his clothing somehow became caught in a conveyor belt which caused asphyxiation.

May 11, 2014 - El Paso, TX - 54 year old Jimmy Joe Broadstreet of Chaparral and 39 year old James Green of Runaway Bay, were struck by a drunk driver who drove through road construction barricades. Green died on scene and Broadstreet at hospital. It is unclear if they were employed by the Texas Department of Transportation or a subcontractor.

May 12, 2014 - Boone County, WV - 46 year old Gary Hensley of Chapmanville and 48 year old Eric D. Legg of Twilight were killed on the job when a mine roof collapsed during a retreat mining operation at Patriot Coal's Brody Mine No. 1 in Boone County.

May 12, 2014 - Savannah, GA - 66 year old Bounpheng Srirtanakoul died after his chest was crushed by a large pipe fell on him while working on a repair project at Savannah's city waste water treatment plant. He was a senior mechanic for Savannah City.

May 12, 2014 - Brentwood, NH - 48 year old Steve Arkell, an officer for the Brentwood Police Department, was shot after responding to a home for a domestic dispute. The suspect then set the home on fire and an explosion occurred.

May 12, 2014 - Minot, ND - 48 year old Richard Rayburn of Hillsboro, OH died after he fell from a roof at a construction site in Minot. He had just unhooked his harness to move position when a gust of wind caused a piece of plywood to blow up into him. As he tried to catch it, he stepped on another piece that caused him to slide off the roof. It is unclear who he was working for.

May 12, 2014 - New Philadelphia, OH - 29 year old Michael C. Cottrell died at a hospital after he was seriously injured in an explosion. He was inside Sutton Pump and Supply Company welding on a bulk storage tank when the explosion occurred.

May 12, 2014 - Lansing, MI - 35 year old Michael Addo of Mason, and a customer, were shot to death inside the Rite Aid store Addo was working at. It appears there was no other motive than a nearby resident was angry over a domestic dispute so he entered the store and started shooting.

May 13, 2014 - Merritt Island, FL - 43 year old Brian Boyle died on the job after he was struck by a tree limb that fell as another worker was up in the tree, trimming it. He was standing on the ground when the incident occurred. He was employed by Gidney's Tree Experts.

May 13, 2014 - Fort Wayne, IN - 26 year old Robert "Bobby" Houser, Jr. died after he was injured May 9 when the riding lawn mower he was driving flipped, pinning him in a pond. He worked for Adams Landscaping according to his obituary.

May 13, 2014 - Withee, WI - 55 year old Roger R. Petranovich was killed on the job when the dump truck he was driving went off the roadway and rolled into a ditch. He was working for HAAS, Inc.

May 14, 2014 - Houston, TX - An unidentified driver of a tractor trailer died on the job when the rig rolled into him at a loading dock of a Kroger Grocery Store. It is unclear who he was working for and attempts to locate his name have not been successful.

May 14, 2014 - Tacoma, WA - Anthony McPeek of Puyallup suffered a fatal heart attack while in his company truck in a parking lot. He was employed by Acco Engineered Systems, Inc. at Tacoma Art Muse.

May 14, 2014 - Puyallup, WA - Richard Phillips collapsed at work from a heart attack. He was working for Alteon Training LLC dba Flightsafety Boeing Training.

May 14, 2014 - Spanaway, WA - Dennis Eaton died of a heart attack 10 days after he fell from his tractor trailer semi as he was loading it with personal items for an upcoming trip. He was hospitalized until May 13 due to the fall but died in his sleep the next day. He was employed with Aggressive Transportation Ltd USA.

May 15, 2014 - Louisville, KY - 56 year old Craig Smith of Cincinnati, OH died of injuries he received when he fell the day before from the top of a tanker truck trailer, striking his head. He was working at Apollo Oil.

May 15, 2014 - Venice, FL - An unidentified construction worker died after falling from a 25 foot high steel beam, striking his head. He was working at the Tervis Tumbler expansion construction site. It is unclear if he was working for them or a subcontractor.

December 16, 2011 - Taunton, MA - 28 year old Daniel Collazo died on the job when he was pulled into or fell into a slow turning mixer auger while working at the Tribe Mediterranean Foods Hummus plant. OSHA charged them with 18 violations and have paid a hefty fine for contributing factors in Collazo's death.

May 18, 2014 - Riverside, CA - 56 year old Okesene Faasalele of Long Beach was killed on the job when he fell from a railroad bridge that buckled during demolition. He was working for Hard Rock and Cal Trans.

May 19, 2014 - Aston, PA - An unidentified dump truck driver died on the job when the fully loaded dump truck overturned on the roadway. Attempts to locate the drivers name or employer have been unsuccessful.

May 19, 2014 - Millersburg, IN - 71 year old John M. Bontrager of Topeka was killed when he was involved in an incident while working on a farm in Millersburg. He was atop a wagon when something spooked the horses and after he grabbed the reins to try and control the animals, he was pulled off, dragged and run over by the wagon.

May 19, 2014 - Arlington, VA - An unidentified construction worker was killed on the job when he was crushed under a large excavator that tipped over at a townhouse construction site. Repeated attempts to locate his name or employer has not been successful.

May 19, 2014 - Union City, NC - An unidentified tractor trailer driver died when his truck went off the roadway, causing his trailer to flip up and come to rest atop the cab. He tried to stop the rig quickly causing it to jackknife. Attempts to locate his name and company have been unsuccessful.

May 20, 2014 - Clearwater, FL - 70 year old Doug Carey was a beloved crossing guard for Clearwater Police Department. He was killed on the job when a motorist ran a red light, crashed into another vehicle and causing it to careen into Carey.

May 20, 2014 - Dexter, IA - 39 year old David Denger of Panora died on the job after falling 50 feet from a boom truck while hanging power lines. It is unclear who he was employed by.

May 20, 2014 - San Diego, CA - 29 year old Eduardo Lopez was killed on the job when he was crushed under a rebar column that collapsed. He was working on an elevated column of rebar at the San Diego International Airport when it began to collapse and fell onto him as he fell. He was employee/owner of Sundt Construction and Austin/Sundt Joint Venture.

May 20, 2014 - San Mateo, CA - 54 year old Elias Vera of Fairfield died on the job after falling about nine feet from the top of a wall being constructed, hitting his head. He was working for a subcontractor Conco for TRI Pointe Homes in San Mateo.

May 21, 2014 - San Jose, CA - 48 year old Victorino Campos-Tovar was killed on the job after he fell three stories from a lift he was working from. He was attempting to unload drywall from the cherry picker lift when he lost his footing and fell. He was employed by Foundation Building Materials of San Jose.

May 21, 2014 - Monmouth, OR - 30 year old Philip Coward died on the job when a load of rebar fell from a truck being unloaded at his families business. The truck belonged to Gagne & Sons and was making the delivery to Coward's parents business Mid-Maine Foundations.

May 21, 2014 - Winlock, WA - 18 year old Cole Bostwick was working for ENB Logging & Construction, Inc. as a logging chokerman when he was struck by the skyline carriage as it was being lowered down.

May 22, 2014 - Milwaukee, WI - 38 year old Stephen Rauth went to investigate why an alarm was going off at the NUVO Construction cement plant he was working at. After his boss was unable to locate him via radio, he went looking for him. Unfortunately he found Rauth trapped in a cement mixing machine. How is became trapped is not clear.

May 22, 2014 - Silver Lake, CA - 20 year old Luis Sanchez died of massive head trauma when his co-worker picked up a hammer and began hitting Sanchez after the two were arguing. He later died at hospital and the suspect was booked on murder charges. The men were working on an apartment construction site in Silver Lake.

May 23, 2014 - Chicago, IL - 46 year old David Varga of Lockport was killed on the job when a crane hoisting a concrete wall dropped it, crushing him. He was employed by Industrial Erectors, Inc.

May 24, 2014 - Harlingen, TX - 35 year old Juan Carlos Reyes of Brownsville died on the job when he fell from an unstable elevated platform. He was standing on a platform holding supplies that was being lifted by a forklift when the platform shifted, causing him to fall. He was working on a Marriott Hotel construction site for an unknown subcontractor.

May 25, 2014 - Corinth, MS - 66 year old Cleo Henderson was found by a coworker, shot to death outside the Liberty Cab stand where he worked. It appears he was murdered during a robbery attempt.

May 26, 2014 - Carroll, NY - 37 year old Michael T. McCray died after being pinned under an overturned farm tractor.

May 27, 2014 - Lakewood, NJ - 67 year old Angel Martinez died on the job when he became trapped in a bulldozer that overturned. He was attempting to drive the machine onto a ramp when it flipped off the ramp and pinning him. He was employed by NJ Advanced Construction LLC.

May 27, 2014 - Everett, WA - 52 year old Anthony S. Purrington died of what has been determined and confirmed natural causes after he was found unconscious inside a confined space (trommel) he was working in. He was employed by Pacific Topsoils Inc. dba Pacific Topsoils Gray Barn.

May 27, 2014 - Salisbury, NC - 39 year old Kenneth "Scott" Fink of Mt. Utta died after his hand was caught in a piece of machinery and his chest was crushed. He was an employee for Thermoid Inc.

May 27, 2014 - Mattawa, WA - 45 year old Dale Peters of Selah and 53 year old Bruce Patnode of Yakima were both killed in a fiery collision. Each was driving a dump truck for Scout Lake Construction, Inc. when one of the drivers crossed into the other lane slamming head on into the other.

May 28, 2014 - Parkside, PA - An unidentified worker in his 30's died of electrocution while performing demolition work in an apartment building. He used wire cutters to snip what turned out to be a live wire. A contractor defied warnings by the city's License's and Inspection Department to stop work and picked up this unidentified worker and others, taking them back to perform the demo. A "make safe permit" was previously issued to Dagan Construction LLC but it is unclear if this is the same contractor who took the workers to the building.

May 29, 2014 - Freehold, NJ - 31 year old Brian Scott Pancoast died after being struck by a piece of equipment while working at the waste transfer station of Freehold Cartage, Inc., a trash hauling company.

May 30, 2014 - Winters, CA - 25 year old Marcus Zane Powell of Redding and 49 year old Glenn Hodgson of Richmond were killed on the job when a crane hoisted basket they were working from broke free, falling about 80 feet onto timbers. They were employed by Disney Construction working on the Winters Putah Creek Car Bridge.

March 11, 2014 - Middleton, CT - 67 year old Robert Williams was overcome and died from carbon monoxide fumes while operating a forklift inside a closed up warehouse. More than likely because it was cold outside, the warehouse doors were closed which led to the fumes building up. He was employed by Custom Carpets.

May 23, 2014 - Muscatine, IA - 29 year old Torkor Dahn died on the job of an unknown cause while working for Fabricator's Plus, a metal fabrication business.

May 27, 2014 - Aston, NJ - 20 year old Jacob S. Blonsinski was killed on the job when a dump truck he was operating overturned. It is unclear who he was employed by but he was working behind Kokes Family Builders.

May 28, 2014 - Ashford, WA - 38 year old Matthew Hegeman of Truckee, CA and 28 year old Eitan Green of Seattle, WA were both guides for Alpine Ascents International, Inc. when they died, along with four climbers, while leading a tour on MT Rainier in Washington State. It appears an avalanche occurred, causing their deaths.

May 29, 2014 - Rapid City, SD - 74 year old Alvin Howe died after the dump truck he was operating tipped over. He was raising the bed to unload gravel but the heavy load caused the truck to tip on its side. It is unclear who he was employed by.

May 29, 2014 - Chenango, NY - Christopher G. Skinner was a New York State Trooper on a routine traffic stop when a motorist from Florida intentionally drove into him, killing him instantly. It is unclear what caused the motorist to do what he did.

May 29, 2014 - Black Creek, WI - 72 year old Thomas Raymond Fischer died in an incident on his farm.

May 29, 2014 - Edgefield, SC - 62 year old John W. Varner died on the job after he fell from a forklift he was operating and then was run over by it. He was employed by Tides International Lumber yard.

May 30, 2014 - Fife, WA - 58 year old Robert Hoquist of Tacoma was killed on the job when his forklift rolled off the end of a loading dock and landed on top of him as he fell off. He was working for Cogdall Construction Co, Inc dba Contract Furnishings Mart.

May 30, 2014 - Corrigan, TX - 56 year old Charles Wayne Kovar of Moscow, TX has died after he was injured on 4/26/2014 with burns when a fire broke out at the Georgia-Pacific plywood plant which injured seven.

May 30, 2014 - Windsor, CT - 51 year old Danny King was killed on the job when he was pinned between a trench box and backhoe while working inside a trench. He was employed by King Construction of Bloomfield, owned by his father.

May 30, 2014 - Jacksonville, FL - 26 year old Jose Lopez of Axton, VA died on the job after falling from the roof of a hanger at Cecil Field. He was attempting to move from one section of the building to another and had untethered his harness. He was employed by IMC Erectors for Flightstar Aircraft Services.

May 31, 2014 - Cleveland, OH - 51 year old James Eiflander died on the job after falling from the exterior of the Anthony J. Celebreeze Federal Building. He was installing windows on the building when he fell. He was a member of Cleveland Local 17 (Ironworkers) hired for a subcontractor National Enclosure Company of Michigan.

June 1, 2014 - West Pikeland, PA - 67 year old William McGuigan of Ardsley was an equipment operator working in a closed lane of construction for Pennsylvania Turnpike Commission when he was struck by a tractor trailer driver who entered the closed lane and struck he and a coworker.

June 1, 2014 - Carlisle, PA - 52 year old Jody Rhoads died on the job when the pallet truck she was operating crashed into some shelves at Amazon Fulfillment Center warehouse in Carlisle.

June 1, 2014 - Pompano Beach, FL - 55 year old Robert Elliott was died and his coworker injured after they were struck by lightening while working on the roof of Joey Accardi Jeep Dealership. It is unclear who he was employed by.

June 2, 2014 - Saginaw, MI - 31 year old Andrew Beckman was killed on the job when he was crushed under 3,100 pounds of steel rods that shifted as he was loading them onto a flatbed trailer. He was pinned up against a steel shelf. He was employed by Peerless Steel.

June 2, 2014 - Rockbridge, IL - 68 year old Lee Whitehead died after an incident involving his farm tractor.

June 2, 2014 - Central, SC - 56 year old Jerry Wayne Kelly died on the job after he fell from a ladder while painting in a home from the second story. He was a self employed painter.

June 2, 2014 - Clinton, AR - 51 year old Hubert Keith Moore of Chester and 40 year old Ricardo Trochez of Atkins were killed and 21 others injured when a loaded log truck blew a tire, spilling its load onto those workers as it overturned. They were working an Arkansas State Highway Transport work site but it is unclear who they were employed by.

June 3, 2014 - Shaw, MS - 50 year old Henry Butler of Rosedale was killed on the job when his Mississippi Department of Transportation truck was struck from behind by a tractor trailer driver, causing a fiery crash.

June 3, 2014 - Great Falls, MT - 38 year old Barry Ladd died on the job from blood loss after he stepped into an auger and his leg was severed. He was working for Montana Specialty Mills.

June 3, 2014 - Middletown, OH - 24 year old Sarah Rhoads of Miamisburg died of injuries she suffered on June 1 when she inadvertently walked into a spinning plane propeller. She was employed by Start Skydiving of Middletown.

June 4, 2014 - Salt Lake City, UT - 58 year old Ronel Christensen of Burley, ID was killed on the job when his milk tanker overturned on a Utah highway. It is unclear who he was employed with.

June 4, 2014 - Queens, NY - 26 year old Thomas Patane of Rossville died on the job after he fell down a five story elevator shaft while working on construction of PS 313 school for the New York School Construction Authority. It is unclear who specifically he was working for.

June 5, 2014 - Florence, AL - 39 year old Christopher Lane Williamson died on the job when he was electrocuted while attempting to restore power after storms in the area. He was employed by Florence City Electric Department.

June 5, 2014 - Hiram, GA - 42 year old Ron Crist, owner of Crist Roofing and Construction, was killed on the job when a skid steer he was operating malfunctioned, causing his death.

June 5, 2014 - Newark, NJ - 50 year old Jorge Giraldo of Elizabeth drowned inside an overturned crane he was operating when it flipped into the river. He was employed by Global Development of Belleville.

June 6, 2014 - Wanamingo, MN - 26 year old William Pohlman was killed when he became pinned under a piece of hay/farm equipment near Dodge Center Airport. He was employed by Durst Brothers Dairy of Mantorville.

June 6, 2014 - Corrigan, TX - 58 year old Kenny Morris of Lufkin has died after being injured in an explosion at the Corrigan Texas Georgia-Pacific plywood plant. He was the second person to die in the incident. Charles Wayne Kovar, 56, of Moscow died on May 30th.

June 6, 2014 - Woodbridge, VA - 23 year old Junior Montero of Hyattsville was shot and killed by the owner of a truck as he was towing it for repossession. After being shot his truck rolled into a water-filled ditch where police found it. The name of the tow company is not known.

June 7, 2014 - Indianapolis, IN - 45 year old Harry Bigg, a store clerk for Mr. Fuel and Kicks 66 died after being shot in the head on June 4th during a robbery.

June 7, 2014 - Chicago, IL - An unidentified man died on the job when he fell about 35 feet through an opening on a roof he was working on. He was employed by Donaly Roofing and Construction.

June 7, 2014 - Jacksonville, FL - 43 year old Stuart Carson was shot and killed while sitting inside his Gator City Taxi after being robbed.

June 8, 2014 - Las Vegas, NV - 42 year old Alyn Beck and 32 year old Igor Soldo, both Las Vegas Metro officers were murdered in an ambush as they sat in a restaurant having lunch. Both were shot by a husband and wife who were later shot and killed by fellow officers inside a store across the street from the restaurant. A innocent man was also shot by the wife as he tried to stop the pair inside the store, likely saving other people. His name was Joseph Robert Wilcox.

June 8, 2014 - Union Beach, NJ - 54 year old Robert "Bob" Meyer suffered a fatal heart attack while battling a blaze in the borough. He was a member of the Union Beach Host Fire #1.

June 8, 2014 - Hoffman Estates, IL - 48 year old Melba R. Farr of Elgin was a cab driver for an unknown company when her taxi was struck from behind by another motorist causing the cab to burst into flames. She was unable to escape.

June 9, 2014 - Prineville, OR - 77 year old Gordon Stroebel was a farmer who lost his life while attempting to load bales of hay onto his truck. The bales shifted and fell from the truck, striking Stroebel.

June 9, 2014 - Moab, UT - 51 year old Jay L. Stephens died on the job when he was crushed between a lift gate and back of a garbage truck. He was at a transfer station when the driver of a garbage truck had just unloaded at a place where a gate was raised. As he moved forward the gate lowered, trapping Stephens.

June 9, 2014 - Meridian, CT - 26 year old Frank Cline of Dover Plains, NY was struck in the head by a tree limb. His coworker had tied the limb as a precaution but it began swinging and struck Cline. They were employed by Tree Inc., of Houston, working for Connecticut Light and Power who are having trees removed after storm damage.

June 10, 2014 - Knoxville, TN - 42 year old Larry Releford of Powell has died due to burns he received in a furnace fire. He was injured, along with five others on May 15th when a fire broke out in a furnace at the Gerdau Ameristeel plant in Knoxville.

June 10, 2014 - Long Grove, IL - 51 year old Brian Pierce of Milwaukee, WI was killed on the job when the tractor trailer semi he was driving crossed a center line and crashed into a dump truck for an unknown reason. His employer is not listed.

June 10, 2014 - Freeport, ME - 59 year old Scott MacDonald of Limington was working with a landscaping crew to unload materials when the forks of skid steer loader, holding granite steps, suddenly came down on top of him. He was employed by Coastal Landscape, Construction and Snow Services of Cumberland after owning his own business for 30 years.

June 10, 2014 - Weston, FL - 45 year old Ernesto Rodriguez of Hialeah was working in a large water-filled ditch on an underwater pipe when he did not resurface. His coworker became alarmed and went into the ditch to find Rodriguez who later died at hospital. His employer is not listed.

June 10, 2014 - Mint Hill, NC - 46 year old James Kirk Mitchman died on the job when he was buried in a trench collapsed. He had been lowered into a 15 foot deep trench for a sewer line installation to continue digging with a shovel. The trench had not been properly shored. He was employed by One Grade, Inc of Indian Trail.

June 10, 2014 - Brooklyn, NY - 22 year old Gustavo Tapia died after being pinned against a wall by a motorized vehicle. He was using a walk-behind electric forklift, trying to push a Yale forklift with a non-working rear wheel to the store’s rooftop parking lot, when the Yale forklift rolled backwards, pinning Tapia to the wall of the building. He was employed by Moisha's Discount Supermarket.

June 10, 2014 - Indianola, MS - 29 year old Tarares Williams of Itta Bena was killed on the job after falling from an industrial lift. He was working from the top of a scissor lift, or manlift, in the freezer warehouse section of Lewis Grocer Division of Supervalu when he fell about 18 feet.

June 10, 2014 - Las Vegas, NV - 59 year old Ricky J. Robinson died on the job after being pushed out of a third story window when he couldn't get out of the way of a motorized cart that began moving. He was crushed under the machine as it fell down on top of him. He was working for the MGM Grand Hotel.

May 30, 2014 - Maplewood, NJ - 51 year old Guo Tai Chen of New York City was killed on the job when masonry came down on him and two others while they excavated for a new stairwell in a restaurant in Maplewood. They were working at Coda Kitchen and Bar, owned by Saul Fisher, but it is unclear if he was their employer or they worked for a subcontractor.

June 11, 2014 - Bronx, NY - 35 year old Jose Duran of Springfield, MA was driving his tractor trailer rig on the Cross Bronx Expressway when a manhole cover exploded, flying through his windshield and striking him in the head. The sewer cap may have been dislodged by another truck. His employer was not listed.

June 11 - Lucas, KS - 65 year old Gary Ward, a longtime employee for the City of Lucas, was fatally electrocuted while working on power lines. Circumstances around his death are unknown.

June 11 - Houston, TX - Russell Soltani was the owner of a used car dealership. When he arrived for work, he was ambushed by an individual who shot him in the back and stole his briefcase. Security cameras caught the event on tape and are looking for the shooter.

June 11 - Chickasha, OK - 56 year old Floyd Etter Jr. was helping a friend on his farm when he was run over by a combine machine. He was on the ground testing it for moisture when the incident happened.

June 12 - Huger, SC - 44 year old Cisco Agbunag of Goose Creek was working in the mold and equipment shop of Nucor Steel Berkeley plant in Huger when a large piece of equipment fell onto him in the maintenance shop, crushing him. The plant melts down scrap metal and recasts it into sheet metal and I-beams for construction.

June 12 - Hallendale Beach, FL - 27 year old Christopher J. Ricci of Sunrise was working with a construction crew at a residential build when a crane began to tip over. The workers started to scatter when they realized the crane with tipping, but Ricci was unable to get out of its way and was struck. A truck with the logo DLS Prestressed Crane Co. was parked nearby but it is unclear if that company was Ricci's employer.

June 12 - Phoenix, AZ - 28 year old Fr Kenneth Walker was a priest at the Mother of Mercy Mission church in downtown Phoenix when he was confronted by a gunman who was at the church to rob it. The gunman shot and killed Fr Walker and injured another priest while they were in the Rectory.

June 13 - Indianapolis, IN - 25 year old William Issac Simpson was working 250 feet down inside a tunnel when he suffered massive crushing head trauma. He was working for SKJV Construction on the Dig Indy Project/Deep Rock Tunnel connector project. It is unclear what he was struck by in this incident.

June 13 - Allentown, FL - 71 year old James Donald McDaniel of Milton was working in a blueberry patch when he was struck by lightening and then suffered a cardiac event. He was employed by Blue Basket Farms.

June 13 - Whitehall, MI - 46 year old John L. Short died on the job when he was struck in the head by a piece of falling tree. He was working at a private resident to cut down a tree. When the tree began to fall it hit another tree, causing it to break into pieces as it came down.

June 13 - Platteville, WI - 79 year old Merle Forbes was assaulted and murdered and put in the trunk of his taxi cab by a man who has since been arrested. The name of the taxi company was not given.

June 15 - Boston, KY - An unidentified farmer was killed on the job when he became pinned underneath his overturned tractor. No further information, or his name, could be found.

June 16 - Parkville, MO - 35 year old Carlos Enrique Gomez-Escobar, "Carlos Gomez", died on the job when he was electrocuted while working to trim a tree. He was in the tree of a private homeowner when the strap that held him to the tree came loose and a piece of metal touched a high power line. It is unclear the name of the tree company he worked for.

June 16 - Washburn, ND - 27 year old Corneilius Johannes DuPlessis, a farm worker from South Africa, died on the job when he was buried in a corn grain bin owned by Doepke Farm. He and another worker were inside trying to get a load of grain off the sides of the bin. The other worker exited, thinking DuPlessis had followed him and started the auger. DuPlessis had not exited the bin and was sucked down into the grain.

June 16 - Flushing, NY - An unidentified 54 year old warehouse worker was crushed when a disabled forklift fell on his chest. Attempts to locate his name or employer have been unsuccessful.

June 17 - San Angelo, TX - 28 year old Cody Freeman of The Colony, TX died after falling from a telephone tower. Reports state he was having issues with his safety equipment when he fell. He was employed by Microwave Transmission Systems (MTSI), a subcontractor for Goodman Networks.

June 17 - Frankfort, IL - 19 year old Francis "Franky" Licka was operating a riding mower at Union Creek Park for the Frankfort Square Park District when he suffered unknown multiple trauma due to an incident with the mower.

June 18 - Messena, IA - 40 year old Brian Larry Follmann died on the job as he was working on his farm. It is unclear how the incident happened but he was caught in his hay baler.

June 18 - Tenino, WA - 75 year old William Hill of Olympia was killed on the job when an explosion of fireworks occurred as he and another worker were assembling them. He was employed by Entertainment Fireworks, Inc. of Tenino.

June 18 - Orangeville, NY - 44 year old Michael Flint died on the job when he was trapped between a farm tractor and wagon. He was working on the Flint Farm and got off the tractor to unhook the wagon when the wagon rolled forward into Flint.

June 18 - New Hampton, NY - 55 year old George Grumbach of Chester died after he was crushed under his truck that rolled into him. He had just parked a dump truck next to a newly unloaded tree for planting and exited the vehicle. Police said the truck rolled backward, pinning Grumbach beneath the front driver's side tire. He was employed by Mid Hudson Forensic Psychiatric in the maintenance department.

June 19 - Kauai, HI - 55 year old Severino Butay of Kalaheo was stopped along a highway, with other coworkers, attempting to change a tire on a work trailer when the driver of a tractor trailer semi struck the trailer. The semi driver was attempting to stop quickly when the rig veered into the crew. Butay was an employee for Kauai County.

June 19 - Oakland, CA - 65 year old Kil Kim died on the job when he was crushed under a heavy compressor that fell onto him as he attempted to move it. He was the father of the owner of ANP Tire Service but it is not clear if he was actually working for the shop.

June 19 - Frostproof, FL - 54 year old Michael Falk was driving an enclosed cab pulling a spray trailer when a tractor trailer driver struck the trailer from behind. The tractor was pushed and rolled onto its side, breaking out the windows and causing Falk blunt force trauma.

June 21 - Queens, NY - 43 year old Steven Frosch of Garden City South was killed on the job when he was struck by a street sweeper while working inside of a maintenance garage. He was working on one machine when he was struck by the other and was pinned between the two. He was an employee for Queens Sanitation Department.

June 21 - Chicago, IL - 58 year old Jose Tafoya of Merrillville was working behind a paving machine when a drunk driver drove through barricades and struck Tafoya. His coworkers said he normally worked in front of the paving machine but had gone to the rear to help others. They were working on a road project but his employer is not listed.

June 21 - Aurora, CO - 31 year old Jason Lee Nolte suffered fatal crushing injuries when he became trapped by 4,000 pounds of glass panels in two wooden crates inside of a trailer. The window company he worked for was not named.

June 21 - Mansfield, LA - 56 year old Ronnie Meshell was working up in a tree for a private contractor at a residence when his safety harness either broke from age or was possibly was cut by a chain saw. He fell nearly 30 feet from the tree he was trimming.

June 23 - Greencastle, IN - 17 year old Riley Sutherlin was killed on the job when he was crushed by a backhoe. He was working with family members installing new grain bins on the farm and was trying to drive over a pile of dirt, when he lost control of the tractor and it flipped, throwing the teen.

June 23 - Fife, WA - An unidentified 58 year old worker died on his job at Gensco Distribution when he was crushed by machinery. He was moving a nearly 6-ton piece of equipment that the company uses to bend metal when it fell and trapped him underneath. Repeated attempts to locate his name have been unsuccessful so far.

June 23 - Davie, FL - 42 year old Hernan Gutierrez of Hallandale Beach died a horrific death when he fell into the teeth of his operating industrial wood chipper. He had been working all day with the Tech Trees landscaping crew when the incident happened.

June 23 - Decker, MT - 58 year old Stewart Orcutt of Sheridan, WY died on the job when his coal truck crashed through a berm at the Spring Creek Coal mine in Decker. He was employed by Wyoming based Cloud Peak Energy operator of the mine.

June 23 - McHenry, IL - 63 year old Bryan J. Reid was killed on the job when he was struck by his work truck. Reports say he apparently did not secure his truck and he was struck as it rolled. He was employed by the Elmhurst-Chicago Stone Company quarry.

June 23 - Caro, MI - An unidentified farmer died when the tire on a tine rack exploded as he was filling it. Attempts to locate his identity has been unsuccessful.

June 23 - Clavert, AL - 46 year old George L. Anthony Jr. of Semmes has died in hospital after being severely burned on June 2nd while working at Outokumpa Stainless USA. He was working for contractor Ranger Environmental Services at the plant when 90 percent of his body was covered in third-degree burns after an apparent accident in which a group of employees dumped a hot ash-like substance on him. It is unclear how that situation occurred.

June 24 - Birmingham, AL - 26 year old Caleb Bankston was an engineer for the Alabama Warrior Railway. He was checking on something between two rail cars when one derailed, pinning him between the two cars.

June 24 - Moreau, NY - 71 year old Richard Stearns was working with a landscaping crew when a storm broke out and it began to rain. He and others sought shelter from the rain in an equipment trailer when the trailer was struck by a motorist who was not paying proper attention to the road. the name of their employer is unknown.

June 24 - Detroit, MI - 49 year old Gregory Cotton died when the Detroit Dept. of Transportation bus he was operating swerved and crashed, for an unknown reason, into a building causing debris to crush through the windshield.

June 25 - Rocky Mount, NC - 41 year old J. Estanislao Gonzalez, a migrant worker from Mexico, was taking a nap under a bus on a farm when another worker started the bus and drove off. Gonzalez was run over by the bus. The name of the farm he worked for is not listed.

June 25 - San Antonio, TX - 56 year old Dwight Freeman died on the job after being struck by an unknown object while operating machinery. He was working for Southwest Research Institute.

June 25 - St. John's, FL - 24 year old Joshua Edward Hatcher was a contractor working on the roof of a TA Travel Center when he was electrocuted. He later died at hospital. His employer is unknown.

June 25 - Minnetonka, MN - 21 year old Travis Sutherland of St. Cloud was fatally electrocuted while trimming up in a tree. He was working for an unnamed tree trimming company out of Little Falls. Apparently a branch he cut, touched a high voltage line causing him to be electrocuted.

June 25 - Heyworth, IL - 54 year old Thomas Pruser died on the job when he was pinned beneath a utility trailer. He was manager of Tate & Lyle Grain Elevator and was last seen by others on Wednesday. He was reported missing Thursday and found after a brief search.

June 27 - Wyoming, MI - 34 year old Russel Scharenbroch, Jr. was killed on the job when he became caught in an injection molding machine that he was cleaning. He was working for Grand Rapids Plastics, Inc.

June 27 - Minneapolis, MN - 36 year old Travis James Carter was operating a forklift, staking bundles of wood products while working for AA Container and Reload. He was crushed after getting off the machine and a stack of the wood products fell onto him.

June 27 - Sweetwater, FL - 42 year old Jose Felix Obando was working for an unknown subcontractor on an I-95 expansion project when he fell into a deep hole on the side of the roadway. After falling in, the sides collapsed around him. Co-workers were unable to rescue him in time.

June 27 - San Francisco, CA - 32 year old Allen Calloway was shot to death for an unknown reason while working outside during a soccer game. A game which included his son. He was an outreach worker for Herz Playground in the Visitacion Valley neighborhood.

June 28 - St. Augustine, FL - 41 year old Kit Tappen was killed on the job when he was struck by a tractor trailer semi as he worked to help a stranded motorist. He was employed by Complete Collision as a tow truck driver and was helping a motorist when the rear of the semi went into the emergency lane, striking Tappen.

June 29 - McGregor, TX - 55 year old Charl Wynard Swart was killed and two other workers injured in an explosion of a chemical storage tank. The explosion evidently occurred as the men filled large plastic containers with a highly combustible acetylene gas, which they planned to take out into a field to ignite, according to police. As they were filling the containers something ignited the acetylene.

June 29 - Knoxville, TN - 40 year old Paul Shepherd had just gotten his license to drive a taxi cab a few days before he was fatally shot multiple times inside his taxi. He was employed by Absolutely Taxi Company.

June 30 - Lexington, KY - 60 year old Macario Santiago-Cruz was working on a church expansion project when he fell about 30 feet from the St. Peter Claver Catholic Church. It is unclear if he was employed by the diocese of a subcontractor.

June 30 - Fairfax, IA - 57 year old David Lee Fry of Cedar Rapids died on the job when severe weather caused the Intermec by Honeywell Technology Center building to collapse. He was found crushed under the debris of the structure. A co-worker was able to escape the collapse.

June 30 - Eau Claire, WI - 73 year old Gary Henkey has died after apparently collapsing at work on June 27. He was working at Menards Distribution Warranty Center when he was found on the floor, attempting to get up. His manager called emergency responders and he was transported to a local hospital.

June 3, 2014 - Covington, WA - 72 year old Cyntia Lyman was teaching a yoga class when she collapsed and died of a heart related medical event. She was employed by Fitness International LLC dba LA Fitness.

June 11, 2014 - Stockton, CA - 33 year old Scott Hewell, an officer for the Stockton Police Department was seriously injured in a car accident on May 28 as he and his partner headed to help a fellow officer who was dealing with a dangerous suspect. He was initially released from the hospital was had to be rushed back, and he died on the 11th.

June 26, 2014 - Seattle, WA - Freed Aguilar of Bothell was an employee for Metropolitan Building Maintenance and was working a night shift at a location in Seattle. He was working overnight and discovered the next morning having died of a medical event.

June 26, 2014 - Auburn, WA - Leanne Siverson died on the job of natural causes when she was found slumped over her work station. She was an employee for FedEx Trade Networks Transport Inc.

June 28, 2014 - Seattle, WA - John Murphy was working for AAA Auto Club of Washington during the night and was found slumped over the steering wheel of his work vehicle. He died of a coronary medical event.

June 30, 2014 - Cleveland Heights, OH - Jim Brennan was killed on the job when he was shot by robbery suspects at the restaurant he owned named Brennan's Colony. One of suspects worked for the tavern owner.

July 1, 2014 - Livonia, MI - 44 year old John Bieniewicz of Westland was assaulted by a player who had been ousted from a soccer game where Bieniewicz was a referee. He was struck in the head and rendered unconscious by the player. He died later at an area hospital.

July 2, 2014 - Marion, IN - 48 year old James L. Gibson was killed and five others were injured during a chemical explosion that occurred at General Motors Metal Stamping plant in Marion. He was a contractor at the plant but it is not clear who, specifically, he worked for.

July 2, 2014 - Sharonville, OH - 54 year old Kevin Roberts of Clarksville died on the job when his tractor trailer semi truck overturned in a fiery crash. His employer is unknown.

July 2, 2014 - Cynthiana, KY - 28 year old Joel Metz of Plainfield, IN was killed on the job as he worked atop a Verizon cell tower for Fortune Wireless. A report states a heavy antennae fell and a cable struck him.

July 2, 2014 - Memphis, TN - 19 year old Chandler Warren was a college student who had just taken a job at FedEx's World Hub in Memphis when he was injured as a loader that lifts containers fell onto him. He suffered upper body trauma and died later at a local hospital.

July 2, 2014 - Quincy, WA - Scott Engelmohr was amaintenance worker for Quincy Foods, LLC when he felt ill at work. He passed out in the shop and later died of natural causes at a hospital.

July 3, 2014 - Comanche, TX - Dr. Russell Reynolds was helping to set up a Fourth of July fireworks display at Comanche High School when something caused an explosion in a trailer, killing him and injuring others. He was a licensed pyrotechnician and started the communities fireworks display.

July 3, 2014 - San Angelo, TX - 34 year old Roy Michael Jaynes of Rising Star died on the job when he fell from an oil field tower. It appears his harness was not properly functioning. He was employed by Heartland, Inc.

July 4, 2014 - Louisville, KY - 35 year old Jewan Turner was a tow truck driver working along a roadway to help a motorist. He was killed by another motorist who, while drunk, struck Turner. The motorist was later arrested and charged with his murder.

July 4, 2014 - Indianapolis, IN - Officer Perry Renn was a 21 year veteran officer for Indianapolis Police Department. He was gunned down while involved in a shoot out in an alley.

July 4, 2014 - Morehead, KY - 47 year old Lonnie "Curtis" Davis was working on Williams Farm when the tractor he was using to pull a tree out with, flipped up, trapping him underneath.

July 6, 2014 - Brooklyn, NY - Lt. Gordon Ambelas was a firefighter for the Fire Department of New York. His team responded to a fire in a public-housing high rise building and while looking for people who may have been trapped he became trapped and died in the fire.

July 6, 2014 - Augusta, GA - 51 year old Chester Wright was working at the Kellogg's Factory in Augusta when he suffered a neck injury believed from a fall on June 24. Doctors discovered he had a fracture in his neck and scheduled surgery. Unfortunately, due to complications during surgery, Mr. Wright died on July 6th.

July 6, 2014 - Seattle, WA - Jason Keum of Mukilteo was an employee for Korean Air Lines when he was attacked by a contract worker at Sea-Tac Airport in Seattle. During an argument, the contract worker struck Keum in the face causing him to fall and strike his head on the ground. The contract worker was arrested for Keum's murder.

July 7, 2014 - Plymouth, MA - 53 year old Chester Flattery was the head custodian at Manomet Elementary School. The search continues for the cause of his death. What is known is that he died, and 13 others were sent to the hospital, in a hazardous materials accident.

July 7, 2014 - Duball, WA - 19 year old Bradley Hogue of Lake Stevens was killed on the job when he was crushed after being caught in a large auger that he was feeding bark into. He was employed by Pacific Topsoils, Inc. of Everett, WA.

July 7, 2014 - Lakewood, WA - Steve Posler of Maryland Heights, MO was working on a golf course renovating at American Lake Veteran's Golf Course in Lakewood when he suffered a medical event. The golf club owner found him slumped over the steering wheel of his work truck. Posler was employed by Landscapes Ltd, LLC of Nebraska.

July 8, 2014 - Point of Rocks, MD - 38 year old Tyrone Cameron of Hagerstown was killed on the job when he was struck in the neck and head by a metal spool he was attempting to load onto a crate at Canam Steel Company.

July 8, 2014 - Spokane, WA - 30 year old Sheena Henderson was working her usual shift at Deaconess Medical Center/Oncology Department (Rockwood Cancer Treatment Center) in Spokane when the unthinkable happened. Her husband, who was not an employee, came into her workplace and shot her several times before committing suicide. Her husband had made previous threats to kill his wife.

July 9, 2014 - Toledo, OH - 35 year old Ricky Arnez Layson was the owner of R & D Auto Sales and was working inside his business when an armed man burst into the building and opened fire on Layson. He died later in the hospital. The person has been arrested for his murder.

July 9, 2014 - Kingwood, TX - 46 year old Daniel Groover was a Houston Fire Department firefighter collapsed and died inside a vacant home in Kingwood after battling a fire.

July 10, 2014 - Haltom City, TX - 28 year old Julio Menendez of Dallas was killed on the job when a ceiling collapsed inside a vacant building that was being remodeled. The building was owned by Blackmon Mooring and Menendez worked for an unknown subcontractor for Southlake General Contracting.

July 11, 2014 - Hamden, NY - 68 year old Duane Merrill, a well known farmer and former Town Justice, was baling hay on his farm when the tractor's seat detached causing him to fall backward onto the ground between the tractor and baler. He then became entangled in the machine.

July 13, 2014 - Jersey City, NJ - 23 year old Melvin Santiago was an officer for the Jersey City Police Department. He responded to an armed robbery call and was unfortunately shot to death. Other officers returned fire and the suspect was killed.

July 13, 2014 - Pasadena, CA - 24 year old Robert Carrasco of Covina suffered severe head trauma a week earlier when he fell about 15 feet at a construction site in Pasadena. He was an ironworker for Local 433.

July 14, 2014 - Kansas City, KS - 48 year old James Breedlove was a contractor for BNSF - Burlington Northern Santa Fe railroad. While working to transfer grain from a rail car to a smaller semi-trailer, he fell into the rail car and suffocated in the grain.

July 14, 2014 - Kings Mountain, NC - An unidentified man died on the job from a crushed skull while working for Hi-Tex, Inc. which is part of Crypton Fabrics. No other information has been found.

July 14, 2014 - Tampa, FL - 31 year old Bryan Hagan of Mulberry fell to his death on the job while working at a Tampa Electric Power Plant. He fell about 50 feet through a metal grate on a catwalk. He was employed by Southeastern Construction & Maintenance.

July 14, 2014 - Moses Lake, WA - Eusebio Garza was loading a corn truck for National Frozen Foods Corp when he collapsed. It was determined that he died of natural causes.

July 16, 2014 - Clinton, UT - 21 year old Scott Nathan Johnson died on the job when his heart was pierced by a piece of equipment. He was drilling floors into trailers for Utility Trailer in Clearfield when he apparently tripped while holding the drill. When he fell, the drill bit he was holding pierced his heart.

July 17, 2014 - Seward, NE - 28 year old Marcus Lathon of Chesapeake, VA died on the job when the tractor trailer semi truck slammed into the back of another semi truck that had stopped for road construction. The name is his employer is unknown.

July 17, 2014 - Camden, TN - 58 year old Jack Moody was killed on the job when the forklift he was working under fell onto him. He was a maintenance employee for Jones Plastic & Engineering, Camden Division.

July 17, 2014 - Waldwick, NJ - 32 year old Officer Christopher Goodell died on the job when a tractor trailer semi driver failed to stop or slow down and plowed into Goodell's unmarked police vehicle. Goodell was in the parked vehicle using radar to check for speeding motorists when the semi driver veered off the road and into Goodell's vehicle. He was employed by Waldwick Police Department. The truck driver was charged with vehicular homicide.

July 17, 2014 - Louisville, KY - 34 year old Anthony Warner died on the job when he fell down an elevator shaft at a construction project near the University of Louisville’s Belknap Campus. His employer's name is unknown.

July 17, 2014 - Deland, FL - 47 year old Robert Leslie Wagner of Orange Park was fatally electrocuted as he worked on the roof of Kangaroo Gas repairing their cooling unit. He was an employee for Integrity First Refrigeration.

July 17, 2014 - El Paso, TX - 59 year old Robert Narro was a code enforcement officer for the City of El Paso. On July 2 he was conducting an investigation when the driver of a car jumped the curb and struck him. He was hospitalized and died on the 17th. The motorist was arrested.

July 17, 2014 - Santa Fe, NM - 29 year old flight nurse Rebecca Serkey of Rio Rancho, 46 year old pilot David Cavigneaux also of Rio Rancho and 46 year old Base Manager Jamie Butler were aboard Careflight 5 medical helicopter on their way to Tucumcari, NM to pick up a patient when the unthinkable happened. Their Tri State Careflight helicopter crashed into a mountainside just miles from their destination. No cause for the crash is known at this time.

July 17, 2014 - Mission Viejo, CA - 23 year old Daniel Pohl of Apple Valley was an electrician for 5 Star Plastering and was helping build scaffolding and put up a banner for an event at Mission Viejo High School when he touched overhead electrical lines and was electrocuted.

July 18, 2014 - Tulare, CA - 37 year old James Heuer was killed on the job when he became pinned under a spray rig he was driving. He was an Ag Specialist for the College of Sequoias working on a campus farm in Tulare when the incident took place.

July 18, 2014 - Stetten, WI - 77 year old Faye Reinhardt died in a tragic accident on a farm. She was standing near where her husband was loading a tractor onto a trailer when the trailer suddenly detached from a truck, pinning her between the trailer and rear of truck. She was life-flighted to an area hospital when she passed.

July 18, 2014 - Lawrence, KS - 54 year old Thomas R. "Dick" Sheppard died on the job when he came into contact with a power line while working to repair guttering on an historic building. He was Vice President of Rain-Man Guttering.

July 18, 2014 - Britt, IA - 62 year old Ron Eisenman died on the job when he became trapped inside a corn grain bin and suffocated. The name of the farm is not known.

July 19, 2014 - Leigh, NE - 87 year old Edwin "Eugene" Bohac died of drowning when he fell into a water tank on his family's farm. He had been working around the tank and fell in.

July 19, 2014 - Elgin, IL - 47 year old Joel Bork was operating a lightweight tractor on his family farm when the machine flipped, pinning him underneath. His family found him after becoming concerned that they had not heard from him.

July 20, 2014 - Sugarcreek, OH - 34 year old Chadrick C. Cornwell of Dayton was killed when an All Terrain Vehicle he was operating flipped over, trapping him underneath. He was an employee of Tom's Mulch and Landscaping.

July 20, 2014 - Las Vegas, NV - 57 year old Robert Lee Lujan was killed on the job when he was struck by a drunk driver in a pick up who veered off the road and into a bus lane that Mr. Lujan was working in. He was using a concrete saw to repair the bus lane. His obituary says he was a project manager/estimator for Mikon Construction.

July 21, 2014 - Port Ludlow, WA - 61 year old Wayne Eldon Hostetler died when the tractor trailer semi truck he was driving went off the roadway and down an embankment before hitting several trees, crushing the cab. He may have suffered a medical event before losing control of the truck. He was employed by D A Development Co., Inc.

July 21, 2014 - Pine River, MI - 46 year old William Courter was killed on the job while working with a crew on a farm completing maintenance on a barn. He was struck and run over by an unmanned bulldozer.

July 21, 2014 - Sioux Falls, SD - 59 year old David Lee VanVoorst died on the job when he was crushed by a 3,200 pound concrete culvert that fell onto him. He was working underneath the culvert, which was hoisted in the air, when it slipped and came down on him. He was employed by Josten Concrete Products.

July 21, 2014 - Bellevue, WA - 31 year old Theodore Merry of Puyallup died on the job when he fell about 42 feet. After stepping off a ladder, he jumped down on a covered floor opening that gave way under him, causing the fall. He was employed by Evergreen Erectors, Inc. doing work for Bayley Construction.

July 21, 2014 - Summerville, SC - 29 year old Kyle Martin was working with co-workers to pump sand out of a pond when he somehow fell into the pond, drowning. He was employed by Murray Mines, Inc.

July 22, 2014 - Ft. Lauderdale, FL - 29 year old Jose Alfredo Isagirrez-Mejia died on the job when a beam collapsed, knocking him and two other workers off a ladder and scaffolding. He and the others were working for Miller Construction Co.

July 22, 2014 - Kansas City, MO - 53 year old David Craven of Kearney died on the job when he fell from a bridge after being shocked or for another reason. He was working to repair bridge joints on the bridge deck when the incident occurred. He was an employee for Mar-Jim Contracting of Oak Grove, a subcontractor for a Missouri DOT project.

July 22, 2014 - Kansas City, MO - 22 year old Dylan Joseph Snyder was an employee for an unnamed construction company walking along a pick up truck, handing the others in the back caution cones when a trash truck struck him. The trash truck driver was involved in a multi-vehicle crash before crashing into Snyder. Several others were injured in the crash but survived.

July 22, 2014 - Echo, OR - 54 year old Donald Kendall of Ukiah was working on a road construction project for his employer, Oregon Dept. of Transportation when he was struck by a dump truck that backed over him.

July 22, 2014 - Homewood, PA - 39 year old Glenn G. Walker, Jr. of McKeesport was shot to death after an argument erupted during a discussion about a building rehab project in Homewood. Another person was shot and injured but authorities are not clear at this time who did the shooting. An investigation continues.

July 22, 2014 - Austin, TX - Ernest Gonzales, a long time employee for the City of Austin/Austin Resource Recovery, was killed on the job when his work vehicle crossed into oncoming traffic and was struck by a tractor trailer semi truck.

July 22, 2014 - Van Nuys, CA - 48 year old Francisco Miguel Tomas was part of a private cleaning crew power-washing a Target Stores parking lot when he was attacked and murdered by a known gang member who was later arrested. It appears he was stabbed and shot in a random act of violence.

July 23, 2014 - Battle Creek, MI - 27 year old Nicole Gates, of Hastings,was a mother and station attendant for Citgo Gas in Battle Creek. After being taunted and frightened by a man outside the store, he came into the store and stabbed her to death before robbing the store. The suspect was arrested.

July 23, 2014 - Brooksville, FL - 57 year old Ismael Santana of Spring Hill died after being pinned against the cab of a tractor trailer semi truck by a front end loader. He was exiting the semi truck and was apparently not seen by the loader operator when the incident happened. He was an employee for Cemex Cement plant.

July 23, 2014 - Brookhaven, MS - 41 year old John T. Johnson of DuPont, GA was killed on the job when he fell from scaffolding on a water tower in Brookhaven. He was employed by the Lake Park, GA based Southeast Tank and Tower.

July 23, 2014 - Galax, VA - 37 year old Benjamin Lee Hancock died on the job when he was trapped inside a piece of machinery and suffocated. No details on how he became trapped are available. He was an employee for American Mirror Co.

July 23, 2014 - Boardman, OR - 64 year old Glen McCoy of Sparks, NV was struck and killed while working for a subcontractor installing rumble strips at a paving project for Oregon Dept. of Transportation. He was struck by a dump truck driver who backed up over him.

July 23, 2014 - Lakeland, FL - 44 year old Shelby K. Shull of Eustis was struck and killed by a truck driver that barreled through a construction zone, knocking down several orange traffic barrels before striking Shull. He was wearing relective safety clothing as he worked for Florida Safety Contractors on a Florida Dept. of Transportation project.

July 23, 2014 - Chelan, WA - 40 year old Tobey Vigil of Beaverton, OR was killed on the job when the helicopter he was piloting crashed either on take off or landing at a refueling site. He was using the helicopter to hover over cherry orchards, using the blades down draft to blow rain water from the cherries of an orchard. He was employed by Applebee Aviation, Inc. of Banks, OR.

July 24, 2014 - Darby, PA - 53 year old Theresa Hunt, a case worker, was shot to death and a doctor wounded when a patient pulled a gun in an office of a psychiatric clinic and began firing. After killing Hunt, the gunman shot and wounded the doctor who then pulled a weapon and shot the patient. After running into the hallway, the patient was wrestled to the ground and arrested by police.

July 24, 2014 - SeaTac, WA - 68 year old Ronald Sperry of Woodinville was at SeaTac Airport loading and unloading animals for his employer, Dog Gone Taxi, LLC, when he was seen collapsing on security video. He died of heart related natural cause.

July 26, 2014, Henderson, NV - 53 year old Harry Lentz, Jr. of Las Vegas died after falling through a roof skylight at Ocean Spray's Black Mountain Distribution Center in Henderson. He was an mechanical and electrical technician for 20 years for the company.

July 26, 2014 - Napoleon, ND - 83 year old Marvin Grenz was killed on the job when he was trapped inside a grain bin on his farm. He was in the bin trying to get the grain to loosen up when he was partially engulfed.

July 26, 2014 - Old Forge, PA - 52 year old James Joseph "Jimmy" Talariso suffered fatal head trauma when he fell from a lift inside a Family Dollar Store under construction. The name of the company he worked for is not known.

July 27, 2014 - Barneveld, WI - 71 year old Kenneth Powell was killed on the job when a tractor fell through an upper level of a barn, crushing him.

July 28, Moss Point, MS - 25 year old Jerry Lee Taylor II of Helena, AL was killed and three co-workers were injured when a tank they were repairing exploded. At least one worker was inside the tank welding, and Taylor was on top of the tank when the explosion occurred. He was struck in the head. Taylor and the other workers were employed by Omega Protein plant in Moss Point.

July 28, 2014 - Mobile, AL - 53 year old Denise Bowen was killed on the job when she was struck by a motorist as she worked in and along a roadway. She was using a leaf blower, wearing a bright yellow safety vest, when she was struck. She was an employee for the University of Alabama.

July 28, 2014 - Stone Mountain, GA - 29 year old Robert A. Rust of Snellville was a maintenance employee for Smoke Rise Country (golf) Club. He died on the job after the tractor he was operating overturned on a slope. He was working alone and was found by fellow co-workers.

July 28, 2014 - Airway Heights, WA - Donald Bradley, of Spokane Valley, was working at his desk when he experienced a heart related medical event. He was an employee for Northstar Sea Foods, Inc.

July 29, 2014 - Redmond, WA - Kenneth Andrews was a general property manager for Property Maintenance Service Co., Inc. when he suffered a fatal heart attack. He was completing cement work to repair a sidewalk in front of the property and the exertion caused his collapse.

July 29, 2014 - Selinsgrove, PA - 57 year old Elias Hoover died on the job when the front end loader he was using to move hay bales overturned on a steep hill, crushing him.

July 30, 2014 - Ocala, FL - 42 year old Jeffrey Earl Phillips of Anthony was killed on the job when he fell about 50 feet out of a bucket truck while attempting to cut a piece of a tree. He was in a Conrad Tree Surgeon truck but the owner said Phillips worked for himself and was a subcontractor.

July 30, 2014 - Everett, WA - John Snedeker of Shoreline collapsed at his desk and died of a heart related medical event. He was a supervisor for Snohomish County Planning & Development Services Department.

July 30, 2014 - Waterbury, CT - David Shand was murdered on the job when a robbery suspect entered the Buck's Hill Market on July 20th and shot him. Shand died 10 days later at an area hospital. The suspect who shot Shand was arrested for his murder.

July 30, 2014 - Burlington, VT - 61 year old Robert Stanhope of Richford was killed on the job when he was struck by a young motorist as he was removing fog line markers at a road construction site. He was an employee for Whitcomb Construction Company.

July 30, 2014 - Artesia, NM - 21 year old Fabian Martinez died on the job while working for Diversified Oilfield Services. He and other workers were working on Kincaid Ranch cutting a pipe line when an explosion occurred. All workers except Martinez were able to scramble away but Martinez was trapped by the pipe and exposed to hydrogen sulfide gasses while caused his death.

July 31, 2014 - Morea, PA - 66 year old William R. Hoffman of Pottsville was a tree service worker, working along a road way when he was struck by a motorist in a van. It is not clear who he was working for.

July 31, 2014 - Carlsbad, NM - 47 year old William Welchhance of Bowie, TX was crushed under a load of pipes being off-loaded by a forklift at an oil field production site. He was working as a truck driver for Star Trucking.

July 29, 2014 - Westport, WA - 52 year old Kenneth Birdsong was working for South Beach Go-Karts, Inc when he struck his head on a work bench while working on a go-cart. He did not want to go to hospital after his speech became slurred but later was found unresponsive in a break room.

July 30, 2014 - Lehigh Acres, FL - 39 year old Jamel Gardener was killed on the job after a drunk driver sped into a work zone, striking him and a co-worker. He was employed by DBI Services, a Florida Dept. of Transportation contractor.

August 1, 2014 - Chandler, AZ - A 27 year old unidentified contractor was electrocuted while completing a job. Attempts for find the man's identity and his employer have been unsuccessful.

August 4, 2014 - Memphis, TN - 29 year old Pierre Davis did on the job after the commercial sized lawn mower he was using flipped on an embankment. He was an employee for Shelby County Public Works.

August 4, 2014 - Nacogdoches, TX - 50 year old Bobby Joe Beall died he was electrocuted while working on a piece of machinery at Pilgrim's Pride in Nacogdoches.

August 4, 2014 - South Middleton, PA - 56 year old Roger Fortune of Garland, TX died after being pinned between the rear of his semi truck and another parked behind him that rolled forward. He was at Allen Distribution warehouse making a delivery when the truck driver who parked behind him didn't fully set the truck's brakes causing it to roll into Fortune. His employer is unknown.

August 4, 2014 - Bensalem, PA - Erasmo Santana was making mechanical repairs to a vehicle at Negrete's Auto when the jack failed, causing the vehicle to fall down crushing Santana.

August 5, 2014 - Niles, OH - 49 year old Daniel Banks of Leavittsburg was killed on the job when his arm was crushed in an aluminum press. He was employed by BRT Extrusions in Niles.

August 5, 2014 - New Carlisle, IN - 41 year old Jamie Middlebrook, an Assistant Chief for the New Carlisle Fire Department, died on the job when a roof collapsed at a large commercial building fire he was helping fight.

August 5, 2014 - Stanley, ND - 51 year old Alan Eberhand of Harrisburg, AR died after being pinned by a piece of equipment. He had been working on a pay loader hose at Dalton Logistics when the forklift portion of the machine collapsed, crushing him.

August 5, 2014 - Laurel, NE - 60 year old Timothy Hansen died on the job when he was engulfed in corn grain. He was inside a grain bin attempting to break up the grain and when it did, he was buried.

August 5, 2014 - Houston, TX - David Hebert died on the job when he fell from an elevated rail track after losing his footing. He was a Bombardier Transportation employee working from the George Bush Airport in Houston.

August 5, 2014 - Bronx, NY - 62 year old Nick Cavataio of East Stroudsburg, PA was working outside the MTA Bridges & Tunnels Building when he was crushed by a 2,700 pound crate he was helping to unload. He was an employee for Hellman Electric.

August 6, 2014 - Houston, TX - 21 year old Clifton Wright died after suffering burns to over 80% of his body in an explosion inside a "melting room" at CMC Steel plant near Seguin. It is not clear what caused the explosion. A co-worker was also injured.

August 6, 2014 - Glendale, KY - 25 year old Jonathan French was a volunteer firefighter for Glendale Fire Department, as was his mother Lisa. Both were helping to put out a vehicle fire along an Interstate when they were struck by a tractor trailer driver. Jonathan died at the scene and his mother is recovering from her injuries.

August 6, 2014 - Houston, TX - Jeremi Chavarria was crushed by an excavator being loaded onto a trailer at the end of a work day. He was working with his father when something went wrong and came to be trapped under the machine. **Note: the article does not list his name, and an updated article was not found, but comments under the article gave clue as to who he was**

August 7, 2014 - Pomeroy, WA - 31 year old Connor Finerty of Seattle, was killed on the job when the loaded dump truck he was operating crashed through a guardrail, rolled and landed in the Snake River. He was employed by Par Electrical Contractors, Inc. in Pomeroy.

August 10, 2014 - Stockton, IL - 49 year old Thomas W. Lucas of Toledo, IL died on the job after he fell approximately 90 feet from a tower he was painting. He was part of a crew for Sherwood Tower Services of Terre Haute hired to paint the tower.

August 11, 2014 - Snoqualmie Pass, WA - 41 year old Salvador Aguirre Lopez died on the job after he fell about six feet off a ladder. He was employed by American Construction Northwest, LLC.

August 11, 2014 - Crossett, AR - 41 year old Robert Wesson of Hamburg suffered fatal head injuries when he became tangled in a piece of machinery at the Georgia Pacific mill in Crossett. He was working at a paper loading machine when something went wrong.

August 12, 2014 - Ketchikan, AK - 51 year old Kenneth Butkovich of Castle Rock, WA was working with a logging crew in Alaska when he was struck and pinned by an uprooted tree on a logging site. He was employed by Logging Crown Alaska, Inc.

August 12, 2014 - Edmond, OK - 57 year old Robert "Bob" Waterson was killed on the job when a BNSF train struck his work truck. His Edmond Electric truck became stuck in tall brush and plants as he worked to de-energize lines along the track that were no longer in use. A tow truck came to help get the truck but before they could get away from the track, his truck was struck by the train as the tow truck driver was attempting to move his truck.

August 12, 2014 - Pittsburg, KS - 28 year old Kenny L. Clark was one of two men burned in a fireworks explosion and fire that was triggered from an unknown source. Clark died from those injuries. He and the coworker were attempting to move the last of the companies fireworks after the business moved to a new locations. Both men worked for Jake's Fireworks.

August 13, 2014 - Lenoir, NC - 31 year old Jimmy Halliburton of Morganton was killed on the job when he was struck in his upper body by a tree. It happened as he was helping other Caldwell County forestry workers remove a tree that fell into a road.

August 13, 2014 - Brookings, SD - Joey McKenzie was working on a construction site at the South Dakota State University campus when he fell in unknown circumstances. He was employed by Bittner Masonry, a subcontractor for Sioux Falls Construction.

August 13, 2014 - Lamar County, TX - Jorge Alejandio Palmeros Osorni was a contractor working to remove a "paint booth" at PJ Trailers when something caused a huge explosion. No other details are known.

August 13, 2014 - Mount Pleasant, SC - 28 year old Raymond Muniz of Virginia was stabbed to death on the job by a co-worker. The men were working on a construction site at the Belk store when, allegedly, the co-worker picked up a sharp construction object and stabbed Muniz in the chest.

August 13, 2014 - Corona, CA - 31 year old Raul Soto-Olvera was killed on the job when he was electrocuted while trimming a tree. It is unknown who he was working for at the time of his death.

August 14, 2014 - New Orleans, LA - 46 year old Michael Collins, a tugboat captain, was killed on the job when he was crushed under a crane arm. The tugboat, Cory Michael, was pushing a barge with a crane on it. When the tugboat and the barge were passing under the open Florida Avenue Bridge, the crane hit the bridge causing the crane to fall on the tugboat's wheelhouse, trapping Collins.

June 22, 2013 - Mouthcard, KY - 35 year old Donald Castor was killed on the job when he was struck by an impaired driver as he was weed-eating along a roadway. He was employed by Blue Ridge Cable Company.

September 3, 2013 - Somerset, KY - 57 year old Louanne Ellis was a passenger in a vehicle that was involved in a multi-car accident. The vehicle she was in was struck by another motorist and pushed into oncoming traffic. She was working for Bethany House Abuse Shelter, Inc.

September 9, 2013 - Shelbyville, KY - 33 year old Jamaal Wood was trying to be a good Samaritan on the day he died. He was delivering medical equipment for his employer, SCM True Air Technologies, Inc., when he came upon an accident on the interstate. As Wood was walking back to his truck, he was struck by a semi truck that caused a second accident.

November 25, 2013 - Newport News, VA - 45 year old Tony Mays was an installer for Verizon when he was found on the ground of a private residence by the homeowner. The circumstances surrounding his death are not clear.

November 27, 2013 - Vancouver, WA - 67 year old Robert Holsinger suffered a medical event on November 7th while working and was admitted to the hospital. He was an employee for JR Gettier & Associates, Inc. and died of natural causes on the 27th.

November 29, 2013 - Greenville, SC - 22 year old Bruce Gordon III died on the job as the result of a miscommunication with co-workers. They were on a disposal pick-up route when he was struck and run over by the garbage truck he was working from. He was employed by the City of Greenville Public Works.

December 2, 2013 - August, GA - 42 year old Ryan Eskew suffered an unfortunate cardiac event and died while working for Covidien, in Augusta.

December 2, 2013 - Maybrook, NY - 53 year old Timothy Lang of Saugerties was killed on the job when he was crushed under a retaining wall collapse at a construction site in Maybrook. Two co-workers were injured. He was employed by Halmar International.

December 2, 2013 - Farmington, ME - 62 year old Linda Branville suffered a fatal heart attack while working at Walmart Super Store in Farmington.

December 3, 2013 - Burleson, TX - 45 year old Guillermo Medina of Cleburne was working on a piece of machinery called an "auto-reamer" when his clothing was caught in the high speed machine. He died at a local hospital from traumatic blunt force trauma. He was employed by KWS Manufacturing Company, Ltd.

December 3, 2013 - College Station, TX - 28 year old Angel Garcia died on the job after falling from the fourth level of Texas A&M's Kyle Field Stadium. He was working for the Dallas based Lindamood Demolition Inc.

December 3, 2013 - Barnhart, TX - 32 year old Brandon R. Allums of Hall Summit, LA died on the job after falling from an elevation while working for Savanna Energy Services Corp. Rig #649. It is unclear how or from where he fell.

December 4, 2013 - New York, NY - 67 year old Jaroslaw Mychajluk was killed on the job when he fell down an elevator shaft. No other details are available. He was working for FM Ring Associates, Inc.

December 5, 2013 - Clewiston, FL - 66 year old Edward Stiles was working for United States Sugar Corp. when he inadvertently came into contact with a swarm of bees. He died as a result of an allergic reaction to the great number of bee stings he endured.

December 5, 2013 - Cypress, TX - 26 year old Mondale Armstrong died on the job in an unknown incident while working for Municipal District Services, LLC. No further information has been found.

December 6, 2013 - Henderson, NV - 47 year old Jerry Bondi died on the job after he fell from a ladder, striking his head. He was climbing the ladder when it slid out from under him. He was working for Chermac Builders, Inc.

December 7, 2013 - Schaumburg, IL - 43 year old Isidro Garcia-Mendez was killed on the job when he fell from an unknown elevation. He was employed by Nation Pizza and Foods.

December 8, 2013 - Jackman, ME - 38 year old James Russell Jr. was killed in the job when he was struck by an unknown object and suffered blunt force trauma. He was employed by Sabatis Mitchell.

December 8, 2013 - Commerce, CA - 59 year old Richard Kulow was the owner of Lacny Logistics, Inc. and was working in the warehouse of his business when he collapsed from natural causes. He was found unconscious by a co-worker.

December 9, 2013 - Boston, MA - 46 year old Marcos Mancilla was working for Boston Bridge & Steel, Inc. when a large beam fell, striking Mancilla.

December 9, 2013 - Eunice, LA - 34 year old Walter Washington suffered fatal crushing injuries when he was trapped between a truck and garbage truck following a crash between the two vehicles. He was an employee for Progressive Waste Solutions of LA, Inc.

December 9, 2013 - Accomac, VA - 48 year old Perry Simpkins was working for Perdue Farms Chicken Processing plant when he fell ill. While not work related we still feel he belongs on our list.

December 10, 2013 - Garden City, ID - 50 year old Mark Hatten died on the job when he fell from an unknown elevation. He was working for Western Heating & Air Conditioning, Inc.

December 10, 2013 - Mendham, NJ - 49 year old Segundo Pinto was performing his regular duties for Morris County Municipal Utilities Authority when he was injured. He was climbing onto his recycling truck when he slipped, causing a hip injury. After his injury he was unable to walk well and was admitted to the hospital. He died during surgery for his injury from an infection that developed due to the fall.

December 11, 2013 - Elk Grove Village, IL - 38 year old Santiago Cervantes died on the job after becoming caught in or between an unknown object. He was working for Danco Packaging Supply Company.

December 12, 2013 - Livingston, NJ - 67 year old Gregory Johnston was working at the Livingston Public Works garage when he was struck by a township owned dump truck. He was working at the garage for Fairfield Maintenance Inc.

December 13, 2013 - New Iberia, LA - 33 year old Daniel James Vice of Youngsville was killed on the job when he became caught in or between an unknown piece of equipment. He was a supervisor for Cajun Sugar Cooperative, Inc in New Iberia.

December 16, 2013 - Putnam, CT - 52 year old James E. Bianchi died of natural causes while working for Putnam Precision Molding, Inc.

December 16, 2013 - Miami, FL - 79 year old Joaquin Gonzalez was killed at work when he was struck against an unknown object while working for Pardel Export Company, Inc.

December 16, 2013 - Boise, ID - 51 year old Todd Wolf died on the job when he was pinned under a forklift as it was being uploaded from a trailer. He was an employee for Pacific Steel & Recycling.

December 16, 2013 - Los Angeles, CA - 45 year old Nicholas Orozco was a freelance, self employed tree service worker who died on the job after falling from a date palm tree at a private residence.

December 16, 2013 - Fairfield, IA - 50 year old Larry Hanshaw died on the job after being struck in the face by a drum that became a projectile after it was thrown into a fire pit. The drum may have contained fuel vapors. He was a City of Fairfield Wastewater Plant employee.

December 17, 2013 - Moulton, TX - 45 year old Joe Torres died on the job from unknown injuries he received while working for Green Hunter Water.

December 17, 2013 - Belton, TX - 19 year old Cristian Hefner was working for Jasdem, Inc., a company hired by Tarco of Texas, Inc. to clean out an asphalt tank that belonged to the roofing contractor. He was using a power washer inside the tank while dislodged debris that struck him.

December 17, 2013 - Reno, NV - 46 year old Garo Gholdoian was a physician at Urology Nevada Ltd. when a gunman walked into an office an opened fire, killing him. The shooter injured two others before turning the gun onto himself.

December 18, 2013 - Preston, CT - 49 year old John Stagg was injured on December 12 when he was caught in or between an unknown object causing asphyxiation. Details on what occurred were not found. He was working for both TW Ferguson Corp. and covanta of Southeaster Connecticut at the time of his death.

December 18, 2013 - San Antonio, TX - 36 year old Lewis Winston III was working to pick up road work signs along Interstate 37 when he was struck and killed by a motorist who was attempting to slow down for a vehicle in front of him causing him to crash into Winston. Winston was employed by ISI Construction, Repair & Maintenance, Inc.

December 18, 2013 - Haslet, TX - 32 year old Ruben Puga Garcia was killed on the job after being struck by an unknown object. He was working for Carlos Bernal Concrete.

December 18, 2013 - Tuckerman, AR - 61 year old Roy Easley died on the job after falling from an unknown elevation while working for Riceland Foods, Inc.

December 19, 2013 - Mechanicsville, OH - 46 year old John Fasig was killed on the job when he was struck by an unknown object while working for The Carrollton Farmers' Exchange Co.

December 19, 2013 - Panhandle, TX - 21 year old Joe Dylan Spears was killed on the job when he was struck by a train causing massive body trauma. He was working for Robinson Grain Company, LLC and was standing near the grain elevator when he was struck.

December 23, 2014 - Billerica, MA - 48 year old Kenneth Schirl of Saugus was an electrical engineer for Nuvera Fuel Cells in Billerica when he died on the job from an unknown cause.

December 24, 2013 - Coulee City, WA - 62 year old Holger "Hal" Jensen of Boise, ID, was a civil engineer for the Bureau of Reclamation/Pacific Northwest Regional Offices. He was performing an inspection of a construction project at the Pinto Dam in Washington State when he fell onto a concrete surface suffering head trauma and died in the hospital a day later.

December 24, 2013 - Bridgewater, NJ - 50 year old William Cotton of Easton, PA received an electric shock on November 23 while working on ground transformers in Bridgewater. He was hospitalized until his death on December 24th. He was an employee for Scholes Electric and Communications of Piscataway.

December 25, 2013 - Chelsea, MA - 41 year old Thomas Archie Ford of Manchester died after being injured on December 20 when he was struck by an unknown object. He was employed by D'Arrigo Brothers Co. of MA and also a member of the Teamsters Union.

December 26, 2013 - Vernon Hills, IL - 25 year old Patrick O'Reilly of Palos Park was killed and two other workers injured when they were struck by falling concrete outside the Westfield Hawthorn mall. They were part of a demolition crew for P.S. Demolition Inc.

December 26, 2013 - Kingsburg, CA - 54 year old Mack Moody suffered a fatal cardiac event while driving a utility vehicle causing him to be thrown from the vehicle into a pond. He was an employee for Kings River Golf & Country Club.

December 27, 2013 - New Providence, NJ - 41 year old Jose Llibicura Lala died on the job after falling from an unknown elevation. Our information states he worked for Heras General Construction, LLC and Four Winds Construction, LLC. No other information was found.

December 28, 2013 - Sauk Village, IL - 63 year old David Miller of Sumava Resorts, IN was killed on the job when he was struck, suffering fractures while working in Sauk Village. He was a truck driver for ABF Freight Systems, Inc.

December 29, 2013 - San Angelo, TX - 49 year old Jose S. Uriegas III died on the job after being caught in or between an unknown object suffering body trauma. He was a truck driver for Blue Line Drilling Company, LLC.

December 30, 2013 - Erie, PA - 32 year old Ryan Kunzler of North East was killed on the job when he was trapped under a semi truck trailer. He was working on separating the truck from an attached trailer when the truck fell on him and trapped him underneath. He was employed by Five Star International, LLC.

December 30, 2013 - Springfield, OH - 55 year old Larry Grierson collasped and died of a heart attack while he was working for FED Excavating, Inc. and Empire Building Co. LLC

December 30, 2013 - 59 year old Herbert Upton of Melrose was struck and killed by a vehicle while he was working near a plane on the tarmac at Logan International Airport. The driver of the vehicle that struck Upton was driving in reverse. Upton was an employee for Signature Flight Support Corp.

December 31, 2013 - Gonzales, TX - 22 year old Matias Pruneda died on the job after suffering trauma due to thermal burns he received while working for Nabors Drilling USA LP.

December 31, 2013 - Palestine, TX - 32 year old Mario Macias was killed on the job after he fell from an elevation from a roof. He was employed by Anchor Roofing Systems, Ltd.

December 31, 2013 - Norfolk, VA - 27 year old John McCullen of Barco, NC was working at the Lyons Shipyard (his employer) in Norfolk moving equipment from a barge when he was struck by a heavy piece of equipment.

August 7, 2014 - Hanover, MD - Rajinder Kumar was a gas station attendant for an Exxon Gas station in Hanover when he was shot to death during a robbery.

August 14, 2014 - Wappingers Falls, NY - 53 year old Edward R. Williams was attempting to open a gate at his work place. He placed the truck in park but failed to put the brake on. The truck rolled and struck Williams, who was alone at the time. He was found the next day by co-workers of Inter-City Tire and Auto Center.

August 15, 2014 - Bronx, NY - 25 year old Brian Jacome was performing routine maintenance on an elevator at New York Avenue Apartments. As he did so, his clothing became caught in the doors as they were closing which then descended and he also fell. He was an employee for US Elevator, Inc.

August 18, 2014 - Watertown, NY - 28 year old Christopher "Moose" Getman of Felts Mills, died on the job after he was crushed under a piece of heavy equipment as he tried to move it. He was an employee of P & M Construction of Watertown at New York Air Brake, also in Watertown.

August 18, 2014 - Sarasota, FL - 45 year old John McMahon was a maintenance worker for an unnamed company, mowing along grass on a median when a motorist struck him due to poor visibility.

August 18, 2014 - San Antonio, TX - 61 year old Ernesto Valdez was killed on the job when a bag of fracking sand, weighing 35,000 pounds, fell onto him causing major blunt force trauma and asphyxiation. He was employed by Chalk Mountain Services of TX, LLC.

August 19, 2014 - Forest Grove, OR - 29 year old Nicole Laube was an employee for Timber Creek Apartments when someone murdered her. She was found stabbed to death in a parking lot and police believe she was hanging lease-renewal notices on apartments when she was murdered.

August 19, 2014 - Tuscaloosa, AL - 63 year old David Carson of Tuscaloosa, and a line specialist for Alabama Power, and 51 year old Matthew Wallace of Hiram, GA, pilot of a helicopter for Rotorworks of Hiram, both died when the helicopter crashed. Wallace was piloting Carson to check power lines when the craft went down and was reported missing.

August 20, 2014 - Kent, WA - In a senseless act of violence, 52 year old David Christianson and 20 year old Carlos Gonzales were gunned down as they were trying to calm down an argument at the gas pumps of West Valley Shell in Kent. A customer entered the store to talk to Gonzales, who then went outside. Christianson heard a commotion and also went outside to tell everyone to calm down when a suspect went to his car, pulled a gun and shot both the employees. The gunman was charged with murder.

August 20, 2014 - Edmonds, WA - 38 year old Jason Vanlandingham of South Prairie died on the job from electrocution. He was working in a deep trench digging under an electrical duct bank and the pneumatic jackhammer he was using penetrated a buried conduit causing his electrocution. He was an employee for HOS Brothers Construction Inc.

August 21, 2014 - Kalispell, MT - 57 year old Larry Wurtz was injured on August 17 when he fell from a wheeled ladder/staircase while working at a local Walmart Super Center. He suffered traumatic head injuries and died at the hospital.

August 21, 2014 - St. Michaels, MD - 40 year old John Kenneth Shahan of Denton was controlling traffic in a work zone in St. Michaels when he was struck by a motorist. He was a subcontractor working for State Highway Administration.

August 21, 2014 - Fort Wayne, IN - Ignacio Casillas died on the job after the tractor he was driving dropped into a manure pit, pinning him. He was an employee for Irish Acres Dairy Farm.

August 21, 2014 - Palemetto, FL - 53 year old John Winchester was killed on the job when a roller machine he was operating rolled down a berm he was working to build, cushing him. He was employed by Woodruff & Sons for Southwest Florida Water Management District at Sawgrass Lake Park.

August 21, 2014 - Eastsound, WA - 18 year old Taylor Shane Ketchum died of complications of a viral condition while working as a camp counselor for Greater Seattle's YMCA Camp Orkila on Orcas Island. He was found unresponsive on a bunk in the staff lounge.

August 21, 2014 - Garfield Heights, OH - 45 year old Elbert Woods was killed on the job due to crushing injuries received when his clothing became caught in a commercial machine and he was pulled into it. He was working for Cleveland Heights Track Material, a railway track and components manufacturer.

August 22, 2014 - Memphis, TN - 52 year old Luis Berrocal of Costa Rico died from injuries he received in a fall. He was in a bucket lift machine trying to step onto a bridge over the Mississippi River when he fell after losing his balance. The name of the construction company he was working for is unknown.

August 25, 2014 - Oxford, IN - 45 year old Stephen Etter was killed on the job when he was struck by an industrial sized tire rim he was attempting to repair. He was trying to repair a split tire rim on a road grader when it blew off, striking him in the chest. He was an employee for a Northwestern Indiana county highway department.

August 25, 2014 - Bath, NY - 44 year old Richard J. Rowland suffered fatal electrocution leading to cardiac arrest after dumping his load in Urbana. After unloading the dump truck, the uplifted box became entangled in overhead power lines. He exited the truck and while attempting to free the vehicle from the wires, he was electrocuted. He was an employee for the Town of Urbana Highway Department.

August 25, 2014 - Jackson, MI - 60 year old Kim Duong suffered blunt force trauma when he was struck in the chest by a falling steel plate. He was an employee for John Crowley, Inc., a Jackson steel company.

August 25, 2014 - Granite City, IL - 46 year old Rodolfo Romo of Fairmont City and 24 year old Tyler Muenstermann of Edwardsville were killed when a mortar shell exploded while working at Totall Metal Recycling. The business handles recycling of ammunition and military items but they are all supposed to be defused first.

August 25, 2014 - Rumford, ME - 35 year old Brant Munster of Newport died on the job when he fell while working on a crane. He was an employee for Somatex Crane of Pittsfield working on a crane at NewPage mill in Rumford when he hit a roof support and fell.

August 25, 2014 - Lake Havasu City, AZ - 39 year old Charles Vacca was accidentally shot in the head at a gun range where he was an instructor. The person he was helping lost control of the weapon which kicked up and he was shot. He was an instructor for Bullets and Burgers Outdoor Range.

August 25, 2014 - Scottsdale, AZ - 41 year old Jarrod Camp and 36 year old Jeremy Johnson, both from Phoenix, died from toxic exposure while working in a confined space. The men, along with a coworker who was injured, were working inside a sewer at a Scottsdale shopping center when they were overcome and trapped. The men, who for an unknown reason did not have any safety equipment with them, were employed by Camp Industries.

August 26, 2014 - Omaha, NE - 38 year old Bryce Dion from Santa Monica, CA was killed by "friendly"gunfire during the taping of an episode for the TV program, 'Cops'. He was working as a sound operator when police responded to a robbery at a local fast food restaurant. As police opened fire, Dion was shot. He was wearing a bullet proof vest but the fatal shot passed through an opening in the vest. He was employed by Langley Productions.

August 27, 2014 - Aberdeen, MD - 45 year old Cesar D. Caetano of North Bergen was struck and killed by a motorist as he worked on the Garden State Parkway construction. The name of the company he worked for is unknown.

August 27, 2014 - Jetersville, VA - 24 year old William Brice Seay of Glen Allen was fatally electrocuted while working on a home construction site. He had been working with a crane to put metal straps around trusses so the crane could hoist them and apparently the crane touched electric power lines in the process. He was working for his father's construction company but the name is unknown.

August 28, 2014 - San Francisco, CA - 52 year old Alvin Andrews was killed on the job when he was struck and run over by a city garbage truck. He was part of a clean up crew for San Francisco Human Services Agency.

August 28, 2014 - Blackfoot, ID - 19 year old Bo Taylor and 46 year old Trent Sorenson died on the job from toxic exposure inside a sewer manhole. The manhole was new and clean but the use of a sealant used may have caused a toxic reaction the PVC liner emitting the toxic fumes. The young Bo was working inside the space and Sorenson may have realized something was wrong and went in after Taylor. Both men worked for Taylor Construction, owned by Bo Taylor's father.

August 29, 2014 - Albion, NY - Luis D. Larios-Hernandez was a farm worker for Kirby Farms when he was killed on the job. He was in a field standing near one truck, when another truck rolled down a hill. The truck hit him, pinning him between the two trucks.

August 30, 2014 - Christiansburg, VA - 50 year old Robert Joseph Tucker died on the job when he was struck by the bucket of a forklift. Tucker was operating the forklift & bucket and a co-worker was inside the lift working to cut trees on a property. The lift began to tip over and Tucker exited the machine and tried to run away but was struck by the bucket. The co-worker was taken to hospital, his condition unknown. It is not known who they were working for.

August 30, 2014 - Birmingham, AL - An unidentified worker died in a trench collapse while working as a plumber for an unnamed company.

August 31, 2014 - Toppenish, WA - David Gonzalez Rebolledo was killed on the job when he was pinned by farm equipment. He was towing a farm trailer, which was improperly secured. The trailer came loose from the tractor causing Rebolledo to be pinned between the steering wheel and trailer. He was an employee for Shinn & Son, Inc.

January 1, 2014 - El Paso, TX - 62 year old Roberto Sanchez-Reyes, who was self employed, died in a fall from an unknown elevation.

January 2, 2014 - Ruskin, FL - 54 year old John Rose died on the job when the ground beneath him gave way and he was buried. He was walking atop a new underground pipe at a water treatment plant when the incident happened. He was an employee for PCL Construction Inc.

January 2, 2014 - Cairo, NY - 53 year old Ronald "Ronnie" Clifford of Earlton was part of the maintenance crew for Greene County Mental Health building. He was found unresponsive by a co-worker who went looking for him. It appears his death was due to natural causes.

January 3, 2014 - Westfield, WI - 56 year old William Meyers died while working of a natural cardiac medical event. He was employed by Brakebush Brothers, Inc.

January 3, 2014 - East Hazel Crest, IL - 70 year old Albert Ewan was salting sidewalks at Calumet Metra Station for the Village of East Hazel Crest when he suffered a fatal heart attack.

January 4, 2014 - Winter Park, FL - 22 year old Delvin Reynoso died on the job when he was electrocuted after becoming tangled in power lines as he was trimming trees. His coworker was critically injured. He was employed by H & L Landscaping, Inc.

January 6, 2014 - Clearfield, UT - 67 year old Robert S. Benton of Kaysville died after being struck by a vehicle at his work site. He was co-owner of Benton Packing LLC and was struck by a truck backing down a loading ramp that was shared with another business, who owned the truck.

January 7, 2014 - Amarillo, TX - 39 year old James Carey Gallaway was killed and a co-worker injured when they were struck by a vehicle. Both were working under a truck up on a forklift, when the truck fell onto them at HD Wrecker Service. He was an employee for Messer Construction Co, Inc.

January 7, 2014 - Chicago, IL - 44 year old Joseph Vandenoever was killed on the job when he was struck by an unknown object, suffering body trauma. He was an employee of Parkway Elevators, Inc.

January 7, 2014 - Grand Ronde, OR - 39 year old Gary Kline of Independence died on the job when his fully loaded log truck crashed and he was ejected from the cab. He was a truck driver for Siletz Trucking, Inc.

January 7, 2014 - Lancaster, NY - 63 year old David Horn was a beloved and tireless worker for Lancaster TN Animal Control. One who worked to find homes for the animals and loved teaching kids how to take care of a pet. He was found deceased at his desk after his family said he had not come home that day. He is missed by many.

January 8, 2014 - Dallas, TX - 39 year old Jason Artinger of Frisco was fatally electrocuted while working for Motion Envelope. There he was Director of Operations for the printing business.

January 9, 2014 - Campo, CO - 24 year old Adam J. Kujawa of Spring, TX was killed on the job when he suffered head trauma when he was struck by an unknown object. He was working for Greenes Energy Group LLC.

January 10, 2014 - Sherman, TX - 39 year old Michael A. Englehaupt III of Calera, OK was a long time employee for Sam's Club in Sherman when he was killed in an unknown incident.

January 10, 2014 - Bronx, NY - 29 year old Jorge Juca died on the job when he fell from an unknown elevation suffering broken bones. He was an employee for R.S. Ecua Contracting Corp.

January 12, 2014 - Brooklyn, NY - 54 year old Arturo Mateos was killed on the job when he was struck by an unknown object while working for Auto Laundry Car Wash Corp.

January 12, 2014 - Scotland, MD - 56 year old Donald Gothard of Lusby was found at the base of a fixed ladder while working at Point Lookout State Park. He was checking water levels in an equalization tank for Maryland Environmental Service.

January 13, 2014 - Mount Orab, OH - 53 year old Duane McQuitty was killed on the job when he was struck against an unknown object while working for F.W. Kibler Milling Company, Inc.

January 13, 2014 - Jerome, ID - 74 year old Harley Randall was working for Hirai Farms LLC when he was caught in or between an unknown object and was killed.

January 13, 2014 - Ferndale, NY - 59 year old Kenneth Squires died on the job when he fell from an unknown elevation and suffered fractures. He was an employee of County Petroleum Products, Inc.

January 13, 2014 - Roswell, NM - 34 year old Jason Leadingham was standing in the wrong place, at the wrong time while trying to load his truck of feed. A 15-20 foot high pile of feed collapsed onto him, causing asphyxia. He was employed by Pirtle Farms, LP.

January 14, 2014 - Canyon Lake, TX - 50 year old Larry Perez died on the job when he fell from an unknown elevation while working for MZ Flooring Partners.

January 15, 2014 - Riverdale, IL - 59 year old Bruce Kamp was killed on the job when he fell into a piece of grinding machinery while working for Riverview Materials, Inc.

January 15, 2014 - Spring, TX - 46 year old Quy Pham died on the job after he was struck by an unknown object. He was employed by H&W Manufacturing.

January 16, 2014 - Miami Beach, FL - 34 year old Henry Galvez Raudales died on the job after he fell from an unknown elevation. He was employed by Fire Stop Systems, Inc.

January 16, 2014 - Lamont, FL - 18 year old David A. "Drew" Kimberl was crushed to death when a bridge panel weighing nearly 1,800 pounds fell on him while he was dismantling a section of the old Aucilla River Bridge in Lamont. He was employed by GLF Construction Corp.

January 16, 2014 - Shady Shores, TX - 20 year old Jacob Hollings was killed on the job when he was struck by an unknown object while working for Sunshine Greens, Inc.

January 16, 2014 - Norfolk, VA - 52 year old Jerry Bazemore was working for Coleman American Moving Services when he fell ill at work. He suffered a fatal heart attack.

January 17, 2014 - Clearwater, FL - 73 year old Bryan F. Gomez Sr. was a machinist for Onicon, Inc. when he was injured in an unknown incident on January 9th when he was struck by something.

January 17, 2014 - Grayson, KY - 58 year old Jerry Jackson was working for Ralph's Food, Inc. when he collapsed while helping a customer and was later found by a co-worker. It is unknown what caused his collapse.

January 18, 2014 - Rankin, TX - 62 year old Armando Godines died on the job after being struck by an unknown object causing fractures. He was working for Precision Drilling Corp.

January 18, 2014 - Stanton, TN - 40 year old Jose Barcenas was working in an aerial lift on a bridge beam when he became pinned between the lift controls and beam as he extended the boom. He was employed by Dement Construction Co. LLC.

January 19, 2014 - Ranger, TX - 49 year old James Keyser was killed on the job when he was struck by an unknown object. He was employed by Robert Caraway Operating.

January 20, 2014 - New Braunfels, TX - 58 year old Bernard Hildebrand suffered fatal head trauma when he fell from an unknown elevation. He was working for Ball Cabinet Shop.

January 20, 2014 - Windsor, PA - 55 year old Benedict Todt of Gardeners suffered fatal crushing injuries when a 2,000 pound electrical box fell onto him as he attempted to adjust it. He was employed by Lobar, Inc.

January 20, 2014 - Sharpsburg, KY - 36 year old Kendale Terrell Ashby was a flagger cautioning drivers to repair work of damaged guardrails when he was struck by a motorist. He was working for George B. Stone Co, LLC.

January 21, 2014 - Garland, TX - 31 year old Yousef Tulu was found shot to death, from a robbery, at the AVRS Stores, Inc. convenience store he worked at.

January 21, 2014 - White City, OR - 42 year old Bernie R. Mickey was working on the Southern Oregon Rehabilitation Center and Clinics when he was struck by and pinned between a dump truck and wall. He was a supervisor for Global Pacific Environmental.

January 21, 2014 - Newport News, VA - 47 year old Dai Pham collapsed from an unknown cause while working on a roof in Newport News. He was working for Beck Roofing Corp. at the time of his death.

January 22, 2014 - Colorado Springs, CO - 42 year old Padro Carreno was killed on the job when he fell from a fire escape while inspecting a roof. The structure then struck him as he fell. He was working for Weathercraft Roofing Co. of Colorado Springs.

January 22, 2014 - Lewisburg, OH - 34 year old Matt Armstrong suffered a fatal heart attack while he was working, and considered natural causes. He was working for Lewisburg Container.

January 22, 2014 - New Iberia, LA - 34 year old Noah Kevin Howard of Lake Providence died on the job when he was caught in or between an unknown object. He was working for Norris International Services, LLC.

January 22, 2014 - Bogalusa, LA - 41 year old Paul Fornea of Franklinton was killed on the job when he drove a piece of heavy equipment into a hole which was in an area he was unfamiliar with. He was an International Paper mill employee.

January 22, 2014 - Pollok, TX - 39 year old Luico Vasquez was working with a logging crew when a section of tree fell onto him. He was working for Linn Dale Logging.

January 22, 2014 - Hamilton, GA - 57 year old Khounseuk Insixiengmay died on the job when he fell from an unknown elevation, suffering trauma to his head. He was employed by TNT Steel Structures, Inc.

January 23, 2014 - Mulberry, FL - 26 year old Sergio Munoz died on the job after being caught in or between an unknown object while working for A-1 Florida Sod, Inc.

January 23, 2014 - VanBuren, AR - 49 year old Kevin Anderson of Springdale was killed on the job when he was struck by a motorist while working on a section or road construction. He was removing traffic-counting cables for The Traffic Group, Inc. when he was struck.

January 23, 2014 - Mattituck, NY - 30 year old Andrew Leone was killed on the job after being struck in the head by a skid steer bucket that fell off the machine that was being operated by another worker. He was working for Romeo Dimon Marine Service, Inc.

January 23, 2014 - Guaynabo, Puerto Rico - 42 year old David Rivera was killed on the job when he was struck by an unknown object. He was employed by US Army Reserve-AMSA.

January 23, 2014 - Houston, TX - 65 year old Jimmy Goff died as the result of a same level fall while working for Native Resource Development Co., Inc. It is unclear how or why he fell.

January 23, 2014 - Gaithersburg, MD - 63 year old Abdul Ghaffar died in an area hospital after being attacked and stabbed in the 7-11 Convenience store he was working at. Robbery does not appear to be a motive, but rather a random act of violence.

January 24, 2014 - Sapulpa, OK - 20 year old William Hunter Miller of Jenks died on the job after being struck and run over by a forklift. He was riding on the outside of the forklift, operated by another, when he slipped off and was run over. He was employed by Paragon Industries, Inc.

January 24, 2014 - Golden Meadow, LA - 46 year old Darren Usui was killed on the job when he was struck against an unknown object while working for Offshore Rental, Ltd.

January 24, 2014 - Standale, MI - 51 year old Robert J. Myler, Jr. of Holland suffered fatal head trauma when he fell from a ladder inside a walk-in cooler of a restaurant. He was employed by Phoenix Refrigeration, Inc.

January 25, 2014 - Atlanta, GA - 57 year old Willis Redmon died on the job after he was struck against an unknown object while working for Snapper Generator & Started Repair.

January 26, 2014 - Senatobia, MI - 68 year old William Cheek was killed on the job when he fell from an elevation, suffering a traumatic head injury. He was employed by Cheek Enterprises, Inc.

January 29, 2014 - Milton, WV - 68 year old John "Big John" Shirkey suffered fatal head trauma when he fell from an unknown elevation. He was working for Piggly Wiggly in Milton at the time of his death.
