This post is comprised of workers we were not previously aware of for the month of February, 2014.  All efforts to find an online article to link has been made.

February 1 - Medley, FL - 61 year old Carlos Rodriguez died on the job due to asphyxia and being struck by an unknown object. He was employed by Xpress Transporters, Inc.

February 1 - Florence, KY - 71 year old Steven McDavid was killed on the job when he was involved in a car accident while traveling to deliver medical supplies to a nursing home. He was employed by Med Car Pharmacy LLC.

February 2 - Cleveland, OH - 43 year old Sean Kilbane died on the job after taking a hard fall down a set of stairs at the business where he was co-owner, Happy Dog, Inc.

February 3 - Ware, MA - 27 year old Brian Smith died on the job after he was struck by an unknown object while working for Jay's Tree Service.

February 3 - Coral Gables, FL - 59 year old Manuel Fernandez was killed on the job after he fell from an unknown elevation. He was employed by NEL & Sons, Inc.

February 4 - Houston, TX - 33 year old Sheldon A. Adams suffered a fatal electric shock while working for Terex Utilities, Inc.

February 5 - Jonesboro, AR - 71 year old James Luther Johnson suffered a fatal cardiac event while working for Fowler Foods, Inc.

February 5 - Watford City, ND - 61 year old Robert P. Ohlmacher of Roy, UT died on the job when he was exposed to a toxic exposure. He was working in Watford City for Armstrong Water Solutions, Inc. at the time of his death.

February 6 - Benton, LA - 35 year old Brandon J. Beaver of Shreveport died and two co-workers injured while working on ground level transformer at Bossier Parish Park. He was electrocuted. The workers were employed by Jack Spring Electrical Contractors, Inc.

February 6 - Chicago, IL - 54 year old Bertin Cisneros died on the job after falling from an unknown elevation while working for John R. Morreale, Inc.

February 7 - Manvel, TX - 32 year old Francisco Meza was crushed under an unknown object causing asphyxia. He was employed by Interfacing Company of Texas, Inc.

February 8 - St. Petersburg, FL - 33 year old Gilpedro Rivera-Torres of Largo was fatally electrocuted while working in a tree, after his cutting pole touched a power line. He was employed by Sierra Lawn Care Service.

February 9 - Wichita, KS - 25 year old Daniel Flores was stabbed to death on the job by the ex-boyfriend of the office manager of Steckline Communications, Inc., a radio station. The man who murdered Flores was apparently looking for the manager. Police say he was in the wrong place at the wrong time.

February 10 - Mount Jewett, PA - 72 year old Melford Teats was killed on the job when he was struck by a semi truck as he stood near his truck. He became trapped under the truck. He was self-employed by Mel Teats Trucking, Inc.

February 10 - Youngstown, OH - 76 year old Richard Vasko died on the job after suffering heat related burns. He was employed by Team Steel Fabricators LLC.

February 11 - Dilliner, PA - 27 year old Ian McKee of Morganstown, WV was killed on the job when an explosion and fire occurred at a Chevron natural gas well site where he was working. His body was not found for a week. He was contractor working for Cameron International Corp. when the explosion happened.

February 11 - Blytheville, AR - 48 year old Tom Dazey was a locomotive operator for Nucor Steel Arkansas as he was riding a train. He suffered fatal head trauma when the train derailed and he was caught between his train and another train.

February 11 - Heyburn, ID - 54 year old Antonio Ponce Briano died on the job after he became tangled in an animal feed mixing machine while working at Anderson Farms, Inc.

February 12 - Kountze, TX - 43 year old D.L. Kirkindall was killed on the job after being struck by a tree while working for Tanner Timber Services, LLC.

February 12 - Providence, RI - 50 year old Jose Carrion (Jose Roberto Guzman Carrion) suffered major blunt force trauma when a pick up truck he was working on fell onto him. He was a mechanic for MCM Auto Sales, LLC.

February 13 - Poland, ME - 38 year old Michael Rowe died on the job after he was caught in or between an unknown object while working for Thomas N. Richards & Son.

February 14 - Clairton, PA - 64 year old Russell Brown of McKees Rock died two weeks after being burned on January 31, 2013 at the U.S. Steel Corp. Clairton works plant. He suffered burns in the quench tower in hot water. Circumstances are not known.

February 14 - Lorain, OH - 62 year old Franklin D. Johnson Sr died on the job after he was caught in or between an unknown object during a railroad incident. He was employed by Republic Steel.

February 14 - Houston, TX - 37 year old Gilberto Salas was killed on the job when he fell from an unknown elevation while working for Rexel Holdings USA Corp.

February 14 - West Middlesex, PA - 69 year old Richard Beck of Springfield died of asphyxiation after being crushed in or between an unknown object. He was employed with Estes Express Lines, Inc.

February 15 - Creston, IA - 58 year old Kelly Monaghan died during surgery after taking a fall at work. He fell on icy steps, bruising his colon and breaking four ribs. He was employed by Bunn-O-Matic Corp.

February 18 - Piti, Guam - 63 year old Kuan-Shih Cheng suffered fatal electrocution while working for E&Y Electric Co. Ltd.

February 19 - Newport, AR - 49 year old Michael Myers of Bald Knob died when a kettle of liquid steel spilled in the melt shop of Arkansas Steel Associates LLC. A cable on the crane system broke as the kettle was being moved, spilling many tons of liquid, causing Myers to be burned.

February 20 - Groton, CT - 60 year old Christine Beaudoin of Jewett City died of an unknown cause while working for the Personal Support Detachment of US Department Naval Submarine Base in CT.

February 21 - Monticello, IA - 62 year old Daniel Kraus was working with a co-worker on the 20th to install an industrial garage door at Frontier Feeds. Something caused the door to fly up and strike him in the head and then he fell from scaffolding. He was employed by Monti Home Improvements, Inc.

February 21 - Shiocton, WI - 63 year old Michael Clauson of Clintonville died on the job when he was pinned between his milk truck and a barn wall after the truck slid on the ice and he couldn't stop it. He was employed by Keith R. Olmstead & Sons, Inc.

February 21 - Cochranville, PA - 33 year old Ulises Malave Ortiz Jr was killed on the job after suffered head trauma during a fall from an unknown elevation. He was working for Ray Eager and Sons LLC.

February 21 - Jackson, MI - 24 year old Kevin Poore was killed on the job from an unknown  incident while working at Tiger Plaza for MidSouth Elevator LLC.

February 22 - Huntington Station, NY - 55 year old Steve Nelson of Copiague died and many people were sickened when a flue pipe malfunctioned causing a build up of carbon monoxide gas in the heating system of Legal Sea Foods LLC restaurant. Nelson was the restaurant's manager.

February 22 - Magee, MI - 57 year old Curtis O'Neal Jackson Jr. died on the job after he was struck by an unknown object, causing head trauma. He was working for Stubbs Heating and Cooling at the time of his death.

February 23 - Las Vegas, NV - 65 year old Medardo Morales was working for the MGM Grand Las Vegas when he tripped and fell on a curb on February 18. He struck his head on the curb and was admitted to the hospital. He was released and then readmitted where he died.

February 24 - Naples, FL - 40 year old Juan Martinez died on the job when he was electrocuted. He was employed by Hogan, Inc.

February 24 - Missouri City, TX - 30 year old Alegandro Gonzalez and 22 year old Thomas Romo, Jr. were killed on the job after receiving an electric shock in an unknown incident.  Both men worked for Maverick Drilling Company.

February 24 - Corpus Christi, TX - 53 year old Jeffrey Bobzian of Highlands died on the job when a pipe fell from an overhead crane, crushing him. He was employed by Pasadena Tank Corp. 

February 25 - LaPorte, TX - 64 year old Eldon Reich died on the job after falling from an unknown elevation while working for Minot Builder.

February 25 - Fremont, IN - 53 year old Robert Fourrous was found unresponsive, slumped over the steering wheel of the fork lift he was operating for Con-Way Freight. It appears he died of natural causes.

February 26 - Houston, TX - 43 year old Redes Funes suffered a fatal electric shock while working for Eliseo Castro.

February 26 - Hollywood, FL - 25 year old Marcelo Zuniga died on the job after falling from an unknown elevation causing blunt force trauma.  He was employed by Simpson Environmental Services, Inc.

February 27 - Oakfield, WI - 42 year old Anthony Shields of West Bend died when he became entangled in a piece of industrial equipment while working for Seneca Foods Corp. 

February 27 - Long Beach Township, NJ - 56 year old Luis Guaman died on the job after falling from an unknown elevation while working for Carlito Andrade.

June 16, 2014 - Houston, TX - An unidentified 58 year old worker was crushed to death under falling rig equipment. 

August 9, 2014 - Lancaster, PA - 15 year old Jonas King died on the job, at a farm, when the skid steer he was operating overturned into a manure pit.

August 28, 2014 - New Castle, DE - An unidentified 47 year old man was killed on the job when he was struck by a skid steer and pinned under it.

August 31, 2014 - Savannah, GA - 58 year old Ricky Salter was a maintenance worker for International Paper, Riverside Paper mill when he was killed on the job. It is unclear how he died, but one report states he was working around an elevator and another says he was in the paper roll handling area. 

August 31, 2014 - Brooklyn Park, MD - 47 year old Luzviminda Monreal was shot and killed by a coworker who was attempting to rob the Popeye's restaurant they were working at.

September 5, 2014 - Detroit, MI - 24 year old VonDre Gordon of Romulus died on the job while working as a baggage handler at Detroit Metropolitan Airport. He suffered leg and other body trauma involving the ramp of a bag belt loader. He was an American Airlines contract worker employed by Prospect Airport Services.

September 5, 2014 - Denver, CO - 52 year old Abel Maldonado died on the job after being struck by a recycling machine. He was attempting to free a jam when the machine lurched, striking him. He was employed by Atlas Metal and Iron Corp.

September 5, 2014 - Jacksonville, FL - An unidentified worker was killed on the job when the ATV he was operating flipped into a lake and he drowned.

September 5, 2014 - New Albany, IN - An unidentified contract worker from North Carolina was killed on the job when he was struck in the head when a pulley failed. He was helping to assemble electrical towers when the incident occurred.

September 6, 2014 - Bridgeton, MO - An unidentified employee of Hussmann Refrigeration died on the job from an incident involving equipment.

September 6, 2014 - Fairbanks, AK - An unidentified worker from Malaysia died when the bus he was operating crashed on a slick road. He was a tour bus driver for Princess Cruises, taking people on a tour.

September 6, 2014 - Houston, TX - 52 year old Marty Carroll was found shot to death at the Keith, Inc. chemical plant where he worked. He also lived on the property.

September 7, 2014 - Bakersfield, CA - Carson Peter-Contesse of Sultan, WA, who was in his 20's, died after attacking his co-workers. He was working with a crew for Railworks Track System near Bakersfield. The other crew members were able to hold him down until police arrived, but it was determined he wasn't breathing so was taken to an area hospital where he died.

September 8, 2014 - Charleston, SC - 58 year old Larry Britton suffered a fatal heart attack while working to communicate information to police officers during an active police situation. He was an employee for Charleston County.

September 8, 2014 - Caledonia, MI - 42 year old Timothy Eugene Dougherty died on the job after being struck by a road grader on a construction site. He was employed by Kent County Road Commission.

September 9, 2014 - Charleston, SC - 45 year old DeputyJoseph Matuskovic was shot a day before while responding to a call at an apartment. He was taken to an area hospital. He was employed by Charleston County Police Department. 

September 9, 2014 - Portland, ME - 56 year old James Laurita was a co-founder of Hope Elephants, a place that cared for retired circus elephants. He was killed when he accidentally fell and was then stepped on by one of his beloved animals.

September 9, 2014 - Gratiot, MI - An unidentified employee for Gratiot Central Market was murdered by an angry co-worker who he was involved in a fight with. He was shot.

September 10, 2014 - Broken Arrow, OK - 52 year old Richard Jones died on the job after he fell under the tires of a vehicle he was attempting to exit. He was an employee for the City of Sand Springs, OK.

September 10, 2014 - Mannsville, OK - 42 year old Ernesto Rodriguez of Gainesville, TX was killed on the job after he was struck by a pipe that came shooting up a well hole while working for XTO Energy near Mannsville.

September 10, 2014 - Columbus, OH - 76 year old Jack L. Martin of Troy died on the job when the front end loader he was operating somehow fell into a sand hopper. He was employed by Snyder Brick & Block Co.

September 12, 2014 - Madison, AL - 47 year old Jason Buckley of Union Grove, AL died on the job after he fell from an elevated bucket. He was working from the bucket to trim a tree, when a limb fell onto the bucket, knocking him to the ground. It is unclear who he was employed by.

September 12, 2014 - Brooks Lake, WY - 31 year old Adam Thomas Stuart of Virgin, UT was working for an ecological assessment service, Nature's Capitol of Boise, ID and was making his first trip into the back country. He was working alone. He went missing on September 7th and was found having been killed by a bear.

September 13, 2014 - Kansas City, MO - An unidentified FedEx employee was killed on the job when his truck rolled into him, pinning him.

September 13, 2014 - Arkansaw, WI - 56 year old John Unser was working in a trench at a home construction site when the trench collapsed. He was looking for a plugged drain tile. Unfortunately, also in the trench was a two year old boy, who according to reports wanted to give "Big John" a hug. Another young man was also in the trench but escaped.

September 14, 2014 - Scranton, PA - 38 year old Cpl. Byron Dickson, a Pennsylvania State Trooper, was killed and another injured when they were ambushed outside a rural barracks in the Pocono Mountains. The man who ambushed the officers is still on the loose at this writing, somewhere in Pennsylvania.

September 14, 2014 - Birmingham, AL - 62 year old Arthur Louie Gaulden of Alabaster was a subcontractor for Allstate Motor Club and was helping a motorist change a flat tire. It was then he was struck by a motorist on the roadside.

September 15, 2014 - Monroe, WI - 18 year old Troy Gorr died on the job as he was working on a farm. He was operating a tractor when it overturned and he was ejected from the machine.

September 15 - Horton, AL - 65 year old Bennie Poole was using a brush hog farm machine on uneven terrain when it tipped over and he died on the job.

September 15 - Panama City Beach, FL - 27 year old Ryan Cook was working for an unnamed company to repair pool lights at a local resident. His coworker left momentarily but when he retured he found Cook in the pool from probably electrocution.

September 15 - Lake Providence, LA - 60 year old Tommy Hart died on the job after he was struck by a vehicle on a work site. He was trying to free a shovel that was being run over by a truck when the incident took place. He was employed by Terral River Services.

September 15 - Clear Lake, MN - 61 year old Brad Braun was killed on the job when his farm tractor flipped. He was operating the tractor to pick up bales of hay. The bales became too heavy, causing the tractor to flip and pinning him underneath. 

September 16 - Carbon County, UT - 46 year old Alejandro Ramirez of Price was crushed inside the cab of his mobile diesel can-setter machine that he was operating. He was working for UtahAmerican Energy Inc.'s West Ridge Mine.

September 16 - Clever, MO - 73 year old Robert Shelby Hill died on the job after he was struck by a front end loader as he walked on a job site. He worked for an unnamed subcontractor for Leo Journagan Construction, Inc. 

September 16 - LaQuinta, CA - 45 year old Jose Juan Vallejo was working with others in a rowboat at PGA West in LaQuinta when the boat overturned and he drowned. He was employed by Main Stream Aquatics.

September 17 - Marvell, AR - 59 year old John Barefield was struck and killed on the job as he was working along a road way replacing signs and was struck by a semi truck driver. His employer is unknown.

September 17 - Omaha, NE - 71 year old James Minshall, owner of JR's Jewelry & Gifts, was shot to death by a suspect who attempted to rob his store.

September 17 - Drumore, PA - 24 year old Moses Fisher was working on a farm as a silo was being filled. He went to check if the silo was being evenly filled and as he climbed up and then inside the silo he was overcome by methane fumes and died.

September 17 - Hiddenite, NC - 56 year old Terry Lee Ledford was performing mining work when a 25 foot hole collapsed, burying him. It is unclear who he was employed by.

September 17 - Las Vegas, NV - 34 year old Gabriel Aguilar-Camacho was a passenger in a landscaping work truck when it collided with another truck on a freeway entrance. The collision caused the truck to crash down an embankment. He was employed by Par 3 Landscape.

September 17 - Mill City, OR - 33 year old Bryan Dodge was working to repair a wood press machine for Freres Lumber Mill when he was crushed in the machine. He was a millwright for the company when the machine suddenly activated causing his death.

September 18 - Dalton, GA - 24 year old Josh Painter was killed on the job when he was struck by machinery. He was attempting to move the machine, which shifted, and fell against him on the floor. He was employed by Marketing Alliance Group.

September 18 - Madison, WI - 44 year old Carlos Martinez of Aurora, IL was driving his semi truck near Madison when a car crossed the center median, causing a fiery crash. It is unknown who he was driving for.

September 18 - Austin, TX - 35 year old Texas Deputy Jessica Hollis was driving her police car on duty when she was caught up in flood waters during a severe storm. She radioed that she was in trouble but before finding her, her car was swept away and later found in Lake Austin.

September 18 - Chandler, AZ - An unidentified man was killed on the job as he fell into a large wood chipper head first. A logo on a work vehicle at the site was for Phoenix Trim-A-Tree.

September 19 - Burke County, NC - 43 year old Flavio James of Mount Giliad died on the job after being struck by a motorist during road construction. His employer is unknown.

September 19 - Yonkers, NY - 79 year old Alfred Thiebaud was a well-known owner of Delicioso Coco Helado ice cream business in Yonkers. He was found crushed under a security gate to the business when fell onto him.

September 19 - Edmond, OK - 54 year old Daniel Birney and his 56 year old brother Michael Birney both died on the job and a coworker was injured when an elevated lift platform at a 2nd story level tipped over, causing their fall. The company they worked for is unknown.

September 20 - Alexander, NY - 23 year old Scott Booth, Jr. of Attica was working on a farm, inside a hay baler machine when he somehow became trapped inside.

September 20 - Farmington, NY - 57 year old Lech Chrostowski died on the job after falling about 20 feet from a roof. It is not known who his employer was.

September 20 - Longview, WA - 50 year old Scott Trapp of Vancouver was working to pull electrical wires through a conduit when he suffered a massive heart attack. His coworkers tried to help him and called for rescue. He died at the scene while working for JH Kelly LLC, a general contractor.

September 21 - Hopkins, MN - 51 year old Perry Goffard was an employee for Supervalu Food Distribution when he was killed on the job. He died after being pinned by a piece of equipment.

September 21 - Fairfield, AL - 53 year old Edward Lamar Bryant of Verbena, AL was an employee for US Steel in Fairfield when he was killed on the job during an explosion at the plant.

September 21 - Hamilton, OH - 54 year old John Sunderhaus was operating a farm tractor when it rolled over, pinning him underneath. The farm name is not known.

September 22 - Rupert, ID - 30 year old Dayne A. Hansen was working at a dairy farm when an explosion occurred on the trailer he was welding. 

September 22 - Bogue Chitto, MS - An unidentified Lincoln Country Road Crew employee was killed on the job when the dump truck he was driving left the roadway, crashed into trees and caught on fire.

September 22 - Tacoma, WA - 57 year old Craig V. Lamotte was operating a belly dump truck when it became high centered on a berm. He was working to dig the truck loose when a coworker offered to use his bull dozer to push him free. Lamotte went back to his truck. After the truck was pushed free, the dozer operator realized the truck was unmanned and saw that Lamotte had been struck.

September 23 - Largo, FL - 34 year old Adam Ferlanie was fatally electrocuted in unknown circumstances while working for Tampa Steel Erecting Company. 

September 23 - Jersey City, NJ - 27 year old Rodalfo Vasquez Galian was killed on the job when he was struck and crushed under a slab of concrete that fell from a building as he stood on the ground. He was working for an unnamed subcontractor at the Tritel hotel building site.

September 23 - Roswell, GA - 40 year old Jose Castillo died on the job after falling down stairs at a construction site. His employer is unknown.

September 23 - Manchester, TN - 50 year old Ricky McCurry was killed on the job after becoming pinned under an overturned backhoe. He was working on the Eagleville School construction site for a subcontractor to Robert Biscan Construction.

September 23 - Birmingham, AL - 33 year old Doug Hutcheson of Odenville, a United Parcel Service supervisor, and 46 year old Brian Callans of Birmingham, a United Parcel Service Business Manager, were murdered by an employee who had been fired from his job. After losing his appeal to be reinstated, the man entered the Birmingham center and shot Hutcheson and Callans before turning the gun on himself, committing suicide.

September 24 - LaBarge, WY - 35 year old Jared Loftiss suffered fatal burns and three others were injured when a natural gas storage tank exploded. All worked for the Houston based EOG Resources. 

September 25 - Moxee, WA - 33 year old Juan Esteban Garcia of Yakima was injured on September 21 as a piece of machinery fell onto his head. He was working for Black Star Ranch, LLC.

September 25 - Tampa, FL - 51 year old Ricardo McCalop was a forklift operator who was using the machine to carry a heavy steel beam when suddenly the forklift broke and the beam swung back and fell onto the cab of the forklift, crushing McCalop. It is not known who he was employed with.

September 25 - Moore, OK - 54 year old Colleen Hufford was killed on the job when she was attacked by an ex coworker who had been fired from Vaughn Foods. The man entered the building and attacked Hufford with a knife and then beheaded her before stabbing another woman. He was stopped from killing that worker by a member of management, who was also a reserve officer, who shot him.

September 25 - Quilcene, WA - 24 year old Jeremy Paapke was killed on the job when he was struck in the head by a felled tree on a logging site. He was employed by Brian Borde Contracting, Inc.

September 26 - Havertown, PA - A 55 year old unidentified man was electrocuted and a coworker injured while apparently working from an aluminum ladder. It is not known who they were employed by.

September 26 - Spokane, WA - 46 year old Richard Carlson died after he fell approximately 85 feet from a tree he was working in. He was using a saw that apparently became pinched and then he fell. He suffered blunt force trauma and died in an area hospital. He was employed by Ever Green Tree Care, Inc.

September 26 - Moncure, NC - 29 year old William Jeffrey Belk of Sanford was killed on the job when a piece of machinery fell onto him. He was working at Boise Cascade Wood Products plant when a logging "C" Clamp (that picks up logs) fell onto him.

September 29 - San Benito, TX - 76 year old Serafino Sanchez died on the job when he was pinned by a piece of falling metal while working for All Star Metal Recycling at Port of Brownsville.

September 29 - Fremont, NE - 54 year old George Young was operating a forklift and died on the job after becoming pinned by the machine. He was driving backwards when it struck a shelving unit causing him to be pinned between.  He was employed by Midwest Manufacturing at Menards Distribution Center.

September 29 - Lumberton, NC - 53 year old Edward M. Reiner of New Milford, CT was in North Carolina to work at the Robeson County Fair when he was shot to death during a robbery behind a convenience store. He was employed by Dreamland Amusements.

September 29 - Lansing, NY - 44 year old Craig Schenk died on the job when he was pinned by a corn harvesting machine. He was performing maintenance work on the machine while working at Bensvue Farm.

September 30 - Roselawn, OH - 54 year old Daniel Muscarella was servicing an ATM machine when he was attacked and shot during a robbery.  He was employed by Diebold.

September 30 - Little Rock, AR - 49 year old Beverly Carter was an agent for Crye Leike Realtors of North Little Rock. She was called by a man to have her show him a home. That man abducted her and she was found murdered a week later. The man was captured and arrested for her murder.

September 30 - Linwood, KS - 85 year old Edna Elder was driving a farm grain truck when the truck struck a pole and caught on fire.

September 30 - Charlotte, SC - An unidentified employee for Source Recycling was killed on the job when he or she was struck by a forklift.

We continue to honor our lost workers who died earlier this year who we were not previously aware of. Our hearts and thoughts are with the families and are available to help in any way.
While often unsuccessful with the later dates, we do our best to find a link for each incident to provide further details. 

March 1, 2014 - Big Spring, TX - 31 year old Donald Wiesner of Abilene was critically injured on February 28, 2014 and died after he was struck by an unknown object while working for Bandera Drilling Co, Inc.

March 1 - Mount Pleasant, TX - 44 year old Danny Allen died on the job after he was struck by an unknown object. He was employed by Chad Menefee Construction.

March 1 - Spring, TX - 42 year old Pedro Sanchez was killed on the job when he fell from an unknown elevation while working for Renee LaPaz.

March 1 - Woodward, OK - 72 year old Michael F. Moore of Mooreland was a pumper on an oilfield site for Sampson Brothers, Inc. when he suffered asphyxiation after being caught in or between an unknown object.

March 3 - Kersey, CO - 51 year old Joe Sherman of Greeley died on the job from inhalation of a toxic substance while working for Now or Never Trucking, Inc. and Suncor.

March 3 - Atlanta, GA - 54 year old Virgil "Joe" Whorton Jr. of Cartersville was killed on the job when he fell from a roof.  He was the owner of All-Top Roofing Services, Inc.

March 3 - Winterville, NC - 27 year old Antonio "Tony" W. Riddick of Elizabeth City was found lying in an isle of a grocery store by his co-worker. He later died at a hospital of unknown causes. He was employed by RGIS, LLS.

March 4 - Pagosa Springs, CO - 38 year old Colin Sutton was part of a ski patrol team who was skiing in the back country and was caught in an avalanche. He co-workers were able to free him but he died later at hospital. He was employed by Wolf Creek Ski Area. 

March 4 - Blue Springs, MO - 44 year old Allen D. Clem of Kansas City was Product Development Technician 2 for Fike Corporation when he suffered fatal head trauma in a fall from an unknown elevation.

March 5 - Shelton, CT - 62 year old Paul E. "Goldy" Goldspink suffered a fatal heart attack while trying to tie down an excavator to a trailer. He was employed by City of Shelton DPW Transfers.

March 6 - Brooklyn, NY - 33 year old Manuel Colorado was killed on the job after he fell from an unknown elevation while working for Adar Steel Inc.

March 6 - Seagraves, TX - 42 year old James L. Croft of Tyler died on the job when he was crushed by a falling silo. Croft was operating an excavator while co-workers were attempting to take down a silo tank when it fell unexpectedly, landing on top of the vehicle cab. He was employed by WV, LLC and K&L Oil Field Demolition, a company which was taking down structures inside the Venture Chemicals plant.

March 6 - Hastings, MI - 42 year old Alicia Avalos-Rebolledo was killed when she was attacked by a bull as she attempted to herd cows to a milking station. She was employed by Valley Grove Dairy Farm.  

March 7 - Buffalo, NY - 56 year old Stephen "Steve" Powlowski, Jr. of Tonawanda was working at his business, Steve's Auto Service, on a car when it caught on fire. The shop was engulfed in flames and he was dragged from the scene by co-workers but died later at hospital of burns. 

March 7 - West Palm Beach, FL - 67 year old Richard Milke died on the job of an unknown cause while working for Home Depot USA, Inc.

March 8 - Los Angeles, CA - 67 year old Richard Chong was critically injured on March 6 when he fell from a ladder on the roof of his business, suffering head trauma.  He was performing HVAC maintenance at his business, Daehung Refrigeration. 

March 9 - Saint Cloud, FL - 45 year old Steven Cockerham died of a natural cardiac event while working for Osceolasc LLC. 

March 9 - Fremont, OH - 28 year old Ramiro Sanchez was working as a bartender at the Last Call Bar when someone came into the bar with a gun and began firing, hitting Sanchez and two others. The other two who were also killed were relatives of Sanchez who were there celebrating a birthday.  

March 9 - Buffalo, NY - 76 year old Leo Sucharski was a letter carrier for Buffalo-Western New York Branch 3 post office.  After his shift on March 6 and parking his work truck, he slipped on the ice and suffered head trauma. He died on the 9th at hospital.

March 10 - South Beloit, WI - 51 year old Alfredo Arrendondo of Beloit was killed on the job when his arm became caught in a conveyor belt at Behr Iron & Steel, Inc.'s recycling plant.

March 10 - Italy, TX - 22 year old Jonathan Dehoyos of San Antonio, died on the job after he was struck by a work vehicle. He was working from a litter truck, picking up trash along an interstate when he jumped off the back of the truck to pick up trash. The truck driver did not realize he had jumped off, and backed into him. He was employed by M&M Striping & Power Washing LLC.

March 10 - Montgomery, AL - 43 year old Vettia Roche was a manager of a Taco Bell restaurant when she was attacked and beaten to death by two teenage employees. Those teens were charged with her murder. The parent company of the restaurant is Monty Mex Corporation.

March 10 - Sun City, AZ - 43 year old Christopher J. Shenker was the owner of CJ's Bathtub Refinishing & Repair at the time of his death. He was working in a customer's home and although he was wearing a respirator he was still overcome by noxious fumes.

March 11 - Newport, AR - 56 year old Larry Ball of Floral was killed on the job when was pinned between two pieces of machinery after an aluminum rolling mill cradle opened unexpectedly. He was employed by Norandal USA, Inc.

March 11 - Sarasota, FL - 63 year old Lam Hong Auy died on the job after she was pinned beneath a 2,000 pound machine that employees were attempting to move. She was employed by Eaton Corporation.

March 11 - Honolulu, HI - 54 year old Wayne Tanaka died on the job after he fell from an unknown elevation. He was employed by Higa Meat and Pork Market, Ltd at the time of his death.

March 11 - Poway, CA - 40 year old Leobardo Valdenegros was killed on the job after being struck by a motorist as he worked along a roadway. He was working for Heaviland Enterprises, Inc., a landscape company, blowing leaves along the road when he was struck.

March 12 - Irvine, PA - 26 year old Nikolai Giles Briggs of Clarendon was killed on the job when a crude oil tank exploded, knocking him some 60 feet away. He was employed by TNT Oil Company.

March 12 - North Smithfield, RI - 64 year old Lionel A. Belisle suffered a fatal heart attack while working as an associate at WalMart.

March 13 - St. Francisville, LA - 37 year old Suyapa Martinez died on the job after being struck by an unknown object, suffered blunt force trauma while working for Bluff's Golf LLC.

March 13 - Brooklyn, NY - 41 year old Shakir Qureshi died on the job from asphyxiation after inhaling a toxic substance while working for Layton Manufacturing Corp.

March 13 - Missoula, MT - 56 year old Ronald Mosher of Iona, ID died of a sudden cardiac arrest while working for Dillard's in Missoula.

March 13 - Habbronville, TX - 68 year old Walter Harrison Hill of Combs was a semi truck driver for Guardian Building Products Distribution when he fell from an unknown elevation and died of head trauma.

March 14 - Garland, TX - 37 year old Kevin Smith died as the result of blunt force trauma after being struck by an unknown object. He was employed by First Graphic Services, Inc.

March 14 - Shreveport, LA - 61 year old Steven Lynn Young of Stanton, TN, died on the job when he suffered head trauma after being struck by an unknown object. He was employed by South Industrial Mechanical Maintenance Company, LLC.

March 14 - Longwood, FL - 48 year old Willard B. Tincher of Sanford, died on the job after falling from an unknown elevation. He was employed by Florida Pool Enclosures, Inc.

March 14 - Golden Meadow, LA - 30 year old Albor Llamas was killed on the job after falling from an unknown elevation, suffering head trauma. He was employed by Scrap Cutting, Inc.

March 14 - Tilden, TX - 45 year old Glenn E. Weigang of Pleasanton died on the job after he suffered burns in an unknown incident. He was employed by Mesa Southern Well Services, LP.

March 15 - Lake Preston, SD - 51 year old Anthony "Tony" DeKnikker died on the job after he was caught in an object causing asphyxia. He was working for Prairie Ag Partners.

March 15 - Port Charlotte, FL - A 42 year old unidentified worker was killed on the job from an electric shock while working for BF Fort Myers, Inc.

March 16 - Santa Rosa Beach, FL - 76 year old Larry Kent Wolfe of Martinsville died after falling from an unknown elevation and suffered head trauma. He was working for his favorite charity, Caring & Sharing of South Walton Co., Inc.

March 17 - Santa Ana, CA - 48 year old Ignacio Garcia died on the job of a heat related illness while working for Mission Landscape Co.

March 18 - Atlanta, GA - 43 year old Vicki Moulder of Lawrenceville died she fell from an unknown elevation while working for The Coca-Cola Company.

March 19 - Converse, TX - 64 year old Edgardo Perez-Suarez was killed on the job after being struck by an unknown object.  He was employed by Ingram Ready Mix, Inc.

March 19 - Chicago, IL - 55 year old Scott L. Tolbert of Joliet died on the job after falling from an unknown elevation. He was employed Chicago Doorways LLC.

March 20 - Tecumseh, NE - 63 year old James Bennet was employed by Midwest Farms Cooperative when he died of inhalation of an unknown substance.

March 20 - Sulphur, OK - 57 year old David Simpson died of asphyxiation after inhaling a toxic substance.  He was employed by Premier Trading Transportation.

March 20 - Jamaica, NY - 44 year old Anthony Cirillo died under unknown circumstances while working for Consolidated Edison.

March 21 - Catoosa, OK - 48 year old Erendira Hernandez Gonzalez was killed on the job when struck by an unknown object while working for Mack's S&S Construction Co., Inc.

March 21 - Boston, MA - 57 year old David Tito Nyombi of Uganda, was killed on the job when he fell down an elevator shaft. He was employed by Dolly DYK Cleaning Corporation.

March 22 - Hannibal, OH - 39 year old Jason Putnam died on the job after being struck by an unknown object.  He was employed by Graham County Land Company.

March 23 - Toms River, NJ - 41 year old Carlos Avilis was an employee for AJH Enterprises LLC when he fell from an unknown elevation, causing his death.

March 23 - Paragould, AR - 41 year old Lori L. Turner died after she suffered an electric shock while employed with Anchor Packaging, Inc., a food packaging company. Details of how she received the shock is not known.

March 24 - San Juan, Puerto Rico - 60 year old Arnaldo Rosa was killed on the job after being struck by an unknown object causing blunt force trauma. He was employed by Horizon Lines of Puerto Rico.

March 24 - Borden, IN - 59 year old Kenneth W. "Kenrod" McKinley of Crystal River, FL was working for Bryan Stumler Excavating Inc. cutting trees for a roadway project when he collapsed for an unknown reason. 

March 25 - Crystal Springs, MS - 39 year old Garvin Barnes was a flagman for Dickerson and Bowen, Inc. working in a work zone near Highway 51 when he was struck by a motorist. One driver rear-ended a car, pushing it into Barnes. The company was a subcontractor for MDOT.

March 26 - Diamond, OH - 50 year old Jon K. Blystone of Youngstown, died on the job after his clothes caught fire as he painted a restroom. It is unclear how his clothes caught fire, but he ran out of the room engulfed in flames. He died later at an area hospital. He was employed by Green Acres Lake Park Resort.

March 28 - League City, TX - 61 year old Barry Benson of Texas City died on the job after he took a fall in an unknown incident. He was employed by Plant Process Equipment.

March 31 - Baton Rouge, LA - 68 year old Joey Rizzo suffered blunt force trauma when he was struck by an unknown object while working for The Home Depot USA, Inc.

March 31 - Coopersburg, PA - 60 year old Neil Horner of Quakertown died on the job after being struck by an unknown object. He was employed by Coopersburg Bagging Company.

September 15, 2014 - Bolingbroke, GA - 24 year old Deputy Michael Norris was fatally shot and another deputy injured as they were attacked by a man with a gun. They were responding to a suicide call at the man's home. Norris was a member of the Monroe County Sheriff's department.

October 1, 2014 - Boonton, NJ - 39 year old Selvin Zelaya and 46 year old Oscar Portillo were killed on the job when they were buried in a trench collapse as they were installing a drain pipe. They were employed by Bednar Landscape Services.

October 2 - Rebecca, GA - 61 year old Isabel Gomez died on the job when he was struck by the falling arms of a front end loader. He was working with his brother on the hydraulic system of the machine when the arms suddenly fell. He was employed by WBT Farm Service. 

October 4 - St. Louis, MO - 33 year old Rolan Mask of Edwardsville, died after being struck by a box truck that backed into him while working for Antler Trucking Co.

October 6 - Chazy, NY - 36 year old Philip Fortsch III of Champlain was killed on the job when a farm tractor he was operating crashed and overturned into a river. He was employed by Rovers Farms in Chazy.

October 6 - Brookhaven, MS - 22 year old Chase D. Curtice was working on an oil rig when he was struck and killed by a pipe. He was employed by Loomis International for Denbury Resources.

October 6 - Tualatin, OR - An unidentied 27 year old worker was killed on the job when he was crushed under heavy metal plates that fell from a forklift. Repeated attempts to locate his name have been unsuccessful. He was employed by Pro Saw.

October 7 - West Point, GA - 57 year old John Edd Dunnivant of Lanett, AL was fatally injured in an unknown incident in the steel press section of Kia Motors Manufacturing plant in West Point.

October 7 - Correctionville, IA - 49 year old Daniel Lewis Uhl of Anthon was pinned by a large piece of earth moving equipment he was operating on a construction site in Correctionville. The name of his employer is unknown.

October 7 - Sparks, MD - 64 year old Michael Ray Mathews died of injuries he received on the job a few days before. He was using a roller compactor machine to flatten out a pile of dirt, when the pile collapsed causing the machine to rolled. He was pinned by the machine. It is not known who he was working for.

October 7 - Chehalis, WA - Zach Whitty was working his normal job for B & M Logging Inc. when he passed out and later died at a hospital of an asthma attack.

October 7 - Yosemite, CA - 62 year old Geoffrey "Craig" Hunt of San Jose, was flying an fire retardant plane helping to fight the Dog Fight Fire in Yosemite National Park when the plane crashed into a canyon wall. He was employed by CynCorp International for Cal Fire.

October 8 - Hicksville, NY - 56 year old David Zimmerly of Lindenhurst was working to make repairs under a postal truck in a shop when a co-worker got into the truck and began to drive away, crushing Zimmerly. He was employed by the United States Postal Service.

October 8 - Douglas, AZ - An unidentified landscaper was killed and his co-worker injured when they were attacked by thousands of bees that were disturbed at a private residence. Repeated attempts to locate his name have been unsuccessful.

October 9 - Ogden, PA - An unidentified man died on the job when he fell from an elevated bucket while working for Lenni Electric. His name has not been located.

October 10 - Baltimore, MD - An unidentified construction worker was killed on the job when he was struck by equipment that fell onto him. Repeated attempts to locate his name and employer have been unsuccessful.

October 10 - Post Falls, ID - 18 year old Jeremy McSpadden Jr. of Spokane Valley, WA was an actor for the Incredible Corn Maze in Post Falls when he was killed on the job. He was a role player in the “Zombie Slayer Paintball Bus” attraction at the maze and was running to the back of a bus when he slipped and fell under the bus.

October 12 - East Orange, NJ - 34 year old Zoungo Sou-Oud was working his normal job at Alexander's Car Wash when he was caught in the cross-fire of a shooting and was shot. He was not the intended target, rather an innocent person.

October 13 - Williston, ND - 28 year old Dustin Payne of Hazel Green, AL was working on a tank October 3rd when an explosion occurred. He died 10 days later of those injuries. He was working in a North Dakota oil field for Nabors Completion & Production Services.

October 13 - Carrick, PA - 29 year old Omar Hodges had just arrived at his work, still sitting in his vehicle, when he was shot to death for an unknown reason. He was employed by Pittsburg Environmental Services Bureau.

October 14 - Jamestown, PA - 65 year old Thomas Thomas of Kinsman, OH, was killed on the job when he was caught in the power take off of a machine as he was feeding cattle. He was working on a farm near Jamestown.

October 14 - Maumee, OH - 40 year old Brad Griffitt of Holland was in his construction vehicle when it was struck by a tractor trailer semi truck on September 19th. He was in a coma until his death on October 14th. He was employed by DG Slurry-Seal of Toledo, Inc. 

October 14 - Erwin, TX - 51 year old Gary Reedy died of injuries he received after falling about 25 feet into a hole at Energy Solutions facility. He was also exposed to radioactive material but that is not what caused his death. He was employed by Erwin Resin Solutions.  

October 15 - Bristol, VA - 50 year old Gary Bowers was a temporary worker at Helm's Candy plant when he died on the job in an unknown incident. His employer was At Work Personnel. 

October 15 - Sundown, TX - An unidentified worker was fatally electrocuted while working at Sundown Independent School District. He was a subcontractor working for K-Bar Texas Electric.  Attempts to locate his name have been unsuccessful.

October 16 - West Carrollton, OH - 33 year old Geoffrey S. Garnett was working as a welder with others to replace the roof of an electrical transformer substation when he was electrocuted. He was employed by Cohen Recycling which is owned by Metal Shredders.

October 17 - Greeley, CO - 46 year old Nicholas Arrendondo-Urbibe was part of a road construction crew working on a flood control project when a construction truck backed up over him. He was employed by Duran Excavating at the time of his death.

October 18 - Middletown, DE - 66 year old George R. Lynam, Jr. was killed on the job when he was struck by a broken tow hook. Lynam was operating a tractor trailer fully loaded with corn when it became stuck. Another worker was attempting to pull the semi free when the tow hook broke loose, crashing into the side window and hitting Lynam. They were employed by Baker Farms. 

October 21 - Columbia, TN - 22 year old Gordon Schaffer was working alone in the Papa John's restaurant where he was employed when he was shot on October 20th in a robbery. 

October 21 - Louisville, KY - 22 year old Tomi Leigh Batts was operating a forklift when it malfunctioned, causing her death from an unknown cause. She was employed by AAA Cooper Transportation.

 October 22 - Florence, OR - 52 year old Mark Fortune was checking equipment at the end of his work day when he somehow backed a work vehicle off the McCoulough Bridge platform, falling into the river below. He was a mechanic for Great Western Corporation.

October 22 - El Paso, TX - Two unidentified workers died of drowning as they performed maintenance on a pump at the Robert Bustamante Wastewater Treatment plant for El Paso Water Utilities. At some point a pit area started to fill with water and one of the workers entered the space to turn off a valve but was unable to escape. The second worker also entered the space to help but could not get out. They were employed by Cubic Water LLC.  Repeated attempts to locate the name of the workers has been unsuccessful. 

October 24 - Sacramento, CA - 42 year old Danny Oliver, a Sacramento Sheriff deputy, and 42 year old Michael David Davis, Jr., a Placer County Sheriff deputy, were both shot to death during a shooting rampage. Deputy Oliver was shot by the suspect who was sitting inside a vehicle in one location. After fleeing the scene and hijacking a truck the suspect made his way to a home where he shot Deputy Davis as he approached the truck.

October 24 - Memphis, TN - 21 year old Zachary "Zach" Wayne Holliday of Hernando, MS was working alongside his father on the Sears Crosstown Building that was undergoing renovations in Memphis. Co-workers became worried after seeing Zach's vehicle in the lot after hours so his father and their boss started looking for him, finding that he had fallen down an unprotected old packaging chute.  They were employed by Chandler Demolition Company.

October 24 - Reily, MI - 25 year old Florian Nilaj of Farmington, MI and 40 year old Gazmend Vukaj of Novi, MI died on the job while working on a water tower for Southwest Regional Water District in Reily Township. Both men fell from scaffolding as they were trying to fix some cabling and it broke. Neither were wearing safety harnesses. Nalaj died at the scene and Vukaj later at hospital. They were employed by V & T Painting LLC.

October 25 - Auburn, WA - Jeffery Syverson was the owner of Prime Air Services in Auburn was found dead without any signs of injuries. The circumstances of the incident is not known.

October 26 - Greenville, SC - 60 year old James "Fly" Irby was a commercial painter for many, many years. He was working with a crew subcontracted to do work at an apartment complex. He was on the roof, securing a ladder, when he apparently lost his balance and fell off backwards. It is not known who he was employed by.

October 27 - Seattle, WA - Timothy Kern was the owner of Fundamental Coffee Company LLC, an eating establishment, and was found in the break room deceased. It was determined he died of natural causes.

October 28 - Statesboro, GA - Salvador Hernandez was working to fix a piece of machinery at Claude Howard Lumber Mill when he was somehow pinned by the machine. The circumstances of the incident are not known. 

October 29 - Grosse Pointe Woods, MI - 59 year old Leland Rumph was partially buried in a trench collapse on a construction site. A secondary collapse occurred when a police officer jumped in the trench attempting a rescue. The officer was pulled free but Rumph was buried to his neck. He was owner of Rumph Construction.

October 30, 2014 - Hutchinson, KS - 29 year old Joshua Tacy was working to help load metal on a rail car for Midwest Iron & Metal, a recycling company, when he was struck by the machine being used to load the scrap.

October 30, 2014 - Wallula, WA - James Pugh of Smelterville, ID was driving his Excel Transport, Inc. semi truck when he suffered a cardiac event and crashed the truck into a utility pole. His death was ruled natural causes. 

October 31, 2014 - Boston, GA - 32 year old Victor Chavez was working to unload a large container of cotton seeds, when he fell and was buried under the seeds. He was unable to free himself. He was working for Boston Gin Company.

November 1, 2014 - Chicago, IL - 44 year old Roberto Topia-Gonzales was working for an unlicensed contractor without a building permit on a construction project when he was crushed and buried under a brick wall that collapsed.

November 1 - Muskegon, MI - 54 year old Michael Yell was working for Continental Dairy when he fell from a moving semi truck trailer and was run over. He was helping to unload the truck and climbed onto the passenger side of the truck when he fell, unseen, by the driver and was struck by the trailer.

November 1 - El Dorado Springs, MO - 25 year old Deputy Matt Chism was making a routine stop for the Cedar County Sheriff Department when he was shot during an altercation with the person he stopped.

November 3 - New Orleans, LA - 57 year old Edward Burton was working for Traffic Solutions on the Lake Ponchartrain Causeway when his work vehicle was struck from behind by garbage truck and pushed into the river below.

November 3 - Guntersville, AL - 41 year old Stephen Paul Woods of Albertville was struck in the head by a pile driver as he worked behind a private residence. He had just completed a pier to a boat house and was moving a platform to another location when the incident happened. He was employed by Greg Weaver Construction of Guntersville.

November 3 - Jersey City, NJ - 58 year old Gary Anderson of Somerdale was making a delivery of drywall to a construction site for George Hildebrandt Inc., who he was a contractor for, when a one pound tape measure fell from a worker's tool belt from the 50th story on the construction site. It hit a piece of equipment on its way down and then struck him in the head. He worked for an unnamed New York based trucking company.

November 3 - Charleston, WV - An unidentified lineman for Virginia based David H. Elliot, working at the Murray Energy McElroy Mine was fatally electrocuted while working to replace copper wire that had been stolen from an electrical line.

November 4 - Lapeer, MI - 34 year old Bradley Davis of Millington was working at the Lapeer Industries machine shop when an over-pressurized tank exploded. The specific circumstances are not clear.

November 4 - York, ME - 50 year old James Woodward was working along the York River Bridge when he was struck by a dump truck driven by his co-worker. Woodward was in the blind spot of the dump truck when he was hit. Both worked for Shaw Brothers Construction.

November 4 - Coos Bay, OR - 55 year old Estella Morgan was driving her Bureau of Land Management SUV and came upon a logging operation. As she neared the site a tree that had just been cut fell onto her vehicle, crushing her inside.

November 5 - Decatur, TX - An unidentified man was operating a backhoe during construction at a private residence when the backhoe flipped and he was pinned underneath. No further information is known.

November 6 - Valley Falls, KS - 58 year old Mike Miller was working on the family farm when he fell into a grain bin on the property.

November 7 - Fairfield, OH - 53 year old David Geiger of Cincinnati was performing maintenance at Northwest Bowling Alley, his employer, when his clothing became caught up and tangled in a pin-setting machine causing asphyxia.

November 8 - Tilden, TX - 50 year old Roel Rodriguez was working at Pioneer Natural Resources Eagle Ford oil rig when it caught on fire and exploded on October 24th. He and other workers were severely hurt in the explosion and he died on November 8th. The condition of the other workers is unknown. 

November 9 - Erie, PA - Jacob Schwab was an Erie Metropolitan Transit Authority employee when he was struck in the head while working. He was repairing a bus when something on the bus exploded, striking him in the head.

November 10 - Huntington Station, NY - Julian Castellanos was working for an unnamed construction company on a project in Long Island when he was killed. He had built two retaining walls and was working on the steps when one of the walls collapsed onto him. 

November 11 - Ocean Springs, MS - 42 year old Gerald Moran of Gulf Park Estates fell to his death from a balcony while working construction on an Ocean Springs home. Apparently he was pulling a board from the roof and it slid faster and more easily than expected, causing him to lose his balance. His employer is not known.

November 11 - Phoenix, AZ - An unidentified man was working under a truck when it fell from the jack, crushing him.  He was a private contractor working at Fred Terry's Contracting. Attempts to locate the worker's name has been unsuccessful.

November 11 - New Albany, OH - 23 year old Kevin Harris of Jenison, MI was a diver who was working to clean out a retention pond in New Albany when something went wrong under water and he drowned. An investigation continues to find out what happened to cause the incident.  He was working for the Chicago based company U.S. Aqua Vac, a company specializing in cleaning various bodies of water.

November 11 - Bridgeton, MO - 31 year old Marreo Travis was doing some testing at Ferro Magnetics, a battery charging plant, when he was electrocuted. The circumstances of the incident are not named.

November 12 - Sarahsville, OH - 48 year old Norman Butler of Newport News, VA was working at a fracking oil well site on a pump for Blue Racer Midstream when an explosion and fire occurred and he was killed. He was employed by Buffalo Gap Instrumental & Electrical.

November 12 - Warwick, RI - 45 year old Paul Matos of Fall River, MA was working on a construction project when he was struck by a large piece of equipment. He died at an area hospital. He was working for AGI Construction on the Apponaug Circulation project.

November 12 - Zelienople, PA - 54 year old Paul Reece of Harmony was working from an elevated bucket trimming trees when he fell or was thrown from the bucket to the ground. He was working at English Lutheran Church in Zelienople for American Tree Service.

November 12 - Staten Island, NY - An unidentified employee for Flag Container Service Company was working to unload a dumpster at Richmond County Country Club in Todt Hill when the container fell, crushing him underneath.

November 12 - Everett, WA - Silvestre Mitra was working at Good Samaritan Adult Family Home, a residential care facility, putting away laundry when he went into cardiac arrest. His death was ruled natural causes.

November 13 - Mead, CO - 36 year old Matthew Smith of Brighton was working with two others trying to heat a frozen high-pressure water line when something went wrong, causing the line to rupture. He was struck by a highly pressurized stream of water.  He was employed Halliburton working on the Anadarko Petroleum Corporation's well site.

November 13 - Raleigh, NC - 42 year old Jose Luis Rosales-Nava was part of a crew working on a pedestrian bridge at Wake Technological Community College in Raleigh when the bridge collapsed. He was trapped under part of the frame structure that collapsed while pouring concrete for the bridge.  He was employed by Central Concrete of North Carolina, a subcontractor for Skanska Construction.

November 13 - Avon Lake, OH - Larry "Gomer" Heczko was working construction on an upscale home near Lake Erie when he was killed as a concrete wall collapsed. The name of his employer is not known, but he was also a beloved member of the Avon Lake Fire Department.

November 15 - LaPorte, TX - Four Dupont Chemical Refinery workers were killed when a lethal gas was released and they were asphyxiated. Crystal Wise, a newer employee, called for help after she had gone to check on a line. Robert Tisnado, 38, and Wade Baker, 60, rushed inside the space without masks and were overcome by the toxic gas. After losing radio contact with his three co-worker's, 48 year old Gilbert "Gibby" Tisnado grabbed a mask and ran in to help. He removed his mask and put it on his little brother Robert but he was overcome as well. An investigation continues.

April 1 - Houston, TX - 49 year old Samuel Palacios died on the job due to a fall from a roof. He was employed by Sam's Roofing.

April 1 - Bradley, IL - 35 year old Thomas Morris of Momence was killed on the job after the forklift he was operating overturned and crushed him. He was employed by C and C General Contractors, Inc. (official), working at Kanga Roof in Bradley.

April 1 - Cary, MI - 56 year old Ernest Case died on the job after falling from an unknown elevation. He was employed with Communications Specialists, Inc.

April 1 - Shamrock, TX - 46 year old David K. Holdren, Jr. died from an electric shock while working for Arrow Water Well, Inc.

April 1 - Tampa, FL - 27 year old Nicholas "Nick" Veley died from an electric shock while working for Power Electric Services, Inc.

April 1 - Staten Island, NY - 50 year old Richard Salvia was sent out to make a routine food delivery on the night he was murdered. Three teenage suspects made a plan and called for the bogus delivery with the intent on causing harm to the person who showed up. While trying to rob Salvia, one of the teens shot him to death. Those teens were arrested for Mr. Salvia's horrible death. Salvia was employed by L & W Restaurant, Inc.

April 1 - Kingfisher, OK - 33 year old Brett Montgomery of Elk City was operating a bulldozer when he was struck and killed. He had parked the machine on an embankment and after stepping off the machine, it rolled back into him. He was employed by Galmor's/G&G Steam Services, Inc.

April 2 - Harrington, DE - 55 year old James Lee Henderson, Jr. was working in an empty warehouse, installing electrical service when he was caught between a forklift and a pole. He was employed by AET Electric, Inc.

April 2 - Sherrill, AR - 71 year old Jay Waymon Ball died on the job as the result of being struck against an unknown object. He was employed by England Liquid Fertilizer, Inc.

April 2 - Bullard, TX - 51 year old Tony Collins was an employee for East Texas Utility Services, Inc. when he died on the job from an electric shock.

April 2 - New York, NY - 58 year old Harmit Singh was killed on the job after falling from an unknown elevation while working for Pak National Gen. Corp.

April 3 - Stuttgart, AR - 65 year old James Boehn died on the job when he was struck against an unknown object, suffering a dislocation. He was working for Blue Seal Petroleum of Stuttgart, Inc.

April 3 - Houma, LA - 41 year old Dutch "Shannon" A. Mire of Thibodaux was working for Gulf Island Fabrication when he was struck by an unknown object, suffering fatal head trauma.

April 4 - Midland, TX - 28 year old Gabriel Trejo of Plainview died on the job after being burned when a tank battery, used for chemical storage, suddenly ignited. He was employed with S&S Equipment & Supply, Inc.

April 8 - Chalfont, PA - 61 year old Victor Garcia of Norristown was killed on the job when he was struck by an unknown object while working for Roll Former Corporation.

April 8 - Johnstown, CO - 30 year old Gabriel Hernandez was working for Gopher Excavation, Inc. when he suffered blunt force trauma after being struck by an unknown object and also receiving lacerations.

April 8 - Blytheville, AR - 36 year old Juan Nieves and 37 year old Nicolas Perez were working on a barge owned by GATX Corp. on the Mississippi River when something went very wrong. Nieves and Perez fell into the water when the barge flipped over while unloading about 700,000 pounds of steel coil at a Kinder Morgan terminal near Blytheville. The men worked for Kinder Morgan through an employment agency. Authorities gave up searching for the men and it is not known if they were ever found.

April 9 - Laredo, TX - 38 year old Efrain Gonzalez died on the job after being struck by a backhoe loader machine while working for Union Pumping, LP and City Ready Mix, Inc.

April 9 - Florence, MT - 51 year old George Fifield Jr. of Missoula was seriously injured in a fall from a roof while working construction in Florence. He died at an area hospital. He was not provided fall protection by his employer, Jared Langley Enterprises, Inc.

April 10 - East Moline, IL - 52 year old David Walters of Moline was killed on the job after being struck by an unknown object while working for K&M Equipment, Inc.

April 10 - Lubbock, TX - 43 year old Jose R. Aguirre was working with a co-worker on a wiring system when he was fatally electrocuted. The circumstances are not clear. He was an employee for Park Tower Apartments.

April 10 - Nantucket, MA - 72 year old Henry Varian was a private contractor working on a construction site when he fell to his death. Owner of Henry J. Varian, Inc., he was working from a scaffold when he fell, landing on his back. 

April 11 - Pago Pago, American Samoa - 49 year old Lolenese Tupau was employed as a stevedore when he was struck by an unknown object and suffered fatal head trauma. He was employed by Peter E. Reid Stevedoring, Inc.

April 12 - Andrews, TX - 54 year old Gabino T. Aguilar was at work for Weatherford International Ltd. when he suffered a fatal heart attack.

April 12 - Dalton, GA - 48 year old Jonathan Wheatof Rossville died on the job after falling from an unknown elevation. He was employed by Porter Roofing Contractors, Inc.

April 12 - Mystic, CT - Susan Masina died on the job from an unknown cause while working for Home Health Care Services, LLC.

April 13 - Greenville, TX - 64 year old Larry Daniel died on the job in unknown circumstances while working for Electric, Inc.

April 13 - Mertzon, TX - 29 year old Curtis Lam was killed on the job after being struck by an unknown object. He was employed by Integrity Services, LLC.

April 14 - Manhattan, NY - 35 year old Lukasz P. Stolarski of Brooklyn was killed on the job after falling from a building.  He and other workers were completing facade restoration work. Stolarski was standing on a ledge between the roof and penthouse level when he fell. Officials say he was not wearing a harness. He was employed by First Astoria/Bas Renovation JV. 

April 14 - Albany, NY - 39 year old Frank Mack of Ballston Spa was killed on the job after he fell from an unknown elevation while employed with Dwight Plumbing and Heating, Inc.

April 14 - Perry, OK - 24 year old Montana Camp died on the job after being struck by an unknown object. He was employed by LaMunyon Drilling, LLC and Dorado E&P Partners LLC.

April 14 - Washington, PA - 60 year old James Zablotny was killed on the job after he fell from an unknown elevation, suffering body fractures. He was employed by Casper Construction at the time of his death.

April 14 - Plainville, MA - 69 year old Robert MacDow died on the job when he tripped and fell (same level) while working at Super Stop and Shop. No other details are known.

April 15 - Pennington, GA - 34 year old Samuel E. Southerland, Jr. of Mobile, AL was killed on the job when he fell from an unknown elevation while employed by S & S Sprinkler Company, LLC.

April 16 - Orlando, FL - 43 year old Jose Nambo Munoz was employed by MAG Construction Group LLC when he died on the job after falling from an unknown elevation.

April 17 - Fulton, MI - 21 year old William McCalpin was killed on the job from unknown injuries received while working for Graham Lumber Company.

April 17 - Bedford, TX - 27 year old Jose Dimas died on the job after falling from an unknown elevation. He was employed by Juan Garcia.

April 17 - Norristown, PA - 41 year old Michael Robinson of Coopersburg was working on the roof of a building at Norristown State Hospital when he fell to his death. He lost his footing and fell three stories. He was employed by Banes Roofing, Inc. 

April 17 - Monroe Township, NJ - 55 year old Edmund Moore III was working at Stencel & Son, Inc. when he suffered a fatal cardiac event.

April 17 - Mesquite, TX - 63 year old Melecio Medina was employed by Madrid Drywall LLC when he died on the job from a fall from an unknown elevation.

April 20 - Rankin, TX - 31 year old Kell de Carlo Mendoza died on the job after being caught in or between an unknown object. He was employed by Sidewinder Drilling.

April 21 - Lakewood Ranch, FL - 33 year old Artemio Valdez was killed on the job after he suffered head trauma in a fall from an unknown elevation.  He was employed by Crown Roofing, LLC.

April 21 - Frostproof, FL - 51 year old Gary Allen was at his job working for F.G. Gamez, Inc. when he suffered a fatal heart attack.

April 21 - South Padre Island, TX - 37 year old Roberto Mendez died on the job after falling from an unknown elevation, suffering head injuries. He was employed by Isla Restoration.

April 22 - Hollywood, FL - 55 year old Fernando Cuellar died of unknown injuries while working for Liliana Albrecht, LLC.

April 23 - Paterson, NJ - 65 year old Jose Maletski was working his shift at Paterson Pickle Company when he suffered a fatal heart attack.

April 23 - Montgomery, AL - 35 year old William Thomas suffered a fatal heart attack while at work for Global Auto Processing Services, LLC.

April 24 - Proctorville, OH - 44 year old Bradley D. Jenkins of Otway was working on a drill bit by a bridge when the ground slipped and he fell into a 10 foot hole filled with water, and was covered by rock and dirt debris. He was employed by Complete General Construction Co.

April 28 - Hialeah Gardens, FL - 49 year old Reinaldo Rodriguez died on the job when he was struck by an unknown object. He was employed by Milan Manufacturing, Inc.

April 28 - Tulsa, OK - 46 year old Ronnie Blagg and 48 year old Michael Turner were both working on a traffic light when a motorist lost control of their vehicle and slammed into the men. Both worked for Traffic & Lighting Systems, LLC.

April 28 - Coronado, CA - 29 year old Coll Perske had five years of experience working with the Navy's dolphins and sea lions in San Diego Bay. During a training exercise for the marine mammal program, he did not resurface and his co-workers rushed to help him. He died of salt water drowning. He was employed by Science Applications International Corp.

April 29 - Houston, TX - 35 year old Timoteo Ramirez died on the job after sustaining head injuries during a fall from an unknown elevation. He was employed by Womble Company, Inc.

April 29 - Gloucester City, NJ - 53 year old Clayton J. Benson of National Park was killed on the job in a shipyard incident on the Delaware River.  He was struck by a two ton shipping tray while at the shipping dock at Holt Logistics. He was employed by Gloucester Terminals LLC.

April 30 - Caldwell, ID - 53 year old Roy Frazier was killed on the job when was crushed under a steel beam. He was moving the 50,000 pound steel beam with a forklift when it fell from the lift, pinning him under it.  He was employed by Rule Steel Tanks, Inc.

November 12, 2014 - Pittsburg, PA - 34 year old Nicole Kotchey of Ross Township was discovered on the floor near her desk at the University of Pittsburg Medical Center.  She died several hours later and it has been determined that she died of cyanide poisoning. A police investigation is underway to determine how she was exposed.

November 13 - Seattle, WA - Gaetano Arico was an employee for Sound Window & Gutter Cleaning when he fell to his death from a ladder. He was cleaning gutters on a two-story home and was discovered on the ground by the homeowner. He died at an area hospital.

November 17 - Merrill, WI - 55 year old Marvin C. Ament of Mosinee died on the job as he worked to install insulation on a piece of equipment at Expera Specialty Solutions of Merrill. He fell from an elevated bucket lift.  He was employed by Taylor Insulation of Mosinee.

November 18 - Bellville, OH -  32 year old Marvin Troyner of Holmesville was working inside a trench on a sewer project when he was trapped by a collapse. He was inside a steel safety box when a boulder came crashing down, hitting the box and trapping him inside. He was employed by Shrock Premier Custom Construction, Inc.

November 19 - Sumas, WA - Mohinder Singh of Nooksack had been off sick from work and collapsed after reporting to work the next day. It was determined he died of a non work related natural death. He was working for Teal Jones Lumber Services, Inc.

November 19 - Horizon City, TX - 40 year old Javier Arteaga Loya died on the job after he was pinned under a trailer that fell off the jack stand. He was a part time employee for S&S Truck Sales.

November 20 - Noblesville, IN - 21 year old Anthony J. Brock was killed on the job when he was crushed between a steel catwalk structure and the railing of an aerial lift. He was working for ID Castings produces iron castings commonly used in the automotive construction and heavy equipment industries. His first child was due in January 2015.

November 20 - New Orleans, LA - 24 year old Jerrel Hancock of Abbeville was killed on the job and a co-worker injured when an explosion occurred on an off shore oil platform.  They were cleaning a piece of equipment during routine maintenance at Fieldwood Energy's Echo oil-and-gas platform in the Gulf of Mexico. He was employed by the Louisiana based Turnkey Cleaning Services.

November 20 - Baytown, TX - 53 year old Walter Warner of Sandy Hook, MS was a subcontractor working for Group when he was involved in a fatal incident at Exxon Mobil's Olefins plant. Details of the cause are not given but there was an expansion project going on at the plant.

November 21 - Waverly, NE - 34 year old Joaquin Danilo Mina Munoz of Lincoln was attempting to clean a grain truck auger when he was killed on the job. He was working for Bevan's Enterprises, Inc., a turkey farm, when his clothing became caught in the auger and he was pulled in. 

November 24 - Forks, WA - 25 year old Nicolas Rodriguez was working at a logging site and standing near an airlift helicopter that was prevented from landing due to weather conditions. He was struck in the head by a swinging lob. He was employed by Engeseth Logging. 

November 28 - Staten Island, NY - 43 year old Delfino Jesus Valazquez Mendizabal was killed on the job when he was buried under the rubble of a roof collapse on a demolition site. He was working for Formica Construction, who did not have proper permits, to demo an old Ford Dealership when the concrete roof collapsed

November 28 - Peabody, MA - 26 year old Joshua Black of Wilmington was working at Waste Support Services when he was struck by a front end loader. Details are not clear but reports say he was lifting up a manhole cover when he was struck. He was an employee for Wayne's Drains.

May 1, 2014 - Makawao, HI - 29 year old Patricia Rabellizsa of Kihei was killed on the job when she fell from a zip line while working for Piiholo Ranch Zipline.

May 2 - Robesonia, PA - 30 year old Benjamin Joe Mast died on the job after being caught in or between an unknown object while working for Z-Weldco, Inc. and Reading Alloys, Inc.

May 2 - Hoover, AL - 59 year old Randy Keith Berry of Cullman was part of a construction crew on an AL DOT repaving project on Interstate 459 when he was struck by a motorist. He was employed by M and W Logistics.

May 2 - Keller, TX - 57 year old Roberto Lariz-Trinidad of Fort Worth was part of a landscaping crew working an apartment complex when he was struck by a truck that was backing up and apparently did not see him. He was employed by F&S Concrete.

May 2 - Rockaway Beach, NY - 65 year old Juan Rodriguez was working for Bryan's Auto East Ltd when he fell from an unknown elevation causing fractures.

May 3 - Buhl, ID - 51 year old Ayala Rigoberto was killed on the job when he was caught in or between an unknown object while working for Eagle View Farms LLC.

May 4 - Port Angeles, WA - 24 year old Samuel Silverstein was a commercial 'geoduck' harvester and was diving for the mollusks when his diving partner found him unconscious in about 50 feet of water. He was transported to hospital but died there. He was employed by Furthur Diving Co.

May 5 - Ursina, PA - 31 year old Jared Ritchey was working to install a concrete manhole form for the city's first ever sewer system when he was pinned beneath the form. He was working for Guyer Brothers, Inc. 

May 5 - Wrightstown, NJ - Marc Cooper was working for Bristol Protective Services when he died in unknown circumstances from inhaling a toxic substance. No further information can be found.

May 5 - Marmarth, ND - 48 year old Darwin Maxwell was working in Marmarth for Continental Resources, Inc and 4J Well Services, Inc. when he died of a heart attack. Circumstances are not known.

May 6 - Ardmore, OK - 31 year old Christopher Cryer died on the job after falling from the roof of an Ardmore plant he was working at. He suffered major head trauma in the fall. He was employed by Atlas Roofing Corporation.

May 6 - Elkhart Lake, WI - 55 year old Jay Brandt was working for Bremer Manufacturing Co. when he died of a sudden cardiac event.

May 7 - Cape Girardeau, MO - 67 year old Sandra K. Christy was working for Regions Bank when she died from a same level fall in unknown circumstances.

May 7 - McCool Junction, NE - 73 year old Marvin Roesti was injured on May 7 when he fellwhile loading a tanker truck at the Farmers Cooperative McCool Junction fertilizer plant. He was working from a height without fall protection. 

May 8 - Mobile, AL - 25 year old David Sells died on the job after being struck by an unknown object while working for Infirmary Health System, Inc.

May 10 - Houston, TX - 61 year old Gabriel Garcia was a longshoreman was helping to load a piece of equipment at a terminal facility when it became disconnected from the traverse that was holding it. The cargo dropped, landing on and severing Garcia's legs. He was an employee for Gulf Stream Marine.  

May 10 - Countyline, OK - 50 year old Robert Dulworth of Duncan was killed on the job when he was crushed under his work truck. He stopped to exit his truck but did not completely set the brake. As the truck began to roll, he tried to stop it but fell and was run over. He was employed by Citation Oil & Gas Corp.

May 11 - El Paso, TX - 39 year old James E. Green of Runaway Bay and 54 year old Jimmy Joe Broadstreet of Chaparral were Crane Service, Inc. contract workers for Texas DOT when they were both struck by a motorist on Interstate 10. Green died on the job and Broadstreet later at a nearby hospital. 

May 11 - Toughkenamon, PA - 64 year old Duane Allen Canipe of Landenberg died on the job after falling from an unknown height, suffering head trauma, in an unknown incident. He was employed by Roman Urethane Corp.

May 12 - McDonough, GA - 41 year old Adonis Javier Maldonado died on the job after falling from an unknown elevation while working for Elite Storage Equipment Erectors, Inc.

May 13 - Mountain Brook, AL - 19 year old Trayshawn Presha of Sarasota, FL was working with a group of movers using a dolly to move a safe into a residence in Mountain Brook when he was crushed by it. The ramp they were using was not long enough so they added a second unsecured ramp extension which slipped, causing the safe to fall. Presha was unable to get out of its way. The workers were employed by Daniel Tatum, a professional van operator for Atlas Van Lines.

May 14 - Caddo, TX - 33 year old Jacob Jackson was killed on the job after he was caught in or between an unknown object. He was employed by Rob Joe Materials, LLC.

May 14 - Washington, DC - 34 year old Christopher Bartholomew died on the job from an electric shock in an unknown incident while employed with Arrow Electrical Solutions, LLC.

May 15 - Newfield, NY - 60 year old Grover Lehman was working for Jim Ray Homes, Inc. when he was killed on the job. He and co-workers were preparing a mobile home to be moved and had it on jacks. They did not, however, have temporary support blocking and when the unit shifted, Lehman was struck in the head. 

May 15 - North Venice, FL - 35 year old Carlos Ochoa died on the job after falling from an unknown height while working for All Steel Consultants, Inc.

It is never easy to deal with a work related death of a loved one, but especially in the time of holiday's. Our thoughts are with all of the families of these workers.

December 1, 2014 - Baltimore, MD - An unidentified worker for Hi-Way Paving was working at the BWI Airport when he was struck by a concrete paving machine.  Repeated attempts to locate his name have been unsuccessful.

December 1 - Quincy, WA - Caesar Salazar was driving a work vehicle hauling a Genie lift when he lost control on an icy road. He attempted to jump out but was pinned under the vehicle. He was employed by WM Construction, Inc.

December 1 - Fountain City, WI - 21 year old Brooke Baures was found dead inside a food elevator/dumbwaiter inside the restaurant she worked at by a co-worker. Details are not clear as to how or why she was in the elevator. She was employed by WingDam Saloon & Grill.

December 2 - Ocean Isle Beach, NC - 46 year old Jose Morales of Calabash died of drowninghttp://www.wncn.com/story/27527731/worker-dies-at-nc-golf-course-after-driving-into-water after the vehicle he was driving went into water.  He was driving the maintenance equipment at Tiger's Eye Golf Course but it is not clear why he ended up in the water.

December 2 - Bloomfield, MO - 54 year old Kenneth Wayne Hurley died on the job when he was buried by grain in a bin. He was inside the grain bin running a vacuum when the grain beneath him collapsed and he became buried. He was working at a farm near Idalia owned by Gilbert Gilooly.

December 4 - North Coventry, PA - 60 year old Lee A. Boyer of Liberty was killed on the job when he was struck by a motorist while setting up construction cones while working for J.D. Eckman, Inc. of Atglen, PA.

December 4 - Michigan's Thumb, MI - 21 year old Jonathan C. Hayden of Kalkaska was a road construction worker and flagman who was standing near a flashing arrow sign trailer when he was struck by a motorist. He died later at a hospital. The name of his employer is not known.

December 6 - Houston, TX - An unidentified employee was fatally electrocuted in unknown circumstances while working for Center Point Energy. Attempts to locate his name have been unsuccessful.

December 6 - Shreveport, LA - 23 year old Se'Destini Fields was a campaign worker for Mary Landrieu. She was out and about reaching out to people on behalf of Landrieu when a driver veered off the roadway, striking her. The person driving the truck was apprehended but no charges as of this writing.

December 7 - Everett, WA - Kenneth Otto was injured on November 13th when he was struck by an airbag inflater. He was a technician for Jamco America, a Boeing supplier, and was installing a passenger seat airbag inflater when it suddenly discharged, striking him violently in the face.

December 8 - Henderson, NV - 42 year old Scott Greenfield was killed on the job after he was struck by a piece of machinery. He was struck in the head by a falling injection machine carrier while working for Berry Plastics.

December 9 - Philadelphia, PA - Joyce Craig Lewis died on the job while fighting a house fire. She was a firefighter for the Philadelphia Fire Department when she died after being trapped in the basement of a burning row house.

December 9 - Savannah, TN - Gus Losleben was driving a tanker truck carrying 2,000 pounds of water when it crossed the center divide hitting a log truck head on.  He was an employee for Hardin County Fire Department.

December 9 - Savannah, TN - 30 year old Christopher Durham of Booneville, MS was driving a log truck for an unknown company when a fire department tanker truck driver crossed the center divide, crashing into his truck head on.

December 9 - Cottage Grove, OR - 31 year old Justin Craig Simons was killed on the job when he was pinned by a piece of machinery while working for Weyerhaeuser Mill in Cottage Grove.

December 10 - Honolulu, HI - 42 year old Joefrey Andrada and 30 year old Justin Saragosa were working in Pearl Harbor on a barge using a crane to replace a 10,000 pound buoy when when it fell. The men were not able to get out of the way fast enough. Both were employed by Healy Tibbitts Builders.

December 11 - Dallas, TX - 36 year old Oscar Esparza-Roma, 43 year old Leroy Carriloo-Solorzano and his uncle, 60 year old Nicacio Carrillo-Martinez were all working in the basement of Thanksgiving Tower in Dallas. They were cleaning four water tanks in the basement and were using a torch to cut rusted and corroded parts from a 30-foot-deep tank when it caught fire. They were discovered dead of smoke inhalation after the fire was extinguished. All worked for Best Mechanical, Inc.

December 13 - Fort Worth, TX - Eric Kent Peterson was a clerk for the Military Gun Store when a man entered the business and shot him. It is unclear why he was shot. The business is a gun supply store.

December 15 - Memphis, TN - 35 year old Jason Webb was injured on November 25th during an explosion and fire while working to fix a gas leak in a residential area but died of his injuries. He was an employee for Memphis Light Gas and Water (MLGW).

December 16 - Escorse, MI - 32 year old Antonino Palazzolo of Rockwood was killed on the job during a blast at US Steel's Great Lakes Works in Escourse. A malfunction caused a spill of molten steel, which then melted into snow causing a flash freeze and explosion. When the steel exploded, the building was hit with a barrage of shrapnel.

Today's Weekly Toll will focus on those workers we lost in May, 2014, that we were previously not aware of (continuing from an earlier post). We are grateful to partners who appreciate our goal of honoring all workers that have lost their lives on the job and sharing information with us. May 16, 2014 - Oakdale, NY - 26 year old Christopher Conklin died on the job after falling from an unknown elevation while working for Optimum.

May 16 - Clewiston, FL - 54 year old Lester Singletary was killed on the job after he was struck by an unknown object while working for Cliff's Trucking, Inc.

May 17 - Austin, TX - 24 year old Manuel Gonzales was working for Axiom Services, LLC when he died on the job after falling from an unknown elevation.

May 19 - Chicago Heights, IL - 33 year old Kevin Bewley of Homewood suffered severe head trauma after being struck in the head by a bundle of steel tubing he was attempting to move. He was working at PTC Alliance for Mid-West Manufacturing, LLC.

May 19 - Saginaw, TX - 67 year old Jack Wilson died on the job after being struck by an unknown object while working for Norco Corporation.

May 20 - Versailles, OH - 37 year old Allyson Mescher was working for Codino's Limited, Inc., a pasta making company, when she became trapped in a piece of machinery. 

May 20 - Mounds, IL - 27 year old Antonio L. Pratt died of a cardiac event while working for Environmental Remediation & Recovery.

May 20 - Decatur, TX - 67 year old Jack Wilson of McGregor died from injuries he suffered at work when he was involved in a forklift incident. He was pinned between two machines. He was employed by Landmark Fabrication.

May 21 - Ft. Lauderdale, FL - 67 year old William Slaugenhaupt was killed on the job when he fell from an unknown elevation, suffering head trauma. He was employed by Charland Construction, Inc.

May 22 - Washington, DC - 71 year old Walter Fitzgerald was killed on the job after being struck by an unknown object while working for Contact Construction, LLC.

May 22 - Stillwater, OK - 22 year old Cindy Long was working for Quapaw Co. when she died after falling from an unknown elevation.

May 23 - Chicago, IL - 35 year old David Vargas was killed on the after he was struck in the head by an unknown object. He was employed by International Erectors, Inc.

May 24 - Harlingen, TX - 35 year old Carlos Juan Reyes of Brownsville died on the job after falling from a platform. He was employed by Angel's AAA Electric, LLC.

May 27 - Chicago, IL - 31 year old Nathan Cottrell of Mount Prospect was killed on the job after he was struck in the head by an unknown object. He was employed by Alpha Baking Co., Inc.

May 28 - Gonzales, LA - 56 year old Danny Deville of Melville died on the job when he drowned during a flooding incident. He was leaving work as directed due to rising flood waters and was later found underneath his truck by his supervisor who noticed his truck still in the parking lot. It is presumed that he fell and possibly struck his head on the ground. He was employed by Pax, Inc.

May 28 - Philadelphia, PA - 31 year old Jose Santiago Vasquez Acevedo was killed on the job when he suffered an electric shock while working for Espana Brothers Corporation.

May 30 - Pontotoc, MS - 50 year old Bobby Williams of Blue Springs, MO was killed on the job as he was working at Three Rivers Landfill. A tractor trailer semi backed into him. He was employed by Waste Management of North Mississippi.

May 30 - Wichita, KS - 41 year old Joshua Loux was killed on the job when he was caught in or between a freight elevator as he worked on a construction site. He was an employee for Restoration & Waterproofing Contractors for General Contractor Key Construction.

May 30 - Jacksonville, FL - 26 year old Jose Mendoza-Lopez died on the job when he fell from an unknown elevation while working for IMC Steel Construction, Inc.

May 30 - Gainesville, FL - 39 year old Eric Gillum was working from an elevated bucket truck when the arm of the bucket suddenly collapsed. The bucket fell onto the cab of the truck, which caused Gillum to be thrown onto the sidewalk below. He was an employee for Advance Tree Service.

June 1, 2014 - Stanton, TX - 48 year old Johnny Plasentillo of Midland was working at a West Texas Gas Compressor Station when he was discovered unconscious next to a spewing natural gas line. He was a gas plant operator for J.L. Davis Company.

This post highlights workers who died on the job in December 2014.  Our thoughts are with the families as we know this time of year is particularly difficult.

December 16, 2014 - Hoquiam, WA - 56 year old David James Dupont of Tahuya was killed on the job when he was struck by the top of a tree that was felled. He was the owner of Double D Logging Co., Inc.  Details of the incident are not available.

December 16 - Sturgis, KY - 34 year old Eli Eldridge died on the job after he was struck by a vehicle shuttling coal to the feeder of an underground belt line at Patriot Coal's Highland No. 9 mine.

December 16 - West Monroe, LA - 30 year old Michael Roberts was killed on the job when he was struck by a motorist. A construction pickup that was equipped with a large arrow board was stopped in the north bound outside highway lane. A motorist was traveling in the outside lane and attempted to move into the inside lane but struck the rear of the construction vehicle and three construction workers. It is unclear who he was employed with.

December 19 - Lynnville, IN - R.V. "Rocky" Schmidt died of suffocation after being buried in grain. Authorities say that Schmidt was likely shoveling grain when apparently it gave way. He did have a safety rope on him at the time. Schmidt owned the farm.

December 19 - Coalgate, OK - 26 year old Gary Keehan of Ada and 27 year old Kelsey Bellah of Tulsa were working on an oil rig near Coalgate when a fire broke out causing an explosion, causing their death. A third worker was critically injured. They were employees for Dan D Drilling for Pablo Energy.  

December 20 - New York City, NY - 40 year old Rafael Ramos and 32 year old Wenjian Liu, both officers for the New York Police Department were shot to death during an ambush as they sat in their police cruiser. The gunman was known to have made anti-police statements.

December 20 - Lynden, WA - Rueben Zimmermann was performing his regular duties, bagging customer's groceries at Safeway Store #1930, when he reported not feeling well.  While resting he slumped over and it was determined he died of a heart attack.

December 21 - Snoqualmie Pass, WA - Robert Benson was a long haul truck driver from Colorado who was on a "run" and suffered a heart attack, crashing the truck on Interstate 90 near Snoqualmie Pass.

December 23 - Everett, WA - Michael Petro was an engineer for The Boeing Company and was performing normal office related duties. He was found unresponsive on the floor of a restroom and paramedics were unable to revive him. He died of natural causes.

December 23 - Woodmere, NY - 43 year old Joseph Sanford, Jr. was a member of Inwood (Long Island) Fire Department when he died four days after battling a house fire in where he fell through the floor into the basement and suffered a heart attack. Officials said Sanford was one of the first on the scene after the initial 911 call. It is unclear if other firefighters even knew he was inside, as his lifeless body was found accidentally by other firefighters making their way through the house.

December 24 - Orlando, FL - 52 year old Robert Belvoir died on the job after falling 30 feet after a harness apparently slipped at an elevated ropes course.  He had just gotten hooked up to the Sky Trail ropes course but a piece of the equipment was not properly secured and he fell, while still wearing the harness. He was employed by Jim Liggitt dba Sky Trails attraction.

December 26 - Las Vegas, NV - 58 year old Antonino Isnit was working his regular shift for Walgreen's when an armed man entered the store to rob it. The gunman took Isnit and a coworker to the part of the store with the safe and it was there that he was shot. It turns out, the gunman had previously worked with Isnit and he may have recognized the robber.  

December 26 - Wilmington, DE - 42 year old Jose Luis Garcia Hernandez of Newark, was killed on the job when he was struck by a crawler tractor. He was standing behind the large machine that was driven by a co-worker to move trash into a large pile. It was then that Hernandez was struck by the machine. He was employed by Cherry Island Landfill in Wilmington. 

December 28 - West Palm Beach, FL - Robert Hoffstetter was a corner marshal for the Palm Beach International Raceway when he was struck by an unmanned motorcycle at the Florida Track Days event. A motorcyclist said that during the race, his bike’s throttle got stuck in the open position, preventing him from being able to stop as he approached a turn in the track. He tried to apply the brakes but couldn’t stop so he jumped off the motorcycle into a grass area. The motorcycle continued down the track and struck Hoffstetter.

December 29 - Wenachee, WA - 34 year old Andrea Carlos Johnson was killed on the job when he was struck by a piece of equipment. He was a mechanic for Stemilt Growers LLC and was changing a hydraulic cylinder on a bin destacker. When he disconnected the hydraulic line, the 600 plus pound destacking mechanism came down on top of his upper body, crushing him.

December 29 - Vienna, VA - 45 year old Hicham "AJ" Najdi was killed instantly after being struck by a motorist along an Interstate. He was a tow truck driver for Fairfax Towing and was prepping a disabled vehicle on to his tow truck. The driver of an on-coming box truck crossed into the shoulder where Najdi was working, hitting him.

December 29 - Staten Island, NY - 62 year old Thomas Choi was struck by a motorist on the Verrazano Bridge in October of 2013. The MTA Bridges and Tunnels employee was struck by a car as he worked to reopen the Brooklyn-bound, lower-level roadway of the Verrazano-Narrows Bridge on Oct. 20. He never emerged from a coma.

December 30 - Avalon, CA - 39 year old Timothy Mitchell was a Harbor Patrol Officer on Santa Catalina Island when strong winds and rough waves began to batter boats in the harbor. He is being called a hero for attempting to save a boat from damage. During the night storm, Mitchell was aboard the King Neptune as it was about to run aground at Avalon Harbor. He tried to jump ashore but didn't make it. Two other officers tried to rescue him, but Mitchell became pinned between the boat and the harbor rocks and was killed.

December 30 - Claycomo, MO - An unidentified employed for KCI, Inc. was killed on the job as he worked on renovations to the Ford Kansas City Assembly plant. He was a millwright who worked on machinery and was crushed while working on the job.

December 30 - Sedro Woolley, WA - Richard Rector of Buckley died on the job after being struck by his logging truck. He reported having trouble with the truck's brakes and stopped the loaded log truck on a hill to fix the problem. It is not clear why but the truck started moving down the hill and he was struck by it. He was employed by Tony Van Hoof Trucking LLC. 

December 31 - Grand Terrace, CA - 48 year old Richard Williamson of Riverside was the owner of Bill Will's Security Services and was working a New Year's Eve party when he was shot to death.  He died confronting a gunman firing at a crowd outside the Cal Skate, Southern California roller rink. Two other security guards were injured. Williamson's actions saved many kids and family's lives that night.

January 2, 2015 - Lubbock, TX - 23 year old Rudy Trevino was working his regular job at Lowe's Home Improvement store when he was crushed under a pile of falling lumber. Details are not given as to how the lumber came to fall.

January 3 - Gibbstown, PA - 63 year old Gary Grimming of Bethlehem was killed on the job when he was struck by a high pressure air hose that broke. He was using an air compressor to sand blast a tank at the time the hose broke. He was employed by G.C. Zarnes & Co., working at the Paulsboro Refinery in Gibbstown.

January 3 - Longs, SC - 40 year old Bala Gopal Paruchuri of Myrtle Beach was fatally shot during an armed robbery by several suspects as he worked in the Sunhouse Convenience Store in Longs.

January 3 - Renton, WA - Lorenzo Zurita was struck by a motorist as he was crossing the street to start work at his second job. He had finished one shift at Freddy's Club and was heading to work at Dianond Lil's when he was struck by the vehicle. Both establishments are owned by Club Monaco Gaming LLC.

January 3 - Oklahoma City, OK - 28 year old Mark Pittman was one of several men who were injured in an explosion on December 19, 2014 while working for Dan D. Drilling.  Two other men, reported last time, died in the same incident. A fire of unknown origin broke out on the Pablo Energy drilling rig and he has spent these weeks in an Oklahoma hospital until his death due to extensive burns.

January 5 - Arcata, CA - 64 year old Mike Vander Veen was testing and repairing equipment at the Sierra Pacific Industries mill in Arcata when he was fatally electrocuted. His boom lift apparently came into contact with a high voltage line at the mill. He was found unconscious in the basket.

January 6 - Bel Air, MD - 29 year old Erik Steciak died as the result of injuries received while responding to a call. Steciak, a paid paramedic with the Bel Air fire company who was also with West Friendship Volunteer Fire Department of Howard County, was hit by a utility vehicle while trying to reach a patient who had been injured in an earlier accident. Weather is being blamed as a contributing factor for the incident.

January 6 - El Paso, TX - 63 year old Timothy Fjordbak was a psychologist working at the El Paso Veterans Affairs clinic when an angry patient sought him out and shot him before taking his own life. Apparently the gunman has made a threat against the doctor for an unknown reason and the doctor had filed a complaint against the man.

January 6 - Owensboro, KY - 67 year old Richard "Preacher" Thessing of St. Libory, IL was working as a truck driver and was unloading a metal door frame when it fell onto him. It is not clear who he was working for. Thessing was a retired coal miner and semi-retired truck driver who served as the mayor of St. Libory for 28 years.

January 7 - Paterson, NJ - 53 year old Stanley Rollins died on the job when he was struck and killed by his garbage truck. Rollins' co-worker was driving and backed up into him. Both men worked for Gaeta Recycling Co., Inc. 

January 8 - Harlingen, TX - 58 year old Margarito Alvarez was killed on the job when he was pinned by a piece of machinery. He and two other employees were replacing brakes on a backhoe when something went wrong. The backhoe unexpectedly turned on and went into gear, pinning Alvarez and crushing him to death. The men were employees of Harlingen Irrigation District.

January 8 - Bristol Township, PA - After a four day search, 49 year old Anthony Gabriel was found buried under concrete powder after a giant silo collapsed. Gabriele was found near an outside office trailer next to the collapsed silo with the help of employees that work at the site after firefighters sifted through several thousands tons of cement and used heavy equipment to remove steel beams while braving the possibility of falling overhead objects. He was employed by Riverside Cement.

January 9 - Sacramento, CA - 31 year old Ramon Lopez was working near a market with co-workers and were approached by three young men, one of whom had a gun. An argument ensued and Lopez was shot. It is unclear who the men were working for except that it was a recycling center.

January 9 - Marlin, TX - 29 year old LaCharles Edmond died on the job when he was buried under debris from a barn collapse. Edmond and two other men were in the process of tearing down the barn when the structure collapsed. The second worker was hospitalized and the third was not injured. Their employer is not listed.

January 9 - Manhattan, NY - An inidentified mechanic was killed when an elevator three floors above him unexpectedly dropped inside an Upper West Side high-rise apartment building. He may have been working on top of one elevator car when, for an unknown reason, a second car came down crushing him in between. His employer is not known and repeated attempts to locate his name have been unsuccessful.

January 9 - South Toms River, NJ - 54 year old Adrian A. Perez of Lakewood died on the job when he was trapped in a concrete crusher as he was performing maintenance on it. The machine "was running in an idle capacity" but there was a spinning axle on top of the conveyor belt and his clothing became tangled in that piece. He was working for Suffolk County Contractors.

January 9 - Shawnee, KS - 44 year old John Bieker was part owner, with his wife, of She's A Pistol gun store which caters to women gun owners. He was shot to death during a shoot out with four teens who came into the store to rob it. Three of the four suspects were shot and all were apprehended by police.

January 10 - Moscow, ID - A gunman opened fire at three different locations, killing three different people before he was apprehended and arrested by police. After murdering his adoptive mother he went to Northwest Mutual Life Insurance where he shot and killed 76 year old David Trail, who was a businessman for the insurance company. The gunman then walked into a Taco Bell and asked for the manager. When 47 year old Belinda Niebuhr came to the front of the store, the gunman opened fire on her.

January 10 - Columbia County, GA - Patrick Wilson died on the job when he was crushed under a trailer. He was killed after a trailer slipped and crushed him while he was working a job at Evergreen Tank Solutions in Columbia County. That company, as well as the equipment hauling company, W.H. Reeves, where Wilson worked, is being investigated by OSHA.

January 10 - Oakville, WA - Erin Van Brocklin of Centralia was working with a logging crew when he was killed on the job. He was getting ready to notch a stump when a log above him rolled down, pinning him between two logs. He was employed by B&M Logging Inc.

January 11 - Las Vegas, NV - 46 year old Daniel Wynn Smith died on the job after being suffocated under palm fronds as he was trimming a palm tree. It is not clear who he was employed by. 

January 11 - Wichita Falls, TX - 53 year old Mark Mere and 72 year old Peggye McNair were both killed on the job when they were trampled by camels. Mere was inside a pen with one male and two females camels when the male charged him. As he attempted to get out, McNair attempted to close the gate the male came after them, trampling them. McNair was the owner of Camel Kisses Farm and Mere worked for her.

January 11 - Carlin, NV - 53 year old Brian L. Homer was a 35 year veteran of the mining industry. He died in an accident in a vent shaft at Newmont Mining Corp.'s Leeville mine north of Carlin. He was an employee for Cementation USA Inc.

January 11 - Reno, NV - An unidentified construction worker was struck and killed by a construction loader machine. He was wearing a reflective safety vest. Attempts to locate his name or employer has been unsuccessful.

January 12 - Edwardsville, IL - 31 year old Justin D. Jokerst of Perryville, MO was killed on the job at a construction site. He was working on or near a large crane when he was crushed under the weight of the crane’s boom. Details are not known. He was employed by Taylor Excavators, a subcontractor for Brinkman Constructors.

January 13 - Tate, GA - 28 year old Kevin Voyles was killed on the job when he was crushed under materials from his job at Georgia Marble.  He was working on a machine when about 7,000 pounds of marble fell on top of him.

January 14 - South Los Angeles, CA - 29 year old Andrew "Andy" Ung was working in his family's doughnut shop when a man came in to buy a doughnut. But then the man tried to rob the store and used a knife to stab Ung, who later died. The suspect in Ung's murder has been arrested.

January 14 - Gulfport, MS - 55 year old Craig Mathis died on the job when his work truck slid off the roadway, slamming into a telephone pole. He was an employee for AT&T.

January 14 - Odessa, TX - 53 year old Christopher Davis and 45 year old Eligio Garcia were killed on the job when the prison bus they were on crashed. The Texas Department of Criminal Justice transport bus carrying prisoners from a unit in Abilene to El Paso's Sanchez Unit when the transport bus hit an icy patch on the roadway, went off a bridge and landed on the railway striking a moving train. Eight prisoners also died and five others, including another corrections officer, survived.

Our thanks go out to Ellen Smith, the editor of Mine Safety and Health News who compiled an ongoing list of miners who have died on the job in 2014. We reported a few of the individuals but many on her list we were not aware of. We have opted to post the full list of miners in memoriam.  

January 16 - Davis County, WV - 20 year old Daniel Lambka was a general inside laborer working at Mettiki Coal WV LLC's Mountain View Mine when he became pinned against the rib by a coal feeder in the underground mine.

February 1 - Iron County, UT - 56 year old Mark Rowley was a belt operator who became entangle in a belt conveyor. He was working for the surface iron ore mine, Mountain Lion Mine, operated by Gilbert Development Corp. (Steve L. Gilbert) under contract to CML Metals (Luxor Capital Group LP/Palladon Ventures Ltd).

February 21 - Buchanan County, VA - 24 year old Arthur "D.J." Gelentser III was a continuous miner operator at the underground coal mine, Dominion Coal Corp. (SunCoke Energy Inc.) Mine No. 30 when he became caught between a continuous mining machine and coal rib.

February 21 - Jefferson County, KY - 37 year old Felipe Vizcaya was a laborer who fell into an open elevator shaft while working as an Independent Contractor for Personal Touch Enterprises LLC at a surface cement plant Cemex Inc. / Kosmos Cement Company.

February 28 - Story County, IA -  27 year old Courtney Johnston was a service technician working as an independent contractor for Interstate Powers Systems, Inc., an underground limestone mine, when he fell from a scaling machine under repair. He was working at Martin Marietta Inc.'s Ames Mine. 

February 28 - Suffold County, NY - 50 year old Declan Boland was working for John B. Tintle's East Coast Mines Ltd. when he was engulfed in backfill material at the surface sand and gravel mine. He was a mechanic/safety director.

March 25 - Gibson County, IN - 41 year old Timothy Memmer was a mechanic trainee working at the underground coal mine Gibson Mine when he became pinned between parts of a coal feeder during repair. The mine owner was Gibson County Coal LLC (Alliance Resource Partners LLC).

March 27 - Virginia Beach, VA - 64 year old Robert Devich was a manager for a sand dredging operation owned by John M. Baillio dba Baillio Sand Co., Inc. when he was struck by a 200 foot long pipe at Lord Farm #5.

April 11 - Genevieve County, MO - 53 year old John Hahl was working for Mississippi Lime Company's Sainte Genevieve Mine as a utility/boom operator when he was struck by falling rib rock in the underground limestone mine.

April 11 - Genevieve County, MO - 29 year old Christopher Rawson was working for Mississippi Lime Company's Sainte Genevieve Mine as a utility/boom operator and died when he was struck by falling rib rock in the underground limestone mine.

April 17 - Wabaunsee County, KS - 58 year old Harold Steege was a truck driver working for Hafenstine Construction at Hafenstine Plant #1 when he was struck by an excavator bucket. 

April 18 - Kanawha County, WV - 58 year old Tommy Reynolds was a security guard who drowned in a water filled ditch. He was an independent contractor for Allied Security working at the surface coal facility, Law River Col., LLC (L.W. Hamilton/Warren Hylton), Crown Hill Dock.

April 24 - Pointe Coupee Parish, LA - 50 year old James Gibson III was a dozer operator who was thrown from a bulldozer while working at the surface clay mine, Big River Industries (CRH Plc), Gravelite Division. He was an independent contractor for Craig Neal and Sons LLC. 

April 28 - Elko County, NV - 53 year old Richard Otto was working for Klondex Mine Operations, Inc. (Klondex Mines Ltd), Midas Mine when he died after becoming entangled in drill steel at the underground gold mine.

May 1 - Pershing County, NV - 57 year old William "Mitch" Hill was president and co-owner of the Silver State Minerals LLC Gypsum Mountain Mine. He died after the all terrain vehicle he was on overturned.

May 9 - Gaines County, TX - 20 year old Miguel Nino was working as a groundman when he became entangled in a mobile conveyor at a surface sand and gravel mine. The mine was owned by Mississippi Sand LLC, Seagraves plant. 

May 12 - Boon County, WV - 46 year old Gary Hensley was a roof bolter/mobile roof support operator when he was struck by a coal outburst during retreat mining at the underground coal mine. He was working at the Brody Mine No. 1 (aka Black Stallion Mine) owned by Brody Mining LLC (Patriot Coal Corp). 

May 12 - Boone County, WV - 48 year old Eric Legg was a continuous miner operator when he was struck by a coal outburst during retreat mining at the underground coal mine Brody Mine No. 1 (aka Black Stallion Mine). The mine is owned by Brody Mining LLC (Patriot Coal Corp.)

May 14 - Franklin County, IL - 25 year old William Daniel Payne was killed when he was pinnede between a roof bolting machine and coal rib while working as a roof bolter for the underground coal mine MC #1 mine, aka Foresight Energy's Sugar Camp Complex. M-Class Mining LLC (Coal field Transport, or Coal Field Transports, Inc., exclusive contractor for Foresight Energy).

June 2 - Shoshone County, ID - 36 year old Nicholas Rounds was a shaft repairman at an underground silver mine, Sunshine Mine, when he was caught between an ore car and shaft timber. The mine's owner is Silver Opportunity Partners LLC (Sunshine Silver Mines Corp.)

June 4 - Campbell County, WY - 25 year old Joshua Wishard was an independent contractor for Rogers Construction Inc., at a surface coal mine when he was caught in a rock crusher machine. The coal mine: Peabody Powder River LLC (Peabody Energy), North Antelope Rochelle. 

June 18 - Chesterfield County, SC - 41 year old Christopher Melton was a production supervisor at a surface sand and gravel mine, Hanson Aggregates Southeast LLC (HeidelbergCement AG), Brewer Sand, when he died after the explosion of a slurry pump that was being repaired. 

June 23 - Big Horn County, MT - 58 year old Stewart "Randy" Orcutt was a haul truck driver at a surface coal mine when his truck went through a berm and fell to the trench bottom. He worked for Spring Creek Coal LLC (Cloud Peak Energy Inc.), Spring Creek Coal Co.

July 21 - Colleton County, SC - 28 year old Kyle Martin was an assistant plant manager for Murray Mines Inc. (James O. Murray Jr.) when he suffered a fatal electric shock inside an aluminum boat during a sand and gravel dredge.

July 23 - Hernando County, FL - 51 year old Ismael Santana was a truck driver who was struck by a front end loader while working as an independent contractor for New Line Transport/Tri-State Carriers/Cemex Tri-State Carriers Transportation Service. He was struck at the surface crushed-limestone operation, Cemex Brooksville South Cement Plant. 

August 2 - Valley County, MT - 53 year old Robert Irwin was a plant operator for Fossum Ready Mix, Belzer Plant, when he was caught under a falling dump truck bed under repair at the the surface sand and gravel mine.

August 20 - Buchanan County, VA - 41 year old Michael "Buddy" Justice was a maintenance supervisor who suffered an electric shock while troubleshooting a roof bolter at the Buchanan Mine #1, an underground coal mine owned by CONSOL Buchanan Mining Co. LLC (CONSUL Energy Inc.)

August 25 - St. Lawrence County, NY - 54 year old Glenn Dibble was working at a limestone operation, Gardenscape of New York (David George Kasmoch) as a truck driver when he fell from a stack of pallets. 

September 15 - Walker County, AL - 53 year old Barry Duncan was a bulldozer operator at Manchester Mine when the dozer went off a high-wall at the surface coal mine owned by Black Warrior Minerals Inc. (Roger W. and Rance Perry/Leah Perry-Haynie). 

September 16 - Carbon County, UT - 46 year old Alejandro Ramirez was a shuttle car operator when he was caught in the pinch point of a loader while setting roof supports. He worked at the underground coal mine, West Ridge Mine, owned by West Ridge Resources, Inc. (Robert E. Murray).

September 26 - Harris County, TX - 55 year old Mark Benoit was a maintenance mechanic at the Hockley Mine when he was struck by a forklift being repaired at a surface area of the underground salt mine owned by United Salt Corp. (Texas United Corp.).

October 7 - Bell County, KY - 31 year old Justin Mize was a utility man at Tinsley Branch HWM 61 when he was struck by rock after entering a high-wall opening at the underground area of the surface coal mine. The mine is owned by Commonwealth Mining LLC (Cox Enterprises/LC Management).

October 10 - Berks County, PA - 66 year old Lance Laity was an independent contractor truck driver for F.T. Silfies when he fell between the bulk truck and a loading rack. He was working at the surface Evansville cement plant, Lehigh Cement Co. LLC (HeidelbergCement AG).

Octobber 18 - Campbell County, WY - 51 year old Darwin Reimer was an independent truck driver for Dennis McCoy and Sons working at a surface coal mine when the truck went over a high-wall. He was working at Peabody Powder River Mining LLC, North Antelope Rochelle. 

November 10 - Monongalia County, WV - 49 year old Raymond Savage was working at an underground coal mine, Red Bone Mining Co. Crawdad No. 1 Mine as a section foreman when he was hit by falling roof rock while operating a bolting machine. 

November 10 - Floyd County, VA - 45 year old Carl Hixon was a crusher operator who was struck by a flying drill bit as it came unstuck. He was working at the surface crushed-granite operation, R&S Stone Inc. (Terri Reed Smith/Terry Garth Reed). 

November 18 - Calhoun County, TX - 42 year old Jerry McClelland was a job site supervisor who became pinned by the fall of a load suspended from a crane. He was an independent contractor for Turner Industries Group LLC at a surface alumina plant, Alcoa World Alumina LLC, Bayer Alumina Plant. 

November 25 - Pike County, PA - 67 year old James Crane was a truck driver operating a dump truck that went backwards over the edge of a dump site. He was working at the surface crushed-sandstone operation, Edwards Sand & Stone (George F. Edwards, Jr.), South Sterling Quarry.

December 1 - Sandusky County, OH - 46 year old Darwin Glover was an independent contractor for Mark's Excavating & Trucking when his vehicle fell into a mine pond at Millersville Quarry. This was a surface crushed-limestone operation operated by MGQ Aggregates Inc. (Tim E. Bell). 

December 16 - Union County, KY - 34 year old Eli Eldridge was a repairman working at Highland 9 Mine when he was struck by a coal hauler. This is an underground coal mine owned by Highland Mining Co LLC (Patriot Coal Corp.).

December 29 - McMullen County, TX - 21 year old Adrian Moreno was a warehouse bagger who died when his forklift overturned while he worked at a surface clay operation at Zeotech Corp. (O. Paul Leonard, Jr.) Tilden Plant.

January 9, 2014 - Buffalo, WY - 26 year old Joshua Adams of Gillette died on the job from a partial rig collapse at Basic Energy Services oil derrick. He suffered blunt force trauma when he was pinned under several 'sucker' or drilling rods. Details were not given as to how the incident happened.

January 14 - Watford, ND - 37 year old Wes Herrmann suffered fatal burns at a QEP Resources oil well site explosion near Mandaree. He was the owner of a start-up roustabout company, Legendary Field Services, doing a fairly routine tube change on the heater-treater unit when a sudden ignition created an inferno of fire. He died at an area hospital. 

January 15 - Bismark, ND - An unidentified man was found dead on a catwalk of an oil well tank battery. He was gauging the tank setup at a Denbury Resources well site between Dickinson and Manning. Repeated attempts to locate his name have been unsuccessful.

January 15 - Farmington Hills, MI - 37 year old Greg Messenger of Temperance was severely injured on his arms, hands and face when he was trapped in the Jan. 8 fire inside a large storage tower at Piedmont Concrete in Farmington Hills. Six others who were injured in the fire have since recovered. Messenger died at the hospital on January 15.

January 15 - St. Louis, MO - 50 year old Scott Knopfel was an overnight manager for the Drury Inn Hotel when he was shot to death by an armed robber at the hotel. The suspect came up to the hotel doors, which were locked, but was allowed in by Knopfel. After entering the hotel lobby and engaging in conversation with Knopfel, the suspect showed a handgun and vaulted the counter. Knopfel and the suspect engaged in a struggle while the cash drawer was opening before shooting Knopfel in the head.

January 16 - Augusta, GA - 59 year old Ralph Avery Zorn of Barnwell, SC died on the job due to a fall.  He was standing on a scissor lift at Machuch Steel in Augusta when another piece of equipment reportedly struck the lift, causing him to fall approximately 20-30ft. He died later that day at an area hospital.

January 17 - Annapolis Junction, MD - An unidentified worker at Rock's State Park was killed when he was struck by falling jersey wall falling from above, possibly caused by moving equipment. He and several other workers were assisting in the effort to stabilize a portion of Route 24 in the park. He was an employee for Corman Construction who had the lead in the project. C.G Enterprises, Inc. is the parent company for Corman. Repeated attempts to local his name have been unsuccessful.

January 17 - Melbourne, FL - 36 year old Leonardo Coppola died a hero when he was shot to death by an employee's husband as he tried to shield her from harm. Police have confirmed that Coppola tried to move the woman out of the way when her husband turned up at the mall to confront her and also attempted to grab the gun away. The wife was shot but survived. Coppola was the owner of a pizzeria at the mall in Melbourne, name unknown.

January 17 - San Marcos, CA - 48 year old Jabar Issa of El Cajon was a tow truck driver who was helping fix a flat tire for a motorist when he was struck and killed by another motorist. The name of Issa's employer is unknown.

January 17 - Burbank, CA - 37 year old Greg Plitt, Jr., fitness model and Bravo reality star, was apparently shooting a fitness video on a train track as he had before, but this time he was struck and killed by a commuter train near the Burbank train station. Authorities are investigation how he and his crew got onto the restricted track or who gave them permission to shoot there.

January 17 - West Palm Beach, FL - 48 year old Hermino Padilla, Jr. was a plant supervisor for West Palm Beach Water Treatment Facility. He died on the job after he fell through a possible unsafe grate, through the system and into a sewage tank.

January 19 - Joliet, IL - 24 year old Mohammad Y. Eid was a store clerk for Gardner Food Mart when an armed gunman came into the store to rob it. He pulled out his own gun and a gunfight ensure and that is when he was shot. The gunman also died after being shot.

January 19 - Augusta, KY - 35 year old Brandon William Carl died on the job when the Hopple Street overpass bridge suffered a "catastrophic pancake" collapse during demolition. He was working from an excavator under the overpass when it collapsed and he was crushed. He was employed by Kokosing Construction working for the Ohio Department of Transportation.

January 20 - Center Point, AL - 20 year old Chance Richard Rolland was killed on the job when he was struck by a motorist during road construction while directing traffic. He was employed by T.E. Stevens Construction.

January 20 - Boston, MA - 44 year old Dr. Michael Davidson of Wellesly died on the job when he was confronted by a gunman who shot him while working at Brigham and Woman's Hospital. The gunman was reported the son of a patient of Dr. Davidson's when she died.

January 20 - Rock's State Park, MD - An unidentified worker was killed when he was struck by debris from a falling jersey wall. He and another worker went into the base of a ravine to retrieve some equipment. A lane separator, known as a jersey wall collapsed and fell on top of him. Repeated attempts to locate his name and employer have been unsuccessful.

January 21 - Upper West Side, NY - 61 year old Frank Dragotta of Morganville, NJ died on the job when he fell down an elevator shaft as he working in a building in Manhattan, NY. It is unknown who he was employed by.

January 21 - Cumming, GA - 27 year old Brian Johnson was killed on the job when he was struck by a falling tree while working with a crew that was clearing trees for new power lines. He was an employee for Shawnee Electric Corporation (EMC).

January 21 - Bunker, MO - John Hoodenpyle, Sr. was a miner working at the Fletcher Mine when he was fatally struck by falling rock. He was checking for and removing loose rock from the Doe Run Company’s mine. According to the company, the employee's equipment cab was hit by loose material, which fatally injured him.

January 21 - Everett, WA - Kenneth Staberow was at work for Boeing Company when he collapsed and died of a non work related medical event.

January 22 - Carrollton, GA - 45 year old Charles Pollard of Ranburne, AL was shoveling mud and gravel from a roadway in Georgia when he was struck by a motorist in the road. According to his obituary he was working for McIntyre Lumber Company as an equipment operator.

January 23 - Middletown, CT - 40 year old Jason Strycharz of Portland died on the job after he was struck by a piece of steel that was swinging from a crane inside a warehouse. He was an employee for Kloeckner Metals factory in Middletown.

January 23 - Ticonderoga, NY - 57 year old Jorg Borowski of Schroon Lake died at a local hospital from burns he received earlier in the day at International Paper's Ticonderoga paper mill. Borowski was on the maintenance crew at the mill and was injured in mill’s Power House, which burns fuel oil and bark to generate electricity for the plant’s two paper machines.

January 23 - Phoenix, AZ - 21 year old Grant Ronnebeckwas well liked by his co-workers and loved by family and friends.  Many of them are still in shock over his murder. He was working at the Quik Trip when an armed robbery suspect walked in and shot him over a pack of cigarettes. 


January 25 - Manhattan, NY - 38 year old Moctar Sy was the manager of a Home Depot store in the Chelsea neighborhood of Manhattan. A disgruntled ex-employee who had been fired went into the store and was speaking to another employee when he saw his old boss. He then pulled a gun and shot Sy to death before turning the gun on himself.

January 25 - Brooklyn, NY - 49 year old Rafael Pujols was shot to death during a robbery when he refused to hand over money to the alleged suspect. After shooting Pujols, the suspect fled the Las Vegas Deli & Grocery store without money or property. 

January 28 - Aberdeen, MD - 33 year old Eric Jones of Forest Hill was operating a forklift at Northeastern Supply Distribution Center when another forklift "bumped" into his and he was knocked off. He fell backward approximately 20 feet to the floor.

January 28 - North Charleston, SC - 54 year old David Swain was found unresponsive on the stage of the North Charleston Performing Arts Center next to the basket of a toppled lift. He fell approximately 30 feet to the stage floor. He was the Technical Director and an employee of SMG, the company hired to manage operations at the PAC.

January 28 - Midland, TX - 26 year old Fabian Garduno Martinez died and a co-worker injured when they were knocked off a scissor lift. The two men were demolishing a brick wall with sledge hammers while standing on a scissor lift. The brick wall then collapsed, causing the lift to fall. Garduno-Martinez fell out of the lift and hit his head on the ground. The company the men were working for is unknown.

January 29 - Farmington, CT - 65 year old Neil Hynick of Schofield, WI was a truck driver delivering bundles of aluminum to Stanley Access Technologies when a load of steel and aluminum fell on him. A spokesman for Stanley Black & Decker, which owns Stanley Access Technologies, said Hynick was a third-party contractor but did not name the company he worked for.

January 30 - Chicago, IL - 45 year old Joel Ogiego of Portage, IN died on the job after he was crushed in a construction incident. Ogiego was about 60 feet above ground working inside a crane during the construction of a high-rise residential building. He was leaning into a shaft where a cage, or "elevator," was descending, and pinned him against the crane. The name of the construction company he was working for is unknown.

January 31 - Shawnee, OK - Trooper Nicholas Dees and another Oklahoma Highway Patrol trooper were were responding to a semi truck that had tipped over the median. While they were investigating the incident, the two were hit by another vehicle. The car that struck the officers passed a blocked area instead of staying the lane where traffic was being diverted, going around the police vehicle.

February 1, 2015 - Pueblo, CO - An unidentified man from Denver died on the job after being caught in between equipment while working at Xcel Energy's Comanche Generating Station. A witness said he was walking next to heavy equipment as a large transformer was being moved from one area to another, and was crushed during that operation and later died from his injuries. Repeated attempts to locate his name have been unsuccessful.

February 1 - Renton, WA - Alan Roberts was a shuttle car driver for Shuttle Express, Inc. when he was found unresponsive outside of his vehicle in a store parking lot. It was determined that he died of a heart attack.

February 2 - Lincoln, NE - 30 year old Emil Galabov of Harwood Heights, IL was driving a semi tractor-trailer for an unknown company when he collided with another semi tractor-trailer. Galabov was thrown from the truck during the crash.

February 2 - Carol Stream, IL - 35 year old Chad Nelson of Elgin was killed on the job in an air tank explosion at Maac Machinery manufacturing plant in Carol Stream. The estimated 200 gallon air tank was being tested for leaks when the explosion occurred. There was no fire or smoke associated with the incident.

February 3 - Chattanooga, TN - 54 year old Wallace Scarbro was killed on the job from an unknown cause while working for the Chattanooga Wrigley candy manufacturing plant. The statement by the company does not give a cause of death but an investigation is underway. This is the second death at the plant in 16 months.

February 3 - Hillsboro, TX - 50 year old Michael Fortner of Wiggins, MS was an employee of Strike, the Woodlands-based energy services company when he died on the job. Authorities say it appeared that Fortner, a mechanic, was repairing the brake system on a commercial truck and trailer, when the safety devices failed, causing the trailer to back over and crush him. 

February 3 - Texarkana, TX - 61 year old Jose Andrede of Texarkana, AR was working on a new road in the area when he somehow became pinned between the backhoe and backhoe's boom arm and died. He was an employee for Texarkana Construction Company.

February 3 - Indiantown, FL - An unidentified migrant worker was fatally crushed as he waited to begin his work day at a local farm. Authorities said one worker hopped behind the wheel of the bus when the driver was late to work, pulling it in front of a restaurant. He then tried to back the bus into place, accidentally crashing through a small fence into two men sitting on a bench out front. One worker was killed, the other injured.

February 5 - Glen Ridge, NJ - 31 year old Michael Alfaro of Carteret most likely suffered a fatal electrocution while working on a utility pole near a private residence while PSE&G crews were investigating a power outage in the area. He was found standing on a ladder that was leaning against the utility pole. He was an employee for Comcast.

February 5 - Medora, IL - 59 year old Kenneth Lehr was the beloved Fire Chief of the Medora Fire District. He died when he was struck and killed while on the job.  Authorities say firefighters were in the process of shutting down a section of highway so a medical helicopter could land. That’s when a another firefighter backed up his truck and struck the chief.

February 6 - Highland Park, MI - 33 year old Roderick Jones was an Army Ranger veteran and a member of the Highland Park Reserve Police when he was gunned down in an altercation at a club. Police said Jones went outside to secure the parking lot for club goers and employees when he was ambushed and shot three times in the chest. He started his own security company and was working security for the club.

February 6 - Columbia, SC - 31 year old Marvis L. Myers died after being buried in a trench collapse. He was working for the City of Columbia, four to six feet below ground level working on pipe repairs when the cave-in occurred. He was freed but died later at a local hospital.

February 6 - Manhattan, KS - 30 year old William Wiske of Wamego was working to repair a water main when a trench collapsed around him near a street intersection in Manhattan. He was an employee of the Water Services Division of the City of Manhattan.

February 6 - Huntsville, AL - 51 year old Marie Sanderson of Union Grove and 43 year old Aimee Cothran Bearden of Huntsville were both employees for UltraTec Special Effects in Huntsville, a fireworks plant, when an explosion and fire occurred. Several other workers were injured. Authorities are still investigating the cause of the explosion.

February 6 - Florence, KY - 30 year old Joel Rebennack died on the job after he was struck by an alleged drunk driver. Rebennack was standing behind his bucket truck when another vehicle hit and pinned him. The driver who struck him has been arrested for his death. She was earlier arrested and released on an influence charge. The bucket truck belonged to the Bowlin Group and is subcontracted to Time Warner Cable.

February 7 - Bellingham, WA -  Ted R. Carter was at his job with Parker Corporate Services, Inc. dba Pacific Security when he suffered a fatal medical event that was non work-related. He was found deceased by the company client.

February 8 - Irondequoit, NY - 46 year old Scott Ponticello was a lineman for Rochester Gas & Electric when he died on the job of an unknown cause. Reports state he was found on the ground but gave no details as to why.

February 9 - San Antonio, TX - 45 year old Jesus Ruiz died on the job of what is being called a mysterious death. He had been down inside an eight foot hole at the back of the Alcoa Mill Products property when he told a co-worker he did not feel well. After sitting down in the hole, he lost consciousness. Emergency crews were unable to revive him.

February 9 - Kirkwood, MO - 47 year old Chrissy Wilson-Mueller had clocked out of her shift for the United States Postal Service and was walking to her car when she was struck by a motorist on December 30, 2014. She suffered brain trauma, broken bones and a collapsed lung. She never regained consciousness and was recently released to hospice care before she passed away on the 9th.

February 10 - Albuquerque, NM - 25 year old German Caballero of Kermit, TX was working near a well at the Concho rig site when he was struck in the head by a steel beam. Police say he died on the job after the piece of steel, that was being removed from the well, fell onto him. It is unclear who he was working for.

February 11 - Duncannon, PA - 52 year old Theresa Walsh of Millfont was directing traffic as a modular home was was being transported along the road when she was struck by a motorist. She was standing outside her vehicle when a young woman struck her car and ultimately her. She was unable to be revive at the site. It is not clear who she was employed by.

February 11 - Goliad, TX - 40 year old Scotty O'Neal Stanley of Livingston died after falling from an unnamed oil rig in Texas. Other workers heard a hardhat fall and looked up to see Stanley dangling from his safety harness. Authorities believe Stanley slipped and fell, striking his head and being left unconscious. A co-worker was able to lower him down but he died by the time emergency personnel arrived.

February 12 - Lennox, SD - 45 year old Jon Richter was a dispatcher at the Lennox Sioux Steel Company. He was shot to death in a case of workplace violence. Richter was assigning trucks to deliver trailers to different locations and asked an individual to transport a trailer to another state. An argument ensued for an unknown reason and the other worker returned with a weapon and shot Richter and another worker, who survived.

February 12 - Cannon Falls, MN - 81 year old John Simon Schafer of Goodhue was working for O'Reilly Auto Parts in Cannon Falls when he suffered injuries due to a fall. He was 8 feet to 10 feet off the ground while working in a confined area with metal racking. His fall was not observed by others.

February 13 - Joliet, IL - 22 year old Jeremy Hughes died on the job after he was pinned between two vehicles. He was working at Center Point Properties when his truck became stuck on a patch of ice while moving containers.  He exited his tractor when, unknown to the him, another driver began using his truck to try and push him free from the rear. Hughes was then between his vehicle and another container parked next to him, and he was pinned.

February 13 - Lexington, KY - 27 year old Nicholas A. Hamm of Georgetown was killed on the job when his work truck ran off a two-lane stretch of roadway, snapping a utility pole and hitting two trees. Hamm, who police say was alone in the truck, lost control when his right-side wheels dropped off the steep edge of the pavement and apparently couldn't steer the vehicle back onto the road.He was employed by Matco Tools.

February 13 - Citronelle, AL - 25 year old Marshall James Walton of Mobile died on the job after a pickup truck hit the trailer he was working on. The driver of the vehicle stayed on scene after the accident. Walton was working for his family's business, John G. Walton Construction Co., Inc.

February 14 - Hershey, NE - 59 year old Russ Peterson of Ogallala was killed on the job in an incident at Simplot Grower Solutions. Peterson was trapped inside a ladder safety cage between the grain leg and the roof of the building when the leg he was climbing collapsed onto the roof. Authorities said it appeared the leg was rusted at the base.

December 31, 2014 - Sweden, NY - 53 year old Thomas W. Taylor of Holley died on the job when he was struck by a vehicle. Taylor was plowing driveways when he parked his truck in a driveway and got out of his pickup to look for a customer's address. He was walking on the shoulder when he was struck by an oncoming vehicle. His employment is not known.

January 1, 2015 - Fort Edward, NY - 41 year old Melvin W. Jacobie was killed on the job when he was crushed under a tractor trailer semi he was working under when it rolled down an embankment, running him over. It is not known who he was working for.

Following is the second half of February 2015.

February 11 - Southbridge, MA - 22 year old Kevin J. "George" St. Pierre, Jr. was a tow truck driver attempting help a stranded motorist on MassPike when he was struck and killed by a motorist driving a box truck. He was employed with Sturbridge Service Center for the last year.

February 14 - San Jose, CA - 35 year old Manuel Alvarez Zuniga was a security guard for SJ Security when he was shot to death on the job over a stolen box of cigarillos. He was working security at the BBoss Club when a man grabbed a box of cigarillos at a store next door to the club and began running to a getaway car. Zuniga and a second guard noticed the commotion and spotted the thief running to the car and dropping cigarillos in the process. Soon afterward, someone in the car began firing. Zuniga saved the life of his fellow guard when he stepped in front of him when the shots rang out, said the other guard.

February 15 - Pendleton, SC - 52 year old Kenneth M. Stanton died on the job when he was struck by a drunk motorist as he was directing traffic around a wreck. The Anderson SC Fire Department volunteer was struck when a vehicle went around a tow truck through the scene and striking him.

February 16 - Richland, WA - Robert Wildenborg was an employee for Fred Meyer Stores, Inc. when he suddenly suffered a fatal heart attack. He had gone on break and was found by a co-worker. It was ruled a natural causes death.

February 16 - Marietta, GA - 49 year old Norberto Galicia Romero was killed on job while working inside a silo in Houston, TX.  Romero was an employee for an unnamed company hired by Thomas Concrete to clean inside a silo. As he attempted to unclog a "hopper" in the silo, he fell and there was a collapse and he was buried by sand and gravel. 

February 17 - North Versailles, PA - 48 year old Charles Shahan of Mt. Pleasant was working in an aerial bucket lift trimming trees when he fell some 30 feet out of the bucket. He was an employee for Davey Tree Expert Company. It is unclear how he came to fall from the lift. 

February 17 - Shelbyville, TN - 38 year old James A. Currier died on the job after falling into a running piece of machinery at the Tyson Foods Poultry plant in Shelbyville. Currier worked for QSI, a division of The Vincit Group which provides plant sanitation services.

February 17 - Livermore, CO - 29 year old Joshua Lascelles from Ogden, UT was operating a tractor trailer semi truck in Colorado when the right wheels of the truck went off the road. Authorities report that Lascelles over-corrected causing the semi to rotate and turn on its side. It is not known who his employer was.

February 17 - Houston, TX - 19 year old Frank Madrano was working alone inside a home under construction as his cousin was working in another home nearby. A man entered the home where Madrano was working and shot him while robbing him. The company the men were working for is not known. The cousin was able to give the police a description of someone he saw running from the house after alerting to the gunshot noise.

February 17 - Keizer, OR - 40 year old Christopher Pierson was driving piles on a retaining wall project for Union Pacific near railroad tracks when a 35-foot beam he was placing detached and pierced the cab of his crane. He suffered fatal head injuries. He was an employee for Axis Crane and was a member of Operating Engineers Local 701.

February 18 - Souderton, PA - 25 year old Danny Vasquez was stabbed to death during a confrontation with a co-worker just outside the JBS Meatpacking plant. No known motive for the fight has been given. 

February 20 - Heilwood, PA - 29 year old Todd Trimble of Gallitzin was working in the Rosebud Heilwood mine when a large rock dislodged from the mine roof, crushing him.

February 21 - Alcoa, TN - 64 year old Daniel L. Word of Louisville died on the job while working at the Alcoa Tennessee Operations North plant. The circumstances surrounding the incident are not clear but it involved the aluminum can sheet that are processed there.

February 18 and 22 - Canton, MA - Two unidentified workers died on the job after falling while attempting to clear snow from roofs. One worker fell about 40 feet through a skylight. No further details are known about his death. The other worker was also clearing snow from the Draper Knitting Co. when he fell. Reports state he was 35 years old.

February 23 - Tacoma, WA - Thomas Burzloff was working his normal job at The Home Depot when he fell and hit his head. He was climbing down portable stairs when he fell several steps to the ground. He was knocked unconscious when he hit his head and began to have seizures. He died at a local hospital.

February 24 - Mabton, WA - Randy Vasquez was an employee for Riverview Ranch and was driving a loader with a push blade on front. It is reported that he drove into a collection basin of manure from alleys which is about 6 - 8 feet deep. He died of asphyxiation due to inhalation of dairy waste water sludge.

February 26 - Fort Bliss, TX - Jesus Moreno of El Paso was a construction worker killed at the site of the new William Beaumont Army Medical Center. A co-worker said that Moreno was welding when something 'exploded' causing part of the structure he was on to fall apart, causing him to fall. The name of his direct employer is not known.

February 27 - Rochester, MN - 30 year old Brian Baird of Lehi, UT was killed on the job at a Minnesota hotel construction site after being struck by a piece of equipment.  He was underneath a telescopic forklift when a "very large attachment" fell from the arm of the lift. He was employed by Far West Construction of Utah, contracted by PLC, Inc. for the project.

February 27 - Columbia, SC - 56 year old Paul Lewis Fee of Winnsboro was with a crew doing road work on the interstate when he was struck by a motorist. Fee was an employee for the South Carolina Department of Transportation.

February 27 - Akron, OH - 60 year old William Haring was killed on the job after he was crushed by a piece of equipment. Police confirm Haring was crushed by a machine door or swinging mechanism at Logan Machine Company.

February 28 - West Allis, WI - 33 year old Ryan Calkins of Brookfield was killed on the job working on a road construction project called the Zoo Interchange. A representative from the area OSHA office says that Calkins "came into contact" with a drill rig while he was installing tiebacks on a retaining wall. He was employed by Michels Corporation, which is part of the joint venture Wisconsin Constructors II, LLC for the Wisconson Department of Transportation.

February 26, 2015 - Sarasota, FL - 24 year old Jonathan M. Dennis of Sanford was fatally crushed by a scissor lift as he worked inside an old movie theater at a mall in Sarasota. He was employed by Creative Environmental Services of Orlando, a subcontractor for Gerardi Construction, Inc.

March 2, 2015 - Dallas, TX - 54 year old David Frank Ostang was a mechanic of 30 years for American Airlines. He was closing and locking down the door of a Boeing 767 plane when he somehow slipped and fell from the jet bridge. He was also a member of the Transport Workers Local 591.

March 2 - Johnstown, PA - 38 year old Janice Livingston was a paramedic for West End Ambulance Services when she was struck and killed in the line of duty. She died in the last of a sequence of three crashes that took place in Upper Yoder Township. After a chain reaction of crashes occurred a coal truck came down the icy road and plowed through the emergency scene. The coal truck road the guardrail in an unsuccessful effort to stop. 

March 3 - Sharonville, OH - 70 year old Manuel Aquino of West Chester was fatally crushed by equipment while working for Art's Equipment Rental. He was working with a front-end loader when he became trapped between the bucket on the piece of equipment and the machine itself. 

March 3 - Kanawha, TX - 32 year old Paul Chelius of Paris went missing on The Circle C Ranch in Red River County and was later found dead inside a grain elevator silo. He had apparently fallen into the silo.  

March 3 - Campti, LA - 42 year old David Scott Long was working with a crew at the International Paper's Red River paper mill when he fell from scaffolding. His harness became entangled in a machine, causing him to fall. He was an employee for the Boch Group. 

March 5 - North Philadelphia, PA - 30 year old Robert Wilson III was a member of the Philadelphia Police Department when he was fatally shot during a robbery at a local store. Wilson was at a Game Stop store while on duty to do a "security check" and to buy a gift for his 8-year-old son, who was being rewarded for doing well in school. A shoot out ensued after two men, intending to rob the store, encountered Wilson. The suspects were later captured and charged for Officer Wilson's death.

March 5 - Brownsville, TX - Vidal Garcia, a US Navy veteran, was working his normal shift at Galaxy Bowling Center when he was killed on the job. He was working behind the bowling lanes when his shirt became entangled in the machine that picks up bowling pins. 

March 6 - St. Louis, MO - 52 year old Ronald Rector of Linn was piloting an ARCH Air Medical helicopter when it crashed. He was the only person on board and was heading to pick up another crew after having dropped off a crew and patient. The cause of the crash is not clear.

March 6 - York, PA - 63 year old Stanley Foller was killed on the job while working for New York Wire Company. He died of asphyxiation when a spool of wire weighing several thousand pounds fell on his chest.

March 7 - Coney Island, NY - 31 year old Toni Jackson, a New York City Housing Authority maintenance worker died on the job from an apparent fall. She was found unconscious next to a trash compactor in the basement of a building that is part of the Coney Island Houses. She was rushed to a local hospital but did not recover.

March 9 - Cameron, WV - 45 year old John M. "Mike" Garloch of Neffs, OH was working in a Murray Energy mine when he was struck by falling rock as the mine roof collapsed. He was an assistant longwall coordinator. Other injured workers were released from the hospital. 

March 9 - Moreno Valley, CA - 27 year old Kyle Sebulsky, an employee for Road Builders, Inc. was driving an earth mover truck performing roadway services for Burbank City at it's Landfill No 3 when he was killed on the job. The truck lost traction and rolled down an embankment, causing Sebulsky to be thrown from the truck and down the embankment.

March 10 - Upton County, TX - Three members of one family were killed on the job during an oil rig explosion and fire. Rojelio Salgado, Arturo Martinez, Sr. and Arturo Martinez, Jr., all from Andrews, TX, were working for the independent contractor Mason Well Services at an oil rig owned by Parsley Energy. The cause of the explosion is not clear.

March 10 - Santa Rosa Sound, FL - A Louisiana National Guard helicopter was performing training activities when, during thick fog, it crashed into the water. Following are the military men who perished in the crash.

33 year old Master Sergeant Thomas Saunders of Williamsburg, VA
33 year old Staff Sergeant Liam Flynn of Queens, NY
29 year old Staff Sergeant Trevor P. Blaylock of Lake Orion, MI
Staff Sergeant Kerry Michael Kemp of Port Washington, WI
26 year old Staff Sergeant Andrew Seif of Holland, MI and
27 year old Staff Sergeant Marcus Bawol of Warren, MI

March 11 - Brent, AL - Keith Leverette was killed on the job when an explosion and fire occurred at the Brent Industries plant in Brent. Several other workers were injured in the explosion. Other employees say they make refurbished gloves at the plant and that a cleaning process requires the use of hazardous chemicals. Several of the employees told authorities that a combination of static electricity in the washing and drying section plus those chemicals could have sparked this explosion.

March 12 - Kansas City, MO - 23 year old Colton R. Frisbee was a painter working with others at Inland Truck Parts & Services when the lift they were working from became unstable. As the scissor lift became unstable to the point of tipping Frisbee jumped from the lift but it unfortunately fell onto him, pinning him. It is unclear who their employer was. 

March 12 - Eufaula, OK - Matt Mathews was killed and two other were injured when their Eagle Med helicopter crashed in Eastern Oklahoma. Matthews was the pilot of the helicopter. The cause of the crash is unknown at the time.

March 12 - Medina, WA - 34 year old Joe Arrants was working at a construction site for the new SR 520 bridge across Lake Washington when he fell from the structure. He fell off the edge of a bridge deck about 60 to 70 feet to the ground. Authorities investigating say it appears that his self-retracting lanyard broke during the initial fall from the platform.He was employed by Kiewit General Manson.

March 13 - Forest City, NC - 27 year old Blake Tyler Luckadoo of Rutherfordton died on the job after being thrown from a piece of heavy equipment. He was an operator for Sisk Grading preparing a site for the county. He was found unresponsive next to the machine.

March 13 - Utica, NY - 23 year old Donald R. Frendenburg had just taken his service oath as a New York State Trooper three days prior to his death on the job. During a basic-training run, in his second week of training, he collapsed. He later died at an area hospital.

February 3, 2015 - Kent, WA - 49 year old Peter Ploeger of Cle Elum lost his battle for life after being injured on January 12, 2015 while working for Hermanson Company, LLP.  He fell from a step ladder onto an electrical junction box which caused damage to his leg requiring surgery. Unfortunately he died from complications from the surgery.

March 6 - Wenatchee, WA - Sergio Nuno Camacho was doing his normal job of driving a Stemilt Growners company vehicle to bring trees back from a nursery when he pulled to the side of the road and called for help. Emergency workers were unable to revive him and he died of a heart attack.

March 9 - Tacoma, WA - 52 year old Randal Simmons suffered a medical event as he was helping to load a work van for a painting job. He was transported to an area hospital where he died of a stroke. He was an employee for Saxon Painting Company at the time of his death.


March 16 - McClure, VA - 34 year old David Brummitte of Pound was a certified workplace examiner for Alpha Natural Resources Deep Mine 41near McClure when he died on the job after being struck by rock "sluffing off the mine wall". 

March 17 - Everett, WA - David Whipkey was an office worker for The Boeing Company when he collapsed at his desk on February 25th.  He was transported to an area hospital where he passed away.

March 18 - Omaha, NE - 18 year old Michael Petersen was a collection driver for Abe's Trash when he was killed on the job. He stepped from the drivers seat and was struck by a motorist.

March 18 - Port Angeles, WA - 49 year old Anthony Commeree of Sequim died of a medical event while working on a boat, working in an equipment room. He was found slumped against a beam and unconscious by a co-worker and was not able to be revived. He was an employee for Westport LLC, a boat building company, at the time of his death.


March 19 - Tolland, CT - 50 year old David L. Ridzon of Willington was working to fill pot holes in a road when a town dump truck backed into him. He was transported to an area hospital where he died. Ridzon was an employee for the City of Tolland Public Works Dept.




March 23 - Goldsboro, NC - 35 year old William Grey Bailey of Kenly was a North Carolina Dept. of Transportation employee who was working along a roadway in the median when he was struck and killed by an impaired motorist. The motorist has been arrested.





March 23 - Lebanon, CT - 54 year old Donald Merchant of South Windham was using equipment to move corn from a large mound at the Square A Farm in Lebanon. When he got off the equipment, some of it toppled onto him. Other farm workers dug him out but he was unresponsive.




Raleigh, NC - 41 year old Erasmo Hernandez, 33 year old Lose Luis Lopez-Ramirez and 33 year old Anderson Almeida were killed on the job and another injured when a mast scaffolding the were using fell from the Charter Square Construction Project. It is not clear who their employer was.


March 23 - Montrose, PA - 48 year old Daniel Acker died on the job while working in a quarry leased from owner Peter Kane.  Acker was operating a block saw on a bed of stone with his back to the highwall, and he apparently slipped on loose material or ice and fell several feet with the running saw landing on top of him. This caused severe lacerations to the back of his legs causing him to apparently bleed to death onsite.





March 24 - Fond du Lac, WI - 21 year old Trevor Casper was a Wisconsin State Trooper who was only three months out of the Academy when he was killed during a shoot out with a bank robber. He was on his first solo day when he encountered the suspect who had also killed another person in his attempt to get away.




March 24 - San Jose, CA - 38 year old Michael Johnson was an officer for the San Jose Police Department when he was killed on duty. He and other officers were responding to a call about a suicidal person when they were met with gunfire as they approached the apartment building. the gunman was found dead on his balcony.




March 24 - Minot, ND - 37 year old Brian James Story and 50 year old Christopher Rusk Sluder were shot to death by a worker who was part of a verbal altercation the day before regarding work performance. Story was part of an employee development programat Turtle Mountain Pork, while Sluder was the manager. The worker was arrested for the murders. Turtle Mountain Pork is managed by AMVC, a swine management company based in Audubon, Iowa.



March 24 - Killeen, TX - 61 year old Frank Wilson was a truck operator for the City of Killeen when he was killed on the job at the city's Solid Waste transfer station. He was apparently struck by a piece of motorized equipment. 






March 25 - Sultan, WA - Dennis Blansett was a machinest for Docufeed Technologies, Inc. preparing to set a milling machine when the owner saw him collapse onto the floor. He was resuscitated and transported to an area hospital when he suffered a second cardiac event and did not recover.


March 25 - Inchelium, WA - 54 year old Jeffery Sackman died on the job when he was crushed by a falling tree as he was cutting a separate tree. He was employed by Ram/Z Excavating and Construction Company at the time of his death.






March 25 - Hanford, CA - 51 year old Luis Manuel Santos was killed on the job when he was electrocuted. He was a truck driver for Penny-Newman Grain of Fresno and while unloading grain he came into contact with overhead power lines. He suffered head and neck injuries associated with electrocution. 




March 25 - Bridgeport, CT - 30 year old Jose Araujo of Milford was working inside a trench for Burns Construction when a man walked up to him and shot him. The man who shot Araujo had walked up to a job site foreman inquiring about a job and was told him to fill out an application at the company office. The man walked away but then came back and fired multiple shots, striking Araujo.




March 26 - Blaine, WA - Terry Whight was attempting to climb onto a tractor used to remove a pool liner when he fell backwards onto the ground. It has been determined that he passed away from natural causes. He was employed by Beachwood Resort at the time of his death. 

March 27 - Miami, FL - An unidentified maintenance worker was killed on the job when he was struck by a glass table top that flew off a balcony. He was working on an electric gate at the Flamingo Apartments when the piece of glass flew off one of the balconies on the ninth floor due to the high winds. Repeated attempts to locate his name have been unsuccessful.



March 28 - Morgantown, PA - 27 year old Jacob Lopez of Ephrata was working in the melt shop of Timet Titanium manufacturing plant when an explosion occurred.  State police say a boiler about 35 feet in diameter exploded while using titanium for production inside the plant.
State police said Monday that a five- or six-foot-tall boiler that was 20 to 25 feet in diameter exploded while using titanium for production

March 30 - Harrison County, MS - 40 year old Brandon Seth Ricks, a pilot for T & M Aviation from Blanchard, OK and 55 year old Steven D. Cobb, a member of the US Forest Service were both killed in a helicopter crash. The helicopter was assisting with a prescribed burn when it suddenly crashed into a wooded area. Nearby witnesses said right before the crash, they heard the helicopter overhead, and it sounded like it was having engine troubles. 

March 30 - Mount Vernon, WA - Lisa Eison was an employee for Mert's Taxi when she was shot to death while on duty. Attempts to locate further information has not been found.

March 31 - Battle Mountain, NV - 47 year old Ronald Paul Raiche, Jr. died on the job when he was struck by a motorist. He was working in a two-man crew on the right shoulder of I-80 west of Battle Mountain to repair cracks when a commercial vehicle drifted onto the shoulder. He was an employee of the Nevada Dept. of Transportation.

March 31 - Soundview/Bronx, NY - Victor Chafla was a long time employee for VZ Deli, a bodega in the Bronx. He was the unintended victim of a shooting March 26th outside the store as he was stocking fruits and vegetables in an outdoor display.

April 1, 2015 - Brighton Beach, NY - 50 year old Vidal Sanchez-Roman of Brooklyn was killed on the job when he fell six stories from a scaffold on a construction site. We were not able to determine who he was employed by.


April 1 - Warwick, NY - 38 year old Tonny Teskera was working with a crew at Hickory Hills Golf Course on a tree clearing project when he was struck and killed by a fallen tree. He was rushed to a rear by hospital where he died. He was an employee for Orange County Parks and Recreation.



April 1 - Richmond, VA - 54 year old Kevin Lee Eskew of Williamsburg was killed on the job when he was struck by a train in the CSX Acca Yard near Richmond. Circumstances for the incident are not known. He was an employee for CSX Railway Corp. 

April 2 - Clermont, FL - An elderly driver attempting to make a turn, pulled into the path of another driver who ran off the road way and struck three Service Electric Company utility workers, killing all three.  The following workers were from different cities working in Clermont:  

35 year old Jeremy Scott Bradshaw was from Albany (photo not found)

 

34 year old Dewey Summerlin was from Franklin


  
 
40 year old Jeffrey Estes resided in Ringgold.




April 2 - Argyle, TX - 36 year old Julio Ledesma of Fort Worth died on the job as the result of a structure collapse. He was working on the frame of the new Argyle High School athletic center when it collapsed under him. He was employed by Warnick Metal Building Erectors.


April 3 - Wintersville, OH - 25 year old Nick Desarro of Wellsville was operating a forklift at a Walmart Distribution Center when he fell from the forklift. Authorities say was standing up in the forklift when he fell onto boxes with his feet were still inside the forklift. The sheriff said there was a small puddle of blood inside the forklift, leading them to suspect that maybe Desarro had suffered a medical issue. He was taken to an area hospital where he died of his injuries.

April 6 - Manhattan, NY - An unidentified 22 year old construction worker died on the job when he was buried in a trench collapse. He was working in a hole at the Meatpacking District construction site when the dirt gave way and collapsed in on him, police said. Repeated attempts to locate his name have been unsuccessful.


April 6 - Culpeper, VA - 27 year old Sonny L. Streightiff of Orange was working with a crew for an unknown employer to clear trees on a piece of property. As he was cutting a tree, one of the limbs broke off and fell onto Streightiff. 



April 7 - Argyle, TX - 22 year old Daniel Moran of Haslet was working on the construction site for the Argyle High School when he was killed on the job. Moran was guiding a tractor trailer semi truck from behind it when he somehow got caught by the back right tires of the last trailer. It is not clear who Moran's employer was but Northstar Builders Group suspended work since Moran was the second worker killed in a week on the job site.

April 10 - Fall Creek, WI - 29 year old Dustin John Keith of Altoona was a public works employee for the Village of Fall Creek when he was killed on the job. He was operating a lawn tractor equipped with a brush on the front for clearing snow when he was struck by a Union Pacific train. The bike/walking path he was clearing crossed paths with the railroad tracks. 

April 11 - Schuyler, NE - 26 year old Craig Allen Anderson of Columbus was killed on the job of a construction site south of Schuyler. Anderson was driving a payloader while attempting to free a cement truck that had become stuck by pulling the vehicle with a chain. The chain snapped and struck Anderson. He was employed by Gehring Construction and Ready Mix.


April 11 - Mattawa, WA - 57 year old Daniel Martinez Cortez died on the job when a tractor he was operating overturned. He was pulling an empty sprayer when he apparently over corrected after going off the side of the road, causing the tractor to overturn. He was employed by Evans Fruit Company, Inc.

April 12 - Seattle, WA - Pedro Lopez was injured on March 31 when he fell 19 feet to the ground when he unhooked from his fall protection system.  Exact circumstances are not known but he died on April 12 after being hospitalized. He was an employee for Key Real Estate Services LLC. 

April 13 - Orlando, FL - 36 year old Gary Terry of Davenport was a professional driving instructor and senior operations manager at the Exotic Driving Experience at Walt Disney World. He was the passenger in a Lamborghini with a man who was celebrating his birthday with a driving experience when the driver failed to maneuver the high-powered vehicle through the course and the sports car's passenger side struck the guardrail. Terry was employed by Petty Holdings who operates the Exotic Driving Experience.

April 13 - Goldsboro, NC - 44 year old Ronald Dwight Lane had just arrived at work at Wayne Community College when he was sought out for a targeted attack by another man who shot him. Lane was the Print Shop supervisor at the college. The man targeted Lane was arrested and has been charged with murder.


April 15 - Elk Grove Village, IL - 50 year old Wayne Follman of Lake in the Hills was killed on the job after he was caught in a piece of machinery while working at Acme Industries plant. He somehow because caught in a power lathe, used to shape wood or metal products.



April 15 - Petaluma, CA - 28 year old Jared Overfield of Novato worked as a pipe layer for the San Rafael based Maggiora & Ghilotti Construction. He was on a construction job along Highway 101 when he was crushed by a giant rolling metal pipe. Just prior, a forklift operator set down a large metal pipe sealed in concrete but the pipe broke loose and started heading towards the highway. Overfield reacted and tried to stop the rolling pipe, but was crushed in the process, according to investigators.  (video photo in link)
